Tetravalent (A, C, Y, W 135) meningococcal vaccine in children: immunogenicity and safety.

Tetravalent meningococcal polysaccharide vaccine (groups A, C, Y and W 135) was administered (s.c.) to 26 healthy children, aged three to 13 years old, in order to assess safety and immunogenicity of the vaccine. Side effects were very mild in all children, generally consisting of local soreness for a few hours following injection. A fourfold increase in bactericidal antibody titres was obtained for the four components in 100% of initially seronegative children. This vaccine thus appears as safe and immunogenic in children as it is in adults.

The in-vitro activity of pristinamycin against Haemophilus influenzae and Neisseria meningitidis.

The in-vitro activity of erythromycin, oleandomycin, spiramycin, josamycin and pristinamycin was tested by a plate-dilution method against strains of Haemophilus influenzae and Neisseria meningitidis. Pristinamycin was the most active product tested with minimal inhibitory concentrations (MIC) ranging between 0.5 and 4 mg/l for H. influenzae (modal value 1 mg/l) and between 0.03 and 0.12 mg/l for N. meningitidis (modal value 0.06 mg/l).

[Pneumococcal vaccination of the pregnant woman in Africa and passive immunity of the child. Immunologic control by the ELISA method].

Thirty-seven african women (mean: 23 years old) were immunized with a pneumococcal vaccine and tetanus vaccine, in a comparative study with seventeen (mean: 24 years old) women only tetanus immunized. Immune response to pneumococcal polysaccharide was studied by ELISA. All women had antibodies before vaccination. Their kinetics in mothers and infants, did not show a significant difference between the two groups. In infants, the rate of IgG antibodies significatively decreases from the date of birth to the fourth month of life. There is no foetal immunisation as shows the total lack IgM antibodies in umbilical cord blood. These results suggest that pneumococcal vaccination pregnant women in order to protect infants till 6-9 months, is not valuable.

[Pneumococcal postoperative endophthalmitis].

Three cases of pneumococcal endophthalmitis proven by paracentesis and direct bacteriologic evaluation were studied. Cultures were positive for pneumococcus in two the three cases. The fulminant course, and poor visual outcome emphasize the urgency of proper diagnosis by microscopic identification, and of intra-vitreal of injection of antibiotics sometimes associated with vitrectomy. Subconjunctival gentamicin commonly used in ophthalmologic surgery as prophylactic treatment, is often not effective against pneumococcus. It seems advisable to associate subconjunctival chloramphenicol with the gentamicin to help prevent this bacterial infection.

Pharmacokinetics of rifampicin and desacetylrifampicin in tuberculous patients after different rates of infusion.

Four rates of rifampicin infusion ranging from 3.3 to 15 mg/min in 12 tuberculous patients were studied. Blood samples (n = 10) were drawn during infusion and 8 h later. Urine samples were collected in six fractions during a 24-h period. Rifampicin and desacetylrifampicin were measured by high-pressure liquid chromatography. Results show that the maximum plasma concentrations increase linearly for each dose with the rate of infusion, and that the amounts excreted in the urines are highly dependent on the administered dose. Simulation of plasma concentrations after different dosage regimens shows that a double rate of infusion--20 mg min-1 during 1 h and then 200 mg h-1--allows plasma concentrations to be quickly reached and maintained at a 20 mg L-1 level, far higher than the minimum inhibitory concentrations of most germs.

[Evaluation of the clinical activity of cefotiam (SCE 963). Multicentric study in 7 centers].

Cefotiam (SCE 963), a new, broad-spectrum, third generation cephalosporin was used in the treatment of 136 patients suffering from respiratory tract infections, urinary tract infections, septicemia, meningitis, biliary tract infections and osteoarthritis infections. Cefotiam was administered in monotherapy to 98 patients at the mean posology of two grams per day (extreme doses: 1 to 6 g). The following clinical effectiveness was noted: 83 successes and 18 failures on 101 available clinical reports. The general, biological tolerance and renal tolerance was good in all patients.

[Comparative study of tinidazole and ornidazole for the prevention of septic complications of colonic surgery].

In a previous comparative study, authors have proved the significantly reduction of post-operative infections under elective colonic surgery by a preventive treatment with ornidazole IV (versus placebo). They have compared preventive effect of tinidazole IV short treatment (1,60 g in 24 h) and ornidazole (4 g in 4 days) in 2 randomised groups each of 20 patients. The same incidence of post-operative complications suggest an equal efficacy of two treatments.

[Cervical tuberculous adenitis in the adult. Diagnostic, therapeutic considerations apropos of 27 cases].

The authors review 27 cases of cervical tuberculosis in the adult, complete and homogeneous and essentially describe their experience concerning positive diagnostic factors as well as their therapeutic approach. Considering the results obtained, they favour early surgical treatment ranging from simple adenectomy to complete and conservative cellulo-lympho-adenectomy which is the essential treatment of choice in multi-nodal forms. This surgical attitude makes possible to considerably shorten the duration of the local course by eliminating recurrences but on the essential condition that it is accompanied by prolonged medical treatment consisting of a triple association of major anti-tuberculous drugs.

[Antibacterian activity modifications of serum isoniazid when using it with rifampicin (author's transl)].

When dosing isoniazid in patients' serum after ingestion, the authors find a difference in the isoniazid levels when isoniazid is used alone, then, when isoniazid is used simultaneously with rifampicin. The level decreases or increases according to the inactivation index. They analyse the therapeutic consequences which can result from it. They infer, from the practical point of view, the necessity of : dosing isoniazid after having ingested it simultaneously with rifampicin and not alone when the efficient therapeutic dose is to be determined ; testing serum levels during antituberculous therapy for an eventual readjustment.

[Incidence of infectious diseases in general practice. Results of a survey conducted in 1979 (author's transl)].

In 1979, a sample survey was conducted among 53 general practitioners to determine the incidence of infectious diseases in general medical practice. The survey showed that 45% of the patients seen by these doctors were suffering from an infection, that 53% of these (i.e. 24% of all patients) were treated with antibiotics and that G.P.'s play an important role in preventive medicine. These results might exert a useful influence on the teaching of infectious diseases to medical students and physicians.

[The pneumococcal infection in a general hospital of a parisian suburb: a prospective study in Villeneuve-Saint-Georges hospital (author's transl)].

The authors have carried during one year, in the hospital of Villeneuve-Saint-Georges, a prospective study on 229 cases of pneumococcal infections with bacteriological findings. They emphazise the frequency, the high-exposed groups and the gravity of these infections despite of the antibiotherapy. The emergence of multiply resistant pneumococcus makes out another argument for proposing a preventive vaccination taking notice of the most frequent serotypes found in France.
