Penicillinase-producing Neisseria gonorrhoeae ophthalmia neonatorum in Singapore.

The problem of ophthalmia neonatorum due to penicillinase-producing Neisseria gonorrhoeae (PPNG) has grown with the increasing prevalence of PPNG strains in Singapore. The epidemiological trends, clinical features, and treatment of ophthalmia neonatorum due to PPNG have been determined from the case histories of 11 babies with this condition treated between 1978 and 1980. All 11 cases were cured with a single intramuscular injection of kanamycin 0.5 g and 1% kanamycin eyedrops instilled for three days. In six of the 11 mothers, a non-PPNG strain was isolated.

Ceftriaxone in the treatment of ordinary and penicillinase-producing strains of Neisseria gonorrhoeae.

Ceftriaxone, a third generation cephalosporin, was used in a single intramuscular dose with oral probenecid to treat 124 men with infections due to non-penicillinase-producing Neisseria gonorrhoeae (non-PPNG) and 64 men with infections due to PPNG. Three different doses of ceftriaxone were used--125 mg, 62.5 mg, ad 32.5 mg, and 32.5. The cure rate for all PPNG infections with the different doses was 100%. The cure rate for the non-PPNG infections with ceftriaxone 125 mg was 100%; those for non-PPNG infections treated with ceftriaxone 62.5 mg and 32.5 mg were 96.2% and 97.3% respectively. The 160 strains of non-PPNG and 60 strains of PPNG isolated were all susceptible to ceftriaxone with minimum inhibitory concentrations of 0.008 microgram/ml. These results are compared with those using kanamycin 2 g. Ceftriaxone is a safe and effective treatment for PPNG and non-PPNG infections.

Contact sensitivity to topical medicaments.

Of patients patch tested at a contact dermatitis clinic between 1985 and 1986, 22.5% (379/1685) had medicament sensitivity. The prevalence in men was 22.8% (193/847) and in women, 22.1% (186/838) (difference not significant). The rate appears to increase with age, 17.9% (216/1205) for patients less than 40 years old and 34.0% (163/480) for patients over 39 years old (difference, p less than 0.001). The differences in prevalence between Chinese (22.8%), Malay (21.3%), Indians (20.4%), and others (33.3%) were not statistically significant. Common sensitizers included neomycin (7.8%), proflavine (7.1%), clioquinol (4.3%), colophony (3.3%), and wool alcohol (2.7%). Proflavine sensitivity was significantly more common in men (9.9%) than women (4.2%) (p less than 0.001). Most dermatitis occurs on the limbs (upper limbs, 44.3%; lower limbs, 43.0%). Facial dermatitis was more common in women (37.4%) than men (19.2%) (difference not significant).

Evaluation of a safe-sex intervention programme among sex workers in Singapore.

The purpose of this study was to evaluate the effects of a brief intervention programme on STD knowledge, condom use and gonorrhoea incidence among sex workers in Singapore. A controlled before-and-after study design with non-random assignment of sex workers was used, supplemented by multivariate analysis to adjust for baseline differences. Control (n = 221, denoted C1) and experimental (n = 221, denoted E1) groups were interviewed on 2 occasions 3 months apart. Two supplementary groups were interviewed once each at the end of the 3-month period (n = 145 who had received the intervention and n = 151 who had not). Basic knowledge of STD symptoms and HIV transmission was high in all groups. There were misconceptions about casual transmission of HIV which improved dramatically at the second interview for group E1 (from 37-56% correct responses to 82-90%). Overall condom use was high (about 75%) and did not change after the intervention. Gonorrhoea rates were correspondingly low (0.4 episodes/worker/year) and also did not change. This brief intervention improved the STD knowledge of sex workers. However, behaviour as measured by reported condom use and gonorrhoea incidence did not change. Implications for future intervention programmes are discussed.

Chromate allergy: total chromium and hexavalent chromate in the air.

This is a study on atmospheric concentration of total chromium and hexavalent chromate and its role in chromate sensitivity. Air concentration of total chromium and hexavalent chromate in a construction factory, a busy city area, a suburban area, a residential area, and a heavy industrial area were measured by air sampling pumps. Hexavalent chromate was not detected in any sampled areas. Two (concreting areas) of 8 locations in the construction factory had total chromium of 0.2 and 2.3 micrograms/m3 in the atmosphere. It appeared that the atmospheric concentration of total chromium and hexavalent chromate was negligible. These findings indicate that unexplained chromate sensitivity, as so often seen in patients attending a contact dermatitis clinic, is not attributable to exposure to hexavalent chromate in the air.

An outbreak of acneiform eruption in a polyvinyl chloride manufacturing factory.

An outbreak of acneiform eruptions occurred in 14 workers in a polyvinyl chloride (PVC) manufacturing factory. The cause was probably a combination of heat, high humidity and irritation from PVC powder. PVC slurry, scales and powder, to which the workers were exposed, were not comedogenic on rabbit ear. Lowering the temperature and humidity by airconditioning and improving ventilation, together with reduction of dust level at the workplaces, eliminated further outbreak's.

Bullous contact allergy from cinnamon.

A young Malay female presented with a bullous eruption as a manifestation of contact allergy to cinnamic aldehyde in cinnamon. The clinical and histological features resembled bullous pemphigoid but immunological markers for pemphigoid were absent. The patient responded to a short course of oral steroids. Contact allergy can mimic pemphigoid.
