Average evoked responses and psychophysical performance in patients with pseudohypoparathyroidism.

Perceptual function in eight patients with pseudohypoparathyroidism (PHP) was studied with two neurophysiological measures, the average evoked response (AER) and nerve conduction velocity. A battery of psychophysical tasks including reaction time, size estimation, `hidden pictures,' and the rod and frame procedure was also used. Patients with PHP had significantly longer latency visual AER and slower reaction time than a group of normal volunteers, and they performed erratically and poorly on the other psychophysical tasks. Differentiating these patients with diffuse mental deficiency were two relatively specific perceptual response patterns: (1) AER amplitude decreased with increasing stimulus intensity, and (2) reaction time was abnormally influenced by the duration of the preparatory interval. Both of these patterns have previously been found in other patient groups.

Subacute toxicity of several ring-substituted dialkylanilines in the rat.

Aniline, o-toluidine, 2,4-dimethylaniline, 2,6-dimethylaniline, 2,6-diethylaniline, 2,6-methylethylaniline, 2,6-diisopropylaniline, and methylene-bis-2,6-diisopropylaniline were administered to male Fischer 344 rats daily for 5, 10 or 20 days. Histopathologic evaluation of selected tissues revealed splenic congestion, increased hematopoiesis and hemosiderosis, and bone marrow hyperplasia in aniline- and o-toluidine-treated animals. These changes, characteristics consistent with enhanced erythrocytic destruction, were not observed in any of the dialkylaniline-treated animals. Hepatoxicity, characterized by biliary hyperplasia, periacinar vacuolar degeneration, hepatocytic cloudy swelling and periacinar necrosis, was observed in methylene-bis-2,6-diisopropyl-, aniline- and 2,4-dimethylaniline-treated animals. Multifocal discrete areas of necrosis were also observed in livers of animals treated with the latter compound. There were no histopathologic changes which could be attributed to any of the alkylanilines studied in kidney, esophagus, trachea, thyroid, parathyroid or urinary bladder.

Quality of life measures and gender comparisons in North Carolina Cardiac Rehabilitation Programs.

BACKGROUND: Quality of life (QOL) is an important health-related outcome measure in patients with cardiovascular disease. The North Carolina Cardiopulmonary Rehabilitation Association (NCCRA) is a coalition of 72 state-certified, multidisciplinary cardiac rehabilitation (CR) programs. In 1997, the NCCRA Executive Board and Research Committee agreed to collect ongoing data to assess QOL changes following CR participation using the Ferrans & Powers QOL Index Cardiac Version III. The purpose of this study was to determine if changes were present in QOL scores after 12 weeks of CR in men and women from multiple outpatient centers. METHODS: Data were analyzed from 420 patients from 19 Phase II early outpatient CR programs who completed the QOL index survey within the first week of program entry and upon discharge. The four QOL index domains measured were health/function, psychological/spiritual status, socioeconomic status, and family interaction. Overall QOL was tabulated as the aggregate score from the four domains. Each domain, as well as overall QOL, was compared pre- and post-CR participation with a repeated measures analysis of variance. RESULTS: Across programs, statistically significant improvements in QOL indices were observed following 12 weeks of CR in each of the four domains and on the overall score in all patients. Men reported greater CR entry and discharge scores than women on the health/function, family interaction, and overall scores. Women showed a greater positive change in scores than men on the socioeconomic, family interaction, and overall scores. CONCLUSIONS: Patients who participated in Phase II multidisciplinary North Carolina CR programs and completed the protocol improved QOL parameters. While men had higher physical function and overall QOL scores both pre- and post-CR, women showed greater improvements in overall scores, as well as in family and socioeconomic parameters. These findings suggest that North Carolina CR patients experience a positive change in QOL following short-term participation in CR. Further study with a larger number of patients benchmarked with programs from other regions with comparisons to patients who do not participate in formalized CR programs is indicated from this investigation.

Spontaneous transpyloric passage and performance of 'fine bore' polyurethane feeding tubes: a controlled clinical trial.

Certain groups of patients requiring enteral nutritional support are at increased risk of regurgitation and pulmonary aspiration of feed. Positioning of enteral feeding tubes distal to the pylorus has been advocated as a method of reducing such complications. Various techniques have been suggested to achieve postpyloric siting. Reports have indicated that lengthening the tube or altering the distal end tip configuration, by varying the tip profile or by the addition of a weight, may facilitate spontaneous transpyloric passage of the tube. This prospective controlled clinical study using three new polyurethane tubes demonstrates that the frequency of spontaneous transpyloric passage of the tube is not affected by tip profile or by the addition of a weight. Indeed, with all three tube designs only about one-third had passed spontaneously through the pylorus at 24 hr. Once through the pylorus the unweighted tube stayed in position significantly longer than the weighted tubes (p less than 0.005). We suggest that in those patients requiring post-pyloric feeding, endoscopic or fluoroscopic techniques should be used to position the tubes at the time of insertion, and that an unweighted tube should be used to prolong tube usage.

Automated methods in urinalysis.

Automation has been an integral part of clinical laboratory testing for many years. In most moderate, or large laboratory settings, it is common to have a piece of equipment available to automate at least some, if not all portions of the urinalysis (UA). Examples range from the simple hand-held refractometer for the measurement of specific gravity, to a very sophisticated floor model instrument that performs the entire assay.

Peripheral blood stem cell transfusion for marrow replacement.

A patient is presented who was treated with ablative therapy for Hodgkin's disease and rescued by reinfusion of peripheral blood stem cells (PBSC). The PBSC were used because previous therapy (chemotherapy and radiation to the pelvis) had resulted in fatty hypocellular marrow which was inadequate for marrow transplantation. The PBSC were collected by leukapheresis before and after recovery of the marrow from suppression with cyclophosphamide to bring the stem cells into cohort cycle and to increase the proportion of stem cells in the peripheral blood for collection. The patient showed a successful recovery on a time scale somewhat longer cells administered, the absence of stimulation by granulocyte macrophage-colony stimulating factor or other cytokine, or potential damage done to stromal elements during previous radiation and chemotherapy. The patient remains in clinical complete remission, fully engrafted, more than one year since his autologous transplant.
