Temporal lobe epilepsy caused by intrahippocampal calcified cysticercus: a case report.

The major differential diagnoses of epilepsy associated with small solitary lesion are tuberculous or cysticercus granuloma which are enhanced in CT and/or MRI study. We report of a 47-year-old man with intractable temporal lobe epilepsy as the presenting feature of a solitary intrahippocampal calcified mass without enhancement, which turned out to be a Taenia solium cysticercus. There was no apparent evidence of systemic cysticercosis. Imaging studies revealed a small solitary intrahippocampal calcification without perilesional enhancement, and atrophy of the hippocampal head portion. Cysticercosis should be considered as an etiology in the differential diagnosis in lesional medial temporal lobe epilepsy even without perilesional enhancement.

Bilateral kainic acid lesions in the rat hilus induce non-linear additive mossy fiber neoinnervation.

Kainic acid (KA) lesions of the rat hilus model hippocampal sclerosis and temporal lobe epilepsy. Unilateral hilar cell loss denervates the associational afferents normally projecting to the inner molecular layer (IML) granule cell dendrites, followed by ipsilateral mossy fiber (MF) sprouting. Hilar neurons also project through the hippocampal commissure to the contralateral IML. This study compared densities of IML MF sprouting following unilateral versus bilateral low dose KA lesions, using Neo-Timm stain 30 days later. Unilateral KA (0.4 microg) caused only dense ipsilateral MF sprouting. Bilateral lesions with lower doses of KA (0.1 with 0.2 or 0.3 microg) induced dense bilateral MF sprouting. However, the same low doses of KA injected unilaterally did not induce significant sprouting ipsilaterally or contralaterally. These data show that denervations of both associational and commissural afferents to the same IML dendritic zones of granule cells induce non-linear, additive bilateral MF neoinnervations.

One human case of natural infection by Heterophyopsis continua and three other species of intestinal trematodes.

The third human case of natural infection by Heterophyopsis continua in Korea was found in Chinju, Kyongsangnam-do. The case was a 53-year-old man. He used to eat raw brackish and fresh water fish. After praziquantel treatment and purgation, H. continua were collected from the diarrheal stool together with Metagonimus yokogawai, Heterophyes nocens and Echinostoma hortense. His clinical complaints were indigestion, epigastric discomfort, poor appetite and fatigue. The complaints were considered rather due to heavy M. yokogawai infection.

Unilateral fetal mesencephalic grafting in two patients with Parkinson's disease: short-term result after transplantation.

Fundamental pathological and neurochemical changes in Parkinson's disease are loss of midbrain dopamine neurons that innervate the caudate and putamen. In an effort to replenish the striatal dopaminergic innervation, fetal mesencephalic tissue containing dopamine cells was implanted into the unilateral putamen in two patients with severe Parkinson's disease. The tissue was obtained from three fetuses with gestational ages of 7 to 9 weeks. The cell suspension was stereotactically injected into the unilateral putamen using 5 needle trajectories. Postoperative immune suppression was not performed. Clinical improvement appeared after 2 months. Both patients showed improvement according to the Activities of Daily Living Scale during the off and practically-defined off state 9 and 14 months after surgery. The motor scores of the Unified Parkinson's Disease Rating Scale improved during the off and practically-defined off state 9 and 14 months after surgery. Dyskinesia and off state were shorter and less severe than before the transplantation. Although the long-term effects need to be ascertained, our short-term observation in these two patients with unilateral transplantation is encouraging and justifies further research trials in selected patients.

Aneurysm at the origin of accessory middle cerebral artery associated with middle cerebral artery aplasia: case report.

A case of ruptured aneurysm at the origin of accessory middle cerebral artery associated with ipsilateral middle cerebral artery aplasia is reported. The angiograms did not show a middle cerebral artery shadow on the lesion side. Instead, the accessory middle cerebral artery ran toward the Sylvian fissure and supplied the middle cerebral artery territory along with multiple hypertrophied perforating branches. The operative findings confirmed that the cord-like, rudimentary middle cerebral artery had no internal blood flow. The effect of these combined anomalies on the hemodynamic equilibrium between the arteries of the same embryologic trees and the genesis of aneurysm is worthy of notice.

Radiotherapy for cancer of the larynx: review of a community hospital experience.

Records of 106 patients with squamous cell carcinoma of the larynx treated with radiation therapy (RT) at Washington Adventist Hospital between 1976 and 1988 were analyzed for patterns of failure and survival, with a minimum of five-year follow-up. In patients treated with RT alone, initial local control rate was 72% for tumors involving supraglottic larynx and 90% for glottic larynx; in postoperative RT patients, the rate was 65% in the supraglottic and 67% in the glottic larynx group. The laryngeal preservation rate in patients treated with RT alone was 87% in those with early lesions (T1 and T2) and 63% in those with advanced tumors (T3 and T4). The overall five-year determinate survival rates of the RT-only group and the postoperative RT group were 90% and 57%, respectively. In patients with advanced tumors (stages III and IV), survival rates were comparable for RT alone and postoperative RT (56% and 52%, respectively).

Expression of c-sis/platelet-derived growth factor B, insulin-like growth factor I, and transforming growth factor alpha messenger RNAs and their respective receptor messenger RNAs in primary human gastric carcinomas: in vivo studies with in situ hybridization and immunocytochemistry.

In situ hybridization and immunocytochemistry have been applied to investigate the expression of c-sis/platelet-derived growth factor (PDGF)-B, insulin-like growth factor (IGF)-I, and transforming growth factor alpha mRNAs and their respective receptor mRNAs in three primary human gastric carcinomas and in their adjacent nonmalignant mucosas. Expression of c-sis/PDGF-B mRNA and PDGF-receptor beta mRNA was seen in the tumor cells of the three gastric cancer specimens but not in their adjacent nonmalignant mucosa. The mRNA expression was accompanied by the expression of their respective protein products. IGF-I, IGF-I receptor, and epidermal growth factor receptor mRNAs were seen in both the tumor cells of the gastric cancer specimens and in nonmalignant mucosa. Transforming growth factor alpha mRNA was expressed in gastric tumor cells but not in nonmalignant mucosa. The coexpression of a potent "competence" growth factor, PDGF, and "progression" growth factors, IGF-I and transforming growth factor alpha, in the tumor cells of gastric carcinomas may contribute to their growth and maintenance.

Fungal contamination in organ culture.

Fungal contamination of organ-cultured corneas was studied retrospectively. Over a 3 1/2-year period, 5.2% of 696 corneas became contaminated by fungi in organ culture. The average length of time for fungal contamination to occur in organ culture was 11 days. The most frequent contaminating fungi were those of the Candida species. In addition to previously identified risk factors for donor cornea contamination, those corneas from donors given mechanical respiratory assistance at the time of death, those corneas with long postmortem times, and those corneas with positive postwash cultures were identified as being at higher risk for fungal contamination in organ culture.

Patterns of failure following surgery alone for colorectal carcinoma.

Two hundred fifty-one patients with colorectal carcinoma were studied following complete primary resection to determine patterns of failure. Seventy-two patients (29%) subsequently developed failures: Local failure (LF) occurred as the only failure in 49% of the failure group and as some component in 81%; distant metastases (DM) occurred in 19% and 47%, respectively. The groups at highest risk for local failure were those with extension of tumor through the bowel wall whether the nodes were involved or not. Furthermore, those with gross extension of disease through the wall developed a significantly higher incidence of distant metastases compared to those with microscopic extension through the wall (P less than 0.005). The absolute 5-year survival rate for those with tumor through the wall vs within the wall was 40% and 79%, respectively. Adjuvant therapy was discussed in view of the ability to identify subgroups of patients at highest risk for local vs distant failures.

Treatment outcome of recurrent cervical cancer.

A total of 85 patients with recurrent cervical cancer were reviewed: 17 patients with recurrences were treated by radical surgery, 18 by radiotherapy, 29 by chemotherapy, and 21 cases received no further treatment. Survival was presented according to the site of recurrence and the mode of therapy. All patients were followed for a minimum of 24 months after recurrence. Of the total group, 14% are living without evidence of disease, 29% died of metastatic disease with no involvement in the pelvis, and 45% died of pelvic cancer. Overall, 22% were living more than 2 years, and only 2% lived over 5 years after recurrence. The NED (no evidence of disease) rate for radical surgery group was 47 and 44% for the radiotherapy group. There were no significant differences in median survival between the chemotherapy group and the no-treatment group (6.8 versus 4.8 months). New chemotherapy agents and adjuvant systemic therapy are discussed.

Colestipol hydrochloride prophylaxis of diarrhea during pelvic radiotherapy.

Thirty-three patients were randomized prior to pelvic radiotherapy to receive the bile acid-sequestering resin colestipol hydrochloride, 5 grams qid, during the entire time of their therapy or diphenoxylate hydrochloride and atropine sulfate 2.5-20 mg per day (control) if they experienced diarrhea. The colestipol patients also took diphenoxylate if they had diarrhea. The patients in the colestipol group often experienced nausea, vomiting, and abdominal cramps and 8 were forced to discontinue the drug. There was no difference in the weekly stool frequency between the colestipol and the control patients but the colestipol patients who took at least 50% of the prescribed dose required fewer diphenoxylate tablets than the controls. The data suggest that colestipol hydrochloride is not of value in preventing radiation-induced diarrhea because of the side effects associated with the drug, but the theory on which the use of bile acid-sequestering agents is based may be correct.

The prognostic significance of vascular channel involvement and deep stromal penetration in early cervical carcinoma.

Eighty-eight patients with FIGO Stage Ib and IIa squamous cell carcinoma of the cervix underwent radical hysterectomy with pelvic and paraaortic lymphadenectomy. The records and histopathologic material were reviewed to determine the prognostic significance of vascular channel involvement and deep stromal penetration by tumor. Seventy-four patients (84%) were alive and free of disease for more than 2 years and 14 (16%) developed recurrent carcinoma within that time. A positive correlation was found between depth of stromal penetration by tumor and the degree of vascular channel involvement (p less than 0.05). Vascular involvement in itself did not significantly affect nodal status, survival or the rate of recurrence. Depth of penetration was associated with a higher incidence of positive nodes (p less than 0.05). There was a trend towards a lower survival rate and a higher recurrence rate in patients with deep stromal penetration as compared to those with superficial tumors. Microscopic nodal disease increased the rate of recurrence and had an adverse effect on survival. The combination of deep tumor penetration and positive nodes in the same patient was associated with the highest recurrence rate and the lowest survival (p less than 0.05). Nodal status was a more significant prognostic indicator than depth of tumor penetration because patients with deeply penetrating tumors and positive nodes had more than twice the recurrence rate than did patients with deep tumors and negative nodes. Postoperative radiation therapy was beneficial to patients whose tumors demonstrated deep stromal penetration and microscopic metastases to pelvic lymph nodes.
