[Assessment of regional wall motion of the hypertrophic heart by phase analysis of thallium-201 images].

Combined technique with gated planner thallium (Tl)-201 images and gated cardiac pool images were applied to evaluate cardiac hypertrophy and regional wall motion for 13 persons. These subjects consisted of four normal persons, four patients with hypertrophic obstructive cardiomyopathy (HOCM) and five patients with asymmetrical apical hypertrophy (AAH). The Tl-201 image and cardiac pool image were obtained using the gated technique, in which the R-R interval was divided into 16 parts, in the 30 degree left anterior oblique projection. The region of interest (ROI) in the left ventricle was defined by the threshold method and radially divided into 16 segments in the center of the left ventricular area. The count ratio, considered as an indicator of regional myocardial contractility, was calculated using the formula (EDC-ESC/EDC) in each ROI. The functional image and histogram, an indicators of the synchronization of left ventricular wall motion, were constructed by the fluctuation of the phase distribution in volume curves of Tl-201. These indicators were evaluated for the normals and the two groups of patients. The count ratio indicated a higher value in the region of the left ventricular free wall and the cardiac apex in the normals, HOCM and AAH. The count ratio at the septal region indicated a lower value in HOCM, in spite of marked septal hypertrophy. A significant fluctuation of the phase distribution was demonstrated in the septal region in HOCM, and also at the apical region in AAH.(ABSTRACT TRUNCATED AT 250 WORDS)

[Neurinoma with intrasellar extension: a case report].

A 64-year-old female visited Kawatetsu Chiba Hospital complaining of left retro-orbital pain. A month before, she had experienced ptosis and diplopia, that had been releaved two days later by corticosteroid. On admission, she had no neurological deficits except for minimal anisocoria, with the left pupil larger than the right. There was no cutaneous manifestation of von Recklinghausen's disease. Skull X-ray films showed depression of the floor of the sella turcica on the left side. CT scans demonstrated a parasellar enhancing mass with intrasellar extension. Left carotid angiogram showed intracavernous portion of the internal carotid artery displaced laterally, inferiorly, and anteriorly. With a tentative diagnosis of laterally extending pituitary adenoma, a transsphenoidal operation was carried out, which disclosed a solid tumor locating beside the medially-displaced pituitary gland. The histological diagnosis was typical neurinoma. Parasellar neurinoma is not so common. It is usually difficult to determine the origin of the parasellar neurinoma. Trigeminal neurinoma arising from the Gasserian ganglion is generally recognized to be the most frequent. However, the absence of the trigeminal nerve involvement, unusual CT findings, and angiographical changes in the present case were all different from those of the typical trigeminal neurinoma. We believe that the tumor of this case originated from the oculomotor nerve. Fifteen cases of neurinoma of the oculomotor, trochlear, and abducens nerve have been reported to date. Their clinical features were reviewed.

Effect of antianginal agents on arteriolar diameter in normal and cholesterol-fed rabbits.

The effect of antianginal agents on the cutaneous arteriolar diameter in normal and cholesterol-fed rabbits was investigated. The change in the potency of trapidil to dilate arterioles in cholesterol-fed rabbits was relatively minor when compared with normal rabbits. On the other hand, the potencies of other agents decreased markedly in cholesterol-fed rabbits as compared with normal rabbits. For example, in arterioles of 45-50 micron in diameter, the effective doses of tapidil and dilazep at which the agents dilated the vessel by 20% (ED20) were 1.0 and 0.07 mg/kg i.v., respectively, in normal rabbits, and 2.3 and 2.0 mg/kg i.v., respectively, in cholesterol-fed rabbits. The other agents, such as dipyridamole, diltiazem and nifedipine, virtually did not show any dilating effects on arterioles of cholesterol-fed rabbits at the doses which were effective in normal rabbits. The maximum changes in systemic blood pressure and carotid blood flow induced by the administration of trapidil, dilazep or dipyridamole did not decrease in cholesterol-fed rabbits as compared with normal rabbits. These results suggest that it is more desirable to evaluate the therapeutic potency of antianginal agents in cholesterol-fed rabbits than in normal rabbits, and that the action of antianginal agents on arterioles is not dependent on its action on systemic hemodynamics.

Tofisopam, a new 2,3-benzodiazepine. Inhibition of changes induced by stress loading and hypothalamic stimulation.

Effects of tofisopam, a new 2,3-benzodiazepine compound, were investigated on the following: gastric ulceration, induced by water-immersion stress in normal rats and by immobilization stress in olfactory-bulbectomized (OB) rats; and propulsion of the small intestine caused by water-immersion stress in rats and autonomic responses to electrical stimulation of the hypothalamus in rabbits. In the latter, the results were compared with those of diazepam and gamma-oryzanol. Tofisopam (30 and 100 mg/kg, po) significantly inhibited the gastric ulceration induced by water-immersion stress in normal rats in a dose-dependent manner. Immobilization-stress loading increased the incidence and average index of gastric ulceration in OB rats, compared with nonstressed rats. Tofisopam (100 mg/kg, po) significantly inhibited the gastric ulceration induced by stress loading in OB rats. Water-immersion stress loading induced a significant increase in intestinal propulsion in rats. This increase was reversed to control levels by tofisopam (100 mg/kg, po). Tofisopam (1.0 mg/kg, iv, or 0.1 mg/kg by intracerebrospinal injection) inhibited the constriction of ear microvessels, the decrease in earlobe temperature, and mydriasis induced by electrical stimulation of the medial hypothalamic area in rabbits. However, diazepam and gamma-oryzanol failed to inhibit the autonomic responses to medial hypothalamic stimulation. From these results, it can be concluded that tofisopam restores the autonomic abnormality induced by stress loading possibly via intervention in the central autonomic area, i.e., the hypothalamus, by an action different from that of diazepam.

Epidemiological study of diabetes mellitus in the Hiroshima area-prevalence of diabetes mellitus and follow-up studies using the glucose tolerance test 5-19 years after initial testing.

Tests for diabetes mellitus have been performed annually since 1963 on about 100,000 A-bomb survivors living in Hiroshima. We report here on the trends over the 19 years leading up to 1981 for the glycosuria positive rate, the 75 g GTT results of positive cases and the findings of the 5-19 year follow-up. The glycosuria positive rate was 3-4 times higher in males. During the period of 1963-1979, the positive rate increased annually in both sexes, reaching 2.6 and 3.0 times the male-female values for 1963, but subsequently levelling off. When Body Mass Index (BMI) less than 25, 24.9% of the sample was diabetic (including those under treatment), 26.0% had Impaired Glucose Tolerance (IGT) and 49.1% were normal, whereas when BMI greater than or equal to 25, the rates were significantly higher being 45.0%, 27.4% and 27.6% respectively (p less than 0.001). Frequency increased with BMI, reaching 75.0% at BMI 33.3). Results of the 5-19 year follow-up showed the annual diabetes onset rate by the person year method in those with BMI less than 25 at initial GTT to be 1.72% among 848 normals, while it was a significantly higher 4.82% in those suffering from IGT. For those with BMI greater than or equal to 25, the rates were 3.77% among 161 normals and 7.93% among 132 IGT cases.

Review of criteria for diagnosis of diabetes mellitus based on results of follow-up study.

Comparison of the 75-g oral glucose tolerance test (OGTT) method was made with the conventional 50-g method in a fixed population followed for 1--17 yr. The possibility of using the results in establishing the diagnosis of diabetes mellitus was also considered. The following results were obtained: (1) The 75-g method showed significantly higher 2-h and 3-h postchallenge plasma glucose (PG) and immunoreactive insulin values. (2) The two methods showed good correlation in PG values at various time periods, and there was no difference between the two at 1/2 h and 1 h. The 2-h standard value of 200 mg/dl used to diagnose diabetes with the 75-g method was equivalent to 180 mg/dl by the 50-g method. The upper limits of normals were 140 mg/dl and 120 mg/dl, respectively, for the two tests. (3) Those subjects diagnosed as diabetic on the basis of fasting plasma glucose (FPG) values of 140--149 mg/dl only had a high rate of reverting to normal over time. The frequency of "nondiabetic" plasma glucose values after glucose loading steadily decreases as FPG increased, with separation into two asymptotic lines at 150 mg/dl level. Thus, the logical value for diagnosis of diabetes when based only on FPG value is considered to be 150 mg/dl. (4) A smaller number of individuals had 2-h PG values that satisfied criteria for diagnosis of diabetes mellitus, but 1/2-h and 1-h values that were less than 200 mg/dl. Nevertheless, follow-up of these subjects showed a high development rate of diabetes. Thus, the 1/2-h and 1-h values are not considered necessary to establish diagnosis of diabetes mellitus.

[Clinical study on dosage schedule of tobramycin: controlled comparison between 180 mg b.i.d. and t.i.d].

The efficacy and safety of tobramycin (TOB) administration at 90 mg twice a day (90 mg group) to patients with respiratory tract infections (RTI) were objectively investigated by means of a well-controlled comparative study. The results in this 90 mg group were compared with the case of administering 3 times a day of 60 mg each (60 mg group) as the control. In both of these patient groups, TOB was administered by intramuscular injection for 10 consecutive days. This study included 85 patients (90 mg group: 43 patients; 60 mg group: 42 patients). The Committee, however, excluded 19 patients consisting of 13 cases of infection other than RTI and 6 cases of out of protocol drug administration. Thus, 66 patients (90 mg group: 32 patients; 60 mg group: 34 patients) were used for the efficacy analysis. All 85 patients were used, however, for the evaluation of the safety of TOB and for the usefulness of TOB as judged by the physicians in charge. A brief summary of the results is presented below. The cases employed by the Committee consisted of 8 and 17 cases in the 90 mg and 60 mg groups, respectively, with bacterial pneumonia and lung abscess, and 24 and 17 cases in the 90 mg and 60 mg groups, respectively, with chronic RTI. There was a tendency for the distribution of disease cases in the 2 drug groups to be significant. Within the chronic RTI disease classification, the number of patients with chronic bronchitis was almost equal in the 2 drug groups; 16 cases in the 90 mg group and 15 cases in the 60 mg group. However, secondary infections were diagnosed in 8 cases in the 90 mg group and 2 cases in the 60 mg group. The clinical efficacy judged for all cases investigated by the Committee showed efficacy ratios of 56.3% for the 90 mg group and 70.6% for the 60 mg group. The difference between the 2 drug groups was not statistically significant. In the treatment of bacterial pneumonia and lung abscess, the efficacy rate in the 90 mg group was 87.5%, while it was 88.2% in the 60 mg group; these high values were practically identical. Against chronic RTI, the efficacy rates were 45.8% in the 90 mg group and 52.9% in the 60 mg group; within that disease classification, the efficacy rates against the chronic bronchitis cases were nearly the same at 43.8% and 46.7%, respectively. [Hypertrophic cardiomyopathy presenting ECG changes mimicking myocardial infarction during 18 years: report of a case].

This report describes a 54-year-old man with hypertrophic cardiomyopathy whose electrocardiogram had developed abnormal changes characterized by progressive ST-segment elevation with Q waves during eighteen years period. The ECG taken on the initial examination at his age of 36 showed a high voltage QRS complex with deep inverted T waves in V3-6. Twelve years later, these inverted T waves became shallow and ST-segment elevation was recognized. Since the age of 49, the ECG showed persistent ST-segment elevation and Q waves in V3-6 mimicking anterolateral myocardial infarction. At the age of 54, he suddenly died of ventricular tachycardia. At necropsy the heart showed the thickened ventricular septum and transmurally infarcted scar in the anteroseptal area near the apex with patent epicardial coronary arteries. Microscopic examination revealed myofibrillar disarray in the ventricular septum and significant intramural coronary artery narrowing in the left ventricular free wall and ventricular septum. The association of myocardial infarction in hypertrophic cardiomyopathy with the patent epicardial coronary artery has been reported by Maron et al, who stressed the role of the narrowing of intramural coronary arteries in such cases. In the present report, we discussed the pathogenesis of myocardial infarction in hypertrophic cardiomyopathy with a review of literature.

[Effects of protizinic acid on the prostaglandins system and the production of oxygen radicals].

Effects of protizinic acid (PRT) on prostaglandins (PG) and the production of oxygen radicals were compared with those of other non-steroidal anti-inflammatory agents. Oral administration of 30 mg/kg of PRT, indomethacin (IM), or ibuprofen (IB) significantly inhibited arachidonic acid-induced erythema in guinea pigs. Although 30 mg/kg of PRT significantly inhibited PGE2-induced erythema, IM and IB did not significantly inhibit it. PRT inhibited phospholipase A2 (PLA2) activity, and the IC50 value was 2.1 X 10-4 M. On the other hand, IM and IB exerted no effect on the PLA2 activity at 3 X 10-4 M. These results suggest that PRT possesses a broader pharmacological activity on the PG system than IM and IB. As for effects on the production of oxygen radicals, in order of relative inhibitory potency was PRT greater than metiazinic acid (MA) = IM greater than IB = phenylbutazone (PB) in the xanthine oxidase assay, PB great than IM greater than PRT greater than MA = IB in the rabbit neutrophil myeloperoxidase assay, and IM greater than PB greater than PRT greater than MA greater than IB in the guinea pig macrophage assay. In the rabbit neutrophil and aggregated IgG-bound micropore filter assay, the order was PRT greater than MA greater than PB greater than IM = IB. Thus, the inhibitory effects of PRT was verified in all experiments on the production of oxygen radicals in contrast to IB. In particular, it could be especially meaningful that PRT showed the most potent activity in the aggregated IgG-bound micropore filter assay which has been reported to be a good model for studying the pathogenesis of inflammatory diseases believed to be caused by immune complexes.

[Effects of tofisopam on the physiological changes induced by stress loading and hypothalamic stimulation (author's transl)].

Effects of tofisopam on the gastric ulceration induced by immobilization stress in olfactory-bulbectomized rats, propulsion of the small intestine caused by water immersion-stress in rats and autonomic responses to electrical stimulation of the hypothalamus in rabbits were investigated. Immobilization stress loading of 16.5 hours each for 10 days caused the augmentation of incidence and average index of gastric ulceration in olfactory-bulbectomized rats, compared with non-treated rats. Tofisopam 100 mg/kg, p.o. significantly inhibited the gastric ulceration in olfactory-bulbectomized rats. Water immersion-stress loading for 2 hours caused a significant increase in propulsion of the small intestine in rats. This increase was reversed to control levels after the oral administration of tofisopam in a dose of 100 mg/kg. Tofisopam at dose of 1 mg/kg i.v. inhibited the contraction of ear microvessels, the decrease in earlobe temperature and the mydriasis induced by electrical stimulation of the medial hypothalamic area in rabbits, Moreover, these inhibitions were also shown by the intra-cerebrospinal injection of tofisopam at a dose of 0.1 mg/kg. From these results, it is concluded that tofisopam could restore the autonomic abnormality induced by stress-loading and exerts such effects by acting on the hypothalamus, an area of the brain, which regulates autonomic nervous functions.
