[A case of a foreign body in the urinary bladder--a surgical suture needle was found in urinary bladder].

We report a case of an unusual foreign body in the urinary bladder. A 66-year-old male had undergone an operation of rectal cancer 6 years previously. He visited our hospital with irritative urinary symptoms. Urinalysis showed hematopyuria, and X-ray examination revealed a curved fine shadow (about 2 cm in length and 3 mm in width) at the vesical region. In cystoscopic examination, a calcified surgical suture needle was identified on the trigonal region. The needle was removed transurethrally by a stone crushing forceps. Thus, it was assumed that the needle had been left in the abdominal cavity at the operation 6 years ago and it had migrated into the bladder during these 6 years. Many cases of foreign bodies in the urinary bladder have been reported. However, to our knowledge, this is the first report of a surgical suture needle, in the bladder.

[A case of idiopathic calcinosis of the scrotum].

We report a case of idiopathic calcinosis of the scrotum. The patient was a 33-year-old male with a complaint of multiple scrotal nodules, from which white chalky materials occasionally broke out. The nodules were first noticed 17 years ago, and since then gradually increased in size and number. Sometimes he had felt itching on the scrotum, but otherwise he had been in good health. Blood analysis and biochemical analysis revealed normal findings. In addition, there were no abnormal findings in serum electrolyte analysis including calcium and phosphate concentrations. Analysis of the intra-nodular content by infrared spectrophotometry showed that it consisted of calcium phosphate (52%) and magnesium ammonium phosphate (48%). We reviewed the literature and discuss the etiology of this disease.

[Ureteral endometriosis: a case report].

A 32-year-old female visited our clinic with the chief complaints of macroscopic hematuria and pollakisuria in February 18, 1987. Cystoscopic examination revealed findings of cystitis but bleeding from the ureteral orifice was not observed. IVP showed right non visualized kidney and retrograde pyelography demonstrated right ureteral stenosis on the lower ureter and right hydronephrosis. Total hysterectomy, right oophorectomy and right ureterolysis were carried out in March 5. Right lower ureter was buried in the fibrous tissue approximately over 2 cm above the crossing with iliac vessels. Dark reddish colored small tumor was noticed in the stenosed ureter and resected. The histological diagnosis confirmed endometriosis. A total of 37 cases of ureteral endometriosis in the Japanese literature was reviewed.

[A case of adrenal black adenoma associated with Cushing's syndrome].

The patient was a 24-year-old female, who had clinical and laboratory findings of Cushing's syndrome with slight virilizative changes. CT scan, ultrasound sonography and scintigraphy demonstrated that there was a functioning tumor at the right adrenal region. Thus we performed right adrenalectomy. The tumor was a characteristic black adenoma of adrenal gland, and it microscopically consisted of compact cells with remarkable intracellular pigmented granules. Thirty-two cases of functioning adrenal black adenoma have been reported in our country and all of them were associated with Cushing's syndrome.

[Prostatic cancer with cystic degeneration: a case report].

A 73-year-old man with prostatic cancer with cystic degeneration is reported. He visited our clinic for postoperative examination of left metastatic lung cancer and treatment of prostatic hypertrophy. When the prostatic biopsy was performed, bloody fluid was aspirated and prostate size on digital examination was reduced. The obtained specimen consisted of scar tissue. Urethrography showed an elongation of prostatic urethra in addition to the compression of urinary bladder from the rear. CT scan demonstrated a prostatic cyst approximately 8 cm in diameter with an irregular margin. Following the transurethral resection of the prostate, the prostatic cyst was opened and papillary tumor observed. Histological examination revealed a well differentiated medullary cancer which coincided with the pathological finding of left metastatic lung cancer. He died from dyspnea caused by pleuritis carcinomatosa 6 years later. Nine cases of prostatic cancer with cystic degeneration in the Japanese literature are reviewed.

[Localized amyloidosis of the ureter: a case report].

A 58-year-old woman was introduced to our department complaining of lower abdominal pain and macrohematuria in March, 1978. Intravenous pyelography revealed stenosis and a ureteral stone on the right distal ureter. In May, 1983, she visited our department again. Endoscopically, bleeding from the right ureteral orifice was observed. Retrograde pyelography demonstrated the right hydronephrosis and irregular margin of the right lower ureter. However, the urine cytology was negative, and CT scan and ureteroscopy showed no tumorous lesion on the portion. On 7 August, 1989, partial ureterectomy and end to end anastomosis was performed. Histopathological examination by H-E and Congo red stain proved amyloid deposition. Postoperative course was uneventful. A total of 40 cases of amyloidosis of the ureter in the world are reviewed.

[A case of medullary sponge disease associated with horseshoe kidney].

A 60-year-old man visited our hospital with complaints of micturition pain and interruption of urinary stream. X-ray examinations were performed, showing no bladder and urethral stone. However, KUB revealed bilateral multiple renal stones. Excretory urography showed a horseshoe kidney and a concentration of the contrast medium in the ectatic tubules at the pyramidal apices. Calculi were also located in these ectatic tubules. Thus, we considered that this was a very rare case of medullary sponge disease associated with horseshoe kidney. So far there has been no case report of medullary sponge disease with horseshoe kidney in our country.

[Hemodynamics of the canine penis during erection].

To elucidate the change of hemodynamics of the penis during erection induced by neurostimulation to the pelvic nerve, we measured the blood flow rates (BFR) through the dorsal penile artery and vein (DPA and DPV), and the internal pudendal artery (IPA) with the intracavernosal and intraspongiosal pressures (ICP and ISP). After DPA were ligated bilaterally, we compared BFR through DPV and ICP with those before ligations. In addition, PVO2 within DPV was determined. When neurostimulation was switched on, BFR through IPA and DPA began to increase. But during erection, BFR through IPA was lower than the flow in the tumescence phase, although the BFR through DPA remained unchanged. Then ISP elevated. When the full erection was achieved, the pressure remained above the prestimulation level, although a gradual decrease in the pressure was observed. On the other hand, ICP dropped transiently for 5 to 10 seconds, and then increased rapidly. BFR through DPV increased rapidly, which coincided with the transient drop in ICP. As soon as neurostimulation was switched off, BFR through IPA, DPA, DPV and ISP decreased to the base line, although ICP still remained elevated. The BFR through DPV and ICP were compared before and after ligation of bilateral DPA. BFR through DPV decreased after ligation during the non-erectile period, but no significant changes were found during the erectile period. ICP did not change significantly after ligation in either period. PVO2 within DPV was higher than that in the internal iliac vein and peripheral vein during both non-erectile and erectile period. In the erectile period, it rose up to a level near that within the aorta.(ABSTRACT TRUNCATED AT 250 WORDS)

[Adrenal hemangioma: a case report].

We report a rare case of adrenal hemangioma incidentally discovered by computed tomography (CT) in a 75-year-old female, the hormonal examination showed within the normal range, and ultrasound-sonography, CT scan and angiography disclosed the mass to be a non-functioning adrenal tumor. The tumor was removed operatively and the pathological finding was adrenal hemangioma. Twenty reported cases found in the Japanese literature are reviewed.

[A case of urachal abscess].

A 26-year-old male with urachal abscess was demonstrated. Ultrasonography, CT and MRI revealed that there was a mass at the supravesical region, which was connected to the umbilicus. However, a differential diagnosis as to whether the mass was a tumor or an abscess was difficult from these examinations. Thus we performed an open biopsy. A pathological examination revealed no malignant change in the biopsy specimen. On the other hand, the pathological diagnosis of a follicular tumor located at the roof of the urinary bladder was TCC G2, but we considered that, this change was a reactive hyperplasia with dysplasia resulting from chronic inflammation of urachal abscess. Thus, the complete excision of urachal abscess and partial cystectomy were performed. The serial sections of the bladder wall revealed no malignant changes. We reviewed similar cases reported in our country and discussed various factors causing urachal abscess, especially related to the infectious root.

[Intracavernous indurations after injuries to the external genital parts caused by a motorcycle tank].

We have experienced 2 cases of intracavernous induration after injuries to the external genital parts caused by a motorcycle tank. Case 1. A 28-year-old male was admitted to our department complaining of painless indurations of the penile radix. Cavernosography showed segmental filling defect in left corpus cavernosum. Because erectile disturbance was noted, resection of the induration was carried out. Microscopic section of the excised tissue showed only fibrosis. Case 2. A 20-year-old male visited our clinic with chief complaints of induration of the penile radix and erectile disturbance. Corpus cavernosography demonstrated filling defect in bilateral corpus cavernosum. We recommended the resection of the indurations, but the patient refused. A brief review on etiology and therapy of intracavernous fibrosis was made.

[Renocolic fistula caused by xanthogranulomatous pyelonephritis: a case report].

Renocolic fistulas caused by typical xanthogranulomatous pyelonephritis are rare. We present a case of renocolic fistula caused by xanthogranulomatous pyelonephritis. A 58-year-old woman was admitted to the Department of Urology, Sakata City Hospital, with the chief complaint of pain in the left flank region. The plain film showed no calculi. An intravenous pyelogram showed left non-functioning kidney. A left retrograde pyelogram demonstrated a communication between the left renal calyx and the descending colon. A computed tomography done after the retrograde pyelography revealed contrast medium in the descending colon and a heterogenous left renal mass and thickness of Gerota's fascia. On the basis of the radiologic studies a diagnosis of renocolic fistula was made. The patient underwent a nephrectomy with closure of the fistula of the colon. The pelvicaliceal system of the resected kidney was distended and contained yellow fatty like tissue. Histologically xanthogranulomatous pyelonephritis was confirmed. The patient made an uneventful recovery.

Intracavernous injection of prostaglandin E1 for the treatment of erectile impotence.

We administered intracavernous injections of 20 micrograms. prostaglandin E1 to 135 patients with impotence, and evaluated the effects and side effects. Among 135 patients who underwent intracavernous injection of prostaglandin E1 complete erection was observed in 83 (62 per cent), while incomplete erection was noted in 33 (24 per cent). In both groups the erection was sufficient for sexual intercourse. Tumescence without rigidity was noted in 12 patients and no response was obtained in 7. Poor response was seen frequently in patients with disorders of the vascular system and/or damage to the cavernous body of the penis. Priapism after the injection was not observed. Moreover, we never observed any other severe side effects. Intracavernous injection of prostaglandin E1 could be applicable to the therapy of impotence, especially that due to neurogenic disturbance. Since prostaglandin E1 acts quickly and loses its validity rapidly it is considered to be a more suitable agent than other vasoactive drugs.

[Bilateral testicular tumors of germ cell origin: a case report--a review of 136 cases in Japanese literature].

A case of bilateral testicular germ cell tumors in a 23-year-old male is reported. He was admitted to the Department of Urology, Yamagata Prefecture Kahoku Hospital with the chief complaint of painless swelling in the left intrascrotal contents. Left high inguinal orchiectomy was carried out. At the same time, right hydrocele operation and right testicular biopsy were performed. Subsequent histological examination revealed anaplastic seminoma in the left testis and typical seminoma in the right. On July 21, 1988, he was referred to our clinic and right high inguinal orchiectomy was carried out. Postoperative chemotherapy was performed with CDDP, VP-16 and peplomycin, and postoperative course was uneventful with no distant metastasis or local recurrence. A total of 136 cases of bilateral testicular germ cell tumors in the Japanese literature are reviewed.

[A case of metastatic tumor of the spermatic cord from gastric carcinoma].

A case of metastatic tumor of right spermatic cord in a 54-year-old man is reported. He visited our department with the complaint of a painless mass in right inguinal lesion. The initial diagnosis was a right spermatic cord tumor, and then orchiectomy was performed. Since pathological findings showed a poorly differentiated adenocarcinoma, further examinations were done, postoperatively. An upper gastrointestinal series revealed the presence of gastric carcinoma. Thus, these results suggested that the spermatic cord tumor was a metastatic lesion from the gastric carcinoma. Forty eight cases of metastatic tumor of the spermatic cord have been reported in our country. In 26 of these cases, gastric carcinoma metastasized to the spermatic cord.

[A case of pathological dilatation of seminal vesicle with chief complaint of male infertility].

A 32-year-old man with the chief complaint of male infertility for two years was admitted to our clinic for extensive examination of azoospermia. Physical examination revealed no abnormal findings in the bilateral testis, epididymis or prostate except for hard thickening of bilateral deferens ducts. Serum follicle stimulating hormone, luteinizing hormone, prolactin and testosterone levels were within normal range, and specimens from testicular biopsy demonstrated normospermatogenesis. Examination of urinary tract disclosed no abnormal findings. Seminal vesiculography, pelvic computed tomographic scan, and pelvic plain X-ray after seminal vesiculography showed that bilateral seminal vesicles were dilated remarkably without filling defect. A cap-shaped silicone prosthesis was installed on caudal portion of right epididymis. Fourteen days later, we punctured it, but enough seminal fluid to provide homologous artificial insemination was not obtained. Sixty five cases of pathological dilatation of seminal vesicle were collected from Japanese literature. A brief review on age, symptoms, complications, therapies and transferrin in seminal fluid were discussed.
