An improved procedure for the synthesis of 14C-labeled phosphatidylserine from cerebral phosphatidic acid.

A complete procedure to prepare a highly labeled phosphatidyl-L-[U-14C]serine possessing the same fatty acid composition of brain phospholipids is reported. CDP-diglyceride was synthesized by reaction between phosphatidic acid and CMP-morpholidate as the dicyclohexylcarboxamidium salt. The reaction between CDP-diglyceride and L-[U-14C]serine to produce the labeled phosphatidylserine was catalyzed by the CDP-diglyceride: L-serine phosphatidyl transferase (EC 2.7.8.8) from E. coli. A selective inhibition of phosphatidylserine decarboxylase activity, present as contaminant in the enzyme extract, was introduced in order to avoid a low yield of product. Traces of phosphatidylethanolamine (about 1%) were easily removed by preparative thin-layer chromatography. The yield of the labeled product was as high as 87% and it specific radioactivity was 170 mCi/mmol.

Artificial gut for total parenteral nutrition through the peritoneal cavity.

Total parenteral nutrition was attempted by using the peritoneal route. In this way it was possible to convey to the patients all elemental nutrients used in hyperalimentation. Three patients were studied while treated for an episode of catabolic stress in the course of acute or chronic uremia. Adequacy of treatment was controlled by measuring urea generation rates, plasma concentrations of 3-methyl-histidine, glucose disappearance from dialysate and kinetics of lipid particles removal from plasma. The data collected demonstrate that peritoneal hyperalimentation is is feasible for adequate for clinical purposes.

Amino acid losses during CAPD.

Amino acid losses in peritoneal dialyzate during CAPD were less than 2 g a day (15, 128 micrometers). CAPD does not correct the tyrosine/phenylalanine ratio in plasma and does not prevent accumulation of 3-methylhistidine.

Cold carbon haemofiltration apparatus.

Four uraemic patients underwent haemofiltration (HF) for two months, three times a week, with an infusion of about 20L of electrolyte solution. For another two months, HF was performed with diafiltrate reinfusion after regeneration on cold carbon and ionic exchange resins. Carbon (26.8g) and resins (10g/kg b.w) were needed. The data obtained in HF and in closed-circuit HF periods were similar, but during the closed-circuit HF treatment there was better control of acid-base balance.

Protein requirement of patients on CAPD: a study on nitrogen balance.

Nitrogen balance studies were performed in 8 patients undergoing CAPD. Patients were studied in a metabolic ward for 14 days while eating a diet providing 1.2 g/kg B.W. of proteins and supplying 166-188 KG/Kg. From these studies it is concluded that 1.2 g/Kg are a safe intake for patients on CAPD. Nevertheless it is strongly suggested that patients on CAPD should undergo a nitrogen balance study in order to ascertain whether such a diet is safe or should be modified for energy and/or protein supply.

Stability of peritoneal urea clearance in continuous ambulatory peritoneal dialysis (18 months experience with CAPD).

A group of 13 patients on Continous Ambulatory Peritoneal Dialysis up to 18 months was studied. During the study period, Peritoneal Urea Clearance was stable. In all but 2 cases peritonitis did not depress the efficiency in clearing urea. When Peritoneal Urea Clearance was corrected for 1.73 m2 surface area, there was on the average an 11.8% increase over not corrected clearance rates. This increase was statistically significant.

Nitrogen balance in uremic patients on different amino acid and keto acid formulations--a proposed reference pattern.

Nitrogen balance was studied in 12 uremic patients given nitrogen intakes in varying formulations, either solely as amino acids or as keto acid and amino acid mixtures. It was shown that of the three formulas studied, the one that was most effective in promoting positive balance was the one having threonine as reference unit, with a relatively high tyrosine intake and lower methionine content, and histidine in the proportions found in human milk and in the egg pattern. Also, in all instances in which an amino acid formulation caused a deviation in nitrogen balance, the same effect was obtained with formulations containing keto acids. However, the effect with the keto acids was less pronounced.

Effects of catabolic stress in acute and chronic renal failure.

The degree of catabolism was studied in a group of seven patients with postsurgical acute renal failure, and net protein catabolism of 43.1 to 927 g/day. In 70% of a group of 30 patients with stable chronic renal failure receiving and 18 g high biological value protein diet, urea synthesis was equivalent to what could be accounted for by the protein content of the food. This equivalence was not found in studies in both adults and children who were treated for catabolic chronic renal failure. Data obtained show the superiority of feeding essential amino acids with an adequated caloric supply. The need for studies aimed at adapting the uremic patient to protein depletive mechanisms is strongly stressed.

Gamma-globulins patterns in CSF of inflammatory neurological diseases in tropical Africa.

400 CSF electrophoreses were performed on Black West Africans (Ivory Coast) with inflammatory infections of the nervous system. In the normal state, the CSF of the Black African contains a higher concentration of gamma-globulins than the European (15.8% compared to 8--10%). This study permits us to confirm indirectly the classical idea of the extreme rarity of MS in tropical areas: an increase in the gamma-globulins of the CSF has been shown in 22% of the cases and the two principal responsible infections are trypanosomiasis (57.3%) and SSPE (16.8%). The oligoclonal distribution by various authors in trypanosomiases was not found. On the contrary, a nonclonal aspect of gamma-zone was constantly found (49/50). It is possible that the differences are in accordance with the different evolutionary phases of the disease, and that the oligoclonal distribution is a marker of the autoimmune state of the disease.

Short daily peritoneal dialysis: 3 years' experience.

During the last 3 years 3,964 short daily peritoneal dialyses (71.4% at home) have been performed in 18 patients aged between 8 and 82 years. 6-10 liters of dialysate have been associated with high caloric-low protein (0.6 g/kg) diets. In 330 predialysis determinations serum urea was 1.30 +/- 0.09 g/l and serum creatinine was 13.9 +/- 1.9 mg/dl. The incidence of peritonitis was 0.18%. The control of anemia, plasma levels of proteins, albumins, transferrin, C-3c and C-4 were satisfactory. The data support the concept of a wider application of this dialysis technique, which may be extended to every uremic patient.

Lysine in treatment of hyperornithinemia.

A metabolic study in a case of hyperornithinemia with gyrate atrophy of choroid and retina is presented. It is demonstrated that the lysine-ornithine antagonism is a physiological, safe, easy to handle therapeutic tool in hyperornithinemia. A lysine load was invariably associated with a reduction of plasma ornithine concentration. This happened at any level of protein intake. It was also shown that diets containing a low protein intake are invariably associated with negative nitrogen balance.
