[Early detection of ureteral injuries associated with hysterectomy. A prospective echographic study].

Among ureteral injuries that occur during hysterectomy some are neglected and remain asymptomatic. They occur even, although not particularly, during so-called easy operations. Their severity and their need for early treatment point to the use of effective screening. It is not possible to carry out intravenous urography routinely. Searching, however, for pyelo-calyceal dilatation by ultrasound of the kidneys can show ureterohydronephrosis and can detect ureteral trauma. The authors carried out such ultrasound controls in 150 consecutive patients the day before, and the fourth and ninth days after hysterectomy. On the fourth day pyelo-calyceal dilatation is seen more frequently but is not large. Large dilatation appearing on the fourth postoperative day may be considered an early and reliable sign of suspect injury. The authors suggest that renal ultrasound should be carried out routinely on the day before and the fourth day after hysterectomy. The authors feel that when dilatation of the pyelo-calyceal area increases by 12 mm or more between the two examinations major ureteral injury must be suspected and urography carried out intravenously.

[Renal ultrasonic diagnosis in pregnancy. Prospective study of pyelo-calyceal changes in the mother and their obstetric implications].

An acute pyelonephritis is still a serious complication of pregnancy. Urinary stasis plays an important role in the pathogenicity of this infection. 232 patients without symptoms were studied to find out how much urinary stasis occurred by measuring dilatations of the pyelo-calices by ultrasound. The authors suggest that a correlation between the bacteriological examination of the urine and the ultrasound appearance of the kidney can be established. Asymptomatic urinary infection occurs more frequently when there is dilatation of the pelvis of the kidney greater than 12 mm in the right kidney and 7 mm in the left. This is why they suggest that an ultrasound examination of the hilum of the kidney should be carried out routinely to screen for patients who are at risk of having pyelonephritis in pregnancy.

[Effects of dithiothreitol and thioredoxines on the in vitro activity of enzymes of the urea cycle in rats].

The five urea cycle enzymes were studied in desactivated extracts of rat liver. After reduction by dithiothreitol (DTT) and in presence of Mg2+ ions, thioredoxines isolated from rat liver were able to activate carbamyl phosphate synthetase-I (CPS-I) and argininosuccinate synthetase (ASS) respectively by 468% and by 370%. Thioredoxines were purified from adult rat liver and an antiserum was raised to these proteins. After immunologic quantitation, their level in adult rat was 0.103 mg/g liver.

[Uterine rupture. Reflections apropos of a spontaneous case in mid-pregnancy].

A case of spontaneous uterine rupture is reported occurring in the midtrimester of pregnancy in a patient who had benign signs of tuberous sclerosis. There was no placenta increta but the myometrial thickness, at the site of placental insertion, was only 160 microns. This finding is discussed. On this occasion, the authors review the aetiology and signs of uterine rupture, its treatment and prevention, especially before the third trimester of pregnancy.

Changes in ornithine transcarbamylase activity and protein level during perinatal period in rat liver. Effects of actinomycin D.

Ornithine transcarbamylase activity and immunoreactive enzyme level are compared during perinatal period and in adult rat. Ornithine transcarbamylase activity regularly rises during late fetal period and presents a marked increase 24 hours after birth. Immunoreactive enzyme level does not correlate with this developmental pattern. Ornithine transcarbamylase level increases from 0.06 mg on day 19.5 of pregnancy to 0.417 mg/g liver on day 21.5 and remains constant after birth (0.418 mg/g liver). These results suggest that inactive mitochondrial ornithine transcarbamylase accumulates before birth and that the postnatal increase in enzyme activity is mainly associated with an activation. Furthermore, the paradoxical effect of actinomycin D on ornithine transcarbamylase activity is associated with an increase in enzyme level (about 25%).

System analysis and nucleic acid sequence banks.

The mass of published nucleic acid sequence data has required the design of several computerized data bases. We show that this activity is related to the methodology of System Analysis and that data bases are a means of modeling biological knowledge. As an example, the ACNUC data base we have created is presented.

Non-parametric statistics for nucleic acid sequence study.

The use of non-parametric statistics for nucleic acid sequence studies is illustrated by some examples. This method is highly flexible and allows design of specific tests for detecting sequence structure. Tests devoted to local repetitivity, codon nearest neighbors, and dinucleotide avoidance are discussed in detail. An appendix indicates all computations required to use these tests.

ACNUC--a portable retrieval system for nucleic acid sequence databases: logical and physical designs and usage.

ACNUC is a database structure and retrieval software for use with either the GenBank or EMBL nucleic acid sequence data collections. The nucleotide and textual data furnished by both collections are each restructured into a database that allows sequence retrieval on a multi-criterion basis. The main selection criteria are: species (or higher order taxon), keyword, reference, journal, author, and organelle; all logical combinations of these criteria can be used. Direct access to sequence regions that code for a specific product (protein, tRNA or rRNA) is provided. A versatile extraction procedure copies selected sequences, or fragments of them, from the database to user files suitable to be analysed by user-supplied application programs. A detailed help mechanism is provided to aid the user at any time during the retrieval session. All software has been written in FORTRAN 77 which guarantees a high degree of transportability to minicomputers or mainframes.

Effects of corticosteroids and pancreatic hormones on carbamyl phosphate synthetase-I and ornithine transcarbamylase activities in fetal rat liver.

The effects of corticosteroids and pancreatic hormones on two mitochondrial enzymes of ureagenesis, carbamyl phosphate synthetase-I (CPS-I) and ornithine transcarbamylase (OTC), were investigated and compared in fetal rat liver. Supplementing hydrocortisone acetate (50 micrograms) to 18.5-day-old fetuses significantly increased CPS-I activity (by 36%) and decreased OTC activity (by 23%). An actinomycin D supply (2 micrograms) to 18.5-day-old fetuses prematurely increased OTC activity and decreased fetal insulin level (by 42%). This treatment had no effect on CPS-I activity. Glucagon supply (25 micrograms) during the late fetal period increased both activities within 2 h, while dibutyryl-cAMP enhanced OTC activity 17 h later. These results suggested that the fetal development of CPS-I activity was under the control of corticosteroids and glucagon. In contrast, corticosteroid hormones produced an inhibitory effect on OTC activity. This might be explained by the permissive effect of corticosteroids on insulin action, since insulin might act as a repressor in utero of enzyme development. Thus, the paradoxical effect of actinomycin D on OTC activity was probably due to the decrease in fetal insulinemia.

Role of insulin on the control of postnatal increase in ornithine transcarbamylase activity in rat liver.

Repeatedly supplementing glucose after birth abolishes postnatal increase in ornithine transcarbamylase activity and increases the plasma insulin level. A single administration of actinomycin D does not affect this activity. When glucose and actinomycin D are administered in association at birth or 13 hours after birth, actinomycin D overcomes the inhibitory effect of glucose on ornithine transcarbamylase activity and decreases insulinemia at the level found in normal neonate. Insulin supply 20 hours after birth to neonates, which previously received glucose and actinomycin D, decreases the enzyme activity and increases plasma insulin level. It is concluded that the postnatal increase in ornithine transcarbamylase activity is bound to the normal postnatal decrease in insulinemia. Furthermore, the paradoxical effect of actinomycin D on enzyme activity might be due to an inhibition of insulin synthesis or a decrease in insulin release.

ACNUC: a nucleic acid sequence data base and analysis system.

Structured as a data base and associated with data analysis tools, ACNUC allows both on-line access to a central computer and local exploitation of published nucleotide sequences. Its data retrieval capabilities seem to be presently the most powerful available.

Prognostic significance of clinical nodal involvement in patients treated by radical radiotherapy for a locally advanced breast cancer.

A group of 230 female patients, treated between 1960-1976 with radical megavoltage radiotherapy for locally advanced breast cancer (T3 greater than 7 cm-T4 Stage), was analyzed retrospectively. Those with inflammatory cancer or evidence of disseminated metastatic disease were excluded. Clinical axillary nodal involvement appears to be a very important prognostic feature. Actuarial disease-free survival at 5 years for the 109 patients with clinically negative nodes (N0N1a) is 52%; whereas it is 28% (p less than 0.001) for the 121 patients with clinically involved nodes (N1b, N2-N3). The 5-year survival following salvage surgery, which was performed on 92/230 patients (40%), correlated with the initial nodal status. It is 62% for the N0 group, but only 27% for those with clinically involved nodes. The survival of the 138 (60%) patients who were treated by radiotherapy only is similar to that obtained in patients who had secondary salvage surgery. This study reinforces the need to stratify patients with locally advanced breast cancer according to the initial clinical status of the axilla when either therapeutic trials are contemplated or the results of treatment are reported. Adjuvant systemic treatment should be strongly considered in patients with clinically involved nodes, since the survival obtained by local treatment alone is poor.
