Rapid estimation of hospitalization charges from a brief medical record review. Evaluation of a multivariate prediction model.

In settings where an itemized hospital bill is not generated, estimation of hospitalization charges for research or administrative purposes can be a laborious task. This article examines the extent to which the number of hospital days spent outside an intensive care unit (ICU), number of days in an ICU, number of laboratory tests performed, number of x-rays, and number of surgeries can be used in a multiple regression equation to impute inpatient charges for a sample of 103 hospitalizations at a Veterans Administration hospital. These predictor variables, all of which are readily ascertained in a brief medical record review, accounted for about 97% of the variance in imputed hospital charges. The bootstrap method was applied for validation of the prediction equation. Application of the method described here may be of value to researchers concerned with hospital charge estimation in non-fee-for-service settings.

Abnormal communication between the left atrium and the coronary sinus. Presentation of 2 cases and review of the literature.

A large communication between coronary sinus and left atrium is a rare cardiac anomaly. Twenty-four cases from the literature and 2 of our own observations are presented. In most cases a considerable left-to-right shunt may exist on the atrial level that can be misinterpreted as an atrial septal defect. The anomaly may be isolated or in combination with an atrial septal defect or complex cardiac malformations. It is difficult to diagnose this abnormality by cardiac catheterization and even during surgery. Continuous oximetry during a fiberoptic catheter pull-back in the coronary sinus ensures the diagnosis in the case of atrial left-to-right shunt. The anatomy, hemodynamics, diagnostic procedures, and the therapy of the cases reported are discussed.

Steroid secretion by the postmenopausal ovary.

The concentrations of certain steroids in arterial and ovarian venous blood were measured in 42 postmenopausal women undergoing hysterectomy for uterine disease. A concentration gradient across the ovary was found for testosterone in 21 women, for androstenedione in 16, for estradiol in 15, for estrone in 8, and for dehydroepiandrosterone in 3. On the basis of postualated ovarian blood blow the gradient would result in a major contribution to the blood production rate for testosterone in about 50% of women and for estradiol in less than 20%. For androstenedione, dehydroepiandrosterone, and estrone the gradients did not signify important ovarian secretion. There was no evidence for secretion of dihydrotestosterone, estrone sulfate, or dehydroepiandrosterone sulfate. There were no apparent clinical correlates to indicate in which women the ovary was secreting steroids.

Estrone sulfate and dehydroepiandrosterone sulfate concentrations in normal subjects and men with cirrhosis.

Circulation levels of estrone sulfate (E1S) and dehydroepiandrosterone sulfate (DHAS) have been measured in plasma using a radioimmunoassay for estrone and dehydroepiandrosterone following extraction and hydrolysis of the sulfate. The mean +/- SE concentrations of E1S and DHAS in normal men were 458 +/- 25 pg/ml and 1.45 +/- 0.19 micrograms/ml, respectively. In normal women the values for days 5-7 of the cycle were 880 +/- 117 pg/ml and 1.25 +/- 0.12 micrograms/ml which were not different than the values for days 20-22 of 1195 +/- 176 pg/ml and 1.58 +/- 0.29 micrograms/ml. The mean values in post-menopausal women were 250 +/- 33 pg/ml and 0.47 +/- 0.07 micrograms/ml, both lower than the values in young women. In a group of cirrhotic men the mean values were 325 +/- 55 pg/ml and 0.38 +/- 0.12 micrograms/ml, both significantly lower than the normal values. This suggests a defect in sulfurylation in men with hepatic cirrhosis.

Androgen and estrogen metabolism in male rhesus monkeys.

Using constant infusions of 3H-labeled androgens and 14C-labeled estrogens, the dynamics of androgen and estrogen metabolism have been studied in male rhesus monkeys. The mean (+/-) MCR, as measured in whole blood, for testosterone (T; 140 +/- 10 liters/day) was significantly smaller than the MCRs for androstenedione (A), estrone (E1), and estradiol (E2; 650 +/- 8, 1010 +/- 160, and 500 +/- 20 liters/day, respectively). These findings are compatible with the specific globulin binding of T in this species. The mean blood productions were 0.42 +/- 0.11 and 0.55 +/- 0.04 mg/day for A and T and 40 +/- 16 and 13 +/- micrograms/day for E1 and E2, respectively. The interconversions of the androgens [fractional conversion rates of A to T ([rho]A,TBB) and T to A ([rho]T,ABB)] were not different but were lower than the interconversions of the estrogens ([rho]E1,E2BB and [rho]E2,E1BB). The mean [rho]T,E2BB values were 0.015 +/- 0.001 and 0.0024 +/- 0.0003, which are remarkably similiar to those values in men. Administrations of PMS gonadotropin did not alter any measured values of androgen or estrogen metabolism, hCG resulted in a decrease in the conversion ratio of A to T, T administration increased the MCR of T, and estrogen administration resulted in no changes. The aromatization rate remained unaffected by all treatments.

[The effect of Solcoseryl on explant cultures of the hippocampus].

Explants of hippocampus from fetal rats were cultivated in Maximow chambers in semisynthetic medium up to 12 days in vitro. The cultures were fixed Bouin, slided 15 micron, coloured with Klüver-Barrera and some morphological parameters were tested. 1. The nerve fiber index increased by influence of 1% Solcoseryl in relation to control cultures, which growed in minimal medium. An essential stimulation was observed by application of placentar serum and embryonal extract into the culture medium. 2. Die decrease of the number of neurons and glial cells per unit of area and a small decrease of the area of neuron nuclei was discussed in relation to the effect of the pharmacon Solcoseryl on O2- consumption. 3. Solcoseryl (firm Solco AG, Base) is an extract of calf blood. It can not substitute other tissue extracts.

[Myocardial stress of different surgically induced types of cardiac arrest in dogs evaluated by myocardial potassium and acid-base balance (author's transl)].

In mongrel dogs myocardial stress of the following types of artificial cardiac arrest in moderate hypothermia and cardiopulmonary bypass was compared: 1. Ischemic arrest after aortic occlusion lasting 30 or 45 minutes. 2. Injection-cardioplegia according to Kirsch lasting 90 minutes. 3. infusion-cardioplegia according to Bretschneider lasting 90 minutes. The following parameters were compared: extracellular potassium activity ([K+]e) during cardiac arrest and [K+] in arterial and coronary venous blood during reflow, total [K+] and acid-base balance during reflow, time course of [K+] and lactic acid release and uptake during reflow after cardiac arrest. In our experimental conditions the lowest alterations of myocardial [K+] and acid-base balance were observed after infusion-cardioplegia according to Bretschneider. Injection-cardioplegia according to Kirsch was less protective for the heart as evidenced by more intensive acidosis, enlarged [K+] loss and prolongation of [K+] uptake during reflow. Ischemic cardiac arrest in moderate hypothermia was the worst type of artificial standstill indicated by development of fatal myocardial acidosis.

[Myocardial contractility in different methods of Cardioplegia. An experimental study (author's transl)].

In 22 mongrel dogs the protective effect on the myocardium of Bretschneider- and Kirsch-Cardioplegia (90 minutes of cardiac arrest) was studied and compared with ischemic arrest (45 minutes at hypothermia of 30 degrees C). Myocardial contractility was evaluated by the following indices of contractility: t--dp/dt max., Krayenbühl-Index of contractility and max. Vce. Depression of contractility of 40 % was observed after ischemic arrest and Kirsch-Cardioplegia, whereas in Bretschneider-Cardioplegia depression was only 7 %. Forty-five minutes after reperfusion contractility had returned to normal in the last group. It is concluded that Bretschneider-Cardioplegia results in little reduction of postischemic contractility, and has a good protective effect on the myocardium.

[Antegrade catheterization of the aorta with a flow-directed balloon catheter following transseptal left atrial puncture: a new technique].

The described heart catheterization technique allows the antegrade catheterization of the aorta from the left ventricle with a Swan-Ganz-flow-directed catheter via the saphenous vein and subsequent puncture of the intact atrial septum. The transseptal Brockenbrough-technique is modified by the use of a long thin polythene sheath which is fitted over the transseptal left heart catheter and can be advanced over the catheter after atrial septal puncture. A Swan-Ganz-flow-directed catheter can be passed directly to the left ventricle toward the outflow tract before it enters the aorta. This technique was used successfully on 18 children. By avoiding the retrograde arterial catheter technique additional information will be obtained.

[Cardiac catheterization of neonates and young infants using a flow-directed balloon catheter (author's transl)].

In cardiac catheterization with the Swan-Ganz-flow-directed balloon catheter the aorta or, in transposition of the great arteries, the pulmonary artery was entered from the left ventricle with a special heart catheterization technique in 68 neonates and young infants. In the same way selective aortography was performed in 29 of these patients. This catheterization technique has the following advantages: 1. the complete evaluation of complex cardiac anomalies even in neonates, 2. selective aortography avoiding the retrograde arterial catheterization, 3. reducing the risk of cardiac perforation or myocardial staining due to the contrast injection using the usual semirigid catheters. Balloon rupture or knotting are specific but avoidable complications.

[Chronic tonsillar hypertrophy as a cause of cor pulmonale, pulmonary oedema, and hypersomnia in children (author's transl)].

Chronic hypertrophied tonsils and adenoids causing partial airway obstruction produced serious illness and a confusing clinical picture in a 51/2-year-old boy. Cardinal signs were cor pulmonale, pulmonary oedema and marked cardio-respiratory changes due to hypoxaemia and hypercapnia, in addition to hypersomnia. Marked improvement of the clinical picture and the abnormal signs followed directly upon tonsillectomy and adenoidectomy. Chronic alveolar hypoventilation is presumably the main cause of the described condition and of others of extracardiac origin with similar signs and symptoms.

[Diphtheria].

[Minor diphtheria epidemic in a Cologne children's home (author's transl)].

Between May and July 1972, 12 children aged from 7 to 14 were admitted to the Cologne University Children's Hospital suspected of having diphtheria. In 10 cases the tonsils or pharyngeal cavity were affected. One case of wound diphtheria and one of nasal diphtheria were observed. The clinical suspicion was confirmed in 8 of 12 cases by detection of the pathogen Coryne-bacterium diphtheroides mitis. 11 patients were discharged from the hospital after being treated for between 2 to 6 weeks, but 5 of them needed ambulant cardiac observation. One patient died of malignant diphtheria with symptoms of cardiovascular failure. He had not been treated specifically for 1 week at first. Not one of the 12 patients had been satisfactorily immunized by active inoculation with diphtheria toxoid.
