The effect of dexamethasone on pyruvate kinase activity in primary cultures of hepatocytes.

Pyruvate kinase activity in primary cultures of hepatocytes isolated from a normal rat was maintained at a constant level similar to that found in vivo (14.0 +/- 2.8 units per mg of DNA) for over 6 days when both dexamethasone and insulin were included in the medium. Yet the pyruvate kinase activity decreased 50% when the cells were cultured for 2 days and 4 days, respectively, in the presence of either dexamethasone or insulin alone. A brief, 10 min incubation of hepatocytes in the presence of dexamethasone was sufficient to maintain the enzyme activity of cells subsequently cultured for 4 days in the presence of insulin. The optimal dexamethasone concentration was 1 microM. Three other glucocorticoids were able to maintain the pyruvate kinase activity in cells cultured in medium containing insulin. The presence of the protein synthesis inhibitors, actinomycin D or cyclohexamide in cells cultured in the presence of dexamethasone and insulin resulted in a 25% decrease in the pyruvate kinase activity. Therefore, it is suggested that the synergistic effect of glucocorticoids and insulin to maintain pyruvate kinase activity in primary cultures of hepatocytes is dependent upon the ability of these cells to maintain protein synthesis.

The effects of screen size and eccentricity on acuity estimates in infants using preferential looking.

The study examined whether screen size (10 degrees vs 19 degrees dia.) and separation (3 degrees vs 10 degrees eccentricity of inner edges) affect the estimates of acuity obtained with 1-3 month infants tested by forced-choice preferential looking. One and 2-month infants (but not 3-month olds) showed higher acuity estimates with the larger screens. Screen separation did not significantly affect acuity estimates for any of the age groups. Possible factors underlying these results are discussed.

Iodine-123 assay using a radionuclide calibrator.

Iodine-123 (123I), an accelerator-produced radionuclide, gained wide acceptance recently in medical applications. Radionuclide dose calibrators are commonly used to assay radioactivities of radiopharmaceuticals, and should be maintained to read within +/- 5%, both for constancy and accuracy. Radionuclidic impurities present in commercial 123I preparations may produce an assay error larger than 10%. The type and quantity of impurities depend on the mode of production of 123I. The impurities can be reduced by either increasing the energy of the proton beam (over 60 MeV) and using the 127I (p,5n) 133Xe leads to 123I indirect reaction or using lower energies and highly enriched (better than 99%) 124Te (or 122Te) target for the direct production of 123I via 124Te (p, 2n) 123I or 122Te (d, n) 123I. A survey of radionuclide calibrators was undertaken to investigate the extent of the 123I assay error due to impurities present in commercially available preparations of 123I. Results from 24 calibrators indicate that the activity of a (p, 2n) 123I capsule is overestimated on average by 22% two half-lives after calibration time, while the activity of a (p, 5n) 123I capsule remains within the accuracy of the radionuclide calibrator of +/- 5%. The 124I fraction increases significantly with time during the useful life of a (p, 2n) 123I capsule introducing large 123I assay errors.

Dynamic urethral function in the assessment of spinal injury patients.

Forty-two spinal injury patients underwent single or sequential urodynamic studies to assess the value of dynamic urethral pressure using transducer tipped catheters. Three groups of patients were identified: those with poor detrusor function, those with unsustained dyssynergia and those with sustained dyssynergia. All patients demonstrated some dyssynergia. Urethral needle EMG mirrored urethral pressure changes accurately. The addition of dynamic profilometry to fluoroscopy, detrusor pressure and urethral EMG has clarified our understanding of lower urinary tract behaviour after cord lesions.

A subjective and urodynamic assessment of unilateral bladder neck incision for bladder neck obstruction.

Thirty-eight patients with bladder neck obstruction were treated by unilateral bladder neck incision. Good subjective and urodynamic results were obtained in 33 patients. Retrograde ejaculation occurred in 16% of the patients on recommencing sexual activity. The value of combined videocystography and pressure flow studies is discussed. The technique is not recommended for patients presenting with acute retention.

Staging lymphography with respect to lymph node histology, treatment, and follow-up in patients with mycosis fungoides.

Extracutaneous involvement in mycosis fungoides (MF) is associated with a poor prognosis. Therefore, a pretreatment staging procedure is used to determine the extent of the disease. As part of the staging procedure lymphography was performed in 36 patients with MF. The radiographic findings and the correlation of these findings with the histopathologic changes in lymph nodes and the clinical course are described. Lymphography in MF shows a variety of abnormalities. However, these abnormalities correlate in only 61% of the patients with the extent and the clinical course of the disease. It is concluded that lymphography is not a useful clinical tool in the staging of MF.

The organization of health services research in the Veterans Administration.

The VA is developing health services research expertise within its health care system. This development and management effort is focused in the Health Services Research and Development Service in the VA Central Office in Washington. The VA development of health services research includes four major programs. Two programs, the Health Services Intramural and Contract Research Programs, provide funding for individual research projects. These programs are analogous in purpose, structure, and administration to the long-established VA biomedical research programs. The third program is the VA-University Health Service Research Affiliation Program. This program consists of about a dozen VA hospital-university affiliations for health services research and is coordinated with the Health Research. Te fourth program is a doctoral and postdoctoral health services training program. By means of these four programs and related efforts, the VA intends to provide knowledge useful for improving teh delivery of health services in the VA health care system.
