Massive incrustation on silicone ureteral stent: a severe complication.

We report the first case of unilateral, upper tract obstruction secondary to incrustation and stone formation on a silicone double-J ureteral stent. We believe that the main cause of this complication was the neurological bladder dysfunction of the patient coupled with urinary tract infection.

Absorbability of mucosa in total replacement of urinary bladder with jejunum. An experimental study.

The absorption of urine components in jejunum substitute for the urinary bladder was studied experimentally in dogs. For this purpose, determination of the blood urea, creatinine, and electrolyte levels as well as measurements of the radioactivity in blood samples after intravesical instillations of radioactive iodine, were done at various intervals after total replacement of the urinary bladder with jejunum. The absorption was found to decrease greatly in one month and to be negligible one year postoperatively.

Genetic markers in renal adenocarcinoma.

The distribution of phenotypes and gene frequencies of the 3rd component of complement (C3), group-specific component (Gc), haptoglobin (Hp) and transferrin (Tf) were studied in 50 patients with renal adenocarcinoma. The statistical analysis of our findings in comparison to the frequency of these genes in the general population does not reveal any correlation between the distribution of the Hp and Tf phenotypes and the disease. On the contrary, a statistically significant association was found between renal adenocarcinoma and C3F and Gc2 genes. The relative risk incidence of this malignancy is 2.07 and 1.94 respectively for the carriers of these genes. These data indicate that genetic factors, possibly related to the immune mechanisms and calcium metabolism, play a role in the pathogenesis of renal adenocarcinoma.

Sensitivity of exfoliative cytology in bladder cancer using the divided one-urine specimen examination.

The urine of 104 patients with bladder cancer was examined by exfoliative cytology. Three staining methods (Giemsa, Papanicolaou and Oil Red O) were used and the results compared with the histological diagnosis. Giemsa was found to be superior to the other two methods but the highest sensitivity (which was statistically significant) was obtained by a combination of the three methods.

Genetic markers in carcinoma of the prostate.

The distribution of phenotypes and gene frequencies of the third component of complement (C3), group-specific component (Gc), haptoglobin (Hp) and transferrin (Tf) were studied in 115 patients with carcinoma of the prostate. The statistical analysis of our findings in comparison to the frequency of these genes in a control group of 155 patients with benign prostatic hyperplasia does not reveal any correlation between the distribution of the Hp and Tf phenotypes and the disease. On the contrary, a statistically significant association was found between carcinoma of prostate and C3F and Gc2 genes. The relative risk incidence of this malignancy is 1.56 and 1.81, respectively, for the carriers of these genes suffering from benign prostate hyperplasia.

[Genetic markers in benign hypertrophy of the prostate].

The distribution of phenotypes and gene frequencies of the C3 component of complement, group-specific component, transferrin and haptoglobin were studied in 155 patients with benign prostatic hyperplasia. A statistical analysis of the findings in comparison with the frequency of these genes in the general population failed to demonstrate any correlation between phenotype distribution and benign prostatic hyperplasia.

Familial cystinuria in Ioannina District (Greece). Diagnosis and treatment.

The "Urocystin Test" has been used as a screening procedure for the diagnosis of the incidence of cystinuria in Ioannina District (Northwest Greece). From the 210 investigated urine samples, eight were positive and four of them were also L-cystine stone formers. All positive cases belong to two cystinuric families. The pedigree in village "Kato-Lapsista" is a genetic type of a "completely recessive cystinuria" while the other one in "Marmara" is the type of an "incompletely recessive cystinuria". All patients with L-cystine urolithiasis, except one child, were treated with the drug Thiola.

X-ray diffractometric analysis of L-cystine stones in 12 patients.

In 12 of 21 cystinuric patients, the components of cystine stones, either previously passed or removed at surgery, were studied by X-ray diffractometry and chemical analysis. The results of diffractometry suggested that, in addition to cystine, the stones contained sodium urate and ammonium magnesium orthophosphate hydrate, although chemical analysis was negative for these components. The role of uric acid in cystinuria is discussed.

[Genetic markers in papillary cancer of the bladder].

The distribution of the phenotypes and the gene frequency of the 3rd fraction of complement (C3), the specific group (Gc), haptoglobin (Hp) and transferrin were studied in 133 patients with transitional cell cancer of the bladder (papillary cancer). Statistical analysis of these results, in comparison with the frequency of these genes in the general population, was unable to demonstrate a correlation between the distribution of these phenotypes and papillary cancer.

The role of acid and alkaline DNases as tumour markers in cancer of the genitourinary tract.

The levels of Acid and Alkaline DNases were measured in the serum of patients with: (a) Cancer of the Genitourinary Tract (confirmed by biopsy), (b) with inflammatory diseases and non-malignant tumours of the Genitourinary tract, (c) healthy blood donors. In the first group the results showed that the Acid DNase level was raised in 62% and Alkaline DNase in 43%. In the second group acid DNase was increased in 30% and Alkaline DNase in 13%. In the third group Acid and Alkaline DNase levels were normal. These results suggest that the measurement of Acid and Alkaline DNases could be considered as malignant diseases markers, in spite of false positive and false negative results in some cases.

Metabolic disturbances in cystinuria.

Metabolic investigations were made in 21 cystinuric patients. The results showed a low output of urinary phosphorus and an increase of plasma IgG. These findings and the presence of uric acid in the urine, even in normal values, could play an important role in pathogenesis and stone formation, and also in cystinuria.

[Effect of acute distension of the bladder on blood flow and the resistance of the peripheral vessels].

The effects of acute, rapid distention of the urinary bladder on the arterial pressure, blood flow and peripheral resistances were studied on dogs and an attempt was made to elucidate the mechanisms of cardiovascular responses observed. In 15 dogs with intact nervous system the arterial blood pressure and blood flow of the femoral, brachial and renal arteries were recorded and the vascular resistances were calculated during an increase of the intra-bladder pressure. Acute distention of the bladder, in the dog, increases arterial blood pressure by 10,25%, the vascular resistance of the femoral and renal vascular bed increases while that of the brachial artery is slightly reduced.

[Changes in tissue enzymes during "in situ" renal ischemia with or without protection of the kidneys].

Three methods of preservation of the in situ temporarily ischaemic kidney were compared in dogs. The study was carried out in 24 dogs, divided in 4 groups. Each group included 4 dogs in which the ischaemic time was 60 minutes, and 3 dogs in which the ischaemic time was 90 minutes. The experimental animals of the first group served as controls (warm ischaemia); in the second group renal hypothermia was accomplished by an arterial infusion of Collins' solution at 4 degrees C; in the third group preservation of the kidney was attempted only by an arterial infusion of inosine (90-120 mg/kg); finally in the fourth group, the infusion of Collins' solution was combined with infusion of inosine (800-1 000 mg/l Collins' solution). The assessment of the damage of the ischaemic kidney was made by histo-enzymological analysis of renal tissue specimens received from each animal just before initiation of the ischaemia and on the 21st post-operative days. The enzymes examined in this analysis were: Alkaline Phosphatase, ATP-ase, Glutamate Dehydrogenase, Glucose-6-phosphatase, Hydroxybutyrate Dehydrogenase and gamma-GT. These studies have shown that the ischaemic kidney is best preserved by the arterial infusion of Collins' solution with simultaneous infusion of inosine.

[Estimation of serum beta 2 microglobulin in carcinoma of the bladder (author's transl)].

beta 2 Microglobulin is a low molecular weight protein synthesised by lymphocytes and neoplastic cells. The authors measured levels of this substance in the plasma of 40 patients with carcinoma of the bladder and normal renal function. 26 subjects were used as controls. Plasma levels were markedly higher in the bladder carcinoma sufferers than in the normal controls. Levels were also higher in invasive carcinomas than in superficial lesions. beta 2 microglobulin may therefore not be considered to be a specific indicator of carcinoma of the bladder but rather as a factor offering the possibility of measurement of the secretory activity of tumour cells.
