[Comparison of in vitro bacteriostatic effect of five betalactamins against Pseudomonas aeruginosa (author's transl)].

In vitro activity of two penicillins (ticarcillin and azlocillin) and three cephalosporins (cefoperazone, cefsulodin and ceftazidime) was compared against one hundred clinical isolates of Pseudomonas aeruginosa originated in two different Paris hospitals. Twenty strains were resistant to carbenicillin at a concentration of 128 mg/l. Minimal inhibitory concentrations (MICs) were determined by a twofold agar dilution method. The required ceftazidime concentration to inhibit 90 p. cent of the carbenicillin-susceptible isolates was 1,9 mg/l. To achieve the same inhibition, 3,9 mg of cefsulodin, 12 mg of cefoperazone, 14 mg of azlocillin or 38 mg of ticarcillin per liter were needed. Against the 209 carbenicillin-resistant isolates, ceftazidime remained active at concentrations lower than 2 mg/l in 90 p. cent of the cases. To obtain such an inhibition with the other antibiotics higher concentrations were needed.

Plasma vasopressin, renin activity, and aldosterone: effect of exercise and training.

The influence of endurance-training on hematocrit, plasma vasopressin, renin activity, and aldosterone changes at rest and at the end of an exercise performed until exhaustion at a given and constant relative work-load (87% of maximal oxygen uptake) has been studied in four untrained subjects submitted to a 5-month training. At the end of this period, maximal oxygen uptake increased of 15.2% (p less than 0.01). Hematocrit at rest slightly rose after training, and if exercise constantly induced increases in hematocrit before (p less than 0.001) and after training (p less than 0.005), the per cent increase after training was lower than before (p less than 0.05). Comparison between the importance of weight loss and hematocrit variation showed that when untrained subjects become trained the variation of hematocrit after exercise becomes smaller while weight loss is more important (p less than 0.01). Plasma renin activity (PRA), aldosterone (Aldo) and vasopressin (AVP) levels, compared to control values, displayed a significant increase after exercise before as well as after training. Control values remained unchanged after training for aldosterone and AVP, but were significantly lower (p less than 0.05) for PRA. This latter observation could be explained by the change in blood volume induced by exercise.

[Disseminated actinomycosis presenting with multiple subcutaneous abscesses (author's transl)].

The case report describes a patient hospitalized for multiple subcutaneous abscesses who presented a four-year history of recurrent skin lesions and a fifteen-year history of non specific bronchopulmonary infections requiring segmental resection. The diagnosis of actinomycosis, A. israeli species, was established by culture of surgical drainage material from a thoraco-abdominal wall abscess. Muscle and bone involvement of the right thigh was subsequently demonstrated. There was no evidence of pulmonary infection at the time of hospitalization. Treatment with Penicillin G was curative. A review of the literature is presented together with a summary of thirteen reported cases. Although disseminated actinomycosis has become very rare since the introduction of antibiotic therapy, patients share similar features and a common clinical pattern which are highly evocative of the disease. All patients described had preceding pleuropulmonary disease, characteristically chronic. Evolution of cutaneous lesions was subacute or chronic. In all cases, there was significant delay (months to years) between the initial appearance of skin lesions and subsequent diagnosis. In most cases, actinomycosis had not been suspected. Diagnosis was usually established by isolation and identification of the micro-organism in cultures of purulent material obtained from cutaneous lesions. Despite the probability of hematogenous disease dissemination, pleuro-pulmonary foci were only rarely demonstrated. Prolonged antibiotic therapy was curative in all treated cases.
