Thromboxane B2, 6-keto-PGF1 alpha, PGE2, PGF2 alpha, and PGA1 plasma levels in arteriosclerosis obliterans: relationship to clinical manifestations, risk factors, and arterial pathoanatomy.

Current concepts of atherogenesis based on animal and human investigations indicate prostaglandins as a key factor in atherosclerotic lesions. The plasma profiles of thromboxane B2 (TXB2), 6-keto-PGF1 alpha, PGE2, PGF2 alpha, and PGA1 were investigated by means of a sensitive radioimmunoassay technique in 40 patients with arteriosclerosis obliterans and in 30 healthy control subjects. Abnormally high levels of TXB2 and PGE2 (222.97 +/- 320.86 pg/ml, mean +/- SD, vs 20 +/- 2.1 and 352.66 +/- 235.54 vs 24.4 +/- 3, p less than 0.01) were detected in arteriosclerosis obliterans patients. The ratio between TXB2 and 6-keto-PGF1 alpha was increased from 1.2 in control subjects to 6.0 in patients. In arteriosclerosis obliterans TXB2 increased in relation to clinical manifestations and to the extension of the vascular damage. In addition, TXB2 was positively related to serum triglyceride content (r = 0.562, p less than 0.05) and inversely related to platelet count (r = 0.727, p less than 0.001). The marked imbalance between the stable metabolites of thromboxane and prostacyclin in arteriosclerosis obliterans patients provides biologic evidence which fits well with the thrombogenic theory of atherosclerosis. These results further support the theory that prostaglandins may be heavily involved in atherosclerosis.

Effect of age on different receptors and functions of phagocytic cells.

Different tests were performed on 15 young (age between 17 and 39) and 13 older (62-83) subjects. Blood polymorphs and monocytes were separately tested for ingestion of 3 different particle species and stimulation of superoxide anion production. The erythrocyte sedimentation rate and the serum concentrations of 10 proteins possibly related to inflammatory processes were also determined. It is concluded that: Individual variations of phagocyte ability to be stimulated by different cell types are independent processes. Aged people displayed selective decrease of Fc receptor-mediated phagocytosis by granulocytes and possibly selective impairment of nonspecific phagocytosis by monocytes. Further, the IgM level was decreased whereas the IgA, C-reactive protein and erythrocyte sedimentation rate were higher in old people than in young subjects. Correlations were found between the serum concentrations of some protein species and phagocytic function, but further experiments were coincidental (if both parameters were age-related) or revealed some causal link. It is suggested that our procedure provides a simple and reproducible way of testing many independent functions of phagocytic cells.

[Carbonic anhydrase activity in male chronic alcoholics. Statistical analysis of correlations apropos of 74 cases].

Changes in cadmium blood levels and erythrocyte contents and in carbonic anhydrase activity (C.A.A.) were studied comparatively in 74 male subjects: 37 chronic alcoholics and 37 healthy age-matched controls. Chronic alcoholics exhibited significantly higher serum and erythrocyte cadmium concentrations and lower CAA than controls. Cadmium levels were correlated with the CAA. There was a negative correlation between the CAA and the mean corpuscular volume, daily alcohol intake and duration of the intemperance, in contrast with a positive correlation demonstrated for cadmium concentrations.

[Plasma thromboxane B2 and capacity of the aorta to produce prostacyclin in arteriopathies of the lower extremities].

Plasma levels of thromboxane B2, and 6-Keto-PGF1 alpha, stable metabolites of thromboxane and of prostacyclin were determined by radioimmunoassay in 17 patients with arterial disease and 10 control subjects. The production of prostacyclin from incubates of aortic microsomal fractions of patients was also studied. There was a significant elevation of thromboxane B2 in the peripheral venous blood of patients with arterial lesions (142 +/- 152 pg/ml vs 22 +/- 2 pg/ml, p less than 0.005); levels of 6-KetoPGF1 alpha were equally low in controls and patients. Rate of production of prostacyclin, which was 450 +/- 24 pmol/50 mg of proteins in 10 minutes in controls, was reduced to 97 +/- 86 pmol/50 mg prot. 10 min (p less than 0.01) in patients. Co-existence of raised plasma thromboxane levels and a reduced capacity for prostacyclin synthesis in the same patients is supportive evidence of the thrombogenic theory of arterial disease.

[Serum profile of inflammatory reaction proteins in arteriopathies of the lower limbs, stage II].

The serum content of proteins involved in the inflammatory process was investigated, partly through kinetic immunodiffusion, in 85 men subdivided as follows: 45 men (aged 57 +/- 5 years) with intermittent claudication due to arteriosclerosis obliterans; 20 controls matched for sex and age; 20 sex-matched controls whose mean age was 25 +/- 3 years. Patients with obliterating arteriosclerosis had signs of both subacute and chronic inflammation. Compared to controls, they had a higher sedimentation rate, a lower percentage of serum albumine, higher percentages and absolute serum contents os alpha 1, alpha 2 and beta globulins, and higher serum concentrations of fibrin, orosomucoid, C3 complement fraction and IgA. In addition, the modifications of these proteins were closely correlated. These results are consistent with previous reports concerning patients with coronary heart disease. They point out the relationship between inflammation and atherosclerosis.

[A case of staphylococcal spondylodiscitis after spinal injury. Post-traumatic spondylodiscitis?].

The authors report the case of a patient who had a D9-D10 staphylococcic spondylodiscitis three month after a fracture of the first lumbar vertebra, which they consider to be posttraumatic despite the fact that it is localised in a different area. They insist on the insufficiency of the literature existing on such cases and on the various pathogenic hypotheses to be applied by the practitionner.

[Thrombocytosis and cancer. Apropos of a chronological series of 100 patients].

The authors report a personal chronological series of 100 cancer patients submitted to an analytical study, and noted thrombocytosis in 18% of cases. They attempt to draw up a correlation between the presence of increased platelets, and the site of the primary tumour, its spread, the state of anemia or iron deficiency in these patients. During a general review of the literature, they compare their results with those of various american and german series and report the various pathogenic hypotheses suggested upto date. They emphasise the fact that routine platelet counts may be of great interest in the detection of certain early carcinomas in spite of the generally moderate levels of thrombocytosis observed. They therefore have a place in routine health checks.

[Beta blockaders and general pathology].

The authors review the literature on the undesirable side effects of beta blockaders (excluding the cardiovascular system) and report the new therapeutic propects in psychiatry and in various metabolic disorders. The necessity of sufficient follow up made us choose propanolol as reference product without neglecting acebutolol, pindolol and oxprenolol.

[Rheumatoid arthritis and trauma. 5 cases].

The authors report 5 cases of post-traumatic rheumatoid polyarthritis. After the traumatism had initiated the first stages of ordinary inflammation, chain reactions, perhaps by means of an "immune" field, intervened to transform it into a self-maintained rheumatoid inflammation. In fact, the etiopathogenic problems of post-traumatic rheumatoid polyarthritis have not yet been elucidated and it cannot be said either that the question of legal compensation for it is any nearer solution. The growing importance of traumatisms in everyday life and of compensation for them, and the gravity of the condition and its disabling sequelae require that great caution should be exercised in the search for criteria for attributing the disease to the traumatism: the almost indefinite evolution of the disease with time makes the choice of the consolidation date arbitrary.
