Isolation of three new avian sarcoma viruses: ASV 9, ASV 17, and ASV 25.

The newly isolated avian sarcoma viruses, ASV 9, 17, and 25, cause fibrosarcomas in young chickens and induce foci of transformed cells in chick embryo fibroblast cultures. They are defective in replication and belong to envelope subgroup A. The sizes of their genomes are 6 kb (ASV 9), 5 kb (ASV 17), and 6 kb (ASV 25), respectively. All three contain long terminal repeat (LTR) and gag sequences but lack pol. env is absent from ASV 9 and ASV 25, but some env sequences are detectable in ASV 17. None of the defective viral genomes hybridized to selected onc probes representing src, fps, yes, myc, myb, and erb A. erb B appears absent from ASV 9 and ASV 17, but some hybridization between the erb B probe and the RNA of ASV 25 was detected. ASV 9 codes for a transformation-specific gag-linked protein of 130kDa. Multiple gag-linked transformation-specific proteins are seen in ASV 17 and 25; they require further study.

Coagulopathy related to dilution and hypotension during massive transfusion.

A retrospective review of 64 patients receiving more than 10 units of red cell concentrate plus crystalloid within 12 h revealed two consecutive patterns of elevation of the partial thromboplastin time (PTT). The PTT at 3 to 4 h (PTT3-4) correlated with the number of liters of crystalloid (LC) infused over the first 3 h (PTT3-4 = 37 + 7 LC, r = .7643, p less than .001); the PTT thereafter (PTT4+) correlated with the number of hours of closely antecedent hypotension (AH) (PTT4+ = 37 + 21AH, r = .8680, p less than .001). These data indicate a transient dilutional coagulopathy, followed by coagulopathy related to the duration of closely AH. Whether this latter is due to impaired production, disseminated intravascular coagulation, or dilution due to internal shifts of fluids and/or proteins, remains to be clarified. Therapeutic implications of these data are discussed.

Cornelia de Lange syndrome in a mother and daughter.

The Cornelia de Lange syndrome was first described in 1933. Since then, more than 250 cases have been described in the medical literature. It has generally been considered to be sporadic, but several authors have raised the possibility of genetic factors. We present a mother and child affected with Cornelia de Lange syndrome and raise the possibility of autosomal dominant inheritance.

Health concerns and teacher training of selected elementary teachers in Michigan.

With reductions in the number of school-based health personnel, increased responsibility for the identification of health problems falls to the classroom teacher. This paper describes a survey of the level of health-related training among a sample of elementary teachers in a Michigan intermediate school district, with the teachers' perceptions of their ability to recognize and deal with selected student health problems. Though some differences existed between classroom and other teachers, and between veteran and newly-trained teachers, the overall results were clear--elementary teachers are not well-prepared for their expanded role. This fact was especially true for more difficult to recognize and potentially more serious health problems such as injury to the head and substance abuse, where teachers expressed considerable discomfort with their ability and lack of preparation. Several recommendations are offered relating to the need for health training.

Occurrence and properties of bacteriophages of Leuconostoc oenos in Australian wines.

Bacteriophages specific for Leuconostoc oenos were isolated from four red wines undergoing malolactic fermentation in one winery. Bacteriophages were not found in samples of 16 other wines. The morphology of the phages was examined by electron microscopy. The phages did not lyse all strains of L. oenos, and susceptibility correlated to some extent with the colony morphology of the strain. Phage survived in wines at pH values greater than 3.5 but was inactivated in wines of lower pH and by the addition of sulfur dioxide or bentonite. Phage did not affect the growth of a sensitive strain of L. oenos in filter-sterilized wine.

A comparison of bacterial aggregation induced by saliva, lysozyme, and zinc.

Aggregation of bacteria by zinc and lysozyme was studied and compared with aggregation induced by a high-molecular-weight salivary agglutinin. Each ligand was found to exhibit a unique profile of properties when examined by both a microradiochemical centrifugation assay and a turbidimetric assay. Significant differences in rate of aggregation and bacterial species specificity were noted. Zinc- and lysozyme-mediated aggregations were shown to be calcium independent and to proceed rapidly at 0 degree C, in contrast to the salivary agglutinin. Zinc produced large, asymmetric aggregates, saliva produced intermediate-sized aggregates, and lysozyme produced the smallest aggregates. These size differences are consistent with many of the observed reaction properties.

Syringoperitoneal shunt for treatment of cord cavitation.

A series of twenty-nine patients with cord cavitation were treated by syringoperitoneal shunt. Twenty-two of them showed improvement after operation, five were unchanged and two worsened. Operation was performed in patients with post-traumatic cystic myelopathy, idiopathic syringomyelia, intramedullary tumours associated with cysts, and patients who had developed cystic myelopathy in association with spinal arachnoiditis. Pain improved in twenty patients, sensory symptoms and signs in eleven and weakness in ten.

Mammography, sonomammography, and diaphanography (lightscanning). A prospective, comparative study with histologic correlation.

While mammography has become a routine method both for the screening and preoperative assessment of breast disease, the role of this modality, especially when compared with newer techniques of breast imaging, needs continued assessment. Recent advances using sonographic principles and diaphanography (light transillumination) must be compared with mammography to define specificity, sensitivity, and accuracy in assessing breast lesions. During a 16-month period between November 1982 and February 1984, 467 women with clinically apparent breast disease (symptoms or palpable lesions) were each studied using all three imaging techniques of mammography, sonomammography, and diaphanography. Of 168 women recommended for biopsy on the basis of these techniques, 84 women had histologic confirmation during this study period. Benign breast disease was diagnosed histologically in 38 women, while carcinoma was found in 46 patients. These techniques showed no significant differences in predicting benign or malignant disease when rates of sensitivity, accuracy, and specificity are computed. Diaphanography (lightscanning), allowed for consistently correct interpretation of cases proven to be histologically malignant and showed a false-negative rate comparable with x-ray mammography. We conclude that diaphanography is a sensitive indicator of both benign and malignant breast disease while serving as a reliable predictor of clinically apparent breast lesions without the potential problems of radiation exposure.

Metastatic neuroblastoma arising in an ovarian teratoma with long-term survival. Case report and review of the literature.

A case of neuroblastoma arising in an immature teratoma of the ovary in a 22-year-old woman is reported. Differentiation was grade 3 in the primary and metastases. Metastases to retroperitoneal, mediastinal, and supraclavicular lymph nodes and to bone were diagnosed 2 years after presentation of the primary tumor. Electron microscopic study demonstrated dense-core granules of the neurosecretory elements. Treatment with combination chemotherapy followed by radiotherapy resulted in complete remission which continues to the time of this report, more than 4 years after diagnosis of the primary tumor. Prolonged survival of metastasizing grade 3 immature teratoma is distinctly uncommon. The literature pertaining to this unusual tumor and to neuroblastoma of adults is reviewed.

Amnesia in a patient with bilateral lesions to the thalamus.

A case of anterograde amnesia is described in a 38-yr-old man with bilateral thalamic lesions. The patient appeared to have suffered no general intellectual loss and performed normally on standard memory tasks involving immediate recall of new material. There was, however, consistent impairment in recalling material, verbal and non-verbal, over delays as brief as a few seconds. Impairment was especially marked on tests involving free recall and partial cueing procedures; recognition memory was also impaired. Premorbid memory tested normally and susceptibility to interference was less than in other organic amnesics. Various interpretations of the patient's amnesia were considered but a deficit at the initial stages of information processing appeared to be indicated.

Multiple meningiomas of the cervical spinal cord associated with Klippel-Feil malformation and atlantooccipital assimilation.

A case of multiple meningiomas confined solely to the cervical spinal canal in association with multiple bony abnormalities of the cervical spine is presented. The relationship of this entity to neurofibromatosis, whether the central type or a form fruste, is explored. It is suggested that multiple meningiomas unassociated with other central or peripheral tumors may present a distinct clinical entity and may occur in an as yet uncharacterized familial pattern.
