Severe lithium intoxication treated by forced diuresis.

In a woman who had been in coma for 4 days because of severe lithium intoxication forced diuresis led to full recovery. Forced diuresis is simpler than hemodialysis and may be as effective in some patients with severe lithium intoxication.

Spectral irradiances of and maximal permissible exposures to two indirect ophthalmoscopes.

Recent reports in the literature have indicated that exposure to certain high intensity sources used in clinical procedures may be damaging to the retina. To investigate this problem, the spectral irradiance of two indirect ophthalmoscopes [American Optical (AO) and Keeler] was measured. The instruments do show operational differences, the most striking of which was the difference in the total irradiance produced at the maximum voltage setting, the Keeler instrument providing nearly 3.6 times greater irradiance than the AO indirect ophthalmoscope. The sources are evaluated as blue light hazards, and maximal permissible exposures (MPE) are calculated and compared to the MPE established by ANSI Z-136 guidelines.

Dietary calcium in human hypertension.

A pilot survey was made of the dietary calcium intake of normotensive and hypertensive individuals. Compared to 44 normotensive controls, 46 subjects with essential hypertension reported significantly less daily calcium ingestion (668 +/- 55 milligrams compared to 886 +/- 89 milligrams). The intake of other nutrients, including sodium and potassium, was very similar in the two groups. The hypertensives differed from the controls primarily in their consumption of nonfluid dairy products. The data suggest that inadequate calcium intake may be a previously unrecognized factor in the development of hypertension.

Eliminating a severely retarded blind adolescent's tantrums using mild behavioral interruption: a case study.

A blind 19 yr old severely retarded man was referred for behavior therapy because of violent and disruptive tantrums. Previous behavioral strategies had failed for various reasons. A very mild, brief, vestibular oriented physical procedure was designed to provide a low level disruptive effect. Intervention consisted of loud teacher demands to stop and work appropriately as well as guiding him through one 360 degrees turn while standing. The data demonstrated rapid and long term success in eliminating the client's tantrums. The results were interpreted in terms of a behavioral interruptor of a chained sequence allowing refocusing of client attention and increased levels of reinforcement.

Simulated stratospheric ozone depletion and increased ultraviolet radiation: effects on photocarcinogenesis in hairless mice.

Solar ultraviolet radiation at the surface of the earth is a recognized cause of skin cancer. Postulated anthropogenic reductions in the thickness of the ozone layer would lead to an increased amount of ultraviolet radiation and hence would be expected to increase the risk of skin carcinogenesis. This study uses hairless (Skh:HR) mice as an animal model to study this increased risk. The mice were exposed 5 days/week to graded doses of ultraviolet radiation from a xenon arc lamp attenuated by five different thicknesses of Schott glass filters (WG320) to simulate various ozone layer thicknesses. A Robertson-Berger sunburning ultraviolet meter was used as one of the forms of dosimetry. The results of the various exposure treatments are expressed as the percentage of animal with tumors (incidence) versus time after commencing irradiation and as cumulative tumor yield (average number of tumors per survivor) versus time. With any given filter, the time to 50% incidence is inversely related to daily dose in Robertson-Berger meter units. The time to 50% incidence for comparable Robertson-Berger meter doses through different filter thicknesses increases with increasing thickness. These results indicate that the effective dose for skin cancer induction may be estimated from the Robertson-Berger meter dose but that the Robertson-Berger meter response spectrum underestimates the photocarcinogenic effectiveness of the shorter wavelengths. The cumulative tumor yield data are also consistent with these conclusions. Alternate weighting of the source spectra with the acute-response action spectrum for mouse skin edema gave a better correlation between unit dose and time to a tumor response, independent of the source spectral distribution. This suggests that the mouse skin edema action spectrum, indistinguishable from a human skin erythema action spectrum for lambda greater than 295 nm, is similar in shape to the mouse skin photocarcinogenesis action spectrum for lambda greater than 295 nm.

Platelet hyporesponsiveness to epinephrine in carriers of Bartter's syndrome.

Platelet aggregation was investigated in the parents of 4 patients with Bartter's syndrome. Aggregation induced by ADP and ristocetin was normal. However, there was some depression of aggregation induced by collagen. All obligatory heterozygotes showed depressed aggregation in response to epinephrine.

Long-term clinical assessment of combined ACAC hemoperfusion-ultrafiltration in uremia.

ACAC hemoperfusion was used in series with a small fluid removal system for a clinical trial in the treatment of uremia. A 22-month trial included a pretest control period, test period and a post-test control period. The most significant observations from this trial are: 1) the predialysis body weight of the patient could be maintained closer to the patient's dry weight; 2) there was a significant increase in hematocrit; and 3) there was insufficient removal of urea. With the development of an effective urea removal system, a more compact artificial kidney than any presently available will become feasible.

Nucleotide sequence analysis of two simian virus 40 mutants with deletions in the late region of the genome.

Two mutants of simian virus 40, dl-1261 and dl-1262, have deletions that map between coordinated 0.90 and 0.95 (Cole et al., J. Virol 24:277--294, 1977). Both affect the structure of the two minor proteins VP2 and VP3. The precise location and size of the deletions have now been determined by nucleotide sequence analysis. Mutant dl-1261 is deleted of 54 base pairs, is temperature sensitive for the protein defined by the D complementation group, and promotes the synthesis of shorter VP2 and VP3 polypeptides. Mutant dl-1262 is viable irrespective of temperature and has a deletion of 36 base pairs, 23 of which overlap the deletion in dl-1261. Since these mutants produce normal VP1, the deleted regions probably have no function in the splicing of precursor RNA to the VP1 mRNA.

Nucleotide sequence analysis of two simian virus 40 mutants with deletions in the region coding for the carboxyl terminus of the T antigen.

Nucleotide sequence analysis of two simian virus 40 early mutants dl1263 and dl1265, which lack a DNA segment around map positions 0.21 and 0.18, respectively (C. Cole, T. Landers, S. Goff, S. MAnteuil-Brutlag, and P. Berg, J. Virol. 24:277--294, 1977), revealed in-phase deletions of 33 nucleotide pairs for dl1263 and 39 nucleotide paris for dl1265. The 33-base-pair deletion in dl1263 does not correspond to an apparent shortening by 6,000 daltons observed for the mutant T antigen. In dl1265 the normal termination signal as well as most of the proline-rich terminal tryptic peptide has been removed, and the carboxyl terminus of the mutant T antigen is a series of three cysteine residues.

The effect of exposure time upon perceived size.

An experiment whereby a standard filled circle is exposed for 100-1000 msec on the tachistoscope for ten subjects. Each is asked to choose a comparison circle, which appears to be the same size, out of a series of eleven filled circles of various angular subtenses larger and smaller than the standard filled circle in the tachistoscope. The results indicate that exposure time has a role in the perception of size.

Clinical evaluation of the clearance profiles of a portable, compact, dialysate-free system incorporating microencapsulated charcoal hemoperfusion for blood purification with ultrafiltration for fluid removal.

A total of 30 procedures have been carried out in two patients using a new portable, compact, dialysate-free system formed by combining 300 gms of albumin--cellulose nitrate microencapsulated activated charcoal (ACAC) in series with a small Amicon ultrafiltrator. Blood passing through the ACAC hemoperfusion system is purified of waste metabolites and toxins. Fluid removal is carried out with the hydrostatic pressure of the blood passing through the dialysate-free ultrafiltrator. Since ACAC hemoperfusion is much more efficient than hemodialysers for blood purification, the combined system with the ultrafiltrator results in a very efficient system for blood purification and fluid removal. Typical clearance data for the combined systems include: 75 ml/min for 2000--5000 MW; 112.7 ml/min for 300--1500 MW; 235 ml/min for creatinine; 240 ml/min for uric acid; 2500--2700 ml/2 hours for water removal; and 17--18 gm/2 hours for NaCl removal. Guanidines, mercaptans, and PTH are also cleared very efficiently.

The development of anti-immunoglobulin antibodies in rhesus monkeys repeatedly exposed to Schistosoma japonicum.

The development of anti-immunoglobulin (Ig) antibodies in rhesus monkeys repeatedly exposed to Schistosoma japonicum cercariae was studied. Anti-Ig developed in all 8 monkeys exposed 5 times to cercariae of the Formosan strain, while none of 4 monkeys exposed once to the Philippine strain developed such antibodies in the same period. All monkeys developing anti-Ig had specificities for IgA, 6 of 8 for IgM and IgG, and 7 of 8 for rabbit Ig. The persistence of anti-Ig was greatly extended in the monkeys exposed initially to the Formosan strain and then challenged with the Philippine strain. A single monkey exposed once to the Philippine strain developed anti-IgA and anti-rabbit Ig 85 weeks postinfection. No relationship between host reaction to trapped eggs and the development of anti-Ig was discerned. The results suggest that immunization protocols designed for humans be carefully examined for their potential immunopathological side effects.
