A cohort of 318 children born in three Sydney hospitals between April 1982 and March 1983 were recruited into a five year prospective study designed to investigate the relationship between low level lead exposures and neurobehavioural development. Blood samples were obtained at the time of birth, then at 6 month intervals to 4 years and then at 5 years; neurobehavioural and physical measures were taken at 6 months, 12 months and hence annually to 5 years. This paper presents some of the findings from the first three years of the study and addresses the issue of the relationship between fetal exposures to lead and child development to three years. Maternal and cord blood lead levels were in range 0-29 microgram/dl with the majority less that 15 micrograms/dl. The geometric means were, respectively, 9.1 micrograms/dl and 8.1 micrograms/dl. The analyses presented do not support the hypothesis of a relationship between maternal and cord blood lead levels in this range and developmental deficits in young children to the age of three years.
The authors analyze the use of mental health care resources before and after the initiation of risperidone therapy in patients of Valley Mental Health, an agency providing aggressive community-based treatment in Salt Lake County, Utah. Sixty-three patients were treated with risperidone during the period from February 1994 through June 1995. Data for at least 6 months before (pre-period) and after (post-period) risperidone therapy were available for 61 patients; the mean period for which data were obtained was 31.8 months (pre- plus post-period). Risperidone use was associated with a reduction, albeit nonsignificant, in the numbers of patients requiring acute inpatient hospital care (-29%), days hospitalized (-1 day per year), residential treatment days (-0.7 days per year), and day-treatment visits (-1.7 visits per year), together with small increases, also nonsignificant, in the need for emergency/crisis intervention (0.1 visit per year), outpatient treatment (0.1 visit per year), and case management (2.3 visits per year). The costs of institutional plus ambulatory care declined from $7949 to $6910 per patient per year (-13%); the greatest reduction was in the cost of acute inpatient care (-18%). With the addition of risperidone, the cost of psychotropic medications doubled, resulting in a statistically nonsignificant mean increase in total costs of $305 per patient per year. Among the 36 patients who received risperidone during > or = 70% of the post-period and the 17 patients who had been hospitalized during the pre-period, total costs declined -3% and -13%, respectively. The results confirm those of previous studies and demonstrate that risperidone can reduce the use of mental health services.

Multiple endocrine neoplasia type 2A (MEN2A) is an autosomal dominantly inherited cancer syndrome characterized by medullary carcinoma of the thyroid, phaeochromocytoma and hyperparathyroidism. Almost all gene carriers can be detected by screening tests before the age of 40, but the nature and location of the predisposing gene are unknown. Simpson et al. recently reported preliminary evidence for linkage between the DNA probe p9-12A on chromosome 10 and MEN2A. We now report linkage between the MEN2A locus and the interstitial retinol-binding protein gene, which is located on chromosome 10p11.2-q11.2.

Fifteen patients with acute deterioration of liver function, high serum ammonia, and an average coma level of 3.9 (of 4) were treated for 8-12 hrs daily with a system that uses the membranes of a cellulosic plate dialyzer to pump blood through a single access at 200-225 ml/min, and includes a sorbent suspension as dialysate. Neurologic status of the patients was declining before treatment, but significantly improved during each treatment and over the course of 1-12 (average, 4) treatments. Diastolic blood pressure and pulse rate normalized. For half the patients, treatment with the BioLogic-DT system served as a bridge to liver transplant or liver recovery.
The home care clients of today are requiring more intensive i.v. therapies in their home setting. The cardiac compromised patient is a prime target for this emerging trend in home infusion therapy. The end-stage cardiac patient has been traditionally treated in the hospital acute-care setting through frequent readmissions. However, there is an increasing desire by cardiac patients and their families to administer inotropic therapies in the home. Cardiac patients receiving inotropic therapy can be successfully treated in the home using specific admission criteria and monitoring guidelines.
Studies show that implants exhibiting peri-implantitis contain elevated levels of the cytokine interleukin-1 beta in the gingival crevicular fluid (GCF). This study further evaluated possible mechanisms of osseous loss in peri-implantitis by examining GCF samples for the presence of prostaglandin E2 (PGE2) and proteolytic enzymes, specifically matrix metalloproteinases (MMPs). Results indicated that levels of PGE2 in healthy sites were not significantly different from those at diseased sites. MMP species migrated at 92 kd and 66 kd. No qualitative difference in bands was seen between healthy implants and those diagnosed with early peri-implantitis. Results suggested that PGE2 and MMP levels are not useful biologic markers for distinguishing between healthy and diseased implants.
