A microchip glucose sensor.

A major problem in development of a glucose sensor for use in an implantable artificial pancreas is the lack of reproducibility in signals from sensor to sensor. Each glucose sensor fabricated with currently used methods has a unique response to varying levels of glucose concentration and thus needs to be individually calibrated before use. We have adapted microchip manufacturing techniques for the fabrication of electrochemically based glucose sensors with standardized and reproducible function. Scanning electron microscopic study of the resulting electrode surfaces shows them to be smooth and featureless at all levels of magnification. X-ray diffraction analysis of the electrodes indicates preferential exposure of the [1,1,1] crystal interface. Cyclic voltammetry evaluation of initial sensor response to varying glucose concentrations shows excellent sensor to sensor reproducibility for all sensors made with the same underlayment. Sensors made with titanium underlayment appear to be more differentiated and thus more sensitive to variations in glucose concentration than are sensors with chromium underlayment. Although the initial response of microchip glucose sensors appears to be standardized and reproducible, additional development of an appropriate electrical insulation material is required before long-term study of signal stability is feasible.

Potentiometric measurement of glucose concentration with an immobilized glucose oxidase/catalase electrode.

A series of enzyme electrodes for measurement of glucose have been constructed. The electrodes contain glucose oxidase immobilized on platinum, either with or without co-immobilization of catalase. When placed in buffered glucose, the enzyme electrodes show a potentiometric response to glucose with respect to a Ag/AgCl reference electrode. This response is reproducible in the physiologic range of glucose concentrations. The immobilization technique, some of the environmental variables such as oxygen concentration and pH, and several compounds that might interfere with the selectivity of the enzyme electrodes for glucose have received preliminary study. This direct potentiometric approach is undergoing further evaluation to determine the basic electrochemical mechanism responsible for the potentiometric signal and whether it can be adapted for continuous in vivo monitoring of the glucose concentration in body fluids.

Fabrication of miniature PO2 and pH sensors using microelectronic techniques.

Miniature PO2 and pH sensors can be used in combination with enzymatic catalysts such as glucose oxidase and glucose dehydrogenase as the basis for the development of a small glucose sensor. The applications of microelectronic techniques such as photolithography techniques and thick- and thin-film metallization are novel approaches in fabricating highly uniform and reproducible sensors that are relatively simple to calibrate and operate.

Dengue virus infects human endothelial cells and induces IL-6 and IL-8 production.

In this study dengue virus (DV) was found to infect primary endothelial cells derived from human umbilical cord veins (HUVEC) and alter their cytokine production. Dengue virus infection of HUVEC was confirmed by an increase in plaque-forming units in the culture supernatant and by immunofluorescence assay. HUVEC produced large amounts of interleukin (IL)-6 and IL-8 but not IL-1beta after DV infection. Both the replication of DV and the production of IL-6 and IL-8 by HUVEC after DV infection were inhibited by ribavirin, an antiviral synthetic guanosine analogue. Additionally, increased serum levels of IL-6 and IL-8 were observed in patients with dengue hemorrhagic fever but not dengue fever. Therefore, our results suggest that endothelial cells can be a target for DV infection, and that DV-induced IL-6 and IL-8 production by endothelial cells may contribute to the pathogenesis of dengue hemorrhagic fever.

Bonded amalgam restorations: using a glass-ionomer as an adhesive liner.

Due to the lack of adhesiveness of amalgam to tooth structure, several adhesive cements have been utilized in bonded amalgam restorations. This study evaluated whether Fuji-II glass-ionomer cement is an appropriate adhesive liner in bonded amalgam restorations. Two adhesive composite luting cements (Amalgambond Plus and Panavia-21) and Copalite cavity liner were compared. The study was conducted in two phases. In the first part, we quantitatively assessed the tensile bond strengths as well as the failure modes of amalgam bonded to human dentin, using different adhesive liners. In each group, the flat dentin surface was treated with the assigned adhesive cement with a Teflon mold, followed by condensation of amalgam (Valiant PhD) onto it. Each group's mean tensile bond strengths were recorded and the statistical analysis by one way ANOVA showed no significant differences among groups (p > 0.05). Similar to the fracture patterns of the Amalgambond Plus and Panavia-21 groups, the failure mode of Fuji-II group was predominantly adhesive fracture. In the second part, the fracture strengths of amalgam restored teeth were measured using different adhesive liners. Standard MOD cavities were prepared in each tooth except for the intact tooth group. After treatment with the assigned adhesives or varnish, the cavities were restored with amalgam. Fracture strengths were then measured and the fractured interfaces examined using a scanning electron microscope. The fracture strengths of the intact tooth, Amalgambond Plus, Panavia-21 and Fuji-II groups were significantly higher than those of the Copalite and prepared cavity without restoration groups (p < 0.01). Accordingly, Fuji-II glass-ionomer cement, when used as an adhesive liner of amalgam restoration, may effectively reinforce the remaining tooth structure and, therefore, enhance the fracture resistance of the amalgam-restored teeth.

Proliferation characteristics of canine transmissible venereal tumor.

Canine transmissible venereal tumor (CTVT) grows progressively (P-phase) in the host and then spontaneously regresses (R-phase). The mechanisms behind the transition from the P-to R-phases are not well understood. In this study, in order to determine the proliferation characteristics of CTVT, we evaluated telomerase activity and enumerated nuclear organizing regions (AgNOR) and proliferating cell nuclear antigen (PCNA). It was found that CTVT cells from the P-and R-phases were both positive for telomerase activity, although it was lower in the R-phase. Evaluations of telomerase activity should take into account the stage of mitosis. Although, in the majority of cases, telomerase activity can be used to differentiate between benign and malignant tumors in dogs, other factors or markers should also be used to obtain accurate diagnoses. The PCNA-positive rate and the number and area of AgNOR per cell increased much more in the P-phase than the R-phase. However, the AgNOR values were always higher. Thus, the AgNOR count can be used to distinguish the P-and R-phases of CTVT. In addition, mitotic figures were much higher in number in the P-phase as compared to the R-phase. We believe that, during spontaneous regression of CTVT cells, slow tumor cell proliferation must contribute to the decrease in tumor size. However, shortening of tumor cell telomeres is not directly involved in this process. Other factors, such as expression of MHC antigens on CTVT cells, humoral immunity, cytokines released by the inflammatory cells and, especially, tumor infiltrating lymphocytes may contribute to CTVT regression.

Comparison of commercial elastomeric impression materials.

The properties of some recently marketed rubber impression materials were compared with some earlier products. In general, the qualities of addition silicones and polyethers were superior to polysulfides and condensation silicones. However, there were significant differences in the properties of products of the same type and class and thus the selection of a product for a particular application should be based on actual property data rather than on the type and class of rubber impression material.

A clinical evaluation of sulbactam/ampicillin in the treatment of pediatric infections.

We have treated 42 episodes of pediatric infections with sulbactam/ampicillin since 1987. Included were 9 cellulitis, 9 urinary tract infections, 5 cervical lymphadenitis, 4 meningitis, 2 thoracic empyema, 2 osteomyelitis, 2 sepsis, 1 furuncle, 1 perianal abscess, 1 dental abscess, 1 peritonsillitis, 1 salmonellosis, 1 shigellosis, 1 peritonitis, 1 suppurative thyroiditis, 1 infective endocarditis. Responsible pathogens were Escherichia coli in 8, Staphylococcus aureus in 6, Hemophilus influenzae in 2, Streptococcus pneumoniae in 3, Streptococcus viridans in 2, Staphylococcus epidermidis in 1, Bacteroides fragilis in 1, Salmonella D1 in 1, Shigella sonnei in 1, Klebsiella pneumoniae in 1, Enterobacter agglomerans in 1, Acinetobacter calcoaceticus in 1, Enterobacter cloacae in 1, group A beta-hemolytic streptococcus in 1, and polymicrobial infection in 4 cases. Thirty-nine out of 41 (95%) clinically evaluable patients cured and all (34/34) bacteriologically evaluable patients eradicated their pathogens after treatment with sulbactam/ampicillin. Side reactions were seen in five patients; one maculopapular skin rash, one hemolytic anemia, two diarrhea, and one liver function impairment plus leukopenia. All these reactions were transient and did not require interruption of therapy. These results indicate that sulbactam/ampicillin is safe and effective in the treatment of common pediatric infections beyond the neonatal period.

Seroepidemiology of hepatitis A virus infection in southern Taiwan.

Hepatitis A virus infection was common in Taiwan. Most of the Taiwan data on the prevalence of Hepatitis A virus antibody (anti-HAV), however have derived from the Taipei area and few reports are available on the age-specific prevalence in other parts of Taiwan. Sera of 738 healthy inhabitants in Tainan District, a rural southern Taiwan, were collected from January to December, 1992, and screened the anti-HAV IgG by radioimmunoassay. The prevalence rate of anti-HAV among infants was 64.6%, which decreased to 3.3% in preschool and school-age children up to 12 years old. The prevalence rate of anti-HAV antibodies was over 92% in adult subjects beyond 30 years of age, and 82.4% among 34 pregnant women. Compared to the data obtained from the children living in Taipei City, our results showed that the prevalence rate of anti-HAV in age group 1 to 10 years in Tainan area was higher than Taipei City (2.3-3.7% vs. 0-0.5%, 0.03 < P < 0.09). The data also indicated that the age of primary infection in children has extended to an older age group. The rarity of HAV infection in Taiwan has created a large growing population of susceptible young adults. Mass vaccination for this population should be considered in the preventive program of public health.

Combined spinal and epidural anesthesia for abdominal hysterectomy in a patient with myotonic dystrophy. Case report.

BACKGROUND AND OBJECTIVES: The authors report a case of myotonic dystrophy in a 34-year-old woman who presented for total abdominal hysterectomy. The goal of anesthetic management is to prevent the known triggers of myotonic crisis, such as hypothermia, shivering, and hyperkalemia; and to avoid depolarizing muscle relaxants and anticholinesterase agents. METHODS: In this patient, the authors used combined spinal and epidural block for intraoperative anesthesia and postoperative analgesia. RESULTS: The advantages of the combined technique offers rapid onset and good muscle relaxation from subarachnoid block, with the ability to supplement analgesia through the epidural catheter both during and after surgery. In addition, the potential complications associated with general anesthesia, including respiratory insufficiency, aspiration pneumonia, cardiac arrhythmia, and heart failure can be avoided. The other measures were directed toward the prevention of shivering, a common problem encountered with general or regional anesthesia. CONCLUSIONS: After the postoperation, optimal analgesia was obtained by infusing local anesthetic (0.125% bupivacaine) via the epidural catheter. No obvious side effects occurred. The authors believe combined spinal and epidural block provides a safe alternative, to other techniques, and minimizes the potential hazards of myotonic dystrophy, while offering effective intraoperative anesthesia and postoperative analgesia.

Investigation and elimination of epidemic methicillin-resistant Staphylococcus aureus in a neonatal intensive care unit.

A recent outbreak of methicillin-resistant Staphylococcus aureus (MRSA) infection in neonatal intensive care unit (NICU) in a teaching hospital was investigated. A total of 25 MRSA isolates, 16 from the patients and nine from the staffs (carriers), was collected for the study. The possible relationship among the isolates was investigated by using antibiograms and restriction enzyme analysis of plasmid DNA. Control strategy of the MRSA nosocomial infection was proposed after the study of antibiograms and plasmid profile. There were four plasmid patterns (I-IV), each containing 7, 8, 2, and 8 isolates. Six of the 16 isolates from the patients had identical plasmid patterns as the carriers (I and II). Person to person transfer via hand contact by medical personnel was found to be the most frequent mode of transmission identified in the outbreak of MRSA nosocomial infection in the NICU. Our strategy for control of the outbreak and elimination of the MRSA from all patients and carriers was successful after intensive surveillance and control measures. These included (a) strict isolation and cohorting; (b) hand washing between patient contacts to prevent transmission; (c) treatment of the carrier state in health care workers and patients with safe and effective topical agents such as mupirocin.

Systemic fungal infection in very low-birth-weight infants.

This retrospective study was designed to investigate the related factors and outcome of systemic fungal infection in very low-birth-weight (VLBW) infants. Medical records of infants admitted to the neonatal intensive care unit of National Cheng Kung University Hospital between January 1990 and June 1994 were reviewed. Of the 262 VLBW infants, 15 (5.7%) had fungemia (14 Candida 1 Cryptococcus) during the study period. Among the fungemic infants, 60% also had urinary tract infection; 18% had central nervous system infection. Their mean birth weight was 1079 +/- 78 g (504-1474 g), and the gestational age was 28.6 +/- 0.6 weeks (23-32 weeks). Thirteen of them (87%) had respiratory distress syndrome and patent ductus arteriosus, while 60% had chronic lung disease. The percentage of antibiotic usage, parenteral hyperalimentation, endotracheal intubation, placement of central venous line and steroid therapy were 100%, 100%, 73%, 67% and 36% respectively. The mean age at diagnosis of fungemia was 40.5 +/- 4.8 days (10-76 days). Common clinical manifestations were respiratory deterioration (93%), poor feeding (58%) and fever (53%). The frequency of side effects of amphotericin B in decreasing order were: hypokalemia (54%), hyponatremia (31%) and decreased urine amount (23%). The mortality rate was 40%. It was concluded that systemic fungal infection in VLBW infants might result in high mortality and the side effects were high in the treated infants.

Comparison of the genetic knowledge among nurses, students, and general public after ten years of implementation of genetic health program in Taiwan.

This study aims to explore the genetic knowledge of different health professional, and non-professional populations after ten years of implementation of genetic health program in Taiwan. A self-administered questionnaire was developed to test the genetic knowledge of 885 respondent who included 361 community health nurses, 74 maternal-child nurses, 364 college students, and 86 members of the general public. The questionnaire included 33 questions which were divided into 4 categories (1) basic genetic knowledge, (2) Mendel's Law and probability, (3) prenatal diagnosis, (4) identification of common genetic disorders. The responses were scored and analyzed statistically with ANOVA tests. Community health nurses had the highest scores of genetic knowledge. However, they need more practice in the application of Mendel's Law and probability. Maternal-child nurses should be involved in genetic education programs and improve their ability to identify common genetic disorders and thus increase their ability to identify cases and provide better information to patients and families. Compared to other groups, college students had lower scores in prenatal diagnosis and identification of common genetic disorders. Therefore, college curricula in genetics need to emphasize more on these subjects. From the study, it was concluded that community health nurses are effective and competent to continue the genetic education as supported by the government in the past ten years. Maternal-child nurses are inevitably involved in genetic service, and they should be provided with suitable continuing education programs. The general public should strengthen their knowledge of prenatal diagnosis and common genetic disorders. Prenatal diagnosis should be added in the school curriculum of college students to expand their knowledge. New genetic technology, such as DNA analysis, should be added in the content of genetic education programs.

Myocarditis with complete atrioventricular block associated with herpes simplex virus infection: report of one case.

Myocarditis with complete atrioventricular block is a very unusual complication of the herpex simplex infection. We report a 10-year-old boy infected very likely by the herpes simplex virus and who presented with high fever, erythema multiforme, complete atrioventricular block, and Adams-Stokes seizures. Emergent temporary pacemaker was performed for bradycardia. A sixteen-fold rise in herpes simplex antibody titer by a complement fixation method occurred within two weeks. Normal cardiac rhythm recovered 11 days later with a sequela of complete right bundle branch block after 2 years follow-up.
