[Formation of autologous spongiosa in bone layers as a function of revascularization].

In 24 sheep standardized holes were drilled into the tibia. These defects were filled differently with autologous cancellous bone: a) The defect was filled only with cancellous bone or b) it was filled like in (a) but additionally covered by a Milliporefilter layer; c) the defect was filled like in (a) but the bottom of the defect was covered by a Milliporefilter layer or d) it was filled like in (a) but the cancellous bone was sandwiched by Milliporefilter layers. Bine remodelling was adversely influenced by the Milliporefilters, but the inhibition in (b) was two times bigger than in (c).

[Biomechanical studies on the stabilization effect of cortico-spongiosa chips in imperfect osteosynthesis].

Stability and plate strain of internally fixed femoral shaft fractures at different sites were tested under bending and compression loads. Stability of osteosynthesis with compression plate was not acutely affected by stresses encountered when standing on one leg. When fractures with a bone defect were treated by plate fixation stability was improved significantly when lateral support was preserved or when a medial cortico-spongious bone chip was used.

[Knee-ligaments--operation and functional postoperative care (author's transl)].

After surgical repair of ruptured knee-ligaments the patients have a functional treatment with the help of a limited-mobilization cast (LMC). Flexion from 20 to 60 degrees exerts no tension on the knee-ligaments. Damage to the cartilage by immobilisation can be prevented, the frequency of post-traumatic arthrosis reduced.

[The change of physical properties of plastics (polyoxymethylenecopolymer, polyethyleneterephthalate, polyethylene, polytetrafluorethylene) after animal implantation and autoclavation (author's transl)].

The change of physical properties of plastics (polyoxymethylene-copolymer, polyethyleneterephthalate, polyethylene, polytetrafluorethylene) and the bio-compatibility of these materials were examined by implantation in the backmuscle of 6-month-old male rabbits for 4 and 12 weeks and after autoclavation. We have found out, that after implantation and autoclavation polyethylene-terephthalate demonstrates a strong diminution of the visco-elastic qualities. Polyethylene and polytetrafluorethylene were not changed by these treatments. After an implantation of 12 weeks and an autoclavation of four times the visco-elastic properties of polyoxymethylene-copolymer were only slightly diminished by 10 to 15%. The histological investigation of the surrounding tissue demonstrated a very good bio-compatibility of polyethylene. After an implantation of 4 weeks polyoxymethylene-copolymer, polyethyleneterephthalate and polytetrafluorethylene produced a comparable foreign body reaction, which, however, was evidently diminished after an implantation of 12 weeks.

[Compound fractures in childhood (author's transl)].

The incidence of compound fractures increases with the age of children. The importance of emergency care at the site of the accident, with sterile coverage of wounds and preliminary fixation, and of fast and careful transportation to hospital, is gaining recognition. Final management should comprise meticulous mechanical cleansing under sterile conditions, treatment of any additional injury, and stabilization. Compound fractures call for more generous application of internal fixation. Tetanus prophylaxis and routine monitoring of wounds are essential when there is any danger of gas gangrene.

[Malignant degeneration of fistulas in chronic osteomyelitis (author's transl)].

The malignant degeneration of a skin ulcer has been reported as early as in 1884. In spite of this, however, the danger of malignant degeneration of sinus tracts associated with recurrences of chronic osteomyelitis is still underestimated. This development is neither restricted to case histories of years and decenniums nor to elderly patients and may also involve any part of the skeleton. Usually patients become aware of suddenly increasing tenderness and purulation, "washed out" structures of osteolytic areas are demonstrable radiographically. Diagnosis is established by mandatory histological examination of sinus tracts. Amputation or exarticulation of the extremity involved including efferent lymphatic vessels and nodi has no alternative. Local measures are insufficient. Apparently the first 3 years are decisive as to the prognosis. The prophylaxis consists of the therapy of the original disease: radical debridement of the osteomyelitic focus and filling of the defect with autogenous cancellous bone grafts.

[Bio-compatibility of different plastics (polyoxymethylene-copolymer, polyethylensterephthalate, polyethylene, polytetrafluorethylene) (author's transl)].

The bio-compatibility of different plastic materials were examined after their implantation in the back-muscle of 6-months-old male rabbits. Twelve weeks after the implantation chemical measurements (DNS-concentration of the tissue) and histological investigations of the surrounding tissue demonstrated for polyethylene the best bio-compatibility.

[Installation and reconstruction of autologous spongiosa and compacta after substitution with ground bone].

Defined tissue defects were made in the tibiae of 24 sheep. In each animal three defects were set; one of these was completely filled with cancellous bone and the other one with ground compact bone, whereas the third one was not grafted and served as control. During the first 3 weeks no significant difference was seen in the formation of new bone at any of the defects. Most of the ground grafts were transformed into new bone between the 4th and the 6th week; in contrast most new bone was made both in the control defect and the cancellous graft between the 7th and 12th week. In the early phase of our experiment the graft made of ground compact bone was superior to the graft formed from cancellous bone with regard to the amount of new bone formed as well as to its structure. Under clinical conditions, ground compact bone could be useful for autologous bone grafting.
