[Bronchial diffusion of piperacillin in 18 intensive care patients].

Eighteen patients (13 under mechanical ventilation) in an intensive care unit received piperacillin for pneumonia (7 cases) or bronchial infection (11 cases), related to Haemophilus influenzae (7), Streptococcus pneumoniae (4) or miscellaneous pathogens (7) including Staphylococcus, E. coli, Klebsiella pneumoniae and Proteus mirabilis. Each patient was given 2g piperacillin intravenously every six hours. Concentrations in serum and bronchial secretion samples were assayed by the agar diffusion technique using a susceptible strain of Bacillus subtilis ATCC 6633. Maximum concentrations were 157 micrograms/ml in serum and 3.60 micrograms/ml in bronchial secretions. These results are similar to those obtained with the same dosage by 0. Cars in serum (150 micrograms/ml) and by G.E. Marlin in the bronchial secretions (3.78 micrograms/ml). They approximate those published for other penicillins with the same dosage.

[Efficacy and tolerability of intravenous ofloxacin. A French multicenter study of 273 patients].

The effectiveness and safety of intravenous ofloxacin were studied in an open, multicentre trial conducted in 31 French hospitals. Among the 273 patients who entered the trial, 62 had septicaemia, 81 an urinary tract infection, 46 a lower respiratory infection and 11 an osteoarticular infection. Admission to an intensive care unit had been necessary for 114 patients. The infection was nosocomial in 76 cases. After treatment with intravenous ofloxacin administered alone (n = 199) or combined with another antibiotic (n = 74), 231 patients (84.6 per cent) were cured. The drug was well tolerated both clinically and biologically.

[False-membrane pharyngitis and vélopalatine paralysis: false diphteria, true infectious mononucleosis (author's transl)].

Paralysis of the soft-palate occurring at the end of, or in conjunction with false-membrane pharyngitis with lymphadenopathies should prompt to think of diphteria. The reported case is concerning an 18 year-old girl in whom the affection of the last cranial nerves (IX-XI and XII) is falling within the rare but deceptive neurological forms of infectious mononucleosis. The evolution was favorable, as is normally the case when there are no respiratory complications.

[Urinary infections in pregnant women].

Urinary tract infection is frequent during pregnancy with a high potential risk for mother and child. Based on a review of the literature and a retrospective survey conducted in 20 representative French university hospitals during 1993, the authors propose a practical review designed to standardize the therapeutic approach to this disease. They define a high-risk group which requires systematic screening and close surveillance during pregnancy. They evaluate the need for complementary investigations in relation to the 3 clinical presentations encountered (asymptomatic bacteriuria, cystitis and acute pyelonephritis) taking into account their respective adverse effects. The therapeutic modalities of the three clinical forms are then described, including drainage of the urinary tract.
