Expression and functional characterization of a soluble form of vascular cell adhesion molecule 1.

Vascular cell adhesion molecule 1 (VCAM1) is a leukocyte adhesion molecule induced on human endothelium in vitro and in vivo by inflammatory stimuli. A truncated cDNA for VCAM1 was constructed, stably expressed in Chinese Hamster Ovary (CHO) cells, and the secreted recombinant soluble form of VCAM1 (rsVCAM1) purified to homogeneity by immunoaffinity chromatography. Immobilized rsVCAM1 is a functional adhesion protein, and selectively binds only VLA4-expressing cells, including human B and T lymphocytes, NK cells, and certain lymphoblastoid cell lines. T cell subset analyses indicate preferential binding of CD8+ memory cells. rsVCAM1 should prove valuable for the further study of the role of VCAM1 during inflammatory and immune responses in vivo.

Human monocytes bind to two cytokine-induced adhesive ligands on cultured human endothelial cells: endothelial-leukocyte adhesion molecule-1 and vascular cell adhesion molecule-1.

Vascular cell adhesion molecule-1 (VCAM-1) and endothelial-leukocyte adhesion molecule-1 (ELAM-1) are adhesive proteins induced on endothelium by cytokines. We examined the contribution of these adhesive proteins to human peripheral blood monocyte adherence to endothelium using transfected Chinese hamster ovary (CHO) cells stably expressing these proteins and monoclonal antibodies (MoAbs) to ELAM-1, VCAM-1, or CD49d/CD29 (VLA-4), the leukocyte receptor for VCAM-1. Monocytes bound to CHO cells transfected with cDNA of ELAM-1 or VCAM-1. Binding to ELAM-1 was inhibited by MoAb to ELAM-1 and binding to VCAM-1 was inhibited by MoAb to VCAM-1 or the alpha-chain of very late activation antigen-4 (VLA-4) (CD49d). Additive inhibition of adherence to unstimulated human umbilical vein endothelium (HUVE) was observed when monocytes were pretreated with both MoAb to CD49d and MoAb to CD18, the common beta-chain of the leukocyte beta 2 integrin receptors. Adherence of monocytes to HUVE stimulated by recombinant human tumor necrosis factor-alpha was not reduced by MoAbs to CD18, CD49d, or ELAM-1 when used singly, but combinations of these MoAbs produced significant inhibition. We conclude that multiple receptor-ligand systems are involved in monocyte adherence to endothelium.

Von Hippel-Lindau disease: a genetic study.

Genetic aspects of von Hippel-Lindau (VHL) disease were studied in familial and isolated cases. Complex segregation analysis with pointers was performed in 38 kindreds with two or more affected members. Dominant inheritance with almost complete penetrance in the highest age classes (0.96 at 51 to 60 and 0.99 at 61 to 70 years) was confirmed and there was no evidence of heterogeneity between families ascertained through complete and incomplete selection. The point prevalence of heterozygotes in East Anglia was 1.89/100,000 (1/53,000) persons with an estimated birth incidence of 2.73/100,000 (1/36,000) live births. Reproductive fitness was 0.83. Direct and indirect estimates of the mutation rate were 4.4 (95% CI 0.9 to 7.9) x 10(-6)/gene/generation and 2.32 x 10(-6)/gene/generation respectively. There was no significant association between parental age or birth order and new mutations for VHL disease.

Review of auditory subliminal psychodynamic activation experiments.

Subliminal psychodynamic activation experiments using auditory stimuli have yielded only a modicum of support for the contention that such activation produces predictable behavioral changes. Problems in many auditory subliminal psychodynamic activation experiments indicate that those predictions have not been tested adequately. The auditory mode of presentation, however, has several methodological advantages over the visual one, the method used in the vast majority of subliminal psychodynamic activation experiments. Consequently, it should be considered in subsequent research in this area.

A blocking monoclonal antibody to endothelial-leukocyte adhesion molecule-1 (ELAM1).

ELAM1 is a leukocyte adhesion molecule induced on human umbilical vein endothelial cells (HUVECs) by inflammatory cytokines. Balb/C mice were immunized with COS cells transiently expressing cell-surface ELAM1 after transfection with ELAM1 cDNA. After fusion, ELAM1-specific monoclonal antibodies (Mabs) were identified by selective adhesion to ELAM1-expressing, but not control, CHO cells, and to cytokine-treated but not untreated HUVECs. One Mab, designated BB11, binds to and immunoprecipitates ELAM1 expressed on HUVECs, COS and CHO cells. BB11 blocks the interaction of ELAM1 with human PMN, the human myelomonocytic cell line HL60, and the human colon carcinoma line HT29.

Existence of irritable bowel syndrome supported by factor analysis of symptoms in two community samples.

To determine whether bowel symptoms covary in a pattern consistent with the existence of irritable bowel as a distinct syndrome, bowel symptom questionnaires from 2 independent samples were factor analyzed. Samples consisted of 351 18-40-yr-old women who visited Planned Parenthood clinics for contraception and 149 18-89-yr-old women recruited through church women's societies. Factor analysis of 23 bowel symptoms identified 4 factors (clusters of symptoms that were correlated with each other) in both samples. The factor accounting for the most variance in both samples included relief of pain with defecation, looser stools with pain onset, more frequent stools with pain, and gastrointestinal reactions to eating. This irritable bowel factor was not correlated with an objective measure of lactose intolerance. An independent constipation factor was found in both samples to include self-reported constipation, straining with bowel movements, feeling of incomplete evacuation, and rectal bleeding. Thus factor analysis of bowel symptoms supports the existence of a specific irritable bowel syndrome and suggests symptoms that may be used to diagnose this syndrome.

Evidence for exacerbation of irritable bowel syndrome during menses.

Many women report that bowel symptoms are associated with menstruation, but neither the prevalence of these complaints nor their physiological basis is known. This study aimed to estimate prevalence, to determine whether patients with irritable bowel syndrome are more likely to make such complaints, and to determine whether bowel complaints during menstruation are attributable to psychological traits such as increased somatization. To estimate prevalence, 369 clients of Planned Parenthood of Maryland were asked whether gas, diarrhea, or constipation occurred during menstruation. These subjects were compared with women referred to a gastroenterology clinic and found to have irritable bowel syndrome or functional bowel disorder (abdominal pain plus altered bowel habits but not satisfying restrictive criteria for irritable bowel syndrome). Thirty-four percent of 233 Planned Parenthood clients who denied symptoms of irritable bowel syndrome or functional bowel disorder reported that menstruation was associated with one or more bowel symptoms. Gastroenterology clinic patients with irritable bowel syndrome were significantly more likely to experience exacerbations of each of these bowel symptoms, but especially increased bowel gas. Self-reports of bowel symptoms during menstruation were not associated with psychological traits or with menses-related changes in affect.

Mapping of von Hippel-Lindau disease to chromosome 3p confirmed by genetic linkage analysis.

Genetic linkage studies were performed in 12 British families with von Hippel-Lindau disease (VHL) using RFLPs at three loci (DNF15S2, THRB, RAF1) on the short arm of chromosome 3. Linkage was detected between the VHL disease locus and RAF1 with a maximum lod score of 3.88 at a recombination fraction of 0.05 (confidence interval 0.003-0.18). Multipoint linkage analysis suggested that the most likely location for the VHL disease locus is telomeric to THRB. These results confirm earlier reports localizing the VHL gene to the short arm of chromosome 3, and provide no evidence for genetic heterogeneity.

Clinical features and natural history of von Hippel-Lindau disease.

The clinical features, age at onset and survival of 152 patients with von Hippel-Lindau disease were studied. Mean age at onset was 26.3 years and 97 per cent of patients had presented by aged 60 years. Retinal angioma was the first manifestation in 65 patients (43 per cent), followed by cerebellar haemangioblastoma (n = 60, 39 per cent) and renal cell carcinoma (n = 15, 10 per cent). Overall, 89 patients (59 per cent) developed a cerebellar haemangioblastoma, 89 (59 per cent) a retinal angioma, 43 (28 per cent) renal cell carcinoma, 20 (13 per cent) spinal haemangioblastoma and 11 (7 per cent) a phaeochromocytoma. Renal, pancreatic and epididymal cysts were frequent findings but their exact incidence was not accurately assessed. Mean age at diagnosis of renal cell carcinoma (44.0 +/- 10.9 years) was significantly older than that for cerebellar haemangioblastoma (29.0 +/- 10.0 years) and retinal angioma (25.4 +/- 12.7 years). The probability of a patient with von Hippel-Lindan disease developing a cerebellar haemangioblastoma, retinal angioma or renal cell carcinoma by age 60 years was 0.84, 0.7 and 0.69, respectively. A comprehensive screening protocol for affected patients and at-risk relatives is presented, based on detailed analysis of age at onset data for each of the major complications. Median actuarial survival was 49 years, with renal cell carcinoma the leading cause of death.

Phobic anxiety and clumsiness in a 10-year-old girl.

During assessment of a girl with a long-standing phobic state, it was found that she also had previously unrecognised perceptuo-motor problems, which were associated with continuing problems in balance and spatial awareness. Her phobia improved after a desensitisation programme, but the physical disorder persisted, despite physiotherapy. The authors conclude that the girl's phobic anxiety may have had its origin in her experience of the perceptual disorder during development.

The use of health care resources by young adults with spina bifida.

Residents of Sheffield, U.K., aged 16-21 years with myelomeningocele were interviewed to ascertain morbidity in the preceding 12 mths., current and past use of hospital and community health care resources, and dependence on carers. Twenty-one patients, representing 49% of those known to paediatric services, responded. In the previous year, 71% of the group had been ill due to their spina bifida. 81% of subjects received hospital supervision, mostly by a urologist. Many hospital contacts had been lost, mostly by age 18 years. Little use was made of community health services or general practitioners. The majority of patients were dependent on their parents for management of incontinence, and care of skin vulnerable to pressure sores. 52% of respondents were dissatisfied with current provision of health care. Particular need was expressed for community nurses and physiotherapists.

Impact of the medical record credential on data quality.

This research study was funded by the Foundation of Record Education of the American Medical Record Association. The Department of Medical Record Administration at the University of Illinois, Chicago, was awarded the grant in the summer of 1984. The purpose of the study was to evaluate the quality of coded and abstracted medical record data and to determine if a relationship existed between data quality and the professional credentials of the individuals who manage and/or supervise the collection of these data. The study consisted of three phases: a telephone interview conducted of 83 hospitals, recoding and reabstracting of medical records performed by the staffs of 59 hospitals, and 34 random on-site visits for a reliability check. Data was collected and analyzed from each of the three phases of the study.

Xenogeneic cytotoxic T-cell clones recognize alloantigenic determinants on HLA-A2.

Long-term murine cytotoxic T-cell clones arising in response to stimulation with human lymphoblastoid cells and reactive with the HLA-A2 antigen are characterized. These clones distinguish between HLA-A2 and 21 other serologically defined HLA-A and -B antigens. In addition, most clones discriminate between prototypical HLA-A2 antigens, expressed by the majority of HLA-A2-positive individuals, and variant HLA-A2 antigens, which are serologically identical with the prototype, but distinguishable by human cytotoxic T cells and by biochemical analysis. This discrimination is reflected as an inability to cause any significant lysis of variant HLA-A2-expressing target cells at effector-to-target ratios 10- to 100-fold greater than those giving 50% lysis of prototype HLA-A2-expressing cells. By screening a panel of serologically HLA-A2-positive cells, a new variant HLA-A2-expressing cell line has been defined. The recognition patterns of these xenogeneic clones are suggested to reflect recognition of alloantigenic polymorphic determinants. Based on the strong bias in the xenogeneic T-cell repertoire for such determinants, we propose a model for T-cell recognition of class I products of the major histocompatibility complex.

[An overview of internal migration in the United States].

"This article briefly looks at internal migration in the United States, with [particular] reference to the reversal of the tendency that was observed in the early seventies, when the South became a region that clearly gained from migratory exchanges with the other regions. The phenomenal growth of the Sun Belt during the seventies constitutes, without doubt, one of the outstanding demographic features of this decade, in the United States. With data from the 1980 Census, this article examines the migratory situation at the state level, during the 1975-1980 period." Consideration is given to the likelihood of the continuation of the migration balance in favor of the Sun Belt in the future. (summary in ENG, SPA)

Expression of nucleolus organizer regions (NORs) in human acrocentric chromosomes by NSG, QFQ and RFA techniques: are they identical?

The morphology of nucleolus organizer regions (NORs) of human acrocentric chromosomes has been studied in a family by QFQ, RFA, and NSG banding techniques. The pale green colour seen by RFA was found darkly stained by NSG. However, in some instances, NOR regions were expressed by NSG technique but the pale green colour was not seen. Therefore, it is concluded that there is no direct relationship between a heteromorphism identified by one technique and that identified by another.
