In order to determine the diagnostic usefulness of the dosage of carbohydrate antigen (CA 19-9) serum levels in pancreatic diseases, 38 patients with pancreatic lesions proved at laparotomy were studied. Pancreatic lesions were classified into two groups: a) group I: pancreatic adenocarcinoma (17 cases) b) group II: benign pancreatic diseases (21 cases). CA 19-9 serum levels were statistically higher in patients of the first group (p less than 0.001). All patients except one with CA 19-9 higher than 60 U/ml had pancreatic carcinoma. All patients except two with CA 19-9 less than 60 U/ml had a benign lesion. The sensitivity and specificity of CA 19-9 were 88 and 95 p. 100 respectively. These results show that CA 19-9 dosage is useful in distinguishing between benign and malign lesions of the pancreas.
Every ninth patient with a previous history of vertigo at the ENT Hospital of the University of Freiburg suffers from BPPV. The incidence, some important pathogenetic factors and the clinical picture of BPPV are discussed. In young men posttraumatic aetiology dominates, whereas idiopathic BPPV prevails in perimenopausal women. The affected ear tends to be hard of hearing in the high frequencies and is sometimes hypoexcitable by caloric stimulation. The diagnosis is best made by personal examination with Frenzel's glasses in darkness. Electronystagmography, brainstem evoked response audiometry, and CT scanning are of no particular value in confirming the diagnosis of BPPV.
1. Immune RNA (iRNA) was extracted from the spleen of a sheep immunized with human immunodeficiency virus (HIV) antigens. 2. The transfer of cell-mediated immunity to HIV antigens was accomplished by injecting iRNA into a Cebus monkey, as evaluated in vitro by leukocyte migration inhibition. The in vitro treatment of normal human leukocytes with iRNA also promoted the inhibition of leukocyte migration in the presence of HIV antigens. 3. These findings have important theoretical and potential practical applications in the field of Acquired Immunodeficiency Syndrome (AIDS).
Three hundred fifty-nine patients with herniated nucleus pulposus syndrome were treated by chemonucleolysis with chymopapain. Good to excellent results were achieved in 71% with a minimum of six months follow-up observation. In a group followed for an average of ten years, the result was favorable in 86.6%; 58% of all patients injected had a good to excellent long-term result. Disc space narrowing averaged 2 mm, with no adverse effect on the result. Firm conclusions about the cause of failure of chemonucleolysis could not be drawn. Injection at two levels was recommended when a single offending level could not be identified. There were no cases of anaphylaxis with a premedication regimen of dexamethasone, diphenhydramine, and cimetidine. The procedure was safe and the results were reproducible.
72 patients with supraglottic carcinomas were stratified into two groups with similar cancerological data. 36 underwent horizontal and 36 total laryngectomy. Both groups were radiated postoperatively. In N0, elective neck dissection was omitted. Superior survival was achieved in patients after total laryngectomy. Recurrence rate in the neck for both groups was 6%. There was considerable morbidity after horizontal laryngectomy with aspiration being the main factor. 30.6% of the patients were unable to swallow at all and 30% had a permanent tracheostomy because of laryngeal stenosis. Technical modifications of horizontal laryngectomy including a change in postoperative radiotherapy as described seem to provide better functional results.
Preservation of the spinal accessory nerve in radical or modified neck dissection is oncologically accepted in order to avoid the sequelae of the so called "shoulder syndrome". In 23 patients, on whom 29 neck dissections had been performed, the functional results were evaluated on an average 2.6 years postoperatively. 19 patients were radiated post surgery. Clinical and electromyographic examination revealed paresis of 62% of the preserved accessory nerves. Nevertheless, only 48% of the patients suffered from the shoulder syndrome, the remaining were able to innervate or compensate for the resulting deficit. Postoperative functional exercises are mandatory to prevent the shoulder syndrome and to make the preservation technique a worthwhile concept.
135 patients with BPPV and 56 patients with Meniere's disease were evaluated critically. The reliability of the diagnostic criteria (history and physical examination, audiograms and vestibular function tests) were compared. History, physical examination and patients' complaints were inconclusive. Moderate or severe medio- or pancochlear inner ear damage was more reliable, but not pathognomonic of Meniere's disease. Patients with BPPV had normal hearing or a high-frequency loss. Electronystagmographically recorded vestibular function tests were unreliable in establishing the diagnosis. The most valid test to distinguish BPPV and Meniere's disease was Hallpike's positional test, using Frenzel's spectacles for better observation of the provoked nystagmus.
In 257 parotid adenomas lumpectomy led to tumour recurrence in 49.2%, superficial parotidectomy in 5.7% and total parotidectomy in 5.3%. 40 patients with recurrent tumours experienced eventually eight true malignant neoplasms. There were five carcinomas ex pleomorphic adenomas, one true malignant mixed tumour, and two undifferentiated carcinomas. Malignant transformation usually occurred with the 4th recurrence after 15.8 years. To prevent recurrence and secondary malignancy in parotid adenomas the lumpectomy technique should be omitted.

A radio-immunological assay with monoclonal antibodies was used to measure the C 19-9 antigen in 51 patients to determine its diagnostic value in cancer of the pancreas. The results show that the C 19-9 antigen is a good marker for carcinoma of the pancreas and that it can be commonly used.
The origin of a ranula is due to obliteration of a minor duct of the sublingual gland. The main excretory duct remains patent.

The high incidence of pneumopathy in intensive care units might be due to the pulmonary aspiration of gastric juice following gastro-oesophageal reflux. The paper describes a radio-isotopic method using material easy to install at the patient's bedside. This technique demonstrated aspiration of gastric juice in the lungs of 8 of 25 intensive care patients investigated. Such a method might be useful later to demonstrate that silent bronchial aspirations of gastric juice are responsible for pulmonary complications.
The most common causes auf sensorineural deafness lie in the cochlea. Other causes are less numerous, such that our presentation will be limited to cochleal disturbances. In a general introduction we shall attempt to analyse the structural reactions, metabolism and circulation of the cochlea and the behaviour of the inner ear lymph to a noxa. The monotony of the reactions will be described as well as the reversibility of some of the changes which are observed. All these reactions play a part in the normal functioning of the inner ear. The special part of the presentation will discuss the different forms of sensorineural deafness with which we are confronted daily, taking particular account of morphological and metabolic characteristics. We shall attempt to summarise wellknown material, and discuss newer information in greater detail. We shall pont out possible causes of sensorineural deafness which have seldomly been discussed.
We were able to carry out a partial resection on 133 (35.1%) of 379 patients with glottic and supraglotic tumours. The results are discussed and compared with the literature.
