Improving dressing behavior in cognitively impaired nursing home residents.

This study tested the extent to which a behavioral intervention, Strategies to Promote Independence in Dressing (SPID), improved dressing independence among 90 cognitively impaired nursing home residents (average score on Mini Mental Status Exam = 7.35 +/- .69). The effect of SPID on caregiving efficiency, the time required for nursing assistants to use the strategies, was also examined. The results showed improved independence (decrease in assistance) from 6.08 +/- .12 at baseline to 4.93 +/- .19 following 6 intervention weeks. This significant improvement in dressing independence occurred without a clinically relevant increase in caregiver time (less than 1 min). Seventy-five percent of the subjects improved one or more levels of dressing independence, and more than 20% achieved their maximum intervention effect during the first week of treatment.

Ultrasound appearance of an equine testicular seminoma.

A 16-year-old thoroughbred stallion developed sudden swelling of the left testicle. The stallion had previously been regarded as a unilateral cryptorchid. Ultrasound examination of the left testicle revealed a diffusely heterogeneous parenchyma. The testicle was diffusely hypoechoic with ill defined regions of hyperechogenicity giving the appearance of hypoechoic nodules throughout the testicular parenchyma. No normal testicular tissue was identifiable. An echogenic band, representing a pseudocapsule could be seen surrounding the testicle. Histopathologic diagnosis a seminoma.

Diagnosis of sciatic nerve tumour in two dogs by electromyography and magnetic resonance imaging.

Sciatic nerve tumour was diagnosed in a Staffordshire Bull Terrier cross-bred and a Bichon Frise, both presenting with chronic left hind limb lameness. Neurological examination in each case was consistent with left sciatic nerve deficits and this was confirmed by EMG studies. Rectal examination of both dogs revealed a palpable intrapelvic mass that was not apparent on survey radiographs. A sciatic nerve tumour was identified using MRI in each case. Histological examination of tissue taken from the Staffordshire Bull Terrier cross-bred was consistent with a malignant peripheral nerve sheath tumour.

A conceptual framework for achieving high-quality care in nursing homes.

Nursing home care accounts for 12% of healthcare expenditures in the United States, yet serious concerns remain about the quality of care provided in these settings. This article describes a comprehensive systems model that views organizational environments as consisting of four interacting dimensions: organizing arrangements, social factors, technology, and physical setting. This model is then used as a conceptual framework (vis-à-vis an extensive literature review) to identify factors that contribute to poor-quality care in nursing homes and to highlight previous research efforts. The literature review also provides a useful framework to guide nursing home administrators and healthcare quality professionals in their implementation of quality improvement processes and serves to help direct future research in this very important arena.

Instrumented testing of functional knee braces.

The ability of seven functional knee braces to control anterior tibial displacement in three severely lax ACL deficient knees using two instrumented testing devices was studied. Some braces were statistically shown to be much better in this regard than others, but not all data obtained was statistically significant. This material should aid one in determining which braces offer the greatest degree of control of anterior tibial displacement in patients with ACL insufficiency.

The synergistic action of the anterior cruciate ligament and thigh muscles in maintaining joint stability.

The synergistic action of the ACL and the thigh muscles in maintaining joint stability was studied experimentally. The EMG from the quadriceps and hamstring muscle groups was recorded and analyzed in three separate experimental procedures in which the knee was stressed. The test revealed that direct stress of the ACL has a moderate inhibitory effect on the quadriceps, but simultaneously it directly excites the hamstrings. Similar responses were also obtained in patients with ACL damage during loaded knee extension with tibia subluxation, indicating that an alternative reflex arc unrelated to ACL receptors was available to maintain joint integrity. The antagonist muscles (hamstrings) were clearly demonstrated to assume the role of joint stabilizers in the patient who has a deficient ACL. The importance of an appropriate muscle-conditioning rehabilitation program in such a patient is substantiated.

The role of matrix vesicles in the pathogenesis of tympanosclerosis.

In a histological and electron microscopical study the authors investigated the pathogenesis of tympanosclerosis. Emphasis was placed on the role of extracellular lysosomes (= matrix lysosomes). The function of such matrix lysosomes is connected with the two-stage degradation of the connective tissue. This system gets out of control in tympanosclerosis. Inflammatory and immunological processes are suggested to be superimposed on this lysosomal action.

The inner laryngeal lymphatics. A lymphangioscopical and electron microscopical study.

Lymphangioscopical and ultrastructural studies of the larynx reveal a lymphatic capillary and a collecting vessel system. Collecting vessels show luminal valves and elastic structural components within the wall. Lymphangioscopy reveals that a strict compartmentation of different laryngeal areas is unjustifiable with the exception made for the capillaries of the free margin of the true vocal cords. Multiple anastomoses of capillaries and collecting vessels enhance midline- and vertical plane-crossing lymphatic drainage. These collaterals are gaining importance in case of impediment of the physiologic drainage pathway.

Dysplastic collagen fibrils in the human vocal fold. An electron microscope study.

Electron microscope studies made on biopsy samples of the human vocal cord from patients suffering from chronic hoarseness revealed pathological changes in both the collagen fibrils and the elastic fibres. There was splitting and fracturing of the collagen fibrils and lacuna or cyst formation in the elastic fibres, or even the complete fibre degeneration. Further investigations must be undertaken in order to establish whether the changes observed are of a primary or secondary nature.
