Forelimb performance by squirrel monkeys (Saimiri sciureus) before and after dorsal column lesions.

Old World monkeys with dorsal column lesions have exhibited slow, inept reaching movements in extrapersonal space under neurological examination. The purpose of the present study was to obtain quantified observations of such performance in similarly prepared squirrel monkeys. Squirrel monkeys with high cervical lesions of the dorsal columns were tested preoperatively and postoperatively. They were compared with sham-operated controls on three tests requiring fast reaching for stationary bait and on one test requiring catching of a falling bait. The experimental animals were impaired relative to the controls postoperatively only on bait catching. Possible reasons for the apparent discrepancy between the forelimb performance of New World monkeys and Old World monkeys are discussed.

[Functional results of tracheoplasties].

There is a direct correlation between airway resistance and residual tracheal lumen in tracheostenosis. Airway resistance has been recorded in 185 patients with tracheostenosis using body plethysmograph. High grade stenosis of the trachea is better diagnosed with body plethysmograph compared with direct inspection, a pertinent observation mainly in bordering cases. Tracheoplasties were performed in 74 patients using the body plethysmograph. Postoperative courses and functional results were observed. The average tracheal diameter was 8-9 mm at the end of our study with corresponding airway resistance of 24-34 mm H2O/l u. sec. These functional results reflect a good tolerance to average labour impound.

[Allogenic and xenogenic aortic segmental transplants in inbred rats angiographically demonstrable vascular changes (author's transl)].

In order to make a systematic study of the significance of immunological reactions in vascular grafts, segments of abdominal aorta (4-5 mm long) were grafted into inbred rats. Four experimental combinations were used: I. syngeneic (CDF leads to CDF), II. weakly allogeneic (LEW leads to CDF), III. strongly allogeneic (BD 5 leads to CDF), IV. xenogeneic (guinea pig leads to CDF). After 100 days the gaft was macroscopically evaluated, angiographed and measured. Whereas no alterations of the vascular wall were found in group I, the degree of aneurysm formation increased significantly corresponding to the degree of histoincompatibility in the other groups: group II showed little aneurysm formation (medicum size 1.27 fold), group III more pronounced (1.71 fold) and group IV very pronounced (3.9 fold) with partial thrombosis in 3/5 of the cases. These findings are interpreted, as functional and morphological consequences of the immunogenicity of the aorta tissue. The conclusion was reached that the degree of histoincompatibility should be more carefully considered, also in vascular grafting.

Persistent cutaneous fistula in the neck.

A cutaneous fistula may develop from an infected tooth. The lesion usually arises without dental symptoms and for this reason is often incorrectly diagnosed. For correct diagnosis intraoral radiographs and essential. Elimination of the infectious process and curettage of the periapical lesion are indicated. Usually no further treatment is warranted.

[Examinations of the thresholds of discomfort of the ears treated by stapedectomy (author's transl)].

Ears treated by stapedectomy often are extremely irritated by noise. To clear up this phenomenon were plotted the thresholds of discomfort by tone- and speechaudiograms in the operated ears as well as in the non-operated. All our patients being examined had the stapedius muscle tendons cut. There was no clinical otosclerosis in the non-operated ears and the audition was the same on either side. A significant increase of the discomfort threshold was seen in the operated ears (93.8% in the toneaudiograms and 78.1% in the speech audiograms). In the tone audiograms the frequencies of 500, 1000 and 2000 cps were the most concerned. We consider this increase of discomfort as a consecutive result of the stapedius tendon cutting. It verifies the protecting function of the stapedius muscle against low frequencies.

[Objective measurements of airway resistance in recurrent nerve paralysis and hypotonic dysphonia].

Respiratory obstructions of the glottis may be reliably diagnosed at any time with a body plethysmograph used in pulmonary function testing. While respiratory resistance at 27.1 mm H2O/l and sec was only slightly increased in 35 patients with unilateral recurrent nerve paralysis and not even perceived by most of them, considerably increased resistance was found in 65 patients with bilateral paralysis. Critical lifeendangering obstructions of the respiratory system were not observed. Intra- and extralaryngeal lateral fixations of the vocal cord were carried out in 29 subjects. We observed unrestricted breathing and alveolar ventilation with respiratory resistance of 25 to 44 mm H2O/l and sec even under physical strain. Additional optimal phoniatric results and speech function is present in this range. 14 patients operated according to the endolaryngeal method of Thornell 1948 showed the best functional results. In cases of unilateral recurrent paralysis and hypotonic dysphonia we were able to measure the glottic resistance during phonation by using the modified interruption technique. The values were clearly subnormal at a median of 65 to 79 mm H2O/l and sec. In 43 cases treated with teflon injections a 50% increase of the airway resistance during phonation was observed. These findings correlate with the positive subjective experiences of the patients.
