Immune complexes and acute phase proteins in human cancer: preliminary evaluation by laser nephelometric techniques.

The serum levels of circulating immune complexes (IC) were evaluated by a new 'direct' laser nephelometric assay, concomitantly with four acute phase proteins (APP), in 167 patients with various neoplasms at different stages. The present study confirms the presence of IC in the sera of cancer patients and, in the breast cancer group, there is a significant correlation with the progression of the disease. Among the APP, in several cancer groups, C-reactive protein, haptoglobin and alpha 1-acid glycoprotein are more elevated than in controls, and a trend to display higher values is observed in the subjects with metastatic disease; particularly, the frequency of patients with 3 or more proteins above the normal levels is significantly higher in metastatic than in localized cancers (p less than 0.09).

Primary aldosteronism: complete remission after adrenal venography.

A remission of adrenal adenoma producing aldosterone is described as a consequence of adrenal venous catheterization. The patient did not undergo surgery but she was kept on frequent clinical controls. The complete remission of the syndrome is still persisting three years after the incident. Since adrenal insufficiency is described after adrenal venous catheterization, the authors suggest that this procedure is to be restricted to well selected patients. When this incident occurs in pathological glands, a remission of endocrine syndrome may be expected.

The ultrasonographic spectrum of fetal omphalocele.

Four cases of fetal omphalocele diagnosed in utero by ultrasonography represent variations in the sonographic appearance of this rare anomaly. A possible pathogenesis of omphalocele is presented, with a discussion of the associated complications and the effect of diagnosing omphalocele prenatally on the management of the pregnancy itself. The association between fetal omphalocele and elevated alpha-fetoprotein is significant, and may prompt a search for a small omphalocele that would otherwise be missed on a routine obstetric sonogram. The differentiation of omphalocele from gastroschisis is difficult, yet the two anomalies can be reliably differentiated sonographically.

[Hepatic hemangioma: limitations of echo-scintigraphic diagnosis].

The Authors report a case of percutaneous liver biopsy of a solitary hepatic lesion complicated by a serious haemorrhage. By a timely surgical operation an hepatic hemangioma has been detected. The lack of diagnostic accuracy of the echo-scintigraphic detection in assessing a solitary hepatic lesion is analyzed.

Immunochemical and ultrastructural study of multiple myeloma with a heavy chain protein in the serum.

A patient with multiple myeloma had antigenically related monoclonal Fc-gamma fragments and complete IgG-kappa molecules in the serum. The urine contained only Fc-gamma fragments in the absence of Bence-Jones protein. The two distinct M-components in the serum showed electrophoretic identity but could be separated by chromatography. The simultaneous presence of complete monoclonal IgG molecules and Fc-gamma fragments, though difficult to detect, could be a frequent occurrence in multiple myeloma, and it could be defined as 'double paraproteinaemia'. A detailed ultrastructural study was performed in this case and showed fibril bundles being released from the malignant plasma cells; such fibrils could be the supramolecular organisation of the neosynthesised heavy chain fragments.

Non-Hodgkin lymphoma associated with double monoclonal immunoglobulin.

We describe an unusual case displaying the features of double monoclonal gammapathy (IgM-kappa plus IgG-lambda) associated with non-Hodgkin lymphoma (NHL). In the last years monoclonal gammapathies have been sometimes found to be associated with NHLs; it is a very peculiar occurrence the association between a different class double monoclonal gammapathy and NHL, as the case reported in this paper.

[Neomycin versus plus metronidazol for large bowel preparation in elective colon surgery].

Two series of 50 consecutive elective colonic resections with bowel preparation either with Neomycin (group A) or Neomycin combined with Metronidazole (group B) are presented. 17 bacterial inflammatory complications in group A versus 10 in group B were noted. It is concluded that the addition of an antianaerobic antibiotic agent in elective colonic surgery might be of beneficial effect.

[Adenocarcinoma of the thoracic esophagus in reflux disease].

Organic reflux disease is a pre-cancerous state. Adenocarcinoma characterized by aggressive growth, poor prognosis, rapid lymphatic and haematological spread and a technically challenging operation were found to develop in the middle or distal third of the oesophagus in 26 of a series of 262 cases (endobrachyoesophagus, grade 2 reflux oesophagitis). Only 7 of the 26 were operable of whom only 2 survived for 2--3 years. This stresses the importance of clear guidelines for a timely operation in lessening the danger of the evolving reflux disease. Comprehensive long-term controls are necessary to know whether surgery aimed at impeding the reflux element has been successful.

Effects of phenobarbital and 3-methylcolanthrene treatment on microsomes of Morris hepatoma 3924-A, tumour-bearing and normal rat liver.

Microsomal cytochromes and some oxidative activities were determined in normal rat liver, tumour-bearing rat liver and Morris hepatoma 3924-A. Except for a moderate lowering of cytochromes and enzymes in host livers, the relation between TPNH-cytochrome c reductase activity and cytochrome P-450 TPNH reduction, both increased by phenobarbital (PB) and decreased by 3-methylcolanthrene (3-MC) treatment, is noteworthy. In tumour cytochromes b5 and P-450 are absent and TPNH-cytochrome c reductase is unmeasurable and not induced by PB or 3-MC treatment. Aminopyrine demethylase activity, instead, is comparable with normal or host liver and it is modified by PB or 3-MC treatment in the same way, despite the microsomal enzymes pathway disorganization. Microsomal enzymatic defect selectivity in tumours may be due to a deranged microsome-linked growth control.

Gray scale echography of unilateral renal disease.

Gray scale ultrasound is a reliable noninvasive method of demonstrating renal pathology. It is simple nontraumatic, nonionizing and can be readily utilized in the differentiation of cystic and solid lesions.

Hoffman's syndrome: a peculiar case.

The Authors describe the case of a 50-yr-old woman with painful muscular cramps and high muscular enzymes level secondary to adult-onset myxedema, and where no histological and ultrastructural muscle alterations could be detected, probably owing to the early observation of the patient.

[Experiences with percutaneous K-wire osteosynthesis in distal radius fractures].

Follow-up controls were made on 300 patients whose fractures of the distal radius were treated by a primary operative fixation with percutaneous K-wire. This group showed significantly better results than a control group of 300 patients given conservative management. We recommend a primary percutaneous K-wire osteosynthesis as treatment of choice for the management of distal radius fractures.
