Chronic facial pain following diseases affecting the maxillary antrum. The influence of peripheral impulses from the maxillary air sinus is not significant in their maintenance.

For the purpose of examining the significance of a pain focus in the maxillary air sinus denervation of the mucosa of the sinus by transantral maxillary nerve section was performed. The material comprises 20 patients with pain in the distribution of the infraorbital nerve and previous disease of the relevant sinus. All patients had been treated previously without any benefit from peripheral surgery of the infraorbital nerve. Only 20% had any benefit of the proximal avulsion for more than three months. It is concluded that impulses from the mucosa of the maxillary sinus do not play any part in the occurrence or persistence of facial pain in this highly selected material.

Otitis media in Greenland children: acute, chronic and secretory otitis media in three- to eight-year-olds.

The middle ear status was examined in 142 children aged three to eight years from Sukkertoppen and Kangamiut in Greenland. The investigation is based on social-pediatric and otologic histories and on objective examination using otomicroscopy and tympanometry. Three-fourths of the small children had a previous history of otitis, some with repeated instances. Otomicroscopic findings revealed suppurative, chronic otitis in 6% and sequelae of chronic otitis in 13%. Tympanometry disclosed a reduced middle ear pressure in another 26% of the ears examined. Comparison of children with healthy ears with the groups with affections of the middle ear showed a tendency to an increased risk of otitis in families at a low social stratum, whereas the mode of habitation seemed insignificant. Finally, a large milk intake both in connection with breast-feeding and later in life seemed to reduce the frequency of otitis. The investigation showed that acute otitis and chronic otitis media in particular are more prominent in Greenland than in Denmark. Attention is drawn to the handicap associated with a high incidence of chronic otitis media and the concomitant hearing loss to a population isolated in point of language. The successful achievements of a health information campaign and an active therapeutic program, which has reduced the prevalence of chronic otitis among the Innuit population native to Alaska and Canada, provides the rationale for an increased otologic effort in Greenland.

Large versus small fenestration technique in stapedectomy. A comparative investigation of House and Fisch prostheses in stapedectomy.

Sixty patients with otosclerosis have been operated on by 2 different techniques using either a small fenestra or a large fenestra. The hearing improvement by using a Fisch prosthesis (small fenestra) was significantly better in the high frequency range compared to results obtained by using a House prosthesis on a vein (large fenestra). Due to significantly better hearing in the higher frequencies the Fisch prosthesis is recommended. In technically difficult cases of stapedectomy, where extraction of the footplate is difficult or impossible the Fisch prosthesis can be valuable.

A population study of otitis media in childhood.

A total of 494 children with an average age of 3 1/2 years were studied with a view to elucidating the frequency of both acute and secretory otitis media and their relationship, if any, to a number of social factors. Of the children, 41% had had at least one attack of otitis media, in 49% of these associated with aural discharge. 70% of the children were attending day-care centres, and the incidence of otitis media was highest in this group. According to Jerger's classification of the tympanometric curves, 51% of these were of type A, 17% of type B, and 32% of type C. Significantly higher frequencies of types B and C curves were found among children attending day-care centres.

Roentgenography of paranasal sinuses during treatment of nasal polyposis.

Seventeen patients had severe nasal polyposis. Local treatment with beclomethasone dipropionate caused marked reduction in the size of polyps and the nasal obstruction, and the effect continued throughout the treatment period. In all cases roentgenographic examination showed clouding of the nasal cavity and the paranasal sinuses, especially of the maxillary and ethmoidal sinuses. After three months of treatment, no change in the opacity of the sinuses was found. In spite of that, the symptomatic effect of the treatment was successful.

Treatment of nasal polyps with intranasal beclomethasone dipropionate aerosol.

In a double-blind trial thirty-five patients with moderately-severe nasal polyposis were treated with intranasal beclomethasone dipropionate aerosol for 3 weeks. The dose given (400 mug/day) had only local effect on the symptoms. Judged by diary card scores the nasal symptoms were reduced to 52% of the pre-trial level for the whole group. Corrected for the placebo effect the percentage was 68. The reduction of symptoms was equally apportioned to the three symptoms, sneezing, nasal secretion and blockage. The treatment was tolerated well, and it is concluded that intranasal treatment with beclomethasone dipropionate aerosol offers most patients with nasal polyps a good response without any risk of systemic steroid side-effects.
