[Long-term outcomes and analysis of its relative factors of nasal endoscopic sinus surgery in children].

OBJECTIVE: To evaluate the results of endoscopic sinus surgery (ESS) in children with chronic sinusitis. The long-term outcomes and the relative factors were analyzed retrospectively. MATERIALS AND METHODS: A total of 268 children (432 sides), 186 males and 82 females, aged 3 to 17 years (average, 14.3) were included in this study (underwent ESS). The postoperative follow-up time of 188 cases (305 sides, 70.1%) was over one year. The management during follow-up was adopted under nasal endoscope and local anesthesia. The postoperative treatment included cleaning operative cavity, nasal irrigation, antibiotics, steroid, immune regulator, etc. RESULTS: Overall cases included 89 cases (147 sides) of chronic sinusitis, 86 cases (142 sides) of nasal polyps and 13 cases (16 sides) of mucocele respectively. The cure rate was 70.2% (132 cases). 43 cases (22.9%) were improved, while other 13 cases were of no effect. Overall symptoms, 93.1% children with headache were improved, 85.1% children were free of nasal obstruction and 60.1% children gained a relief of purulent discharge. 11 cases had no change and 2 cases were worse compared with before surgery. 34 cases with anosmia were improved or recovered after ESS. The total effective rate of ESS was 93.1%. 8 cases received a revision ESS. CONCLUSIONS: ESS in children with chronic sinusitis and polyps has a good long-term outcomes. The reasonable reservation of mucosa during ESS must be emphasized. The quality of follow-up is an important factor affecting the results of ESS. Whether the disciplinary preoperative management was made or not is the hypothesis of ESS indications.

[Interleukin-5 regulate the bone marrow response in the model of allergic rhinitis].

OBJECTIVE: To elucidate the mechanism of bone marrow response regulated by interleukin-5 (IL-5) in the model of allergic rhinitis. METHODS: Twenty SD rats were randomly divided into allergic rhinitis (AR) group and control group. The leucocytes in the smears of bone marrow and peripheral blood were counted, and the expression of IL-5 was detected by immunohistochemistry. RESULTS: The ratio of eosinophils to white cells in bone marrow smears of AR group was significantly higher than that of control group(P < 0.01). The ratio of basophils to white cells in bone marrow smears of AR group was significantly higher than that of control group(P < 0.01). The ratio of eosinophils to white cells in peripheral blood smears of AR group was significantly higher than that of control group(P < 0.01). The ratio of IL-5 positive cells to white cells in bone marrow smears of AR group was significantly higher than that of control group (P < 0.05). The ratio of IL-5 positive cells to white cells was significantly positively correlated with the ratio of eosinophils to white cells in bone marrow smears of AR group (R = 0.85, P < 0.01). CONCLUSION: IL-5 regulates the proliferation and differentiation of eosinophils in bone marrow.

[Surgery of ossifying fibroma of the sinuses].

OBJECTIVE: To explore the different surgical choices for treating the ossifying fibroma of the sinuses. To summarize the management and characteristics of each surgical operation. METHODS: A retrospective evaluation of thirty-five patients with ossifying fibroma of the sinuses from August 1994 to July 2001 was presented. RESULTS: Among 22 patients operated by nasal endoscopic management, complete ossifying fibroma removed was achieved in 8 cases, and the majority part of tumor removed in 14 cases. Six patients were operated through a lateral rhinotomy with radical operation in 4 cases. Five ossifying fibromas were removed with a coronal incision. Two cases underwent Caldwell-Luc' surgery. The clinical symptoms, location of ossifying fibroma, and surgical procedures were analyzed. All patients outcomes were successful, no serious complication from the surgical technique occurred. Thirty-three cases were followed-up for 1 to 8 years with an average of three and half years. Fourteen patients had no recurrence, fourteen cases lived with the remains of ossifying fibroma, and five cases recurred. CONCLUSIONS: The choice of surgical operations on ossifying fibroma of the sinuses was mainly decided by the location of ossifying fibroma, in the meanwhile, the organ function, the cosmetology, the surgical degree of difficulty, and the doctor's experience were taken into account.

Glucose-induced changes in integrins and matrix-related functions in cultured human glomerular epithelial cells.

In cultured human glomerular epithelial cells (HGEC), 25 mM glucose resulted in decreased expression of alpha(3)-, alpha(2)-, and beta(1)-integrins and increased expression of alpha(5)- and alpha(v)beta(3)-integrins. This change was accompanied by decreased binding of HGEC to type IV collagen. In the presence of normal (5 mM) glucose concentration, cell binding to type IV collagen was primarily mediated by alpha(2)beta(1)- and alpha(5)beta(1)-integrins, as indicated by experiments in which cell adhesion to type IV collagen was competed by specific anti-integrin monoclonal antibodies. In the presence of high (25 mM) glucose, the upregulated alpha(5)- and alpha(v)beta(3)-integrins were mainly involved in cell binding to type IV collagen. Furthermore, high glucose decreased expression of matrix metalloproteinase-2 (MMP-2), a collagenase regulated in part by alpha(3)beta(1)-integrin, as suggested by the use of ligand-mimicking antibodies against these integrins, which resulted in release of increased amounts of MMP-2 in the culture medium. Finally, tissue inhibitor of metalloproteinase-2, the specific inhibitor of MMP-2, was upregulated in high glucose and could contribute to matrix accumulation. These changes could help explain basement membrane thickening in diabetes.

A novel member of a zinc transporter family is defective in acrodermatitis enteropathica.

The rare inherited condition acrodermatitis enteropathica (AE) results from a defect in the absorption of dietary zinc. Recently, we used homozygosity mapping in consanguineous Middle Eastern kindreds to localize the AE gene to an approximately 3.5-cM region on 8q24. In this article, we identify a gene, SLC39A4, located in the candidate region and, in patients with AE, document mutations that likely lead to the disease. The gene encodes a histidine-rich protein, which we refer to as "hZIP4," which is a member of a large family of transmembrane proteins, some of which are known to serve as zinc-uptake proteins. We show that Slc39A4 is abundantly expressed in mouse enterocytes and that the protein resides in the apical membrane of these cells. These findings suggest that the hZIP4 transporter is responsible for intestinal absorption of zinc.

Proximal tubular epithelial cell integrins respond to high glucose by altered cell-matrix interactions and differentially regulate matrixin expression.

Thickening of the tubular basement membrane (TBM) occurs in diabetic nephropathy, but the effects of high glucose on the functional aspects of proximal tubular epithelial cells are not clearly understood. In the present study, we examined the effects of elevated glucose concentrations on (a) integrin expression by human proximal tubular epithelial cells (HK-2) and integrin-mediated interactions with type IV collagen (colIV) and laminin, major components of TBM; (b) the expression of matrixins/matrix metalloproteinases (MMPs), which is regulated by integrins; and (c) the expression of tissue inhibitors of metalloproteinases (TIMPs). HK-2 cells cultured in 25 mM glucose underwent a reduction of the expression of alpha3, beta1, alpha(v)beta3, and alpha5 integrin subunits, with a concomitant increase of the alpha2 subunit, compared with cells grown in 5 mM glucose. Adhesion experiments demonstrated that high glucose led to increased cell adhesion on either colIV or laminin. Experiments of competition of adhesion using anti-integrin antibodies indicated that HK-2 cells in 5 mM glucose used mainly alpha(v)beta3 and alpha5beta1 integrins to adhere to colIV, whereas in 25 mM glucose they additionally used alpha2beta1. In the case of laminin, a beta1-mediated adhesion was observed when HK-2 cells were in 5 mM glucose, whereas in 25 mM glucose, alpha2beta1 and alpha(v)beta3 were also involved. Elevated glucose concentrations resulted in decreased expression of MMP-9 and MMP-2, whereas an increase in TIMP-1 and a decrease in TIMP-2 expression were observed. We also examined which integrins mediated the expression and secretion of matrixins MMP-2 and MMP-9. Ligation of alpha3beta1 with mAbs resulted in induction of MMP-2 expression and secretion, whereas antibody ligation of alpha(v)beta3 led to down-regulation of MMP-9. The above data implicate integrins of proximal tubular epithelial cells in the regulation of MMPs and in the development of TBM thickening in diabetic nephropathy.

[Related structures of the lateral sphenoid wall anatomy studies in CT and MRI].

OBJECTIVE: To study the anatomy of the Lateral Sphenoid Wall in CT and MRI in normal adults so as to provide a theoretical basis for imagine science. METHOD: High resolution CT in axial and coronal planes was conducted on 25 normal adults. MRI in axial and coronal planes, using SE T1 WI and FSE T2 WI was conducted on 20 volunteers. RESULT: 1. The pneumatisation of sphenoid sinus can be divided into two types: Pneumatisation of the sphenoid body and the one of sphenoid process. The former one can be subdivided into three categories: concha (2%), pre sellar (20%), sellar (78%). The latter can be subdivided into four categories: lesser wing (38%), greater wing (40%), pterygoid process (34%) and dorsum sellar (6%). 2. The thinnest part (< 1 mm) of the LSW is located at the inner wall of the optic nerve canal (96%), at the bony wall in the plane of sphenoethmoid recess (86%), the bony wall in the plane of the internal carotid artery (66%). 3. Over pneumatised sphenoid sinus could make the foramen rotumdum (64%) and pterygoid canal (44%) protrudes into the sphenoid cavity. CONCLUSION: Attention should be paid to the various mode of pneumatization around the optic nerve canal.

[Inhibitory effect of flavones from Ageratum conyzoides on the major pathogens in citrus orchard].

Inter-cropping with Ageratum conyzoides may effectively control major pathogens in citrus orchard. The research showed that flavones produced by and released from A. conyzoides could inhibit the major fungal pathogens, such as Elsimoe fawcetti Bit. Et Jenk, Collectrichum gloeosporimdes Penz, Oidium tingitaninum Carter, Capondium citri Berk et Desm in citrus orchard. 10 flavones, including one glycoside, from A. conyzoides plant were isolated and identified. Most of them could gradually degraded in soil and only 3 flavones could be accumulated and last a long time. Just these 3 flavones could obviously possess inhibitory effect on fungal pathogens in citrus orchard. The inhibitory effect of the 3 flavones was stronger, not only than those of other allelochemicals produced by and released from A. conyzoides, but also than that of Carbenzin, a commercial fungicide. Therefore, the flavones produced by and released from A. conyzoides played an important role in pathogens control in citrus orchard.

[Allergic fungal sinusitis: report of three cases].

OBJECTIVE: To discuss and analyze the diagnosis and management of allergic fungal sinusitis. METHOD: Three middle-aged patients with allergic fungal sinusitis (AFS) were observed. The histological diagnosis was made on the mucous material and allergy evaluation based on the strong history of inhalant mold allergies, a positive result of skin test, total immunoglobulin E level and radioallergosorbent test to fungal antigens. The management included wide local endoscopic sinus debridement, adequate sinus aeration, postoperative use of systemic and intranasal corticosteroid. Systemic antifungal therapy was not used. RESULTS: All 3 patients were immunocompetent and none demonstrated histologic evidence of tissue invasion. Two patients remained disease-free during follow-up ranging for a mean of 16 months, 1 patient recurred 17 months after operation and symptom-free for 2 months after re-cleaning of the sinus cavity. CONCLUSION: Diagnosis of AFS required a high index of suspicion. A confirmatory diagnosis was made from the inspissated mucus, clinical and CT findings along with careful communication with mycologists and immunologists about the possibilities of fungal growth and allergy. Costly and lengthy courses of antibiotics, aggressive surgery should be avoided. Surgical debridement and systemic corticosteroids followed by intranasal corticosteroids might provide long-term control of AFS.

[Measurement of nasal airway resistance and olfactory function before and after endoscopic sinus surgery].

OBJECTIVE: To quantitatively analyse the changes of nasal airway resistance (NAR) and olfactory function in patients before and after endoscopic sinus surgery(ESS). METHODS: NAR and olfactory functions in 127 patients suffering from chronic sinusitis and/or nasal polyps were measured with anterior rhinomanometry and T&T olfactometer standard odors. RESULTS: 1. The degree of NAR increased and olfactory dysfunction were accompanied with varied clinic classifications. 2. After the ESS, NAR decreased and olfactory functions improved obviously, the effective rates were 93.4% (85/91) and 71.9% (64/89) respectively. 3. Allergy was one of the major factors that affected the outcome of nasal function after ESS. CONCLUSIONS: 1. The improvements of NAR and olfactory function in patients with chronic sinusitis and polyps were significant after ESS. 2. As the determining method of nasal function, the measurement of NAR and olfactory function ought to be used widely in clinical practice.

Puromycin aminonucleoside suppresses integrin expression in cultured glomerular epithelial cells.

Puromycin aminonucleoside (PAN)-induced nephrosis is a well-described model of human idiopathic nephrotic syndrome, but the mechanism of PAN's effect is not completely understood. Because PAN injection into rats results in retraction of glomerular epithelial cell foot processes and glomerular epithelial cell detachment, it was hypothesized that PAN might alter the contacts between these cells and the glomerular basement membrane. The major integrin expressed by glomerular epithelial cells is alpha3beta1, which mediates attachment of these cells to extracellular matrix proteins including type IV collagen. T-SV 40 immortalized human glomerular epithelial cells were used to study PAN's effects on alpha3beta1 expression, as well as that of podocalyxin and the slit diaphragm component ZO-1. Glomerular epithelial cells were seeded into plastic flasks and allowed to attach and proliferate for 48 h. The cells were then incubated for another 48 h in media containing 0, 0.5, or 5.0 microg/ml PAN. PAN exposure resulted in dose-dependent decreases in alpha3 and beta1 expression, both at the protein level and at the mRNA level. This was accompanied by a significant decrease in the adhesion of glomerular epithelial cells to type IV collagen. PAN did not affect ZO-1 protein expression. Treatment with PAN increased the expression of podocalyxin at the protein and mRNA levels. Reduced glomerular epithelial cell expression of alpha3beta1 integrins and impaired adhesion to type IV collagen may contribute to the glomerular epithelial cell detachment from glomerular basement membrane seen in the PAN nephrosis model.

Identification of two alternatively spliced forms of human tubulointerstitial nephritis antigen (TIN-Ag).

Tubulointerstitial nephritis antigen (TIN-Ag) is a recently described basement membrane glycoprotein reactive with autoantibodies in some forms of immunologically mediated human tubulointerstitial nephritis. This report presents the complete cDNA and predicted amino acid sequences of two human TIN-Ag mRNA species referred to as TIN1 and TIN2. Translation through the open reading frames of these clones indicates the presence of a signal peptide and putative pre-propeptide. TIN1 additionally contains a characteristic laminin-like epidermal growth factor (EGF) motif and significant homology within the carboxy terminus with the cysteine proteinase family of enzymes. The EGF motif bears important similarities in the positions of cysteines with two motifs in the propeptide of von Willebrand factor. The EGF motif and part of the region that is homologous with the cysteine proteinase family are removed from the TIN2 cDNA. However, the rest of the sequence is identical in these two forms, indicating an alternatively spliced TIN-Ag mRNA product. Both forms contain putative calcium-binding sites. Secondary structure predictions strongly suggest differences between TIN1 and TIN2 leading to the hypothesis that these two forms of TIN-Ag may exhibit differences in their function. Expression studies with appropriate probes demonstrate expression mainly in the kidney and in the intestinal epithelium and lack of expression in other tissues. In the kidney, both TIN1 and TIN2 transcripts are detected, however, TIN1 appears to be the predominant form.

Parkinson's disease in China: prevalence in Beijing, Xian, and Shanghai.

A cross-sectional prevalence study of Parkinson's disease in China was undertaken in 1997-98. We assessed 29?454 residents aged > or =55 years from 79 rural and 58 urban communities in Beijing, Xian, and Shanghai, who were selected through stratified, multistage, cluster sampling. With a standardised diagnostic protocol, we identified 277 people with the disease. Prevalence of Parkinson's disease for those aged > or =65 years was 1.7% (95% CI 1.5-1.9). We estimate that 1.7 million people in China have the disease (aged > or =55 years). Our findings suggest that prevalence of Parkinson's disease in China is similar to that in developed countries.
