[Studies of treatment strategy and prognosis on acute myeloid leukemia with chromosome 8 and 21 translocation].

OBJECTIVE: To investigate the relationship between the biological features and the treatment efficacy and prognosis in acute myeloid leukemia subtype M2 (AML-M2) patients with chromosome 8 and 21 translocation. METHODS: By using Cox regression model and Kaplan-Meier analyses, prognostic factors in 54 cases of de novo adult AML with t(8;21) in our institute from 1990 to 2003 were retrospectively analyzed. RESULT: The complete remission (CR) rates were 81.9% for all M2 patients, 82.4% for patients with normal karyotype, 88.5% for patients with t(8;21) [P > 0.05 for normal karyotype vs t(8;21)], 100.0% for 28 patients with t(8;21) alone and 75.0% for 24 patients with additional chromosome abnormalities (P < 0.01). The actuarial 3 year overall survival(OS) was 26% for M2 patients with normal karyotype, 25% for patients with t(8;21) [P > 0.05 for normal karyotype vs t(8;21)], in whole t(8;21) group, 46.4% for patients with t(8;21) alone and 0% for patients with additional chromosome abnormalities (P < 0.01). Multivariate analysis of prognostic factors showed that chromosome abnormalities besides t(8;21) was the only factor affecting CR, disease-free survival (DFS) and OS. DFS of allogeneic hematopoietic stem cell transplantation (HSCT) and intermediate-dose cytarabine/high dose cytarabine (IDAC) groups were better than the group received routine dose cytarabine as postremission therapy (P < 0.01). CONCLUSION: AML with t(8;21) is not a single defined AML subset, and patients with additional chromosome abnormalities have a worse prognosis. HSCT and IDAC could improve the outcome. HSCT is the best choice for patients with high risks, especially with additional chromosome abnormalities.

[Effective evaluation of liver transplantation for hepatocellular carcinoma].

OBJECTIVE: To evaluate the effect of liver transplantation for hepatocellular carcinoma and to explore the suitable indication for liver transplantation for advanced primary hepatocellular carcinoma. METHOD: Clinical data were collected and studied with 43 patients with cirrhosis who had unresectable hepatocellular carcinoma and who underwent liver transplantation from August, 2000 to December, 2003. All the survived patients were followed up. RESULTS: The longest following period was 45 months. 18 patients died by the end of the April, 2004. The 1-year survival rate was 85.5%, the 2-year survival rate 65.5%, the 3-year survival rate was 43.0%. CONCLUSION: In our country, some advanced primary hepatocellular carcinoma without extrahepatic metastasis is still indicated for liver transplantation. Some patients could have the opportunity to survive relatively long time.

Extinction-free electron diffraction refinement of bonding in SrTiO3.

Accurate low-order Fourier coefficients of the crystal potential of SrTiO(3) are measured by quantitative convergent-beam electron diffraction. The accuracy in the corresponding derived X-ray structure factors is about 0.1% for the strong low-order reflections (sin theta/lambda < 0.3 A(-1)). This accuracy is better than for conventional X-ray diffraction and equivalent to the accuracy of the X-ray Pendellosung method. Combination of these structure factors with high-order X-ray diffraction measurements allows accurate bonding information to be obtained from a multipole model fitted to the experimental data. It is shown that Ti-O has a covalent component and that the Sr-O bond is mainly ionic. The role of Ti 3d electrons in Ti-O bonding is also discussed.

Effects of urban community intervention on 3-year survival and recurrence after first-ever stroke.

BACKGROUND AND PURPOSE: For the past 2 decades, stroke has been a principal cause of death in China, and stroke incidence tends to increase with the increase of stroke-related risk factors. The purpose of this study was to evaluate the effects of urban community-based intervention on 3-year survival and recurrence after first-ever stroke. METHODS: Two communities with a registered population of approximately 50 000 each were selected as either intervention or control communities in Beijing during 1991 to 2000. Comprehensive intervention measures including the management of high-risk population and the health education of whole community population were regularly implemented. Then the influence of community intervention on 3-year survival and recurrence after initial stroke was evaluated. RESULTS: Within 3 years, 41.85% of 736 patients in the intervention community died whereas 40.34% of 818 patients in the control community died. Of 223 cases from the intervention community, 26 (11.66%) had a recurrent stroke within 3 years versus 52 (20.80%) of 250 cases from the control community. The statistical difference was found. Compared with the control community, the death risk of first-ever stroke in the intervention community decreased by 26% (relative risk [RR]=0.74; 95% confidence interval [CI]: 0.61 to 0.89; P=0.002); especially, that of hemorrhagic stroke decreased by 39% (RR=0.61; 95%CI: 0.46 to 0.81; P=0.001). Compared with the control community, the recurrence risk of first-ever stroke from the intervention community decreased by 42% (RR=0.58; 95% CI: 0.34 to 1.00; P=0.048). CONCLUSIONS: Community intervention may be effective and beneficial to the recurrence prevention and survival improvement of stroke, especially hemorrhagic stroke.

Brain-derived neurotrophic factor acutely depresses excitatory synaptic transmission to GABAergic neurons in visual cortical slices.

Brain-derived neurotrophic factor (BDNF) acutely modulates synaptic transmission to excitatory neurons in hippocampus and neocortex. The question of whether BDNF acts similarly on excitatory synaptic transmission to GABAergic neurons was eluded in previous studies using cortical slices. To address this question, we used transgenic mice in which expression of green fluorescence protein (GFP) is regulated by glutamic acid decarboxylase 67 (GAD67) promoter. In cortical slices prepared from these GAD67-GFP knock-in mice, we could detect GABAergic neurons under a fluorescent microscope. An application of BDNF rapidly depressed excitatory postsynaptic currents (EPSCs) evoked by layer IV stimulation in most GFP-positive neurons in layer II/III of the cortex. This effect was seen at synapses activated during the BDNF application and blocked by anti-TrkB IgG, indicating that the acute inhibitory action of BDNF is activity-dependent and mediated through TrkB. Paired-pulse ratios of the amplitude of EPSCs to paired stimulation at intervals of 10-100 ms were not significantly changed after BDNF application, suggesting that the site of depression may be postsynaptic. Responses to directly applied glutamate were also depressed by BDNF in most of neurons, being consistent with the interpretation of postsynaptic action of BDNF. The depressive action of BDNF was blocked by an intracellular injection of a Ca(2+) chelator, suggesting that a rise in Ca(2+) is involved in the acute depression of EPSCs. This action of BDNF was seen in 67% of parvalbumin (PV)-positive neurons, but in only 19% of PV-negative neurons, indicating that the depressive action is biased to PV-positive GABAergic neurons.

[Control effect of health education on body mass index of community population].

OBJECTIVE: To examine the effect of community-based health education and health promotion on body mass index (BMI), overweight and obesity. METHODS: We separately selected 100,000 population in Beijing, Shanghai and Changsha Cities during 1991-2000. Each of the cities was then randomly separated intervention and control community of 500,000 population. In the intervention community various kinds of health education and health promotion had been carried out. The level of knowledge, attitude and behavior (KAB) and body mass index (BMI) were evaluated. RESULTS: The net increases of KAB level and regularly exercising rate in three city and different target population were statistically differences in comparison with baseline survey of 1992, and the net decreases of the BMI, overweight rate and obesity rate of the whole population were statistically differences. CONCLUSION: Health education and health promotion can effectively control the rising of population BMI, and have great meaningful for population preventing from cardio-cerebro-vascular diseases.

[Bone marrow morphologic features in patients treated with imatinib for Philadelphia chromosome positive chronic myeloid leukemia].

OBJECTIVES: To assess bone marrow morphologic changes in Philadelphia-chromosome positive chronic myeloid leukemia (Ph(+)-CML) patients treated with Imatinib, and to evaluate the correlation of the morphologic changes with hematological or cytogenetic responses. METHODS: One hundred and seventeen patients with Ph(+) CML: 54 in chronic phase but failed to interferon-alpha treatment, 41 in accelerated phase, 22 in blastic phase received oral administration of Imatinib 400 or 600 mg once daily for more than 18 months. RESULTS: All of the patients responded to the treatment, including complete hematological response, bone marrow response and return to chronic phase, bone marrow cellularity and myeloblast count reduced significantly to non-CML picture. Myeloid/erythroid ratio and megkaryocyte count were decreased significantly in most patients in chronic and accelerated phases (P < 0.05). Bone marrow hypoplasia or aplasia was associated with lower cytogenetic response rates in patients in chronic phase (58.8% vs 86.5%, P = 0.035), lower complete hematological response in patients in accelerated phase (26.3% vs 75.0%, P = 0.004), and 6-month overall survival in patients in blastic phase (77.8% vs 16.7%, P = 0.009). Patients in advanced stage obtained non-CML marrow picture in 1 month of treatment had better prognosis. 18-month disease progression rates were lower (25% vs 75%, P = 0.028) and overall survival rates higher (75.0% vs 11.8%, P = 0.004) in patients obtained non-CML picture marrows than in those with CML marrows picture in accelerated phase. Hematological response rate and overall survival of more than 6 months were higher in patients with non-CML marrows picture than those with CML marrows picture (100.0% vs 40.0%, P = 0.017 and 83.3% vs 26.7%, P = 0.046 respectively) in blastic phase. CONCLUSIONS: Normal marrow appearance can be sustained under continuous treatment of Imatinib in CML patients who achieved hematological responses.

[Arsenic absorption and retention in acute promyelocytic leukemia patients treated with tetra-arsenic tetra-sulfide].

The objective of this study was to investigate arsenic absorption and retention in acute promyelocytic leukemia (APL) patients treated with tetra-arsenic tetra-sulfide (As(4)S(4)). Arsenic concentrations in samples from APL patients were quantitatively determinated by Hydride Generation Atomic Absorption Spectrometry. The results showed that blood arsenic level was 51.7 +/- 18.7 microg/L (n = 41), urine arsenic level was 2359.1 +/- 1910.6 microg/L (n = 36), and the average daily urinary arsenic excretion was 5.1 +/- 4.06 mg/day (n = 32) on the 7th-9th day after As(4)S(4) administration; blood arsenic level was 61.7 +/- 22.7 microg/L (n = 30), urine arsenic level was 2834.0 +/- 1958.3 microg/L (n = 27), the average daily urinary arsenic excretion was 6.3 +/- 4.98 mg/day (n = 24) on the 10th-12th day after As(4)S(4) administration; blood arsenic level was 62.8 +/- 25.1 microg/L (n = 34), urine arsenic level was 2859.3 +/- 2298.2 microg/L (n = 32) and the average daily urinary arsenic excretion was 6.82 +/- 5.58 mg/day (n = 32) on the 13th-15th day after As(4)S(4) administration. Blood arsenic level was 20.7 +/- 10.9 microg/L (n = 31), urine arsenic level was 525.5 +/- 337.1 microg/L (n = 28), and the average daily urinary arsenic excretion was 1.76 +/- 1.3 mg/day (n = 17) on the 7th-9th day after stop of treatment; blood arsenic level was 16.1 +/- 10.1 microg/L (n = 34), urine arsenic level was 207.1 +/- 164.5 microg/L (n = 28) and the average daily urinary arsenic excretion was 0.42 +/- 0.27 mg/day (n = 22) on the 13th-15th day after stop of treatment. Blood arsenic concentration, urine arsenic concentration and daily urinary arsenic excretion reach a steady state after treatment with As(4)S(4) for 10 days. The average urinary arsenic excretion was 6.82 +/- 5.58 mg/day (n = 32), arsenic absorption level was 9.74 +/- 7.97 mg/day and arsenic absorption efficiency was 0.25 +/- 0.20% during treatment. In conclusion, the majority of absorbed arsenic was excreted from urine and other ways, only a small part of absorbed arsenic was retained in body at the 14th day after therapy was discontinued, blood arsenic concentration, urine arsenic concentration, daily urinary arsenic excretion and hair arsenic concentration could be considered as useful biomarkers for monitoring arsenic absorption and retention in APL patients treated with As(4)S(4).

[Efficacy and safety evaluation of imatinib in the treatment of patients with chronic granulocytic leukemia in accelerated phase].

OBJECTIVE: To evaluate the efficacy and safety of imatinib in the treatment of adult patients with chronic granulocytic leukemia (CGL) in accelerated phase. METHODS: Thirty patients with CGL in accelerated phase were orally administered with imatinib 400 or 600 mg daily for 7 approximately 9 months. RESULTS: Hematological responses occurred in 28 of 30 patients (93.3%) in the treatment: 14 (46.7%) had a complete hematological response, 10 (33.3%) had a marrow response, and 4 (13.3%) returned to chronic phase. Bone pain and splenomegaly disappeared soon after the administration of imatinib. Eight patients relapsed 30 approximately 172 days after hematological responses. Six of them received imatinib with daily dose increment to 800 mg. Four of these 6 patients had no response and 2 returned to chronic phase again. The risk factors for relapse were blasts > or = 15% in bone marrow or in peripheral blood, extramedullary leukemia involvement, hemoglobin < 100 g/L before the administration of imatinib, and lack of a complete hematological response after the treatment. Cytogenetic remission occurred in 6 of 27 patients (21.4%) after 3 months treatment: 4 (14.3%) were complete cytogenetic response and 2 (7.1%) major cytogenetic response. Mild non-hematologic adverse effects occurred in most of the patients, but were manageable and tolerable, or disappeared automatically. Severe neutropenia or thrombocytopenia appeared in more than half of the patients. CONCLUSIONS: Imatinib has substantial activity and is well-tolerated in the treatment of accelerated phase of CGL.

[Effect of the tetra-arsenic tetra-sulfide (As4S4) on the corrected QT interval in the treatment of acute promyelocytic leukemia].

OBJECTIVE: To evaluate the effect of tetra-arsenic tetra-sulfide (As4S4) therapy on the corrected QT interval (QTc) in the acute promyelocytic leukemia (APL) patients. METHODS: Ninety cases of APL treated with As4S4 were divided into two groups--the remission induction group and maintenance therapy group. Blood arsenic concentration was measured and a 12-lead electrocardiogram (ECG) was simultaneously performed before treatment and after remission in the induction group, and before and 2, 4, 6, 8, 10 courses after the treatment in the maintenance therapy group. QT interval on each ECG was measured and corrected by the Bazett formula. RESULTS: Oral administration of As4S4 could lead to the prolongation of QTc both in remission induction and maintenance therapy groups. QTc prolongation was related to the doses of As4S4 and blood arsenic levels. QTc prolongation and its variation range were increased with accumulative doses of As4S4 and the blood arsenic levels. In ten courses maintenance therapy patients, the average abnormal rate of QTc was 37.7%. Blood arsenic concentration was increased slowly with courses, but the variation had no statistical difference (P > 0.05). All the patients whose QTc was abnormal (> or = 440 ms) had neither symptoms nor serious cardiac events, such as ventricular tachycardia and Torsade de pointes and could complete the As4S4 therapy. CONCLUSION: Although As4S4 therapy can lead to QTc prolongation in the treatment of APL patients, it does not preclude the completion of the therapy.

[MICM classification and prognostic analysis of 80 adolescents with acute lymphoblastic leukemia].

OBJECTIVES: To explore MICM classification and adverse prognostic factors in adolescents with acute lymphoblastic leukemia (ALL). METHODS: The MICM classification, clinical characteristics of 80 adolescents with ALL admitted to our hospital from January 1998 to December 2002 were retrospectively analyzed. Survival data were estimated by the Kaplan-Meier method and the prognostic factors were analyzed with the COX regression model. RESULTS: In the 80 patients, B-ALL and T-ALL accounted for 69.12% and 26.47%, respectively. The percentage of Ph(+)ALL was 18.37% (9/49), and that of hyperdiploidy was 4.08%. Patients at diagnosis with high leukocyte counts (> 50 x 10(9)/L) accounted for 27.94%. Among the 78 cases treated with VDP(L) or CODP(L) regimens, 73 (91.03%) obtained CR in 4 weeks. After a median follow-up of 24 months, the estimated 3-year disease-free survival (DFS) rates of patients receiving chemotherapy or allo-HSCT were (32.55 +/- 16.50)% and (69.58 +/- 8.72)%, respectively (P < 0.05). In COX analysis, high initial leukocyte counts (> 50 x 10(9)/L) and Philadelphia chromosome positivity were adverse prognostic factors for long-term survival. CONCLUSIONS: MICM classification has important clinical and prognostic significance in the risk-directed therapy of adolescents with ALL. The adverse prognostic features for these patients were high leukocyte counts, less incidence of chromosome hyperdiploidy and Ph chromosome positivity.

[A case-control study on the risk factors of Alzheimer's disease in military elderly men].

OBJECTIVE: To investigate the risk of Alzheimer's disease (AD) associated with life style, early exposure to magnetic fields, family history of dementia and other risk factors. METHODS: We conducted a case-control study among the inpatients of Chinese PLA General Hospital in 2000 - 2003. Sixty-two AD cases and 124 controls were selected and matched for age. Univariate and multivariate analyses were conducted using logistic regression model. RESULTS: All subjects were males aged 66 to 102. In univariate analysis, lack of social activities, more physical exercises, early exposure to magnetic fields, suffering from negative life events and family history of dementia were statistically different between two groups (P < 0.05). Multivariate analysis showed that after adjusting for potential confounders, suffering from negative life events, family histories of dementia increased the risk of AD with odds ratio (OR) and 95% confidence interval (CI) 3.27 (1.53 - 6.97) and 5.78 (1.39 - 24.10). Early exposure to magnetic fields seemed a possible risk factor for AD, with OR (95% CI) 2.49 (0.96 - 6.45). The amount of social activities, cigarette smoking and history of cancers were negatively correlated with AD and their ORs (95% CI) were 0.81 (0.72 - 0.92), 0.46 (0.21 - 1.00) and 0.31 (0.12 - 0.82) respectively. CONCLUSION: The study demonstrated that suffering from negative life events and family history of dementia were risk factors for AD, and the early exposure to magnetic fields might also play a role.

Human epithelial beta-defensins 2 and 3 inhibit HIV-1 replication.

OBJECTIVE: Mechanisms underlying mucosal transmission of HIV-1 are incompletely understood. We describe the anti-HIV-1 activity of human beta-defensins (hBD), small cationic molecules that provide protection at mucosal surfaces. METHODS AND RESULTS: HIV-1 induced expression of hBD-2 and -3 mRNA (but not that of hBD-1) 4- to 78-fold, respectively, above baseline in normal human oral epithelial cells. HIV-1 failed to infect these cells, even after 5 days of exposure. Recombinant hBD-1 had no antiviral activity, while rhBD-2 and rhBD-3 showed concentration-dependent inhibition of HIV-1 replication without cellular toxicity. Inhibition was greater against CXCR4-tropic than against the CCR5-tropic HIV-1 isolates. hBD-2 and hBD-3 induced an irreversible effect on virion infectivity, with electron microscopy confirming binding of hBDs to viral particles. Finally, hBD-2 and -3 induced downmodulation of the HIV-1 coreceptor CXCR4 (but not CCR5) in peripheral blood mononuclear cells and T lymphocytic cells as shown by confocal microscopy and flow cytometry. CONCLUSIONS: This study shows for the first time that HIV-1 induces beta-defensin expression in human oral epithelial cells and that beta-defensins block HIV-1 replication via a direct interaction with virions and through modulation of the CXCR4 coreceptor. These properties may be exploited as strategies for mucosal protection against HIV-1 transmission.

[Analysis of the correlated factors of the early mortality after orthotopic liver transplantation].

OBJECTIVE: To investigate the correlated factors of early mortality after orthotopic liver transplantation. METHODS: The preoperative liver function, coagulative status, the graft's warm and cold ischemia times and the details of the operations of 42 patients were summarized retrospectively and the comparison between survival and death patients performed to analyse the correlated factors of early death after orthotopic liver transplantation. RESULTS: Thirteen of the 42 patients died early after the transplantation. Two died of vascular complication and two fo graft primary nonfunction. The comparison between those without identified causes (9 patients, in death group) and survivors (29 patients, in survival group) showed that the patients in death group had a poorer preoperative liver function and coagulative status than those in survival group, the blood loss was more than that in survival group, the length of time of operation, the duration of hypotension and anhepatic time were also longer than those in survival group. CONCLUSION: The poorer preoperative liver function, and especially the coagulative status were the main causes of early mortality after orthotopic liver transplantation, the vascular complications and the graft primary nonfunction were the other reasons that led to some patients' deaths.

Long-term depression is not induced by low-frequency stimulation in rat visual cortex in vivo: a possible preventing role of endogenous brain-derived neurotrophic factor.

Low-frequency stimulation (LFS) at 1 Hz for 15 min is an effective protocol to induce homosynaptic long-term depression (LTD) in visual cortical slices. It is reported that LFS becomes ineffective when brain-derived neurotrophic factor (BDNF) is applied to slices. It is not known, however, whether such a protocol induces LTD in visual cortex in vivo, and whether endogenous BDNF has the same or similar action. To address these questions, we recorded field potentials of rat visual cortex evoked by stimulation of lateral geniculate nucleus, white matter, or cortical layer IV. We found that LFS did not induce LTD of cortical responses in vivo. To test the possibility that spontaneous activity from retinas would interfere with the induction of LTD, both eyes were removed or inactivated by tetrodotoxin. LTD was not induced in these conditions either. To test whether the difference in temperature between the two preparations is a factor for the discrepancy, the temperature of slices was increased from 31 to 37 degrees C. LTD was induced in slices at either temperature. Then, we hypothesized that endogenous BNDF and its receptors, TrkB, prevent the induction of LTD. To test this, we infused the cortex with an inhibitor of Trk receptor tyrosine kinases, anti-TrkB IgG1, anti-BDNF, and anti-neurotrophin 4/5 antibodies. LTD was induced when the BDNF-TrkB system was blocked. In slices, the level of phosphorylation of Trks was found to decrease with time. These results indicate that activation of TrkB signal pathway prevents LFS from inducing synaptic depression in visual cortex in vivo.

[Relationship between cytopenia and cytogenetic response in imatinib mesylate treated Ph-positive chronic myeloid leukemia in chronic phase patients].

OBJECTIVES: To evaluate the relationship between cytopenia and cytogenetic response in Imatinib mesylate treated Ph-positive chronic myeloid leukemia (CML) in chronic phase patients. METHODS: Fifty-four patients with Ph + CML in chronic phase received oral administration of Imatinib 400 or 600 mg once a day for 18 months. RESULTS: In the early phase of Imatinib treatment, rates of severe leukopenia (leukocyte < 2.0 x 10(9) L-1), anemia (hemoglobin < 100 g.L-1) and severe thrombocytopenia (platelet < 50 x 10(9) L-1) were 14.8%, 37.0% and 27.8%, respectively. Hemocytes recovered in most patients with continued therapy. Treatment was interrupted or dosage reduced in a few patients. The lower the hemoglobin and higher the platelet before the regime, the lower the nadir of leukocytes and hemoglobin counts during the treatment. The lower the platelet count before the regime, the lower the nadir of platelets during the treatment. Risk factors for severe leukopenia were thrombocytosis (> or = 500 x 10(9) L-1) and basophilia > or = 5% before the treatment. Risk factors for severe thrombocytopenia were thrombocytopenia (< 100 x 10(9) L-1) and basophilia > or = 5% before the treatment. During the 12-month treatment with Imatinib, the statistically significant lower probabilities of cytogenetic response were observed at all checked points in patients with severe leukopenia, at the end of 3 and 6 months in patients with anemia, at the end of 3 months in patients with severe thrombocytopenia. CONCLUSION: Cytopenia is a common side effect in patients with CML in chronic phase treated with Imatinib mesylate. Severe leukopenia is associated with a sustained lower major cytogenetic response, whereas anemia and severe thrombocytopenia influence more for the first 6 months.

[Liver transplantation for advanced primary hepatocellular carcinoma].

OBJECTIVE: To investigate the value of liver transplantation for late hepatocellular carcinoma. METHODS: Thirty-six patients were treated by liver transplantation from August 2000 to February 2002, of which 15 patients had had advanced hepatocellular carcinoma and thirteen of these 15 patients were evaluated for results. RESULTS: The 1-year survival rate was 86% (6/7). Only one patient died of recurrence within 6 months. The tumor-free survival was 5 to 19 months. Till February 2002, two patients have survived for 10 months and 19 months with recurrence. CONCLUSION: In our country, if the patients can afford liver transplantation, advanced hepatocellular carcinoma without extrahepatic metastasis is still indicated for liver transplantation, since some patients may survive relatively long.
