Timing of administration of dexamethasone or the nitric oxide synthase inhibitor, nitro-L-arginine methyl ester, is critical for effective treatment of ischaemia-reperfusion injury to rat skeletal muscle.

1. The effects of the nitric oxide synthase (NOS) inhibitors, NG-nitro-L-arginine-methyl ester (L-NAME), nitroiminoethyl-L-ornithine and S. methylisothiourea on skeletal muscle survival following 2 h of tourniquet ischaemia and 24 h of reperfusion were compared with those of the anti-inflammatory steroid, dexamethasone. 2. Administration of each of the NOS inhibitors or dexamethasone 30 min before reperfusion reduced the degree of skeletal muscle necrosis 24 h after reperfusion. 3. The influence of timing of drug administration was investigated. L-NAME administered 30 min before reperfusion, at 3 h after reperfusion, but not thereafter, significantly improved muscle survival compared with saline-treated controls. Dexamethasone administered 30 min before, or at 3 or 8 h after reperfusion, but not at 16 h, significantly improved muscle survival, but neither agent had protective effects when administered before ischaemia. 4. After 8 h of reperfusion of ischaemic skeletal muscle, cell-free homogenates contained Ca(2+)-independent (inducible) NOS activity which was reduced in dexamethasone-treated (2.5 mg/kg) rats. Furthermore, inducible NOS mRNA levels, as detected by reverse transcriptase-PCR, were increased after 8 h of reperfusion in saline, but not in dexamethasone-treated rats. 5. These data suggest a significant deleterious effect of endogenous NO which may be restricted to the first 3 h of the reperfusion phase of ischaemia-reperfusion injury, and raise the possibility of effective treatment of incipient reperfusion injury, even after several hours of reperfusion.

Constitutive mutations of the Saccharomyces cerevisiae MAL-activator genes MAL23, MAL43, MAL63, and mal64.

We report the sequence of several MAL-activator genes, including inducible, constitutive, and noninducible alleles of MAL23, MAL43, MAL63, and mal64. Constitutive alleles of MAL23 and MAL43 vary considerably from inducible alleles in their C-terminal domain, with many of the alterations clustered and common to both alleles. The 27 alterations from residues 238-461 of Mal43-C protein are sufficient for constitutivity, but the minimal number of alterations needed for the constitutive phenotype could not be determined. The sequence of mal64, a nonfunctional homologue of MAL63, revealed that Mal64p is 85% identical to Mal63p. Two mutations that activate mal64 and cause constitutivity are nonsense mutations resulting in truncated proteins of 306 and 282 residues. We conclude that the C-terminal region of the MAL-activator, from residues 283-470, contains a maltose-responsive negative regulatory domain, and that extensive mutation or deletion of the entire region causes loss of the negative regulatory function. Additionally, certain sequence elements in the region appear to be necessary for efficient induction of the full-length Mal63 activator protein. These studies highlight the role of ectopic recombination as an important mechanism of mutagenesis of the telomere-associated family of MAL loci.

Clinical diagnostic accuracy of faecal occult blood test for anal diseases.

PURPOSE: To investigate the diagnostic accuracy of faecal occult blood test (FOBT) for anal diseases and to evaluate the relationship of this disorder to the results of FOBT. METHODS: In a hospital-based retrospective study, FOBT using faecal samples collected on two consecutive days was performed on patients with anal disease, such as internal as well as external haemorrhoids and anal fissure, patients with colorectal cancer and healthy subjects to evaluate the diagnostic accuracy of FOBT. In a screening-programme-based cross-sectional study, the subjects who underwent colorectal cancer screening by FOBT using the 2-day method were divided into two groups according to the results of a questionnaire on anal diseases; the positive rate of FOBT as well as the predictive value for colorectal cancer were determined in the two groups. RESULTS: Among 300 cases, including 100 anal diseases, 100 colorectal cancers and 100 healthy subjects, the test was positive in 12 anal diseases, 82 colorectal cancers and six healthy subjects. The sensitivity to anal diseases and colorectal cancer was calculated to be 12% and 82%, respectively, and the specificity was 95%. A significant difference was noted in the sensitivity between anal diseases and colorectal cancer (P < 0.001). Among 21,724 subjects, who underwent colorectal cancer screening, 2395 subjects were positive for anal diseases, and 19,329 subjects were negative. The positive rate of FOBT was 7.2 for the anal diseases group and 6.8 for the non-anal diseases group, and the predictive value for colorectal cancer was 2.9 and 3.0, indicating no substantial difference between the two groups. CONCLUSIONS: These results reveal that FOBT is inferior for the diagnosis of anal diseases, and anal diseases have little relationship to the results of FOBT.

The relationship of various tibia bone measurements in hens.

Tibias from 50 Single Comb White Leghorn hens were used to study the relationships among various bone measurements. The bone ash (BA), length, wet bone volume (V), fat-free dry matter (FFDM), fat-free bone density (BD; mass per unit bone volume), and bone breaking force (BF) were measured, and the BA concentration (BA/V) and BA as a percentage of FFDM (BA/FFDM) were calculated. Bone ash was highly correlated with FFDM (r = 0.975). The BD was poorly correlated with any of the parameters measured in the present study. The BF was highly correlated with BA (r = 0.922) and FFDM (r = 0.918), but less well correlated with BA/FFDM (r = 0.496) and BA/V (r = 0.694). However, the inclusion of volume2 significantly improved the BF predicting models when BA/V was used as predictor (R2 = 0.855), indicating that BF is not only a function of BA density but also of bone size. The BF adjusted for V may better reflect treatment effect upon bone status than BF alone. The coefficient of variation of BA/FFDM and BD was smaller (0.007) than the CV with BA/V (0.026), which may indicate the BA/V is potentially a better indicator of bone status than BA/FFDM and BD. Bone length was poorly correlated with BA, FFDM, and BF, but highly correlated with V (r = 0.844). The volume of the same bone measured by the water weight change method gave the most consistent measurement. The data showed that BA/V may be a better measurement to reflect the bone status changes of hens than other measurements tested. When BA, FFDM, or BF are used in laying hen's bone status studies, the data analysis needs to be adjusted for V, an V is closely correlated with these variables.

The relationship of calcium intake, source, size, solubility in vitro and in vivo, and gizzard limestone retention in laying hens.

A 10-d trial was conducted to investigate the relationship of Ca particle size (CPS), dietary Ca level (DCL), limestone source (LS), limestone solubility in vitro (LST) and in vivo (LSV), and limestone retention in the gizzard (LRG). A total of 120 molted Leghorn hens, 88 wk of age, were randomly assigned into a 2 x 3 x 4 factorial arrangement of treatments with LS (A and B), DCL (1.95, 3.72, and 5.32%), and CPS of each limestone source (average U.S. Screen Nos. 5, 8, 14, and 27). The in vitro solubility of the four respective particle sizes were 29.8, 45.8, 49.3, and 63.1% for Source A; and 36.3, 54.8, 57.7, and 67.6% for Source B. The limestone retention in the gizzard of Source A was greater than that of Source B. The limestone retention in the gizzard was increased as in vitro solubility decreased (P < 0.001) or dietary Ca level increased (P < 0.001). The in vivo solubility of the limestone was decreased as dietary Ca level increased (P < 0.001). The in vivo solubility was negatively correlated (P < 0.05) with in vitro solubility for Source A when Ca was fed at 3.72%. The data showed that larger particle size limestone (> 0.8 mm) with lower in vitro solubility (30 to 50%) was retained in the gizzard for a longer time, which increased the in vivo solubility (94% maximum). The results support the concept that larger particle size or lower in vitro solubility may increase Ca retention for layers.

A feedback loop regulates the switch from one sigma factor to the next in the cascade controlling Bacillus subtilis mother cell gene expression.

Regulation of gene expression in the mother cell compartment of sporulating Bacillus subtilis involves sequential activation and inactivation of several transcription factors. Among them are two sigma factors, sigmaE and sigmaK, and a DNA-binding protein, SpoIIID. A decrease in the level of SpoIIID is thought to relieve its repressive effect on transcription by sigmaK RNA polymerase of certain spore coat genes. Previous studies showed that sigmaK negatively regulates the level of spoIIID mRNA. Here, it is shown that sigmaK does not affect the stability of spoIIID mRNA. Rather, sigmaK appears to negatively regulate the synthesis of spoIIID mRNA by accelerating the disappearance of sigmaE RNA polymerase, which transcribes spoIIID. As sigmaK begins to accumulate by 4 h into sporulation, the sigmaE level drops rapidly in wild-type cells but remains twofold to fivefold higher in sigK mutant cells during the subsequent 4 h. In a strain engineered to produce sigmaK 1 h earlier than normal, twofold less sigmaE than that in wild-type cells accumulates. SigmaK did not detectably alter the stability of sigmaE in pulse-chase experiments. However, beta-galactosidase expression from a sigE-lacZ transcriptional fusion showed a pattern similar to the level of sigmaE protein in sigK mutant cells and cells prematurely expressing sigmaK. These results suggest that the appearance of sigmaK initiates a negative feedback loop controlling not only transcription of spoIIID, but the entire sigmaE regulon, by directly or indirectly inhibiting the transcription of sigE.

Bacillus subtilis SpoIIID protein binds to two sites in the spoVD promoter and represses transcription by sigmaE RNA polymerase.

The Bacillus subtilis spoVD gene encodes a penicillin-binding protein required for spore morphogenesis. SpoIIID is a sequence-specific DNA-binding protein that activates or represses the transcription of many different genes. We have defined the spoVD promoter region and demonstrated that it is recognized by sigmaE RNA polymerase in vitro and that SpoIIID represses spoVD transcription. Two strong SpoIIID-binding sites were mapped in the spoVD promoter region, one overlapping the -35 region and the other encompassing the -10 region and the transcriptional start site.

Autoantibodies to IA-2 in IDDM: location of major antigenic determinants.

Thirty-three IDDM sera that immunoprecipitated full-length IA-2 were tested for reactivity with different fragments of the IA-2 molecule. The fragments were prepared by PCR amplification of IA-2 cDNA and by expression in a rabbit reticulocyte transcription/translation system. Whereas all 33 sera reacted with the intracellular domain (amino acid 604 to 979), none of the sera reacted with the extracellular domain of IA-2 (amino acid 31 to 577). Analysis of the reactivity of IDDM sera with the different regions of the intracellular domain showed that 94% (31 of the 33) reacted with the COOH-terminus (amino acid 771 to 979), 40% reacted with the NH2-terminus (amino acid 604 to 776), and 40% reacted with the middle portion (amino acid 692 to 875). Of the 31 sera that reacted with the COOH-terminus, 14 of these reacted only with the COOH-terminus and with no other region. Of the 13 sera that reacted with the NH2-terminus, only one reacted exclusively with the NH2-terminus. Treatments of the different domains of IA-2 with trypsin showed that only the COOH-terminus was resistant to trypsin, arguing that it is from this region of the IA-2 molecule that the 40-kDa tryptic fragment from insulinoma cells is derived. From these experiments, it is concluded that the major antigenic determinant of IA-2 is located at the COOH-terminus and that minor antigenic determinants are located at the NH2-terminus and middle portion of the intracellular domain.

High-level expression of H-ras and c-myc oncogenes in mycoplasma-mediated malignant cell transformation.

C3H mouse embryo cells, which normally have low inherent spontaneous transformation, underwent malignant transformation while chronically infected with Mycoplasma fermentans or Mycoplasma penetrans. This mycoplasma-mediated oncogenic process had long latency (more than 7 weeks of persistent mycoplasmal infection) and showed multistage progression characterized by reversibility and irreversibility of malignant properties upon removal of M. fermentans from culture. Marked expression of H-ras and c-myc mRNA, but not N-myc, src, N-ras, or p53 mRNA, was found in the mycoplasma-transformed C3H cells that exhibited characteristic malignant properties of morphological changes and uncontrolled cell growth. However, at least up to the eleventh week of persistent mycoplasma infection, the marked expression of H-ras or c-myc mRNA in C3H cells depended on continued presence of the mycoplasma in culture. H-ras or c-myc mRNA rapidly declined to the undetectable low levels of nontransformed parental C3H cells, and all malignant properties of the once-fully-transformed C3H cells quickly reversed, if M. fermentans was eradicated from culture. In comparison, infection with M. penetrans for 7 or 11 weeks also induced a high level of H-ras, but not c-myc, mRNA expression in C3H cells. Despite having prominent amount of steady-state H-ras mRNA, these M. penetrans-infected C3H cells did not show any sign of malignant transformation. Thus, marked expression of H-ras gene alone was not sufficient to effect transformation in C3H cells. Interestingly, after a further prolonged (18 weeks) infection with either M. fermentans or M. penetrans, C3H cells revealed prominent chromosomal changes, expressed constitutively (with or without the presence of the transforming mycoplasmas) at high levels of both H-ras and c-myc mRNA and became permanently transformed. These cells were able to form tumors in animals.

[Methods for detecting mycobacterium tuberculosis in the air of wards for tuberculosis patients].

OBJECTIVE: To explore useful methods for detecting M. tuberculosis in the air of inpatient's wards of tuberculosis. METHODS: Thirty samples were collected by using LWC-I air sampler from the air of wards housing sputum positive patients. The samples were washed with 0.85% sodium chloride solution, then centrifuged, and finally detected M. tuberculosis by L-J medium culture, BACTEC method, guinea pig experiment, PCR and Southern blotting. RESULTS: The positive rates by PCR and Southern blotting were 47% (14/30) and 60% (18/30) respectively. In the experimental group containing 30 guinea pigs, two were positive by culture from the animal tissues (spleens or lungs), and another one was pathologically positive. Ten guinea pigs in control group were negative. Thirty samples were all negative by L-J and BACTEC. CONCLUSIONS: It is indicated that using air sampling to detect M. tuberculosis in the air is practicable. PCR and Southern blotting are very helpful in detecting M. tuberculosis in the air because they have the advantages of high sensibility and specificity, and they remarkably shorten the time of detection. Animal experiment can provide compensation to PCR and Southern blotting.

[Studies on cell senescence induced by D-galactose in cultured neurons and fibroblasts].

The cell senescence induced by D-galactose (D-gal) in cultured rat fetal brain neurons and human fetal lung fibroblast was investigated in vivo and in vitro. The results showed pronounced decreases in nerve cell diameters, number of process and percentage of multiprocess neurons including "pyramidal" and "stellate" shaped and a rather high level of mortality rate observed in D-gal-treated (8 g/L) neurons. Lessening of the proliferating population of lung fibroblast taken from rat treated with D-gal (50 mg/kg.d, subcutanous injection for 6 weeks) was observed. We have also determined the changes of cell cycle in human fetal lung fibroblast cells treated with D-gal (8 g/L): the proportion of the G0-G1 cells increased but the G2-M and DNA level and the results showed that decreased and the cell growth rate declined in comparison with those of the control. Lipid peroxidation was observed in both kinds of cells because D-gal was proved to have action of decreasing SOD activity and increasing MDA content, the results found in D-gal-treated rats were similar to those observed in hypoxanthine-xanthine oxidation reaction system (O2-), indicating that reactive oxygen was generated in the course of D-gal metabolism. It is concluded that D-gal has the hastening effect on cell senescence and oxidative stress might be responsible for this aging effect.

[Effects of endothelin-1 on the progesterone production of rat preovulatory ovarian granulosa cells].

The effect of endothelin-1 (ET) on progesterone production of isolated rat granulosa cells and its mechanism were investigated. ET could inhibit the hCG-stimulated progesterone production significantly at the concentration of 10(-8) mol/L (P < 0.05, n = 6) and highly significantly at the concentration of 10(-7) mol/L (P < 0.05, n = 6), but ET did not inhibit the basal progesterone production at various concentrations (10(-10)-10(-7) mol/L). Further experiments showed that the inhibitory effect of ET on the hCG-stimulated progesterone production could be reversed by rabbit antiserum to ET (ET-A, 1:1000) or cAMP (10(5) mol/L). ET could decrease the binding sites and the affinity constant (ka) of LH/hCG receptor on granulosa cells. All these findings suggest that ET may be an intraovarian regulatory factor which may inhibit progesterone production stimulated by hCG from rat granulosa cells through inerfering LH/hCG receptor function and cAMP formation.

[The injury of Xenopus laevis oocytes membrane and its acetylcholine receptor by free radical and the protection of lycium barbarum polysaccharide].

The injury of the Xenopus laevis oocytes membrane and its acetylcholine receptor by free radicals was observed by exposing Xenopus laevis oocytes to the xanthine oxidase hypoxanthine (XO-HPX) reaction system. The results showed that under the action of free radicals the passive electrical membrane parameters changed significantly, this effect was directly proportional to the concentration of XO-HPX and the reaction time. The membrane function was not affected by 2 hours reaction with XO-HPX, while significant decrease of various membrane functional indices was found after more than 4 hours reaction, the Ach polarization decreased, the rise time prolonged, the degree of depolarization decreased, the decay 1/2 time shortened. SOD may be used to eliminate the effect of free radicals on the above mentioned membrane parameters. Lycium barbarum polysaccharide (LBP) may be used to improve the passive electrical membrane parameters of the injured membrane, but it does not lead to the recovery of Ach depolarization. The results suggest that the injury of cell membrane and Ach receptor by XO-HPX reaction system may be caused by the production of O2- free radicals. LBP may antagonize the action of free radicals on the membrane while it is ineffective in muscarine receptor injury.

[Reoperations on prosthetic heart valves: an analysis of 64 cases].

During the period of Fabruary 1988 to May 1996, 64 reoperations were performed for prosthetic valve malfunction. Degeneration of bioprosthesis occurred in 46 patients, failure of mechanical prosthesis in 9, and periprosthetic leakage without evidence of infection in 9. In accordance with the status of cardiac function, 9 patients with acute dysfunction of mechanical prosthesis received emergency operations, and the remaining 55 patients with bioprosthetic valve failure or periprosthetic leakage received selective operation. There were 8 early deaths with a overall hsopital mortality rate of 12.5%. Three of the deaths occurred among the 55 selective procedures for a mortality of 5.3% and 3 of the deaths occurred among the 9 emergence procedures for mortality of 33.3%. The 56 long-term survivors were followed up from 3 months to 7 years (mean 2.1 years). There were 3 late deaths (5.3%). Forty-eight patients have been living more than 1 year. Their cardiac function after reoperation was class I (42 patients), class II (5), and class III (1). The timing of reoperation and surgical technique were discussed.

[Maze procedure for chronic atrial fibrillation associated with mitral valve disease].

From May 1995 to October 1996, 20 patients with mitral valve disease underwent the maze procedure for chronic atrial fibrillation and mitral valve replacement or mitral valvuloplasty. Epicardial mapping data demonstrated that the large macrorecurre flutter circuit was located in left atrium (14/20) and complex fibrillation in right atrium (18/20) of the majority of patients. No early death occurred. 20 patients were followed up for at least 3 months (range 3-20 months) and 14 patients for at least 1 year after operation, sinus rhythm and atrioventricular synchrony were restored (100%), atrial fibrillation was not induced by electrophysiologic study. The right and left atrial transport function was preserved (100%) by Doppler echocardiogram tracings. One patient died from acute necrotic hepatitis, 4 and half months after operation. Cox maze procedure was modified, atrial flutter and atrial fibrillation never occurred. In this study, the electrophysiologic mechanism of chronic atrial fibrillation associated with mitral valve disease, indications of operation and clinical results are discussed.

[Surgery of substernal thyroid goiter].

A retrospective study on 87 substernal thyroid nodules operated between 1965 and 1994 was made. Among them 73.6% were goiters, 14.9% adenoma and 11.5% malignancy. Resection via cervical collar incision was performed in 70% of all cases, sternotomy in 16% and thoracotomy in 14%. A cervical collar incision was adequate in 91% of 43 cases after 1985, only one goiter was completed by sternotomy. Morbidity rate of complications was 41.4%, while the rate of recurrent laryngeal nerve paralysis was 20.7%. Surgery via cervical collar incision for all retrosternal thyroid nodules was advised and sternotomy or thoracotomy are suggested.
