Affective state and voice: results of a pilot study with six depressive patients.

As part of an investigation into the nonverbal information in human speech, a pilot study with depressive patients was designed in order to model the global affective state as a function of time and in terms of speech parameters. Over a two-week period, six patients were examined six times at two-day intervals under comparable experimental conditions. The psychopathologic status of these patients was assessed during a psychiatric exploration half an hour before the actual examination was carried out. Besides the solution of the baisc methodological problems, a principal goal of this study was to test the ability and cooperation of depressive patients when asked to speak in the laboratory. It turned out that even scalar parameters such as energy and dynamics give insight into the intraindividual variation of global affective state, and that the examination procedure is practicable even it patients are considerably handicapped by their illness.

Unsolved problems in the pharmacotherapy of depression.

Five unsolved problems in the pharmacotherapy of depression are discussed: (a) it is not possible to differentiate endogenous and nonendogenous depression; (b) a selective efficacy of serotonin and noradrenaline reuptake inhibitors cannot be demonstrated; (c) the relationship between plasma levels and antidepressant effect is still unclear: plasma levels are influenced by pharmacogenetic factors, age, route of application, and concomitant treatment with other drugs; (d) evidence is growing for the development of tolerance towards therapeutic effects of antidepressants; (e) no pretreatment variable allows prediction of treatment response: the best predictor is the initial response to treatment.

Acetylation of maprotiline and desmethylmaprotiline in depressive patients phenotyped with sulfamidine, debrisoquine, and mephenytoin.

A gas chromatographic/mass spectrometric method (using either electron impact or chemical ionisation with methane or ammonia) is described for the quantitative analysis of maprotiline (MP, Ludiomil), N-acetylmaprotiline (AcMP) and N-acetyldesmethylmaprotiline (AcDMP) in whole blood or plasma. In two groups (A and B) of 82 and 53 depressive patients treated clinically with MP for 10 and 21 days, respectively, plasma and whole blood MP was monitored during the treatment. In group A, all subjects were phenotyped with debrisoquine and mephenytoin, and 44 with sulfamidine. 5 patients were poor metabolizers of debrisoquine and 2 of mephenytoin; 18 subjects were fast acetylators of sulfamidine. Traces of AcMP were found only in two patients. AcDMP was present in levels below 2 ng/ml in the plasma of most of the patients in group A. In group B, AcDMP levels between 2.4-14.6 ng/ml of whole blood were found in 9 patients. The mass spectral data suggest the presence of another, unknown MP metabolite interfering partly with the analysis of AcDMP. The presence of AcDMP seemed not to be related to the phenotype of the patients as determined by the pharmacogenetic tests.

Light treatment of seasonal affective disorder in Switzerland.

Seasonal Affective Disorder (SAD) has been characterised by two or more depressive episodes in autumn or winter (with remission the following spring or summer), decreased energy, increased sleep, increased appetite, weight gain and carbohydrate craving. SAD patients were identified in a Swiss-German population; 22 participated in a light-therapy protocol (1 week bright white light 2,500 lux or dim yellow light 250 lux, from 06-08 h and 18-20 h). Both observer and self-ratings indicated a significant diminution of depressive symptoms with both lights. One week after withdrawal from yellow light, depression ratings relapsed to previous values; remission lasted longer after bright white light. Global VAS self-rating scales for "mood" and "well-being" however, and the Hamilton scale for atypical SAD symptoms, differentiated clearly between bright and dim light: only bright light showed an improvement that persisted after withdrawal. These results suggest that even though a placebo effect cannot be excluded, 4 h explicit light exposure/day may not be a negligible quantity. Light treatment promises to be a useful non-pharmacological intervention in certain forms of depressive illness.

The AMDP-system in clinical psychopharmacology.

The AMDP-System has been developed in Europe by the Association for Methodology and Documentation in Psychiatry the standardised documentation of psychiatric files and for the measurement of change in drug trials. It is increasingly used in the French-speaking and German-speaking countries by clinical psychopharmacologists for teaching classical German psychopathological concepts to students and for the multicentre and multifactorial evaluation of drug changes. The AMDP-System consists of three anamnestic forms and of two comprehensive rating scales on present psychopathology (100 items + write-in items) and somatic complaints (40 items + write-in items).

Clinical Trials as Topic↗

[The value of clinical studies].

Not only to perform scientific work but also to read scientific periodicals critically some elementary basic knowledge in methodology and statistic is needed. In the present paper are given further particulars on three pairs of concept: Open and double-blind trials, Statistical significance and clinical relevance, Alpha and Beta errors.

Clinical Trials as Topic↗

Comparison of AMP system and comprehensive psychopathological rating scale with regard to contents.

Using a sample of 170 patients the psychopathological contents of the AMP system and the Comprehensive Psychopathological Rating Scale (CPRS) were compared by canonical correlations. The nine primary AMP scales predict 86% of the variance of the three CPRS scales. 64% of the variance of the nine AMP scales is explained by the CPRS scales. The secondary scales are nearly equivalent. The univariate analysis showed that all AMP scales with the exception of the 'hostility syndrome' and the 'catatonic syndrome' correlate highly with one of the psychopathological CPRS scales.

Scale construction of the Swedish CPRS version.

A sample of 161 schizophrenic and depressive patients were interviewed in a Stockholm hospital to find out whether the Comprehensive Psychopathological Rating Scale construction carried out by Maurer et al. [Int. Pharmacopsychiat. 17: 338-353, 1982] and valid for German-speaking areas could be reproduced with a Swedish-speaking sample. Only the superimposed 2-factor solution resembled, in its factor-structure, that of Maurer et al. (depressive and schizophrenic syndrome). The fact that a 'depressive' syndrome was found instead of a 'manic-depressive' syndrome, is to be attributed to the different sample compositions.

[Difficulties in neuroleptic treatment of schizophrenic patients].

During the treatment of schizophrenic patients with antipsychotic drugs the following difficulties must be considered: Lack of predictability of therapeutic outcome, extrapyramidal side effects, "pharmacogenic" depressions. The most important presupposition for a successful handling of these difficulties is an intensive psychopharmacological training of psychiatrists.

Antipsychotic Agents↗

Antipsychotic efficacy of fluperlapine. An open multicenter trial.

In an open multicenter trial 46 schizophrenic patients were treated with fluperlapine for 20 days. A mean daily dosage of 300-400 mg appeared to be an effective antipsychotic treatment in most cases. The marked antipsychotic effect was accompanied by a good improvement of depressive symptoms. Extrapyramidal side effects were extremely rare.

Comparison of the AMP system and the CPRS with regard to interrater reliability.

The interrater reliability of the AMP system and the Comprehensive Psychiatric Rating Scale (CPRS) was compared in a sample of 30 hospitalized schizophrenic or depressive patients. The CPRS proved to have on average a slightly higher reliability on both the level of items and primary scales. The reliability of the secondary scales was nearly identical.

Results of a multicenter AMDP study with fluperlapine in schizophrenic patients.

In an open multicenter trial (7 hospitals) 85 schizophrenic patients were treated with 3-fluoro-6-(4-methyl-piperazinyl)- 11H -dibenz[b,e]azepine ( fluperlapine , NB 106-689) during 20 days. Symptomatology was documented with the AMDP system on days 0, 1, 5, 10, 15 and 20. A mean daily dosage of about 400 mg caused a significant antipsychotic effect and pronounced improvement of depressive symptoms. Fluperlapine was well tolerated. Extra-pyramidal side-effects were very rare.
