Kinetics of interstitially administered monoclonal antibodies for purposes of lymphoscintigraphy.

The clearance rates of radiolabeled murine monoclonal intact IgG, F(ab')2 Fab and of an IgM following subcutaneous administration were evaluated in normal mice and rats using nuclear imaging and counting techniques. These studies suggest no significant difference in clearance rate exists between intact IgG and its F(ab')2 fragment, and little difference between these moieties and intact IgM. Fab is cleared considerably faster than the others, however. While significant differences in clearance rates exist, the magnitude of the differences are not as large as those following intravenous injection particularly when ambulation by the animal is allowed. When ambulation is allowed, clearance rates of all classes and fragments are accelerated and quite similar. Injection into the subcutaneous tissues of the footpad results in consistently faster clearance than an injection into the subcutaneous tissues of the abdomen. Ambulation considerably increased the clearance of antibodies, presumably by increasing lymph flow. These studies imply that the choice of intact antibody versus fragments for kinetic reasons may be less critical (particularly if ambulation is allowed) by the subcutaneous as compared with the intravenous delivery route. This kinetic information should be useful in designing imaging protocols with radiolabeled antibodies administered subcutaneously for purposes of imaging disease processes involving the lymphatics.

Development of a behavioral test of visuospatial neglect.

The Rivermead Behavioral Inattention Test (RBIT), consisting of nine items sampling activities of daily living, was administered to 28 patients after unilateral right (20) or left (8) cerebrovascular accidents, and to 14 non-brain-damaged controls. All patients were tested on two parallel forms of the RBIT with order of presentation balanced and on at least two of six conventional tests of visual neglect. Control subjects were tested on either form 1 or form 2 of the RBIT. Interrater reliability of scoring was tested on seven subjects chosen at random. Using control scores to determine the cutoff point between visual inattention and noninattention, 14 patients (50%) showed evidence of visuospatial neglect on the RBIT. Correlation between the two forms of the test was 0.83. The RBIT also correlated well with five of the conventional tests. Interrater reliability was 100%. The RBIT appears to be a valid and reliable test of visuospatial neglect and one which is likely to provide more information about everyday problems than existing measures of neglect.

Motility of rasH oncogene transformed NIH-3T3 cells.

A series of rasH oncogene-transformed cell lines were established from NIH-3T3 cells using the calcium phosphate precipitation method of transfection. The transfectant lines formed highly-invasive tumors when injected into athymic mice while the parent cells did not. When examined for motility in the micropore filter assay, all of the transfectant lines were motile in response to either laminin or fibronectin while the parent cells were not stimulated by either factor. These studies, therefore, indicate that transformation of NIH-3T3 cells with the rasH oncogene results in the generation of cells with increased capacity for motility relative to the parent cells.

The radiological and urodynamic significance of large bladder diverticula.

Large acquired bladder diverticula in adults may cause severe urodynamic disturbance and exacerbate difficulty in micturition. They may accommodate a large proportion of the contents of the bladder during attempted voiding, thus decreasing the effect of the detrusor muscle contraction. The post-micturition radiographs on intravenous urography, after the diverticula have emptied again, may only show a large intravesical post-micturition residue and give little information as to the size of the diverticula during the act of micturition.

A case of visual agnosia showing a disorder of pre-semantic visual classification.

A case (J.R.) of associative visual agnosia (loss of meaning for visually presented objects without any serious sensory disorder) is reported. When asked to match a given name to sample the patient made both visual and semantic errors. Whereas semantic errors were reduced by priming, visual errors were intractable. It is argued that there is a disorder at the stage of pre-semantic abstraction, similar to that postulated elsewhere for some cases of deep dyslexia, contributing to the agnosia.

Aldosterone studies in obese patients with hypertension.

Ten patients with hypertension and obesity were studied during a program of weight loss on an unrestricted sodium diet. The study showed that weight loss during the ten month period was accompanied by a significant decrease in urinary aldosterone, tetrahydroaldosterone -3-glucuronide and plasma renin activity values. It was also demonstrated that successful reduction in body weight was associated with a reduction in blood pressure. It is postulated that blood pressure reduction in obese patients during weight reduction may depend on decreases in aldosterone and plasma renin activity. The reduction in levels of tetrahydroaldosterone-3-glucuronide is due to an energy related depression of the glucuronidation process caused by carbohydrate deficiency. It is postulated that the lowered rate of metabolism of aldosterone as measured by the diminished formation of the metabolite tetrahydroaldosterone-3-glucuronide is an important determinant of blood pressure status during weight reduction.
