Isotachophoresis as a useful tool for monitoring neurological complications of acute leukaemia in children.

Cerebrospinal fluid proteins from 42 children with acute lymphoblastic leukaemia were analysed by isotachophoresis. The isotachopherograms of cerebrospinal fluid taken from patients undergoing central nervous system prophylaxis with neurological complications showed an increase of several peaks (albumin, prealbumin, and an unidentified peak), and changes in the globulin zone, compared with those from patients who had completed central nervous system prophylaxis for at least six months. The most striking finding was that these alterations were not associated with any other biochemical changes in the cerebrospinal fluid, as assayed by routine analysis. Isotachophoresis may be useful in the monitoring of therapy in children affected with acute lymphoblastic leukaemia.

[Vitamin C in a long-term trial is without effect on experimental carcinogenesis].

This long-term study deals with the effect of ascorbic acid on chemically induced carcinogenesis of the small intestine in rats. Carcinoma was induced in 27 animals by application of N-Ethyl-N'-nitro-N-nitrosoguanidine (ENNG) alone in the drinking water (120 mg/1). The average survival time was 238 ( 40) days. The addition of large amounts of ascorbic acid to the food (3 g/100 g food) did not suppress the development of tumors. On the contrary, a significant reduction in the survival time was seen. All animals receiving ENNG and ascorbic acid only lived 207 ( 45) days on average. Neither histological type of tumor spread was influenced by the use of ascorbic acid. Giving ascorbic acid alone had no effect on the survival time and did not lead to changes in the tissue of the small intestine.

Clostridium difficile isolation in leukemic children on maintenance cancer chemotherapy. A preliminary study.

Between December 1982 and November 1983, stool specimens from 15 children with acute lymphoblastic leukemia, who were on maintenance cancer chemotherapy, were examined weekly for the presence of Clostridium difficile and its toxin. Four out of 15 patients were positive for C. difficile: three patients had stool specimens that did not contain toxin, but cultures yielded growth of toxigenic C. difficile on only one occasion. The fourth patient, who had a recent history of hospitalization, particularly aggressive cancer chemotherapy, neutropenia, and antibiotic therapy, excreted both C. difficile and its toxin for at least 1 month. All children were asymptomatic at the time of positive cultures. This preliminary study reveals a low rate of C. difficile colonization in leukemic children on maintenance cancer chemotherapy.

Effect of ovulation on haem metabolism in rabbits.

To investigate the origin of the cyclic changes in the rate of endogenous carbon-monoxide production (nCO) during the menstrual cycle, haem turnover was determined before and after chorion gonadotropic hormone-induced ovulation in six female rabbits. 14C-labelled delta-aminolevulinic acid and glycine were administered and the excretion rate of 14CO (A14CO) was measured for determination of hepatic and bone-marrow haem turnover, respectively. Carbon-monoxide production (nCO) was measured to estimate total haem turnover. After ovulation A14CO was increased significantly the first 2 h of the early labelled peak after 14C-ALA administration and was increased also at the first determination during the early peak after 14C-glycine but statistically not significantly. The total excretion of labelled CO during the period of the early labelled peak was not increased with any of these precursors for haem synthesis. On the other hand nCO was increased 34% (P less than 0.05) during the post-ovulation period. As the increase in 'unassigned' haem turnover was small and may be unaccompanied by a contemporary increase in bilirubin/CO production, it was concluded that the increase in nCO during the post-ovulation period essentially depends on increased destruction of circulating red cells in the rabbit.

A new method for estimating the trend in the fertility rate.

"This article describes the method devised by Gerard Calot...for estimating monthly TPFRs [total period fertility rates] when only provisional monthly figures for numbers of births are available. The application of the Calot method, together with the US Bureau of the Census X-11 seasonal adjustment package, for computing the trend in the England and Wales fertility rate is also described."

Assessment of hemolysis in regular hemodialysis patients by measuring carbon monoxide production rate.

In 17 patients requiring regular hemodialysis, inter- and intradialytic hemoglobin catabolism measured as endogenous carbon monoxide production (VCO) were studied before (9 patients) and after (8 patients) the introduction of water treatment. Before the water treatment, VCo increased significantly from inter- to intradialytic periods whereas insignificant changes were noted after the water treatment. Thus water treatment leads to less destruction of erythrocytes in connection with hemodialysis. However, hemolysis associated with hemodialysis does not play a major role as a cause of anemia in these patients.

[Ascorbic acid and cancer of the duodenum. An experimental study (author's transl)].

This study applies to the agency of vitamin C on chemical carcinogenesis in the small intestine of rats. Administration of N-Ethyl-N'-nitro-N-nitrosoguanidine (ENNG) in drinking water produced tumors of the small intestine after 18 weeks in more than 90%. The induction of tumors could not be suppressed by a large amount (2-3%) of sodium ascorbate in food, but the depth of tumor infiltration was restricted. Cancer developed in 29 of 36 rats receiving ENNG only. In 25 animals of this group growth of tumor corresponded to a P4 stage. In 24 of 35 animals, additionally receiving vitamin C, P4 stage was observed in only 13 cases. Pathological changes in the small intestine could not be observed after the sole administration of vitamin C.

Treatment of carcinoma of the head and neck with combined irradiation and chemotherapy.

Radiation therapy was combined with simultaneous administration of bleomycin, cyclophosphamide and 5-fluorouracil for the treatment of 33 patients with carcinoma of the head and neck. This combination increased the toxicity to the normal tissue. Complete response was obtained in 46 per cent, partial response in 36 per cent and no response in 18 per cent. Patients with a complete response had a significantly longer survival than patients with a partial or no response. An increased cytotoxicity was noted after the combined treatment when compared with irradiation alone. However, no evidence of an increased therapeutic effect was obtained with the addition of simultaneous chemotherapy to irradiation.

[Diagnosis of the functioning of an interposed part of the colon substituting part of the oesophagus after oesophageal resection (author's transl)].

In 6 patients with carcinoma of the oesophagus, resection of the oesophagus was performed, followed by substitution of part of the oesophagus by part of the colon. The functioning of this interposed part was examined by both magnetic tape recording and conventional fluoroscopy. The following findings and special aspects were noted: 1. Formation of a "pocket" in the area of the upper anastomosis. 2. "Watering-can" phenomenon in case of great length of the interposed part. 3. Pronounced reflux from the stomach to the interposed part in horizontal position of the patient and in head down position. 4. Loss of peristalsis of the interposed part of the colon while preserving the natural sacculation and width of lumen. The findings correlated clearly with the signs and symptoms reported by the patients, such as heartburn, belching and retrosternal pain. Comparison of findings via roentgenography and via fluoroscopy with those determined via magnetic tape recording showed the advantages of the magnetic tape recording in checking on the function of the restored oesophageal tract.

Trisodium phosphonoformate inhibits woodchuck hepatitis virus associated DNA polymerase.

Recently a new virus has been described which infects woodchucks, Marmota monax. This virus, named woodchuck hepatitis virus (WHV) is closely related to human hepatitis virus (HBV). The virions have the same principal antigenic system involving surface and core determinants and a serological relationship has been found. WHV has also a DNA polymerase associated with the core. It has previously been reported that trisodium phosphonoformate (PFA) but not phosphonoacetic acid (PAA) inhibits DNA polymerase associated with HBV. This investigation shows the same type of inhibition pattern by PFA and PAA on WHV DNA polymerase.
