[An experimental study of a new route for Acyclovir administration for anti-keratitis therapeutic treatment].

OBJECTIVE: To study the significance of a collagen shield made by centrifugal method that can deliver Acyclovir. METHODS: 28 white rabbits were divided into four groups for observing releasing of Acyclovir into aqueous at 0.5, 1, 3, 5 hours after adding drugs in two different administrative ways, that were putting drug loaded collagen shields on one eye of the rabbit and subconjunctival injection of drug in another eye of the rabbit as control. The drug concentrations in aqueous of rabbit at different times using different administrative routes were measured by high-performance liquid chromatography (HPLC). RESULTS: At 0.5 hr of drug administration, the Acyclovir concentrations in aqueous of subconjunctival injected rabbits were obviously higher than that in collagen shields loaded rabbits (t = 4.050, P <0.01), but it became not so notable at the time of 1 hr. (t =2.074, P>0.05). At the time of 3 hrs and 5 hrs, the drug concentrations in aqueous of collagen shield loaded eyes of the rabbits not only were higher than that of the subconjunctival injected rabbits (t=4.761, t=4.190, respectively, P <0.01), but also could sustain rather a long time. CONCLUSIONS: A drug delivering way using collagen shield made by centrifugal method can replace the subconjunctival injection.

[Surgical resection of giant hemangioma].

OBJECTIVE: Through the operative experiences in 45 cases of giant hemangioma, it is elucidated that the operative procedure for hemangioma is an effective, radical method of treatment. METHODS: 1. Preoperative DSA can establish the diagnosis, delineate the range of lesion and facilitate the design of operation. 2. "Dissection over the false capsule" is applied in the operation. 3. The operative wound, is repaired with a plastic procedure. RESULTS: Of the 45 patients primary cure was obtained in 27, and secondary cure in 13 with effective rate of 89 percent. The followed-up was as long as 8 years with excellent results. CONCLUSIONS: The establishment of operative scheme is chiefly based on preoperative DSA, and the success is fundamentally determined by complete dissection over the so-called "false capsule" around the tumour during operation. This procedure can totally remove the tumour, eliminate recurrence and fulfil the purpose of radical resection, which is worthy of wide application.

[Blood transfusion, an investigation on its brief history].

Transfusion has developed as a practical clinical technique. Its development has experienced from ignorance to science and from cruelty to civilization for hundreds of year. Transfusion has made great contribution for saving lives and expanding operation coverage. To understand the history of transfusion, we can have reference to promote again the development of transfusion technique.

[Dynamics of community structure in successional process of needle and broad-leaved mixed forest in Heishiding of Guangdong].

This paper dealt with the dynamics of community structure in a ten-year succession of needle and broad-leaved mixed forest in Heishiding of Guangdong Province. The vertical community structure tended to be complex, while the horizontal one increased by a wide range. There was no big change in species composition, but the dominance of conifers Pinus massoniana and Cunninghamia lanceolata was substituted by the broad-leaved species, such as Schima superba, Symplocos adenophylla and Schefflera octophylla. The whole community changed from needle and boad-leaved mixed forest into broad-leaved and needle mixed forest. The species diversity and evenness of tree layer were increased, the ecological dominance was decreased, and the community became more complicated.

[Molecular ecology of Cryptocarya chinensis populations in different communities].

The amplified fragment length polymorphism (AFLP) method was used to analyze two populations of Cryptocarya chinensis, the succession climax species in lower subtropical broadleaved forest. The across two communities of evergreen broadleaved forest and mixed coniferous-broadleaved forest. The analysis of molecular variance (AMOVA) indicated that the genetic variation was mostly due to the difference within individuals of the population (89.55%), with high significant genetic variation (10.45%) due to the different communities. The microenvironment of different communities and population biological characteristics resulted in the genetic divergence.

[Tree species diversity in gaps of different sizes and developmental stages in lower subtropical evergreen broadleaved forest, South China].

The change pattern of tree species diversity in gaps of different sizes and developmental stages in the lower subtropical evergreen broadleaved forest was analyzed in this paper. The species diversity index(Shannon-Wienner) did not change in the gaps with a size of < 400 m2, reached the highest in those with a size of 400-500 m2 and the lowest in the gaps of 500-600 m2 size, and increased in those with a size of > 600 m2. The index was the highest in the middle period of 20-50 years, secondly high within 20 years, and lowest after 50 years of gap formation. The species diversity in regeneration layer(H < 1.5 m) reached a peak in the gaps with a size of 500-600 m2, and the lowest values occurred at the gaps with a size of > 600 m2 and 200-300 m2. Species diversity index in the regeneration layer reached the highest in the first 10 years after gap formation, then revealed a general trend of decreasing, but two relative peaks were formed in the gap ages of about 30-40 years and 50-60 years, respectively. The general trends of species richness were correspondent with those of species diversity index. Gaps of different sizes and ages had a strong influence on the tree species regeneration through changes of ecological factors in gaps, thus, tree species diversity was different in gaps of different sizes and ages. Gap is one of the most important mechanisms for the maintenance of tree species diversity in the lower subtropical evergreen broadleaved forest.

[Molecular markers and their application in plant population research].

This review surveys kinds of markers such as allozyme, AFLP, RAPD, SSR, RFLP and their application in molecular ecology of plant populations. The advantages and disadvantages of these molecular markers in plant population research are discussed based on the comparison of the methods and the experience in research work. It is considered emphatically that the adoption of the molecular markers is dependent on the various research purposes and different quality and quantity of the molecular information are acquired from various molecular marker methods.

Relationship between metabolic phenotype of N-acetylation and bladder cancer.

OBJECTIVE: To study the relationship between metabolic phenotype of acetylation and bladder cancer. METHODS: Totally 203 healthy volunteers and 67 patients with bladder cancer were investigated with caffeine as a metabolic probe. Urine samples were collected in 2-6 hours after a cup of 140 mg coffee was taken, and the caffeine metabolites, 5-acetylamino-6-formylamino-1-methyluracil (AFMU) and 1-methylxanthine (1X) were analyzed by high performance liquid chromatography (HPLC). The frequency histogram and probit plot were constructed to select the critical value which was used to assess slow and fast acetylation status both in healthy volunteers and patients with bladder cancer. RESULTS: The peak height ratios of AFMU and 1X (AFMU/1X) were from 0.06 to 6.50 for healthy volunteers and 0.10 to 6.31 for patients with bladder cancer, both with the critical value of 1.10. Of 203 healthy volunteers involved in this study, 26.3% were slow acetylacors, as compared to 46.3% with slow acetylacors in patients with bladder cancer. The odds ratio is 2.376, and the gene frequency for healthy volunteers and patients with urinary bladder cancer were 0.5218 and 0.6804, respectively. CONCLUSIONS: N-acetylation status in the Chinese population is polymorphic and completely concordant with that determined with other metabolic probes. Slow acetylators are significantly associated with bladder cancer.

[Expression of caspase3 gene does not correlate with tumor cell apoptosis in primary hepatocellular carcinoma].

OBJECTIVE: To investigate the relationship between caspase3 expression and apoptosis in hepatocellular carcinoma (HCC). METHODS: In situ hybridization was employed to determine caspase3 expression, and in situ end-labeling was used to detect apoptotic cells in HCC. RESULTS: Twenty one of 39 (53.8%) cases of HCC were found to express caspase3 transcripts, while 46.2% of HCC failed to express it. Adjacent non-cancerous liver tissues showed more caspase3 positiveity (87.5%, 7/8) as compared with HCC (P < 0.05). The expression of caspase3 was correlated with HCC differentiation, as 72.2% (13/18) of moderately to well differentiated HCC showed positive caspase3 transcripts, while only 38.1% of poorly differentiated HCC was caspase3 positive (P < 0.05). No relationship was found between caspase3 and tumor size or grade or metastasis, although 62.5% (5/8) of HCC with metastasis was caspase3 positive but it did not differ significantly from that without metastasis (P > 0.05). Expression of caspase3 alone did not affect the apoptosis index (AI) of HCC. AI was 7.1@1000 in caspase3-positive tumors (n = 21), as compared to 6.6@1000 (n = 18) in caspase3-negative cases (P > 0.05). CONCLUSION: Although loss of caspase3 expression may contribute to hepato-carcinogenesis, its expression may not be related to cell apoptosis in HCC.

[Cytologic and histopathologic examinations of fine-needle aspirates of pancreatic mass].

OBJECTIVE: To evaluate the usefulness in diagnosis of pancreatic mass by histopathologic and cytologic examinations of fine-needle aspirates obtained under ultrasonic guidance. METHODS: In 83 patients with pancreatic mass, percutaneous fine-needle aspiration was performed under ultrasound guidance. The fluid aspirates were examined cytologically and tissue fragments examined histopathologically. The findings were compared. RESULTS: The rate of accuracy, sensitivity, and specificity of cytologic examination was 98.8%, 98.3% and 100% respectively, and that of histopathologic diagnosis was 95.1%, 93.2% and 100%, respectively. There was no false positive. CONCLUSION: Both histopathologic and cytologic examinations of fine-needle aspirates of pancreatic mass are sensitive and accurate enough for the diagnosis of pancreatic diseases. They can be used with complementarity.

[Development of a video system for three-dimensional cephalometry and dental cast analysis].

OBJECTIVE: The purpose of this article is to introduce the outline of our newly developed video system for three-dimensional cephalometry and dental cast analysis. METHODS: The video measuring technique was utilized in the field of dentistry in our study. Two CCD cameras were used to capture the images both of the dental cast and headfilms. The dental cast analysis and three-dimensional cephalometry shared a set of operating system. The three-dimensional reconstruction was based on the LAT and PA films. RESULTS: The measurement error of dental cast analysis is less than 0.09 mm, and the error of three-dimensional cephalometry is less than 0.45 mm. CONCLUSIONS: The clinical application suggests that the use of our system is feasible not only for clinical evaluation of malocclusion but also for saving time necessary for treatment planing and diagnosis.

[Clinical study on treatment of advanced non-small cell lung cancer with Chinese herbal medicine combined with synchronous radio- and chemotherapy].

OBJECTIVE: To explore the effect of applying Chinese herbal medicine according to Syndrome Differentiation of TCM combined with synchronous radio- and chemotherapy (combination therapy) in treating advanced non-small cell lung cancer (NSCLC). METHODS: The 56 patients in the group A were treated with combination therapy, and the 44 patients in the group B treated with synchronous radio- and chemotherapy, while the 34 patients in the group C treated with non-synchronous radio- and chemotherapy. The efficacy of treatment in the three groups were observed and compared. RESULTS: The immediate effective rate in the A, B, C groups was 87.5%, 84.1% and 55.9% respectively, that in the group A and B was better than that in the group C (P < 0.01). The median survival time in the three groups was 16.4, 11.8 and 10.6 months respectively. The 2-year and 3-year survival rate in the group A were markedly higher than those in the group B (P < 0.05), moreover, the clinical symptoms, Karnovsky scoring and immune function were obviously improved in the group A (P < 0.05, P < 0.01). CONCLUSION: Effect of the combination therapy is superior to that of synchronous radio- and chemotherapy alone in improving immune function, elevating quality of life and prolongating survival time of patients with NSCLC.

[Study of prevention and treatment on acute radioactive injury with huoxue yiqi yangyin recipe].

OBJECTIVE: To reduce the acute toxic and side-effect of radiotherapy with Huoxue Yiqi Yangyin recipe (HYYR). METHODS: Seventy-two nasopharyngeal carcinoma patients that using radiotherapy were divided into two groups: HYYR group (A) and the control group (B) using Wisk lotion for gargling or apply at the skin lesion. RESULTS: The injury on mucous membrane of mouth cavity and skin were 47.2%, 91.7%, 13.9%, 33.3% in group A and B respectively, the degree of injury was different (P < 0.01). The acute reaction of the mucous membrane of mouth cavity did not occur until the cumulative radiation were 41.4 +/- 9.4, 30.9 +/- 8.9 in group A and B respectively, there was significant difference between the two groups (P < 0.01). CONCLUSION: HYYR can reduce the acute radioactive reaction of the skin and mucous membrane of mouth cavity and inhibition of bone marrow.

[p16 gene deletion and point mutation in highly purified fresh breast neoplasm].

OBJECTIVE: To study the form and frequency of p16 gene deletion and point mutation in human breast neoplasm and the effective detection. METHODS: Cell culture was made using repeat-adhering method to purify human breast neoplasm cells and PCR-SSCP analysis in 14 highly purified fresh breast neoplasm, 14 fresh none-purified specimens, 27 paraffin-embedded specimens and corresponding normal tissues beside neoplasms. RESULTS: The rate of p16 gene deletion and point mutation was 42.9% (6/14) in the highly purified group, in which exon 1st - 3rd, showed homozygous deletion (5 cases) and exon 2nd showed mutation (1 case). The rate of p16 gene mutation in the other three groups was 7.1%, 7.4% and 4.9%. The detection rate of mutation was significantly higher in the highly purified group than in the other three groups (P < 0.01), but it was not significantly different in the other three groups (P > 0.05). CONCLUSIONS: p16 gene deletion and point mutation exist in primary breast cancer. Homozygous deletion and small fragment deletion may be an important mechanism of inactivation in human breast neoplasm. Purified cancer cells will enhance the detection rate.

[Sagittal profile restoration of thoracolumbar fracture with rod rotation and in situ bending technique].

OBJECTIVE: To restore the sagittal profile at the time of regaining vertebral height during surgery of thoracolumbar fracture by rotation, screw-hook combination and in situ bending technique. METHODS: In 48 patients who have been treated since 1997, 41 were male and 7 female. Their age ranged from 17 to 54 years. Surgical procedures were (L(1) fracture for example): CD or TSRH pedicle screws inserted in T(12) and L(2), offset laminar hooks placed on T(11) and L(2), and two rods contoured according to the normal sagittal profile of the instrumental segment and inserted into the pedicle screws with the rod contouring curve in the frontal plane. Distraction force was used to regain vertebral height. The rods were rotated by 90 degrees and fixed with the set screws. The contouring curve of the rod at this time was transferred from the frontal plane to the sagittal plane where the normal sagittal profile was restored. The offset laminar hooks were compressed toward the screws. If lordosis was not satisfactory, the rods could be bent in situ. Posterolateral bone grafting was finally performed. RESULTS: No death and no neurological worsening were noted except deep infection in one patient. Average vertebral height was restored by 96%. 39 patients had complete restoration of vertebral height. Sagittal kyphotic Cobb angle was corrected from average 24 degrees before operation to average -2 degrees after operation. Follow-up (20 months on average) showed no pseudarthrosis but two screws broken in one patient. CONCLUSIONS: Rod rotation and in situ rod bending technique can satisfactorily restore the normal sagittal profile and regain vertebral height. Offset laminar hooks can effectively protect pedicle screws, and minimise mechanical failure of instrumentation.

[The susceptible alleles on HLA-DRB 1 of type I diabetes in children in Harbin].

OBJECTIVE: To evaluate association between type I diabetes in children and HLA-DRB1 alleles in Harbin. METHODS: Case-control method was employed, and the PCR-SSP technique was used to identify HLA-DRB1 alleles. RESULTS: The frequencies of DRB1 * 0301 and DR3/4 heterozygosity were significantly higher but the frequency of DR2 was significantly lower in cases than that in controls. In DR4 subtypes, the frequency of DRB1 * 0405 was higher and the frequency of DRB1 * 0406 was lower in cases than that in the controls. The difference between cases and controls in DRB1 * 0901 was not significant. CONCLUSION: DRB1 * 0301 allele and DR3/4 heterozygosity were the important susceptibility and heterozygosity genes while DR2 was the most important protective gene in Harbin. The DR4 possessed double-tendency. DRB1 * 0405 conferred susceptibility and DRB1 * 0406 conferred little protection. The susceptibility conferred by DRB1 * 0901 was not certain in type I diabetes in childhood in China. This study provided useful genetic data on the study of etiology of type I diabetes mellitus.

[Effect of qi replenishing and blood circulation activating drugs in treatment of middle-advanced pancreatic cancer with radio- and chemotherapy].

OBJECTIVE: To study the effect of Qi replenishing and blood circulation activating drugs (QRBCAD) in treatment of middle-advanced pancreatic cancer with radio- and chemotherapy. METHODS: Patients were divided randomly into two groups, 28 patients treated with radiotherapy and intervention treatment as Group A and 30 patients treated with the same therapy but with additional QRBCAD medication as the Group B, and the effects in the two groups were compared. RESULTS: The short-term effective rate in Group A and B was 53.6% and 67.6% respectively (P < 0.05). The remission rates of jaundice and abdominal pain in Group A were 57.1% and 50.0% respectively, while in Group B, 83.3% and 76.7% respectively, there was significant difference between the two groups, P < 0.05. The gastroenteric reaction occurred in Group B was significantly lesser than that in Group A, P < 0.05. The 1- and 2-year survival rate in Group A were 50% and 21.4%, and in Group B, 80.0% and 46.6% respectively, significant difference (P < 0.05) showed in comparison of the two groups. CONCLUSION: QRBCAD could alleviate the gastroenteric reaction caused by radio- and chemotherapy, raise the clinical symptomatic effective rate of treatment, and prolong the 1- and 2-year survival rate of patients.
