[Therapeutic trial on function bandage for military training injuries].

OBJECTIVE: To evaluate therapeutic effectiveness of "Tianniansu" function bandage used for military training injuries (MTI). METHODS: In a randomized tri-blind controlled trial, recruits of all soft tissue injuries and stress fracture (SF) who took part in basic training in 1998 were separated into two groups--A and B with comparability, except the use of bandage. After close follow up on weekly base, part of the patents were subjected by ECT pre and post therapy. RESULTS: Effective rates of bandage A and B on treating soft tissue injuries were 66.7% and 86.4%, respectively. However, in terms of symptoms, detumescence effect of bandage B showed milder than that of A. The effective rate of bandage A and B in treating SF were 67.2% and 75.0%, respectively, but statistically not significant. The treatment effect in 26 injured tibias evaluated by bone scintigraphy pre and post treatment showed that the degree of osteogen injury after treatment was not significantly improved. CONCLUSION: "Tianniansu" function bandage had significant effect on treating soft tissue injuries, but did not have effect in treating SF.

[A study on the risk factors of congenital urogenital malformations].

OBJECTIVE: To explore risk factors for congenital urogenital malformations. METHODS: A hospital-based category matching case-control study was performed to investigate the characteristics of 95 cases with congenital urogenital malformations and 160 controls. Parents were interviewed for their characteristics before and during conception. Unconditional logistic regression analysis was used for single factor and multivariate analysis to estimate odds ratios and their 95% confidence intervals. RESULTS: There were 71 male infants in the overall congenital urogenital malformations (Ratio = 74.4%, Chi(2) = 74.863, P = 0.001) identified. Statistically significant associations were calculated for the pregnant women suffering from cold during first and/or second trimester (OR = 7.034, 95% CI = 3.488 - 14.187), for a low birthweight delivery (OR = 4.075, 95% CI = 1.774 - 9.362), for maternal times of pregnancy >/= 2 (OR = 3.133, 95% CI = 1.514 - 6.483) and exposure to occupational detrimental chemicals before pregnancy (OR = 10.496, 95% CI = 1.053 - 104.651). CONCLUSIONS: The risk factors of congenital urogenital malformations were suffering from cold during first and/or second trimester, male infants, low birthweight delivery, times of pregnancy >/= 2 and exposure to occupational detrimental chemicals before pregnancy.

[Pathomorphologic features of clinical stage A prostate carcinoma and causes for misdiagnosis and missed diagnosis].

OBJECTIVE: To study the pathomorphologic features and causes for misdiagnosis and missed diagnosis of clinical stage A prostate carcinoma. METHODS: Prostate samples from a series of 1 020 prostate resections performed in five Shanghai hospitals were obtained, from which 50 clinical stage A carcinoma were studied by immunohistochemistry, according to tumor differentiation and their volume, they were divided into stage A1 and A2 disease. The pathomorphologic features of stage A1 and A2 disease were compared, and the causes for misdiagnosis were analysed. RESULTS: Stage A1 cancer were well- or moderately well-differentiated, of low volume and tended to be multifocal; Stage A2 cancers were poorly differentiated, of high volume, diffuse infiltration and with high grade prostatic intraepithelial neoplasia. 8 cases were misdiagnosed of which 7 cases were stage A1 and misdiagnosed as benign proliferous small gland lesions and 1 case of A2 disease was misdiagnosed as epithelioid histiocytic reactive proliferation. CONCLUSIONS: Stage A1 cancer tended to be initiated in the transition zone and the central zone of the hyperplastic prostate. Stage A2 cancers were mostly middle grade to high grade cancers initiated in the peripheral region and then invaded the central area. The lower incidence of stage A carcinoma in China is related to the small amount of biopsy tissue, careless microscopic observation, or lack of the ability to diagnose stage A1 cancer.

[The influence of two different casting temperatures on properties of the incorporated keeper].

OBJECTIVE: To probe the clinical possibility of making incorporated keeper cap by high temperature casting. METHODS: The incorporated keepers were equally divided into two groups, and cast by applying medium and high temperatures respectively. The two groups were compared with the following items: the morphological changes and thickness of keeper's oxide film, the combining quality between keeper and alloy, and the magnetic retention force. RESULTS: The two different casting temperatures had the similar influence on the keeper's properties, such as the keeper's metallographical view, magnetic force (medium temperature casting group: 433.6 g; high temperature casting group: 433.8 g), and combining quality with alloy, except for the morphology and thickness of the keeper's oxide film. The oxide films of the high temperature casting group (surface: 7.59 microns combining surface of keeper and alloy: 6.17 microns) were thicker than those of the medium temperature casting group (surface: 4.65 microns; combining surface of keeper and alloy: 3.95 microns). CONCLUSIONS: The incorporated keeper could be made by using high temperature casting method instead of medium temperature one in clinic.

[Discovery of natural variant strain of HIV1 in env gene C2-V3 encoded PND of membrane protein].

OBJECTIVE: To sequence and analyze the env gene C2-V3 region of proviral genome from a HIV1 isolate(WWBH7) which was obtained from Huadong Area in China. METHODS: The env gene C2-V3 DNA fragment was amplified by nested-primer PCR with genome DNA of eripheral blood mononuclear cells from a confirmed HIV1 infected individual as a template. The amplified DNA fragment was inserted into pGEM-T vector. The recombinant plasmid was confirmed by restriction enzyme analysis. The inserted DNA fragment was sequenced by ABI737 autosequencer and analyzed by PROSIS software. RESULTS: The HIV1 strain was the derivatives of HIV1 B subtype. But there was mutation of 192 bp fragment repeated insertion at env C2-V3 region of the HIV1 strain compared with standard HIV1 B subtype such as SF2 strain. The mutation brought about a double V3 region in gene encoded PND (principal neutralizing domains). The DNA sequence was registered in GenBank (AF220245). CONCLUSIONS: This was a natural mutated variant strain of HIV1 whose genome showed a 192 bp repeated insertion at C2-V3 region of env gene encoded PND of membrane protein.

[Detection of HPV16 E6 gene in cervical tissues by quantitative polymerase chain reaction].

OBJECTIVE: To study the relationship between the quantities of HPV16 E6 gene in cervical tissues and the course of cervical disease. METHODS: The number of copies of HPV16 E6 gene was detected by competitive PCR in 44 positive samples including 20 cases of chronic cervicitis, 6 cases of cervical dysplasia and 18 cases of cervical cancer. RESULTS: The mean copies of HPV16 E6 gene in chronic cervicitis, cervical dysplasia and cervical cancer were 6.16, 5.33, and 6.45 copies in each microgram of cervical tissue respectively. The quantities of HPV16 E6 gene were not significantly different between cervical dysplasia and chronic cervicitis (P<0.01). CONCLUSIONS: This study underscores the importance of the relationship between the quantities of HPV16 E6 gene and the course of cervical disease. It also suggests that quantification of HPV16 E6 gene may be useful as a prognostic tool to identify women who are at increased risk of developing cervical cancer.

[Human toll like receptor gene expression in human pulmonary granule epithelial cells and umbilical vein endothelial cells].

OBJECTIVE: To determine whether human Toll like receptors (TLRs) genes are expressed in human lung granule epithelial cells and umbilical vein endothelial cells (UVECs). METHODS: Specific primers for TLR2 and TLR4 were designed and total RNAs were isolated from human monocyte cell line THP-1, human lung granule epithelial cell line SPC-A-1 and the primary culture of human UVECs. RT-PCR and Northern blotting with digoxin labeling were used for the detection of TLR2 and TLR4 mRNA expression. RESULTS: TLR4 mRNA expressed in the three types of cells mentioned above. TLR2 mRNA was detected in THP-1 and UVECs, but not in SPC-A-1 cells. By using DNA sequencing, the RT-PCR products were confirmed to be TLR2 and TLR4 cDNAs. CONCLUSION: This study provided evidence indicating that Toll like receptor genes constitutively express not only in human monocytes but also in human larger vascular endothelial cells and pulmonary granule epithelial cells.

[Transverse deflection study of casted titanium zirconium alloy for dental clinic use].

OBJECTIVE: The aim of this study was to obtain the reasonable thickness of Ti-Zr alloy used for denture base material. METHODS: According to the "Revised ADA Specification No. 12 for denture base polymers", the transverse deflection tests of Ti-Zr alloy after casting were performed. Pure titanium, Ti-6Al-4V alloy and Co-Cr alloy were examined as the control. Three groups of wax patterns with different thickness were designed. Ti-Zr alloy, pure titanium and Ti-6Al-4V alloy were casted into molds at 300 degrees C, which were made of zircon (ZrO2.SiO2) investment, using a casting machine (China), with a program of vacuum, pressure and centrifuge casting. Co-Cr alloy was casted by normal method in clinic. The transverse deflections of these four alloys castings with three different thicknesses were measured using a transverse testing machine. RESULTS: When the thickness is 0.65 mm for Ti-Zr alloy, pure titanium and Ti-6Al-4V alloy, and 0.57 mm for Co-Cr alloy, the transverse deflections could satisfy the requirement. CONCLUSION: The transverse deflection is one of the key indexes for evaluating the ability of dental material to bear biting forces. The reasonable thickness of Ti-Zr alloy used as denture base material is 0.65 mm.

[Diagnosis and restitution of nasal bone fractures in emergency management].

OBJECTIVE: To improve the diagnosis of nasal bone fractures in emergency. METHOD: 161 cases underwent fracture taxis, 118 cases underwent taxis and septum corrected operation, 13 cases underwent taxis and nasal sinus operation. RESULT: 279 cases were cured and 18 cases were improved. CONCLUSION: Patients must be checked carefully also with CT scan, one must pay attention to the symptoms of septum fractures, and the possibility of craniocerebral injury and nasal sinus fractures. Severe injuries must be treated according to the emergency treatment principle; patients with septum fractures may be managed simultaneously with septum corrected operation.

[Substituting esophagus with colon in the treatment of hypopharyngeal and esophageal disease].

OBJECTIVE: To study the feasibility and effect of substituting esophagus with colon in the treatment of advanced stage hypopharyngeal carcinoma, cervical esophagus cancer and serious esophageal stenosis on the basis of laryngeal function preservation. METHODS: From 1989 to 1996. 25 patients in our department were retrospectively reviewed among them, nine were hypopharyngeal carcinoma, T3 (No1, N1 4, N2 2), T4 N1 M0 2 (UICC 1997), thirteen were cervical esophagus cancer, T1N0M0 2, T2 (N0 4, N1 7), there were serious esophageal stenosis, length was 3-5 cm. The patients with carcinoma were given postoperation radiotherapy. RESULT: The total survival rates in 3, 5 years are 54.5%, 42.9% respectively. The 3-year survival rates are 44.4% in the cases with advanced stage hypopharyngeal carcinoma, and 61.5% with cervical esophagus cancer, respectively. CONCLUSION: Substituting esophagus with colon is in conformity with physical demand, can reduce complication and improve life quality.

[Separation and determination of pseudoephedrine in bufferin cold tablet by capillary electrophoresis with hydropropyl-beta-cyclodextrin as chiral selective reagent].

A method for the separation and analysis of the chiral pseudoephedrine enantiomers using capillary electrophoresis was established. The buffer solution for separation was 25 mmol/L Tris-phosphate, including 38 mmol/L hydropropyl-beta-cyclodextrin with pH value of 2.65. (1S, 2S)(+) Pseudoephedrine in Bufferin Cold tablet was determined. The method has good precision, recovery and linear relationship.

[The development of dental silver amalgam glass ionomer cement].

To eliminate the harm of mercury, we, by means of special technology, have developed a new type of dental filling material, comprising of glass ionomer and 100% ultra-fine ball-shaped silver amalgam powder. Clinical applications show it has very good wear-resistant, pressure-resistant and adhesive properties Moreover, it causes no irritation to the dental pulp and is a good kind of dental filling material in clinical applications.

[K. pneumoniea endotoxin induced mice beta-defensin-4 mRNA expression and its signaling transduction].

OBJECTIVE: To investigate the in vivo effects of Klebsiella pneumoniae endotoxin(LPS) on beta-defensin expression and the relevant signaling transduction pathway. METHODS: A LPS tolerant mouse C3H/HeJ with a point mutation at Toll-like receptor-4 (TLR4) gene and its wild type strain C3H/HeN were used in this study. C3H/HeJ and C3H/HeN were injected with 4 mg/kg of LPS intraperitoneally. The tracheas, lungs and kidneys of the C3H/HeJ and C3H/HeN were collected respectively at different LPS-treated time points, and the total RNA of each sample was extracted. The expression of mice beta-defensin-3 and/or beta-defensin-4 mRNA in these tissues was determined by reverse transcriptase-polymerase chain reaction (RT-PCR). The sequence of cDNA amplified from the lung of C3H/HeN treated by LPS for 24 h was analyzed. By using western blot, p-I kappa B alpha (phosphorylated I kappa B alpha) and I kappa B alpha of in the lungs of C3H/HeJ and C3H/HeN were detected at different time points after treatment with LPS or without LPS. RESULTS: 1. beta-defensin-4 mRNA was detected in the lungs of C3H/HeN after 24 h treatment with LPS. In contrast, no signal was determined in C3H/HeJ mice with LPS treatment and the C3H/HeN mice without LPS treatment. 2. Compared with the control, increas of the p-I kappa B alpha was observed in the lungs of C3H/HeN at 4 h after treatment with LPS, while both the p-I kappa B alpha and I kappa B alpha contents showed a tendency to go down at 8 h after treatment and dramatically decreased at 24 h. But there were no changes in the of p-I kappa B alpha and I kappa B alpha content the lungs of C3H/HeJ under the same conditions. CONCLUSION: K. pneumoniea endotoxin could induce the expression of beta-defensin-4 mRNA in the lung of C3H/HeN, and TLR4-mediated NF-kappa B activation signaling pathway may be responsible for this event.

The roles of bcl-2 gene family in the pulmonary artery remodeling of hypoxia pulmonary hypertension in rats.

OBJECTIVE: To investigate the roles of apoptosis in the pulmonary artery remodeling of pulmonary hypertension secondary to hypoxia and illustrate the relative genes expression. METHODS: Thirty rats were divided into hypoxia group (10% O2, 8 h/d) and normal control group. On the 15th day of hypoxia, pulmonary artery pressure and right ventricular hypertrophy index were measured and pulmonary artery vessels were studied by light microscope. Then terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling (TUNEL) technique was used to detect nucleosomal DNA fragmentation of apoptotic cells. In situ hybridization and RT-PCR were used to detect the expression level of bcl-2 and bax. RESULTS: The pulmonary artery pressure and right ventricular hypertrophy index of hypoxia group were increased significantly, the pulmonary artery wall of hypoxic group become incrassate than control group. Apoptotic cells can be found in lung with hypoxia or without hypoxia. Compared with control group, apoptotic index of hypoxic group decreased significantly. Through the methods of in situ hybridization and RT-PCR, we found the expression of bcl-2 increased whereas bax decreased significantly in the hypoxic group. CONCLUSION: The alternation in bcl-2 and bax expression induced by hypoxia play an important role in the pulmonary artery remodeling which is the main pathologic change of pulmonary hypertension secondary to hypoxia.

[Effects of glutamine on the intestinal failure in rats model of acute necrotizing pancreatitis].

OBJECTIVE: To determine the effects of glutamine (Gln) on the intestinal failure in rats with acute necrotizing pancreatitis (ANP) and its possible mechanisms. METHODS: Fifty-four Sprague-Dawley rats were randomly divided into 3 groups: sham operation (SO, n = 18), ANP (n = 18), and ANP treated with Gln (ANP + Gln, n = 18). ANP model was induced by injection of 5% sodium taurocholate solution into bilo-pancreatic duct. The therapy was continuously given with amino acid solution by a mini-pump via a central intravenous line. In addition, the ANP + Gin group was received 3% Gln dipeptide solution (equal to 2% Gln) with a dosage of 0.5g x kg(-1) x d(-1). These groups were isocaloric and isonitrogenous. Bacterial cultures from pancreas, mesenteric lymph node (MLN), liver, spleen and ascites were done at 24, 48, 72 h after operation. Endotoxin level in portal vein was determined. Pathologic changes of intestinal mucosa were also studied. Apoptosis of intestinal mucosa was determined by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) method. Expressions of insulin-like growth factor 1 (IGF-1), Gln synthetase (GSase) and glutaminase (Glnase) mRNA were assayed by reverse transcription polymerase chain reaction (RT-PCR). RESULTS: At 24, 48, 72h, the positive rate of bacterial culture and the endotoxin concentration were increased significantly in ANP group compared to the SO group (P < 0.05), while Gln could decrease them significantly. Pathologic study showed that the height of mucosal villous in ANP group was lower than that in SO group, indicating the intestinal mucosa became more atrophy. However, the height of mucosal villous in ANP + Gln group was no significantly difference compared to that in SO group, indicated Gln could preserve the mucosa well. Apoptotic index was increased in ANP group and decreased in Gln treated rats. Expressions of IGF-1, GSase, Glnase mRNA were down-regulated in ANP group, but were up-regulated in ANP + Gln group. CONCLUSIONS: The intestinal barrier function was impaired in ANP. Gln could protect intestinal barrier function. This action was probably related to its enhancement of IGF-1, GSase and Glnase mRNA expressions and its inhibition of intestinal mucosal apoptosis.

An efficient and convenient approach to the total synthesis of sphingofungin.

As a new member of the sphingofungin family, sphingofungin F exhibits interesting physiological activities with a structural unit of an alpha-substituted alanine. Described herein is an efficient and convenient stereoselective synthesis of sphingofungin F from L-(+)-tartaric acid, which utilizes Sharpless asymmetric epoxidation of allylic alcohol and Lewis acid-catalyzed intramolecular epoxide-opening reaction with an N-nucleophile, to introduce the other two desired stereogenic centers. Side chain functionality was incorporated into the chiral segment using a Wittig reaction.

Entropy and holography constraints for inhomogeneous universes.

We calculated the entropy of a class of inhomogeneous dust universes. Allowing spherical symmetry, we proposed a holographic principle by reflecting all physical freedoms on the surface of the apparent horizon. In contrast to flat homogeneous counterparts, the principle may break down in some models. We refined fractal parabolic solutions to have a reasonable entropy value for the present observable universe and found that the holographic principle always holds in the realistic cases.

Correlation property of length sequences based on global structure of the complete genome.

This paper considers three kinds of length sequences of the complete genome. Detrended fluctuation analysis, spectral analysis, and the mean distance spanned within time L are used to discuss the correlation property of these sequences. The values of the exponents from these methods of these three kinds of length sequences of bacteria indicate that the long-range correlations exist in most of these sequences. The correlations have a rich variety of behaviors including the presence of anti-correlations. Furthermore, using the exponent gamma, it is found that these correlations are all linear (gamma=1.0+/-0.03). It is also found that these sequences exhibit 1/f noise in some interval of frequency (f>1). The length of this interval of frequency depends on the length of the sequence. The shape of the periodogram in f>1 exhibits some periodicity. The period seems to depend on the length and the complexity of the length sequence.
