[Examination of the extracranial carotid artery with a high resolution B scanner. A comparison with carotid angiography (author's transl)].

Patients with cerebro-vascular insufficiency in stages I to IV had their extracranial carotid arteries examined with a high resolution real time ultrasound scanner. In 227 cases, carotid angiograms were available for comparison. Provided good image quality was obtained, sonography was able to distinguish with certainty between a normal and a pathological narrowed vascular lumen. Mural changes could be shown down to 0.5 mm. Under suitable conditions it was possible to differentiate between smooth and irregular plaques. Occasionally, ulcers were visible. High resolutions real time sonography is the only noninvasive method which provides information on the appearance and movements of vascular walls and of changes within these. It is possible to detect atherosclerotic changes at an early stage without risk, and to observe their development.

[Haemodynamic effects of intraarterial contrast injections (author's transl)].

Intra-arterial contrast injections have well known systemic and pulmonary vascular effects; in addition there are local haemodynamic changes which begin immediately after the injection and last for several seconds. Three phases can be recognised: Phase 1 Increased flow, pressure and turbulence due to transmission of kinetic energy from the contrast medium to the vessel contents. Phase 2 Reduced flow due to the high viscosity of the contrast medium. Phase 3 Increased flow, probably a pharmacological results produced by contrast (osmolarity). The effects of contrast injection can be altered by changes to the catheter or to the properties of the contrast medium. This has practical consequences for injection techniques and for flow measurements. While assessing the clinical usefulness of the "spill over" flow measurements described by Olin and Redman in 1966, we concluded that the haemodynamic effects of the contrast lead to significant falsification of the measurements, and this also has some significance for the performance of angiography. We have therefore studied this phenomenon, its causes and the factors which influence it, on a model and in animal experiments. In order to quantify changes in flow caused by the injection during animal experiments, a study was carried out on the effects of contrast media on electromagnetic measurements.

[Pre- and postoperative ultrasonography of portal circulation in portal hypertension in childhood (author's transl)].

The use of ultrasound investigation in children with portal hypertension is discussed and a 14 year old patient is presented. With this non-invasive investigation, the course, diameter and patency of the splenic vein can be demonstrated, repeated post-operative investigations showed a reduction in the size of the splenic vein of at least 2 mm. following patent spleno-renal anastomosis. The ultrasound technique is demonstrated.

Dihydroergotamine--stimulation of intestinal peristalsis. An experimental and clinical study.

In an experimental study in 15 beagle hounds using strain gauges a significantly increased motility in the small and large intestine after dihydroergotamine was observed. Since this assessment did not provide any information about mucus transport, gastro-intestinal passage time in patient and control groups was determined using radioopaque catheter material. While transit time in the patient group was significantly reduced by 2 x 0.5 mg DHE s.c./day, this effect was not observed in controls. We related the significantly reduced passage time after DHE in the patient group to a normalisation of a functional disturbance of regulation. The action of the drug is seen in the blocking of the stress induced increased sympathetic tone, without the side effects often seen after pure alpha-antagonists of the phentolamine type or the adrenergic guanethidine type neuronal blocking agents. The specific efficacy of DHE as a partial alpha-antagonist is pointed out. For the clinician, postoperative gastro-intestinal atony might be an indication for its use.

Late changes in aorto-renal vein transplants.

The results of 152 renal artery reconstructions in 128 hypertensive patients were reviewed 1 to 15 years postoperatively in 89 of 98 survivors. Forty-seven (52.8%) patients were normotensive, 29 (32.6%) are improved, and 13 (14.6%) were not improved. Angiography was performed 1 to 11 years after the operation. Only 2 of the 31 vein implants showed an adequate lumen. Four vein grafts were occluded. In 2 out of 4 stenosed transplants stenosis was functionally significant, and the patients with recurrent hypertension underwent re-reconstruction. The causes of the stenoses were severe constriction by scar tissue and extensive subintimal cellular proliferation of the vein graft. Twelve of 31 vein bypass grafts were dilated by an average of 22%. The average increase in caliber of the remaining 9 vein grafts was 106%. The possible contributory factors were forceful distension, excessive adventitial dissection, inadequate storage of the vein, ischemic damage of the vein wall (especially in younger patients), and the increase flow rate through the transplant.

[Management of traumatic pelvic bleedings using embolization].

Massive retroperitoneal bleeding in pelvic trauma is a difficult problem in surgery, and surgical treatment is rarely attempted due to the risk involved. Angiographic embolization in this region is an uncomplicated and effective measure that may be performed without general anesthesia and simultaneous with shock treatment. Four patients with massive traumatic hemorrhage in the pelvic region were successfully treated with embolization.

Preoperative angiography of the superior mesenteric and portal vein with prostaglandin F2 alpha in portal hypertension.

In a comparative study, we showed that a gain in diagnostic reliability justifies the routine use of PGF in indirect superior mesenteric venograms of patients with portal hypertension. Three radiologists without knowledge of patients' data and treatment analyzed 30 angiograms with and 30 examinations without PGF. Complicating factors, such as presence of varices and dose of contrast medium (CM), were taken into account by a Mantel and Haenszel procedure. When PGF was used, intrahepatic portal branches could be outlined beyond their second ramification more frequently. The superior mesenteric and portal veins, but not the confluence, were visualized significantly more often using this agent, so that patency, constriction, partial thrombosis or occlusion could be reliably diagnosed. Independent of the use of PGF, visualization of the confluence was improved by high CM doses in combination with high flow rates.

Gadolinium-DTPA enhanced MR imaging of spinal dural arteriovenous fistulas.

To evaluate the role of magnetic resonance (MR) in the diagnosis of dural arteriovenous (AV) fistulas and the resulting myelopathy, the MR examinations of 11 patients with symptoms and signs of slowly progressive myelopathy of the lower spinal cord have been reviewed. Patients with intradural or extradural AV malformations were excluded. Six patients have been examined without the use of a contrast agent. The other five patients were studied prior and after intravenous administration of gadolinium-diethylenetriamine pentaacetic acid (DTPA). Serpentine linear areas of low signal due to flow void effects within the subdural space have been detected in seven patients. The T1-weighted sagittal images and T2-weighted sagittal and axial images demonstrated signal intensity changes within the center of the cord due to prolongation of T1 and T2 relaxation times. In addition, a relatively abrupt increase of the sagittal and transverse diameters of the lower thoracic cord was found in all cases. In nine patients T1-weighted, proton-density and T2-weighted sagittal images presented "blurred" margins of the lower thoracic cord. After intravenous administration of Gd-DTPA (0.1 mmol/kg), contrast enhancement was present not only within these vessels but also within the lower spinal cord. There was only slight enhancement of the cord immediately after injection of the contrast agent, but significant enhancement was observed 40-45 min later.

Dynamic MR imaging: a further possibility for characterizing CNS lesions.

To improve the specificity of MR imaging, 45 patients with different lesions in the CNS were studied with gadolinium-DTPA from June to December 1985. With a dosage of 0.2 ml/kg body weight, no adverse effects were seen. Seventeen of these patients were also studied with dynamic MR imaging, using a spin-echo sequence of TR = 100 msec and TE = 30 msec and a display matrix of 128(2). Four parameters were determined and compared with histologic findings: the time to peak of signal intensity, the ascent to peak of signal intensity, the height of peak of signal intensity, and the percentage of peak of signal intensity at the end of measurement. In this small series, arteriovenous malformations had a short time to peak and a steep ascent to peak as compared with tumors. Gliomas had a slow ascent to peak and usually a longer time to peak as compared with tumors of mesodermal origin. Paragliomas according to Zülch's classification had a steeper ascent to peak than gliomas. Dynamic MR with gadolinium-DTPA may lead to further information about lesions in the CNS, as the ascent to peak parameter seems helpful in discriminating among different lesions.

[Measuring the thickness of facial soft tissues using nuclear magnetic resonance tomography for the purpose of identification].

Nuclear spin tomography since its beginnings in the seventies has steadily gained in importance as a method of examination in medical diagnostics as it produces a picture. In the field of forensic medicine the NMR technique as used for anatomic-anthropologic issues attempting to identify skulls this is a valuable supplement to an extension of the existing methods of investigation. The results of a measurement of the thickness of soft facial tissue in a live test person is shown as compared to measures obtained by sonography.
