A 31-year-old woman had an extruding ocular implant in her right orbit. After surgically removing the implant, we transferred an autogenous dermis-fat graft from the hip region to the freshly prepared socket. The graft was sutured to the conjunctiva, and a conformer was placed in the socket. Eight weeks postoperatively, the dermis-fat graft was covered with conjunctival epithelium and a prosthesis was fitted successfully. No evidence of infection has occurred. This technique of using composite dermis-fat grafts in enophthalmos avoids recurrent extrusions and corrects the cosmetic problems produced by migrating or extruding implants.
Alterations in plasma prolactin (PRL) concentrations in response to nursing in puerperally lactating women are often significant beyond the 90th postpartum day, yet the increment appears unrelated to the frequency or duration of the nursing stimulus. Tonic gonadotropin secretion is low, assuming a more episodic secretory pattern either when the frequency of breast-feeding is reduced or when weaning takes place. Significant increments in peripheral concentrations of luteinizing hormone can be seen in response to weaning coincident with a fall in peripheral plasma PRL concentration. At the same time, peripheral estrogen concentrations increase, suggesting that a specific set point for ovarian responsiveness to gonadotropins exists. Whether this set point is related solely to the peripheral concentration of gonadotropins or whether it is also related to the peripheral PRL concentration is not known at this time.

Electrooculographic recordings during left and right conjugate lateral gaze fixation from 10 degrees to 50 degrees were made in 29 patients being treated with phenytoin. At the time of electrooculographic recordings, venous blood samples were drawn for analysis of total and free phenytoin plasma concentrations. Rhythmic horizontal nystagmus occurred in 7 patients, and only at extreme (40 degrees, 50 degrees) later gaze in 6 of these. Measurable free phenytoin levels in the 29 patients ranged from 0.2 to 3.2 mug/ml, and in the 7 patients with nystagmus were 0.6, 0.8, 1.0, 1.0, 1.7, 1.8, and 3.2 mug/ml. Neither the occurrence of nystagmus nor the degree of lateral gaze at onset could be correlated with free phenytoin concentrations in plasma.

Recent reviews have emphasized the value of cytodiagnosis in the evaluation of malignant effusions from patients with lymphoproliferative disorders. We present a case in which the presence of Sternberg-Reed-like cells in pericardial fluid was related to prior radiotherapy and not to recurrent Hodgkin's disease.
An attempt has been made to acquaint the surgeon with the basic principles in the evaluation and treatment of the patient with trauma involving the orbital area. Evaluation must include neurologic, ophthalmic, as well as radiologic examination of the traumatized area. The decision to operate must be based upon the findings in the individual case using sound clinical judgment. Treatment consists of the restoration of the integrity of the traumatized area and the repair of whatever defects may have been caused.

Four cases of non-Hirschsprung megacolon in adults are presented. The histology of the myenteric plexus suggests that this may be an anomaly of development.
Over a seven-year period, a 22-year-old woman developed clinical linear scleroderma with associated ipsilateral coloboma of the upper eyelid, blepharoptosis, enophthalmos, and impairment of extraocular motility. Reconstructive surgery included correction of the enophthalmos by Silastic block implant, closure of the coloboma, and resection of the levator palpebrae superioris muscle for correction of the blepharoptosis.

One hundred thirty-five cases of cicatricial entropion operated by the Oculo-Plastic Service of the Manhattan Eye, Ear & Throat Hospital were reviewed. Results of 114 of these were available for study. There was an overall success rate in primary surgery of 57 percent. Of those cases reoperated, only nine percent had a subsequent successful result. The most common procedure done was the transverse blepharotomy and marginal rotation. There was no statistical correlation between the success rate and the age or sex of the patient. Results on the fellow eye correlated well with results of the first eye in bilateral cases.
