[Interdisciplinary diagnosis and therapy concepts in climacteric osteoporosis].

50 patients with signs of osteoporosis have been selected from special outpatient clinics for climacteric or rheumatic complaints for a complex interdisciplinary programme concerning diagnostics and therapy. 46 patients could be treated successfully for one year with estrogens and progestogens, fluoride and calcium preparations as well as physiotherapeutic treatments including electrotherapy and dietetic advice. X-ray examinations done after this period did not show any progression of the osteoporosis. But only in 4 patients an increase of bone density could be demonstrated. In the other patients health conditions were stabilized. Good results could be obtained with regard to climacteric complaints and improvement of fitness and efficiency. In 2 patients a long-time disablement could be stopped.

[Biotransformation of B 24/76 in the rat, minipig, dog and man].

After oral administration of B 24/76 (1) on experimental animals (rat, minipig, dog) and human beings 11 metabolites could be isolated from urine, faeces and bile. The structure of 7 compounds was elucidated by means of TLC and MS. The investigated species are showing an analogues metabolic pattern with differences in the intensity and main metabolites. In phase II-reaction we could observe qualitative differences.

[Magnesium--a new therapeutic alternative in primary dysmenorrhea].

50 patients suffering from primary dysmenorrhoea were treated with Magnesium (Mg 5-longoral, Artesan GmbH) in a double-blind study. After a six-month period 21 out of 25 women showed a decline of symptoms, only 4 ones reported no therapeutical effect. For monitoring treatment results prostaglandin F2 alpha (PGF2 alpha) was measured every second month. On Mg-therapy conditions we achieved a reduction of PGF2 alpha in menstrual blood to 45% of value before treatment started. As against that 90% of basic concentration were estimated from women who received a placebo. Probably, the specific therapeutical effect of Mg based on inhibition of biosynthesis of PGF2 alpha but also on its direct muscle relaxant and vasodilatory effect. Beside the PG-synthesis and ovulation inhibitors the use of Magnesium is a potential, natural opportunity to treat primary dysmenorrhoea, which is widely free of side effects.

[Treatment of long-term sterility by intra-tubal gamete transfer].

It is reported about a first successful program of gamete intra fallopian transfer (GIFT) as a supplement to intrauterine insemination and to in vitro fertilization (IVF). In 29 patients after HMG stimulation of the ovaries and laparoscopic aspiration of follicles recovered oocytes have been introduced together with prepared sperm via a special catheter directly into the ampulla of the oviduct. In 21 patients with different causes of infertility optimal stimulation conditions could be achieved. A pregnancy rate of 41.4% is the result of 29 GIFT events and 3 of them resulted in spontaneous term deliveries (10.3%).

[Endometriosis within the scope of sterility treatment].

Endometriosis is one of the most frequent diseases in connection with female sterility, 3.2 per cent endometriosis was diagnosed in all operative cases from 1964 to 1984 but in all cases of sterility there were 10.8 per cent. Essentially these were cases of severe endometriosis genitalis externa (78.6 per cent) according to the Acosta-classification. The rate of recoquition of endometriosis as a possible reason for sterility has been increased from 2.4 per cent to 9.9 per cent since the general introduction of laparoscopy into the diagnosis of hitherto unsolved cases of sterility in 1980. A combined laparoscopic-hormonal therapy pattern is presented here. A corrected pregnancy rate below 30% after endometriosis therapy indicates that there are much more possibilities for therapy of this diseases.

[Retrograde vascular catheterization as a differential diagnostic method of studying androgenization in the female].

In 20 women with marked androgynism or testosterone concentrations being suspected for a tumor blood samples were taken by selective retrograde catheterization of the ovarian and suprarenal veins via vena femoralis. Testosterone, 17 beta-estradiol, and progesterone were determined and the concentration gradients to peripheral blood levels were calculated. In 20% of patients an pure adrenal and in 5% an ovarian hypersecretion of testosterone was established. A combined adrenal-ovarian hypersecretion was found in 50%. In 25% of the cases no final estimation was made because anatomic variations in venous size and drainage made catheterization and bilateral blood sampling impossible. There were no references to an androgen-secreting tumor.

[Determination of the ovulation date in stimulated cycles with Clostilbegyt].

Modern sterility therapy presupposes a precise ovulation timing in stimulated cycles. The different contribution and importance clinical hormono-analytical and ultrasonographic methods to and for the determination of the ovulation date were tested in 137 cycles of 84 women with a Clostilbegyt therapy as well as in a control group of 27 spontaneous cycles. It was stated that the results ascertained by ultrasonography are of special importance. Nevertheless the determination of 17 beta-estradiol and the LH estimation must not be neglected as functional criteria. In order to supervise the corpus luteum phase determinations of progesterone, 17 beta-estradiol and prolactin during the second part of the cycle are to be included into a control program.

[Determination of estradiol receptors in the endometrium--a contribution to the diagnosis of functional sterility].

In 86 cases of female infertility an endometrial biopsy was performed. The received tissues were examined by histological methods and the binding of estradiol on the specific cytoplasmic receptor was studied. Serum levels of estradiol and progesterone were estimated at the same day.--The estradiol receptor levels from ovulatory cycles are directly dependent on the histological dated phase of the menstrual cycle. The level was highest in late proliferative phase and declines during the secretory phase.--Luteal phase deficiency and other luteal phase defects are connected with a decrease of estrogen receptor concentrations in secretory endometrium.

[Change in the diagnosis and therapy of Stein-Leventhal syndrome].

Within the period from 1959 to 1984 the development of diagnostic and therapeutic possibilities has been demonstrated in 68 cases of women with a Stein-Leventhal syndrome. In the first group consisting of 11 female patients who were under examination until 1975 the clinical aspects took the leading part in the diagnosis. The ovarian wedge resection meant histological security and therapy at the same time. During the second period until 1980 (26 women) the hormone diagnostics was an essential criterion. The conservative therapy with Clostilbegyt aimed at the correction of these hormonal dysfunction as well. During the last years (31 women) diagnosis has been concentrated on etiological aspects. Correspondingly an individualized therapy was practised. The advances in the medical attendance of infertility become evident in the increase of the pregnancy rate from 9.1 to 61.3%.

Serum drug fluctuations after administration of two sustained-release oral theophylline preparations in asthmatic children.

Serum theophylline levels produced by two bands of sustained-release theophylline tablets, TheoDur and Respbid, were compared in a double-blind crossover study in 13 asthmatic children. The children, aged 9 to 17 years, remained involved in their daily school activities throughout the study. Each patient took 250 mg every 12 hours of one brand of anhydrous theophylline for five days and the other preparation for the remaining five days of the ten-day study. Trough and peak theophylline levels at steady state were measured on the fifth and tenth days of the study. Results indicated no significant differences between peak and trough with either preparation and no difference between the two preparations in mean theophylline concentrations.

[Stability of beta-HCG in old blood stains].

Pregnancy was diagnosed from old blood stains by means of radioimmunoassay of beta-HCG. Up to 45 days, the age of the blood stains did not influence values, and extraction by ultrasound did not improve the results.
