[Implant changes in sliding endoprostheses].

Analysis of 6 failures of resurfacing gliding prostheses of the knee with necessary exchange operations. By this procedure of exchange of the loosened parts of the alloplasty, a good functional result could be achieved with the exception of one case. Importance of careful preoperative indication of gliding prostheses of the knee, where axis deviations of the leg as well as instability of the collateral ligaments should be particularly observed.

[Significance of early meniscectomy in the etiology of severe arthritis of the knee].

With reference to 3579 meniscectomies performed in our clinic during 18 years as well as to cases with severe knee damage treated by arthroplasty, arthrodesis or correction osteotomy and their aetiopathological analysis with regard to previously performed meniscectomies, the following conclusions are drawn: Earlier meniscectomies seem to occur relatively often (up to 20%) in clinical cases of severe destructive processes in the knee e.g. knee arthritis. With reference to 3579 meniscectomies performed in this period this number is very low (1.34%) and - under statistical aspects - therefore not essentially important. The analysis of the cases of our clinic operated on by means of plastics, arthrodesis or correction osteotomies showed, that an earlier meniscectomy was performed because of static axis faults. These axis deviations were the main reason not only for the meniscopathy but also for the further development of knee arthritis. Cases of severe knee arthritis, who could be referred to an earlier meniscectomy (traumatic lesion) without any deviation of the leg axis were very rare. The rarity of the development of a knee arthritis after meniscectomy seems to prove that evidently there is no disadvantage of the mainly performed total meniscectomy compared to the subtotal procedure.

[Causes, therapy and results of operative treatment of recent and old acromioclavicular joint dislocations].

Osteosynthesis with wires and cerclages is the operative technique of choice in case of recent or old breaking of acromio-clavicular joint. In case of an old lesion of the acromio-clavicular joint syndesmoplasty should be performed only if direct suture of the ligaments is no longer possible. The so-called "allo-ligament" proved to be a failure because of the high rate of infections. The "Kirschner wires" should have a thickness of at least 1.5 mm. To avoid stiffness of the operated shoulder there should be an early active functional treatment. The removal of the osteosynthesis material should be performed only after complete healing of the ligaments (3-4 months postoperatively).

Effects of aminoglycoside antibiotics on bound calcium and contraction in guinea-pig atria.

We studied the effect of three aminoglycoside antibiotics which have been shown to replace Ca from lipid monolayers on the superficially bound Ca of isolated beating left atria from guinea-pigs. The cellular Ca content was determined by means of 45Ca after having attained complete exchange. The antibiotics dibekacin, sisomicin, and gentamicin, all reduced the cellular Ca content by 10-20% in a dose-dependent manner. The loss of superficially bound Ca was accompanied by a decline of the contractile force by 40-90%. It is concluded that in isolated atrial muscle it is the amount of Ca bound to the outer surface of the cardiac plasmalemma, rather than the extracellular Ca2+ concentration, that determines contraction height.

Monitoring lens transparency during in vitro incubation by Scheimpflug photography and densitometric image analysis.

Dragomirescu et al. [1983] described a 2-chamber flow system for the incubation of lenses with an inserted photometer to measure changes in lens transparency. Mester et al. [1984] applied the method to simulate conditions occurring during vitrectomy, which occasionally leads to reversible lens opacification. Using Scheimpflug photography (a modified SL 45 Topcon camera) instead of the transmission measurements of incubated lenses has the advantage that disorders in lens transparency can be exactly localized and the sensitivity is much higher than the photometer readings. Application of Scheimpflug photography required a modification of the incubation chamber. The application of the experimental set-up was demonstrated with pig lenses under various incubation conditions. The results obtained by densitometric image analysis of Scheimpflug photos were compared with transmission measurements by photometer.

[Causes of rearthrotomies of the knee joint].

Report on 178 rearthrotomies of the knee joint in 158 patients. Most of these rearthrotomies became necessary subsequent to meniscus operations. A review of the literature shows that these are mainly persisting or recurring complaints appearing usually under the overall pattern of "irritable knee". The principal causes were found to be leftover meniscus residues; on rarer occasions degenerative meniscus regenerates were found, or other causes such as floating cartilage, damaged cartilage of the patella, and instabilities of the condyles and of the ligaments.

[Technic and results of replacement operations for irreparable radial paralysis].

First, the main operative techniques (Franke, Perthes, Bauer and Lindemann) are shortly described. Since 1964 we have performed in our clinic 20 tendon transplantations for radial paralysis, 16 of our patients could be controlled clinically 7.6 years average after the operation. The functional results were altogether very satisfactory.

[Sonographic changes in placental structure during pregnancy].

Modern ultrasonic-B-scanners permit the in-vivo-observation of placental structure during pregnancy. From 3rd Trimenon there are characteristic ultrasonic patterns point to placental maturity with classification grades from 0 to III. Progressive echographic changes begin in the chorionic plate, after that few scattered in the placental substance and finally in different extent and density in the basal layer. Grade III represent the term placenta. In the determination of intrauterine growth retardation and dysmaturity the recognition of structure may be an additional indication and should be a part of ultrasonic routine care.

[Effect of early sonographic placental maturity on birth weight].

Dependence of birth weight from placental maturity could be demonstrated by sonographic judgement of placental structure during pregnancy in 784 pregnant women. Early was children in with low birth weight generally seen placental maturity. In children with birth weight over 3500 grs first findings of placental maturity were in 81.8% after 35 weeks gestation. There may be possible correlations between placental maturity and risk factors typical for low placental perfusion with the consequence of fetal growth retardation as hypertension, hypotension, stress and smoking.

[Rupture of the Achilles tendon, cause - prognosis - therapy - late results].

Reports about 147 ruptures of the Achilles tendon which were mainly caused by sports activities (85%). The authors also report on the operative methods used in our clinic, and on the long-term results (up to 16 years) of our cases. The complication rate was relatively low, and reoperations were necessary in a few cases. Healing of the Achilles tendon was achieved in every case. The disability to work was relatively short. Most of the patients were able to resume their former sports activity.

[Meniscus injuries in childhood and adolescence. Causes--treatment--results].

A brief outline of special morphological features of meniscus structure during growth is followed by an analysis of meniscus lesions during this age period. Between 1964 and 1981, we performed altogether 3,579 meniscectomies in our clinic; at that time, only 19 patients (0.5%) were between 3 and 12 years old, 223 cases (6.2%) between 13 and 18 years. Whereas dysplastic changes of menisci were the main causes for meniscectomy in childhood, traumatic lesions of the menisci were prominent in adolescence. A causal relationship to regular activity in sports is assumed. 188 cases (77.7%) could be controlled clinically after an average postoperative period of 8.5 and 10.0 years, respectively. The subjective results were altogether satisfactory. In 11 cases, x-ray examination showed arthrotic changes of the knee joint.

[Indications, technic and results of knee joint arthrodesis].

Report on about 67 cases of knee arthrodesis (septic and aseptic cases). In aseptic cases we preferred arthrodesis with autocompression plates in 21 patients. In the other cases we preferred Fixateur externe. We could follow up the patients for an average period of 25.4 months. The arthrodesis was consolidated in 98.3%; the infection could be cured in 92.9% of the cases.
