[Simultaneous registration of pancreatic secretion profiles in endoscopic secretion sampling in patients with and without pancreatic diseases].

A method for the continuous detection of secretory profiles of human pure pancreatic juice is described. After retrograde cannulation of the D. pancreaticus the exocrine pancreas was stimulated by an intravenous infusion of secretin and a bolus injection of pancreozymin additionally. The absorbance of pancreatic juice at 280 nm was measured continuously using an Uvicord-UV-Photometer which was connected directly with a Teflon catheter localised in the main pancreatic duct. The secretory profiles obtained from patients without pancreatic diseases (n = 12) indicated a first maximum of absorbance after secretin infusion and a second one in response to pancreozymin bolus injection. Alterations of secretory profiles in patients with chronic pancreatitis (n = 10) were evident after pancreozymin administration principally.

Chromosomal mosaicism confined to chorionic tissue.

Chorionic villus samples from two healthy pregnant females were obtained for first trimester fetal diagnosis. The karyotypes were 46,XX/47,XX,+3 and 46,XX/47,XX,+15, respectively. In both cases fibroblast cultures after termination of pregnancy were shown to have normal karyotypes.

Tandem duplication chromosome 21 in the offspring of a ring chromosome 21 carrier.

A direct tandem duplication chromosome 21 was found in a boy with Down's syndrome. The proband's mother and grandmother both carried a ring chromosome 21. The observed duplication chromosome in the child may be explained either by recombination between the maternal ring and the mother's normal chromosome 21 or by break of a double-sized ring chromosome 21.

[Chorionic biopsy in prenatal chromosome diagnosis].

This paper reports on 110 chromosome analyses of prenatal diagnosis after chorion biopsy. Normal findings were recorded in 105 cases (51 female, 54 male), and 5 chromosomic abnormalities were found, including triploidy, rarely to be observed in the second trimester, and a trisomy 8. Both direct preparation and long-term cultures were employed for chromosome diagnosis. With long-term cultures, three mosaic findings were recorded that were not confirmed in subsequent examinations of amniotic cells. The observation of certain cells with female karyotype in a long-term culture with otherwise definitely male karyotype illustrates the necessity of polymorphic comparison in order to exclude maternal contamination in the analysis of long-term cultures.

Timentin in the treatment of nosocomial bronchopulmonary infections in intensive care units.

A clinical trial with Timentin (ticarcillin plus clavulanic acid) was undertaken in patients with hospital-acquired lower respiratory tract infections. Two formulations, 3.2 and 5.2 g consisting of 200 mg clavulanic acid and 3 or 5 g ticarcillin, respectively were usually given three times daily. Eighty-one patients were evaluable for clinical efficacy and 89 for tolerance. The clinical cure rate was 96% of the assessable cases even though all patients had severe concurrent or underlying diseases. The pronounced synergism between ticarcillin and clavulanic acid resulted in a bacteriological elimination rate of 94%. Adverse effects were very rare and of a mild nature, and restricted to those usually seen with the well-tolerated penicillins. No toxicological abnormalities could be detected in extensive laboratory screening. Timentin is a highly effective broad-spectrum antibiotic with good tolerance. Its potentiated action in comparison to other penicillins against beta-lactamase-producing strains, could reduce the usage of aminoglycosides in the future.

Chromosomal mosaicism and maternal cell contamination in chorionic villi cultures.

In a series of 110 chromosome analyses, after chorionic villus sampling, mosaics were found in 3 out of 106 long term cultures. In all 3 cases, subsequent amniotic fluid cell cultures showed normal karyotypes. Maternal cell contamination was encountered in one long term culture derived from carefully dissected chorionic villi.

Treatment of severe infections with temocillin. Clinical and bacteriological evaluation.

Included in a study of the clinical efficacy of temocillin were 20 hospitalised female patients suffering from infections of the respiratory or urinary tract, septicaemia or an abscess. Clinical evaluation revealed fully effective treatment in all patients, and all pathogens identified were eliminated during therapy with no adverse reactions being observed.

[Immediate effect of x-ray contrast media on thyroid hormone parameters].

Immediately after application of various contrast media a number of functional parameters of thyroid function were determined. Especially after application of Biliscopin which is strongly bound to plasma proteins, a significant rise of the free thyroid hormone fractions, a significant increase in the binding capacity of specific transport proteins and a decrease of the total concentration of thyroid hormones was noticed. It is therefore recommended that neither in the early phase within the first hour, nor in the late phase within 10 days after application of contrast media, blood for determination of parameters of thyroid function should be collected, since otherwise misleading results may be obtained.

Heterochromatin and nucleolus organizer regions in cells of patients with malignant and premalignant lymphatic diseases.

Peripheral blood lymphocytes (short-term cultures) and permanent lymphoid cell lines (long-term cultures) of patients with Hodgkin disease (5), lymphoma (1), plasma cell leukemia (1), angioimmunoblastic lymphadenopathy (1), and infectious mononucleosis (3) were investigated for C-band variants and nucleolus organizer region (NOR)-activities by C-banding and silver-staining, and compared to those of healthy adults (5) and newborn children (3). Heterochromatin polymorphisms were found in malignant diseases (33%) as well as in controls (29%). In the lymphoma patients, heterochromatin of class 3 (Patil and Lubs 1977) was seen more frequently than in the controls (83%:57%). No marker C-band variants could be detected in any of the lymphatic diseases. There was no difference in the heterochromatin polymorphism between short-term cultures (predominantly T-cells) and long-term cultures (B-cells), and there was little but inconsistant difference in the NOR-activities. Silver-staining showed differences between healthy adults (8.8 AgNORs; SD:0.5) and newborn children (6.9; SD:0.4). In the lymphoma patients we found 8.3 (SD:0.7) AgNORs. Thus, using silver staining there was no detectable increase in the number of active NORs in cells of patients with malignant diseases as an expression of increased nucleolus activation in malignancies. The remarkably low NOR-activity in infectious mononucleosis (6.7; SD:1.0) may reflect an influence of acute virus infection (Epstein-Barr virus) on NOR-activity.

Multiple forms of alanine aminopeptidase, alkaline phosphatase and gamma-glutamyltransferase in urine of healthy persons, patients suffering from kidney diseases and patients with kidney transplants.

The catalytic activities of alanine aminopeptidase, alkaline phosphatase and gamma-glutamyltransferase were determined in the soluble and particulate fractions of urine, after ultracentrifugation. In healthy adults the fractional catalytic activities in the supernatants were 0.53, 0.56 and 0.24, respectively. Nearly the same proportions were found in children. In patients suffering from chronic kidney diseases there was a tendency for the proportion of catalytic activity in the soluble fraction to increase. However, the separation into the multiple forms gave no higher diagnostic reliability than the determination of total catalytic activity of the respective enzymes. The determination of multiple forms has no clinical significance in the detection of rejection episodes in renal transplant recipients.

[Varying results of different procedures for determining the FT4 concentration in serum before and after heparin administration].

After application of heparin FT4 was determined by various methods in patients undergoing hemodialysis, and the results were compared with those obtained before application of heparin. Using ultrafiltration (Amicon) and the Dow-Lepetit test a significant increase in FT4 concentration could be registered at all times after heparin application. By use of the Corning test a non-significant increase in FT4 concentration was found one hour after heparin application. When the Amerlex method was employed no change in FT4 concentrations was detectable. Most probably, the observed increase in FT4 concentration after heparin application does, in fact, occur.
