[Addiction-like behavior with continuous self-stimulation of the mediothalamic system (author's transl)].

In a patient with postamputation pain a continuous self-stimulation of the mediothalamic system by a chronic deep brain electrode induced a change in behavior, similar to addiction. At the same time various psychological tests (HAWIE, Benton, Hooper-VOT and concentration tests) were significantly disturbed. After interruption of the deep brain stimulation all induced psychic abnormalities normalized within a few days.

[Diagnosis and treatment of a neonatal isoimmuno-thrombocytopenia].

The diagnosis of neonatal isoimmune thrombocytopenia is based upon the exclusion of other causes of thrombocytopenia in the newborn, and on successful treatment by transfusion of compatible, preferably maternal, platelets, If isoimmunization is suspected, detection of antiplatelet isoantibodies should be attempted using specific serological methods. This case report nevertheless illustrates that failure to confirm isoimmunization serologically does not exclude the diagnosis of neonatal isoimmune thrombocytopenia.

Circular dichroism study of Escherichia coli initiation factor 3 binding to nucleic acids.

The circular dichroic spectral features of (A)10-20, (C)10-20, A8UGU6, poly(A), and poly(C), at both neutral and acidic pH values and in the presence and absence of Mg2+, are significantly altered by Escherichia coli initiation factor 3 (IF3), implying the occurrence of protein-induced changes in nucleic acid secondary structure. Similarly, the circular dichroic spectral characteristics of helical poly(U), poly-(A)-poly(U), and poly(I)-poly(C) are modified by IF3. However, no structural perturbation of poly(A)-poly(U) occurs in the absence of Mg2+ by IF3. The oligonucleotides (A)10-20 and (C)10-20 at both pH 7.5 and 5.5 titrate to end point of 26 +/- 4 nucleotide residues per IF3 [except (C) 10-20 at pH 5.5 which titrates to 17 +/- 1 nucleotide residues per IF3], whereas the hairpin A8UGU6 under similar conditions at neutral pH and in the presence of Mg2+ titrates to an end point of 56 +/- 3 nucleotide residues per IF3, thereby suggesting the presence of multiple binding sites on the protein. By contrast, poly(A) and poly(C) at neutral pH and in the absence of Mg2+ titrate to an end point of 13 +/- 1 nucleotide residues per IF3. The occurrence of significant light-scattering artifacts precluded a determination of the end point stoichiometry in most other cases. The circular dichroic spectra of E. coli tRNA, MS2 RNA, phiX174 DNA, and sonicated calf thymus DNA were unaffected by IF3 at physiological concentrations. Addition of an equimolar mixture of IF3 and ribosomal protein S1 titrates the circular dichroism of poly(C) at acid pH as did S1 alone. However, addition of IF3 to mixture of poly(A) and S1 at neutral pH did not result in significant titration of the optical activity until IF3 was in excess over S1, even though filter binding assays indicate normal IF3 binding to the polynucleotide. The possible relation of these observations to the biological function of IF3 is briefly considered.

Spontaneous and detergent-induced vesiculation of thymocyte plasma membranes.

An analog of lysophosphatidylcholine (1-dodecyl-propanediol-3-phosphocholine) which does not impair membrane-bound enzymes was used for the induction of shedding of membrane vesicles from intact calf thymocytes. Without liberation of intracellular enzymes such as lactate dehydrogenase (EC 1.1.1.27) the shedded membranes contained 15--25% of the total activity of the plasma membrane enzymes alkaline phosphatase (EC 3.1.3.1), nucleotide pyrophosphatase (EC 3.1.4.1) and gamma-glutamyl transferase (EC 2.3.2.2). Membrane-free supernatants only exhibited trace activities of these enzymes. Without further purification, the specific enzyme activities in shedded membranes were of the same order of magnitude as in purified plasma membranes prepared after nitrogen cavitation of thymocytes. Small amounts of membrane vesicles which showed a different composition could be removed without detergent. These membranes exhibited a 3-fold lower specific activity of the gamma-glutamyl transferase while that of the alkaline phosphatase and nucleotide pyrophosphatase was similar as in detergent induced membrane vesicles. Distinct differences also were found in the protein pattern. The content of total cholesterol and phospholipid in vesicles shed spontaneously or after detergent treatment was nearly identical, however, significant differences were found in the fatty acid composition of the main phospholipids. The content of polyunsaturated fatty acids (linoleic and arachidonic acid) increased in the order: spontaneously shedded membranes, detergent induced vesicles, conventional purified plasma membranes. These results are discussed in terms of the heterogeneous composition of areas of the thymocyte plasma membrane.

Mesenteric vein thrombosis. The importance of a second-look operation.

Mesenteric vein thrombosis is an abdominal emergency that is rarely diagnosed early. Review of the literature has shown that most cases have been treated by intestinal resection, a few by thrombectomy with or without resection. Three cases of mesenteric vein thrombosis were treated by resection, heparinization, and a second-look operation after 24 hours. When necessary, resection was carried out at the second surgical procedure followed by a third-look operation after 24 hours. After the last inspecting operation without further resection, full heparinization was continued and anticoagulation was ultimately maintained with warafin sodium (Coumadin) for at least three months. The authors believe that optimal treatment for mesenteric vein thrombosis should include anticoagulation with heparin sodium and a mandatory second-look operation.

Management of post-traumatic vascular malformations of the liver by catheter embolization.

Trauma to the liver occasionally produces vascular malformations, usually pseudoaneurysms of the hepatic artery and sometimes arteriovenous fistulas. Hemorrhagic complications, usually in the form of hemobilia, can result, and portal hypertension is a late complication of an arterial-portal vein fistula. The true incidence of these post-traumatic lesions is apparently much higher than previously realized, and they can regress spontaneously. A case is presented in which both complications appeared simultaneously after a stab wound to the liver. Treatment using selective arterial catheterization and embolization with thrombogenic material was successful. The literature on the use of catheter embolization in hepatic trauma is reviewed. We suggest that catheter embolization be added to the standard therapeutic armamentarium for these lesions. Selective catheter embolization is suggested as the initial therapeutic maneuver in treating both of these lesions because of the relatively low morbidity, the great precision available in limiting the area of devascularization and the good expectation of success on the basis of the reported results of this technique in other traumatized organs, as well as the initially promising reports of its use in hepatic lesions. It should be considered at the time of diagnostic arteriography.

Recent observations on the serology of syphilis.

Routine screening of 404 742 sera by the automated micro-haemagglutination assay (AMHA-TP) and the Venereal Disease Research Laboratory (VDRL) test showed that 9848 specimens gave a reactive result to one of the three assays. Reactive results were confirmed by the fluorescent treponemal antibody absorption (FTA-ABS) test. The possibility of false-positive results varied from 0.04-0.38% of all specimens or from 1.7-15.7% of reactive sera. The VDRL test failed to detect reactivity in 56.54% of sera from patients who had previously been infected with Treponema pallidum. The importance of routine testing by the AMHA-TP is illustrated by the detection of four patients with mesaortitis and two with active neurosyphilis among a selected group of 54 patients who had non-reactive results to the VDRL test. Testing of cerebrospinal fluid specimens by the AMHA-TP test produced more specific results than by the other two tests.

Producing positive interaction among Down syndrome and nonhandicapped teenagers through cooperative goal structuring.

Effects of cooperative, competitive, and individualistic goal structures compared on the dimensions of interpersonal interaction and attraction. Subjects were nonhandicapped and Down syndrome junior high school students who participated in eight weekly sessions of recreational bowling. Results indicated that significantly more positive interactions and evidence of greater interpersonal attraction took place between the nonhandicapped students and the Down syndrome students in the cooperative condition than in the competitive and individualistic conditions. .

[Epsylon-aminocaproic acid in the treatment of severe and dominating hyperfibrinolysis in a premature infant].

A premature infant of 31 weeks gestational age weighing 1650 g developed pronounced generalized bleeding following exchange transfusion for rhesus-incompatibility. As a consequence there was continuous leakage into a subcutaneous haematoma that had been caused by accidental puncture of the infant's flank during diagnostic amniocentesis. The ensuing massive and prolonged loss of blood required replacement by infusions of heparinized fresh blood over several days. The most conspicuous detail in the longitudinal study of haemostasis was hyperfibrinolysis with values for Fibrin-Fibrinogen Split Products exceeding 1280 micrograms/ml. This situation could be treated effectively with epsilon-amino-capronic acid. We suggest that anti-fibrinolytic agents can be recommended under cover of heparin for treating those rare cases in which hyperfibrinolysis predominates and clinical criteria of disseminated intravascular thrombosis are missing. We would, however, strongly discourage the uncritical use of antifibrinolytic agents whenever Fibrin-Fibrinogen-Split Products are detected.

Endorphins in schizophrenia: hemodialysis/hemoperfusion are ineffective in clearing beta-Leu5-endorphin and beta-endorphin from human plasma.

Beta-Leu5-endorphin, a relative of "normal" beta-endorphin in which leucine is substituted for methionine at position 5 of the latter, has previously been found in high concentrations in the dialysate of schizophrenics. Its removal from plasma by means of hemodialysis has been claimed to relive the symptoms of schizophrenia. Using a highly sensitive radioimmunoassay of equal sensitivity to beta-endorphin and beta-leu5-endorphin, we have compared the plasma immunoreactivity of three schizophrenic patients befofe and after performance of their first session of membrane hemoperfusion. As compared to normal subjects, plasma beta-endorphin-like immunoreactivity was not greatly elevated in the schizophrenic patients before hemoperfusion.However, instead of the expected decrease, a consistent increase in the plasma levels of immunoreactive beta-endorphin was detected after hemoperfusion. In vitro experiments in which two different membranes and hemodialysis as well as hemoperfusion were used, revealed that synthetic beta-leu5-endorphin (and beta-endorphin) from human plasma was not cleared with any of these methods. This finding is inconsistent with the hypothesis that the claimed therapeutic effects of hemodialysis in schizophrenics are due to the removal of a beta-endorphin-like material from the plasma. Consequently, it seems to be unprobable that high concentrations of beta-leu5-endorphin occur in the dialysate or ultrafiltrate of schizophrenics.

Purification and some properties of five endo-1,4-beta-D-xylanases and a beta-D-xylosidase produced by a strain of Aspergillus niger.

Five different xylanases and a beta-D-xylosidase in the culture medium of Aspergillus niger have been purified to homogeneity from 13- to 52-fold by a procedure of gel and hydroxylapatite chromatography. The strain was isolated from soil of the African equatorial forest. Gel chromatography of the purified enzymes indicated that three of the xylanases have molecular weights of 31,000 and the other two xylanases have molecular weights of 50,000. beta-D-Xylosidase has a molecular weight of 78,000. The pH curves of the xylanases were quite diverse and showed pH optima ranging from 4.0 to 6.5. Characteristic action patterns were obtained for each of the purified xylanases by gel chromatography of the xylan digests on Bio-Gel P-2. The enzymes degraded arabinoxylan by an endomechanism, producing L-arabinose, D-xylose, xylobiose, and a mixture of branched arabinose-xylose and D-xylose oligosaccharides. All xylanases seemed to be capable of liberating L-arabinose from either arabinoxylan or the arabinose-xylose oligosaccharides. Branched arabinose-containing D-xylose oligosaccharides were slowly hydrolyzed, so that these sugars accumulate in the digest. Two xylanases showed relatively broad substrate specificity and were able to degrade also crystalline cellulose. beta-D-Xylosidase showed optimal activity at pH 6.7 to 7.0 and at 42 degrees C. The Km for o-nitrophenyl-beta-D-xylopyranoside was 0.22 mM and xylotriose was hydrolyzed more rapidly than xylobiose.

[Meridional amblyopia in humans. A simple clinical test].

The term meridional amblyopia (MA) defines an amblyopia in one meridian, orthogonally to which the visual acuity is normal. This applies to astigmatics. The clinically simple application of a test is described, and the results from examinations of 50 patients with astigmatism are reported. If the first optical correction is performed later than at the age of 6 years, an MA is bound to develop. An early and exact prescription of spectacles is therefore necessary.

[Serological demonstration of a Treponema pallidum reinfection].

An improved method for precise separation of 19-S-IgM from other immunoglobulines reveals the reappearance of antitreponemal 19-S-IgM-antibodies after reinfection in five patients. The IgM-FTA-ABS-test with non-separated full-serum was non-reactive in all cases due to competitive inhibition. The new assay apparently permits a clear differentiation between reinfection and non-specific increase of titers in lipoidal antigen tests. Treatment failures can be distinguished from reinfections in cases where this method can continuously be applied.
