Occupational health nursing education. Curricular content in baccalaureate programs.

It is important that occupational health nursing content be offered in academic institutions to support the scope of practice in the specialty. Less than 50% of the schools responding (N = 215) included content specific to occupational health nursing concepts and practice, related fields of knowledge, and management of the occupational health unit. Occupational health nurses in practice settings and educational institutions should collaborate on the development of occupational health content in nursing school curricula.

A study of hospital employee health programs.

Occupational hazards in hospital environments are well documented. Occupational health services, including health promotion and protection for hospital based employees, and environmental survellance activities, need to be increased and targeted for specific groups. Multi-disciplinary approaches to health hazard evaluation and risk reduction should be encouraged.

Diabetic ketoacidosis. Neurologic collapse during treatment followed by severe developmental morbidity.

Diabetic Ketoacidosis (DKA) remains the leading cause of death in children with type 1 diabetes mellitus. Complications occurring during DKA treatment include cerebral edema and neurologic collapse. Developmental outcomes following neurologic deterioration during DKA have varied from no sequelae to severe developmental disabilities. A total of three children developed neurologic deterioration during treatment of DKA at Buffalo Children's Hospital between 1984 and 1987. The authors treated aggressively for cerebral edema. Characteristic findings on the computed tomography (CT) scans and magnetic resonance imaging (MRI) of the brain included hemorrhagic infarctions of the thalami, basal ganglia and lentiform nuclei. The authors conducted developmental follow-up examinations between 1-1/2 - 3 years following recovery from DKA coma. Although they noted significant recoveries over time, developmental disabilities persisted. The clinical courses and neuroradiographic findings of these patients are compatible with sequelae of central brain stem herniation and cytotoxic brain injury. Continued efforts are needed in the prevention and early detection of clinically significant cerebral edema during treatment of DKA.

Occupational health nursing practice, education, and research. Challenges for the future.

The practice of occupational health nursing has expanded enormously and is influenced by company culture and forecasts as well as external factors such as population trends, technology, and the profile of the work force. Future trends include the provision of nurse managed occupational health services. Education and research in occupational health nursing must be strengthened to advance the practice of occupational health nursing. The occupational health nurse as the primary health care manager must take the lead in the development and implementation of a concept of health at the worksite.

Ethics and research.

Research subjects have the right to self-determination, informed consent, confidentiality and privacy, and freedom from harm. The use of coercion, such as a threat of harm, or excessive reward to influence subject participation in research is unethical. The benefits and risks related to participating in a research study should be disclosed to all subjects. In order to conduct the study, the benefits should exceed the risks.

Disparity curvature and the perception of three-dimensional surfaces.

BINOCULAR stereopsis provides information about the relative distance of objects from the differences in the horizontal position of their images on the two retinas. Because the size of the disparity between two points is inversely related to the square of the viewing distance, it is usually assumed that disparities have to be scaled according to distance using the vergence angle of the eyes, or by using the small vertical disparities that also exist between corresponding points of the two images. Here we present evidence that the visual system could extract information about the shapes of surfaces (without the need for scaling) by using the second spatial derivative of disparity--disparity curvature--which remains invariant with viewing distance. Rather than computing the second derivative, we suggest that an approximation to disparity curvature could be derived from the differences in curvature of corresponding line elements in the two eyes.

Employment and salary characteristics of nurse practitioners.

Nurse practitioners provide high-quality, cost-effective care to health care consumers, yet they continue to experience low economic compensation and salary compression. The nursing literature is limited with respect to data regarding salaries, benefits and trends in compensation for nurse practitioners. In order to provide baseline data for nurse practitioners to use in negotiating compensation benefits and packages, 459 certified nurse practitioners in North Carolina were surveyed regarding demographic and employment characteristics and job satisfaction. Of the 83 percent who responded, nearly two-thirds earned less than $30,000 per year. Benefits and salary adjustment for certification are reported, and techniques for compensation negotiation are discussed. Although information obtained is specific to North Carolina, geographic similarities may be apparent.
