Specific immunotherapy (SIT) has been used worldwide since it was introduced near the beginning of this century. Although this mode of therapy has been known for over 80 years, its mechanism of action is still not definitely clear. The problem of the presence of allergen-specific IgE in IgE-containing circulating immune complexes of patients with inhalant allergy has been mentioned in the literature. However, there are no data concerning specific immunotherapy. The aim of our study was to evaluate the influence of SIT in patients with inhalant allergy on allergen-specific IgE in serum and in IgE circulating immune complexes. A total of 112 subjects with allergic rhinitis (57 with grass pollen allergy and 55 sensitive to house dust mites Dermatophagoides pteronyssinus) were included in the study. SIT was administered to 29 patients suffering from pollinosis and 27 patients sensitive to house dust mites. The remaining patients were treated with an H2-receptor antagonist only. The decrease of allergen-specific IgE concentration in IgE circulating immune complexes was parallel to the analogous changes in the serum only in the patients with pollinosis. Immunotherapy of house dust mite-sensitive patients caused a much slower decrease of allergen-specific IgE in circulating immune complexes than in serum. The binding index of allergen-specific IgE in circulating immune complexes increased during the course of the treatment. No significant associations were found between the immunological indices studied and clinical score of the disease activity.
INTRODUCTION: Polyps are a nasal condition considered to be inflammatory in nature. Pathophysiology of that disorder is not fully known. The role of ICAM-1 in inflammatory and immune response through its involvement in leukocyte adhesion dependent phenomena is proved to be important. The aim of the study was to establish the role of sICAM-1 in the inflammatory process in nasal polyps. MATERIAL AND METHODS: 27 patients (23 males and 4 females) with nasal polyposis and 12 (7 males and 5 females) controls with septal deviation were included in the study. All patients underwent routine surgical treatment (polypectomy, septoplasty). The collected tissues of polyps and nasal mucosa were homogenized, placed in 0.04M EDTA and centrifuged. sICAM-1 concentration in supernatant of polyps and nasal mucosa tissue were determined with the ELISA method (Genzyme, UK). RESULTS: In the polyps tissue we found a significantly higher concentration of sICAM-1 compared to the nasal mucosa of healthy subjects (6.74 +/- 5.31 ng/ml vs 0.28 +/- 0.13 ng/ml; p < 0.001). The elevated concentration of sICAM-1 was positively correlated with total tissue eosinophil count (r = 0.51; p < 0.01) and the duration of the disease (r = 0.628; p < 0.001). No correlation was found between sICAM concentration and the age of the patients subjected to examination. CONCLUSIONS: The increased level of sICAM-1 in polyps tissue indicates the role of the molecule in the development of chronic inflammation and probably in the patophysiology of nasal polyps.
The aim of the study was to establish the frequency and follow the clinical course of depression and anxiety in patients with bronchial asthma. 57 patients (37 atopic and 20 nonatopic) with mild and moderate bronchial asthma and 53 subjects matched for age and sex as control groups were included in the study. The following questionnaires: Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI) and Hospital Anxiety and Depression Scale (HADS) were used to evaluate anxiety and mood state. The level and prevalence of anxiety and depression were significantly higher in both groups of asthmatics in comparison to healthy subjects. Symptoms were mild and moderate. Nonatopic patients were more depressed and anxious than atopics.

In 25 house dust mite-sensitive patients with perennial allergic rhinitis, an analysis of platelet aggregation tests (dual-channel aggregometer, Chronolog Corp, 345 model) induced by adenosine diphosphate (ADP) was carried out. The levels of total serum IgE specific antibodies against Dermatophagoides pteronyssinus and the soluble form of the low affinity IgE receptor (sFc epsilon RII/sCD23) were estimated as well. The study was carried out in a dynamic state, before and after 2 years of treatment with specific immunotherapy. We observed a significantly diminished platelet aggregation response, which partially improved after treatment. The results of this study suggest that platelet hyporesponsiveness might be involved in the pathogenesis of house dust mite hypersensitivity.
Randomized, double blind, placebo controlled clinical studies aimed at evaluating the efficiency of nedocromil sodium (Tilade) in the form of metered dosimeter aerosol. Studies involved patients with moderate chronic bronchial asthma controlled with beta 2-agonists and theophylline in the form of sustained release preparations. Forty patients completed the studies. All patients were examined clinically (staging of the symptoms and doses of drugs) and spirometrically prior to and after 4 and 8 weeks of the treatment with nedocromil sodium. Statistically significant clinical improvement and spirometric improvement as well in patients treated with nedocromil sodium were noted. It may be concluded that nedocromil sodium is effective and well tolerated adjuvant therapy in the bronchial asthma.
