[The immunological background of the allergen vaccines application].

The review and critical estimation of the studies concerning the mechanism of action of SIT was performed. The presented data give the reasons to view the allergen vaccines as the immunomodulators motivating simultaneously the applicability of these agents as the causative treatment of the allergic diseases.

Risk and safety of immunotherapy.

The risk and safety of specific immunotherapy (SIT) has been the subject of many well-designed studies since it was introduced. Most authors consider SIT an efficient and safe mode of treatment despite the occurrence of life-threatening anaphylactic reactions. They emphasize the importance of a proper selection of patients as well as the requirement for highly qualified physicians to provide the treatment. Another problem associated with the safety of SIT is the potential enhancement of the synthesis of immune complexes after the treatment. SIT causes a decrease in allergen-specific IgE which is slower in circulating immune complexes than in the serum. No data are available to show that SIT induces immune-complex diseases, in either the immunologic or the clinical sense. As far as anaphylactic reactions are concerned, new alternative methods different from the conventional parenteral mode of delivery seem to be safe. Our increased understanding of the altered functions of the immune system justifies the assumption that the more specific the method of SIT administration, the more effective and safe it is.

[Mechanism of action for corticosteroids].

The article is the review of the literature data concerning the molecular mechanism of action of corticosteroids in bronchial asthma and related diseases. Extensive research has led to the detailed understanding of the mechanisms underlying the cellular actions of steroids, their interactions with beta 2-agonists and reasons of steroid resistance.

[Theophylline in treatment of bronchial obstructive diseases].

The article is the review of the data from literature concerning the new role of theophilline in the management of asthma and chronic bronchoobturative pulmonary disease. Theophilline has been widely used to treat asthma and COPD patients worldwide and is classified as a bronchodilatator, although there is increasing evidence that it has antiinflammatory activity and enhances immunomodulatory effects. The latter is therapeutically relevant to markedly lower serum concentration (even < 10 micrograms/ml) than this required for bronchodilatation. That is particularly clinically important because significant adverse effects have been seen only in the case of doses greater than 10 micrograms/ml.

[The role of imperfect fungi in etiopathogenesis of allergic rhinitis].

In this paper the role of imperfect fungi in etiopathogenesis of perennial rhinitis was examined. In a group of 26 patients the concentration of total IgE and IgE specific of Candida albicans, Aspergillus fumigatus, Alternaria alternata, Mucor racemosus and Cladosporum herbarum was signified. Oversensibility to imperfect fungi was confirmed in 30.8% of patients: to Candida albicans in 3.8%, to Aspergillus fumigatus in 11.5%, to Alternaria alternata in 3.8%, to Mucor racemosus in 7.6% and to Cladosporum herbarum in 3.8%.

[Selected elements of biological activity of circulating immune complexes in serum of patients with atopic diseases. I. Ability to injure guinea pig tissue by injecting atopic patient plasma].

Serum level was measured of circulating complexes (SpA binding method-CIC/SpA) in pollen and house dust mite sensitive patients during specific immunotherapy. Histopathological changes in guinea pigs skin after the intradermal injection of patients serum have been studied. Differences were demonstrated in the observed patho-morphological changes in relation to CICs/SpA serum concentrations.

[Platelet aggregation and receptor expression of low affinity to IgE (Fc epsilon RII CD23) in patients with seasonal allergic rhinitis].

In 12 patients with pollinosis analysis of platelet function was carried out. Platelet function was assessed basing on the thromboelastogram curve and aggregation capability induced by ADP. Total serum IgE levels, as well as, specific antibodies to grass pollens, IgE and IgG4 levels and circulating receptor of low IgE affinity (sFc epsilon RII/sCD23) were also analysed. The study was carried out in a dynamic state, during natural allergen exposition and natural remission. A significant correlation was found between loss of platelet function and sCD23 levels. The observed relations were not influenced by seasonal changes.
