No deleterious pulmonary effects from moderate transfusions of microaggregate-rich blood in dogs.

Dogs were transfused with moderate amounts of blood (28% of calculated blood volume) containing microaggregates as reflected in high screen filtration pressure. In one group of animals, the blood was filtered through a standard commercial filter and in another group a micropore transfusion filter was used. Comparison of the transfused groups with nontransfused control animals revealed a statistically significant decrease in Qt and pH 1 h after blood transfusion. There were no differences between the two transfused groups. It is concluded that moderate blood transfusions with aggregate-rich blood did not seem to seriously compromise lung function. Furthermore, the changes found after transfusion were related to the presence of factors in stored blood other than microaggregates.

The effect of balloon catheter trauma on surface ultrastructure and fibrinolytic activity in the caval vein in rats.

Surgical procedures in veins such as thrombectomy are followed by a high complication rate. Re-occlusion is not uncommon in spite of primarily successful thrombectomy. The venous endothelium is damaged by the thrombotic process and the mechanical trauma from the balloon will be additive. The endothelial cells are the source of fibrinolytic activators. The present study compared changes in fibrinolytic activity in the vessel walls from the caval veins in rats to ultrastructural changes as seen in the scanning electron microscope. The balloon caused an almost complete denudation of the intima and the lesions were fully covered with platelets 1 hour after trauma. After 3 days reendothelialization had started. The fibrinolytic activity was significantly decreased at 1 and 24 hours following trauma. Reendothelialization was accompanied by return to normal fibrinolytic activity.

Cytophotometric determination of nuclear size and DNA distribution in different hyperfunctioning thyroid lesions.

Nuclear enlargement in hyperfunctioning thyroid lesions which has been found in earlier cytophotometric studies is also one of the criteria in the subjective histological estimation of thyroid function. Histopathological assessment is, however, often unreliable. In the present study stage scanning cytophotometric measurements in Feulgen-stained tissue sections were used to determine the nuclear changes encountered in non-toxic and toxic nodular goitre, and in toxic diffuse goitre. To ensure optimal selection of specimens for measurements autoradiography was used. Specimens of toxic diffuse goitre invariably had enlarged nuclei, but no difference was found between nodules in nontoxic and toxic nodular goitre. In fact, the same nuclear area was found in hot nodules, warm nodules and perinodular tissue in non-toxic nodular goitre, and in hot nodules in toxic nodular goitre. Thus there are lesions with clear-cut clinical, biochemical, and autoradiographic hyperfunction that do not have enlarged nuclei. Against this background it is possible that the nuclear enlargement present in toxic diffuse goitre reflects the disorder in itself and not the hyperfunctioning state. Hyperdiploid cell nuclei were found in all cases of toxic diffuse goitre and in a higher percentage than in the other lesions. It was not possible to distinguish nontoxic and toxic nodular goitre on this basis.

The cellular uptake of 5-hydroxytryptamine in the rat lung in vivo--a histochemical study.

The cellular uptake of 5-hydroxytryptamine (5-HT, serotonin) in the rat lung was studied in vivo after bolus injection of the amine (1 mg/100 g b.w.i.v.). The cells responsible for the 5-HT uptake were visualized by fluorescence microscopy according to the Hillarp-Falck technique and further identified as mast cells by Astra Blau staining. The quantitative aspects of the 5-HT uptake were studied by a specific cytofluorimetric method. A significantly increased intracellular content of 5-HT was found in interstitial pulmonary mast cells 1 min after injection. No 5-HT uptake was demonstrated within epithelial cells of the bronchiolar mucosa or within the pulmonary vascular endothelial cells; not even in animals pretreated with monoamine oxidase inhibitor. This finding is in contrast to the endothelial uptake earlier demonstrated by autoradiography in artificially perfused lungs. One explanation for this discrepancy may be a rapid transport of the amine across the vast endothelial area in the lung in vivo resulting in low 5-HT concentrations within the endothelium, not detectable by fluorescence microscopy. After endothelial passage the amine was rapidly taken up and stored in interstitial mast cells, which normally have the amine handling properties.

Residual carcinoma after intracavitary irradiation of endometrial carcinoma, stage I. A study of whole-organ sections.

Surgical specimens from 87 patients with endometrial carcinoma, stage I, preoperatively treated by intracavitary irradiation were investigated by a whole-organ sectioning technique. Re-examination of the curettage material showed cancer stage 0 in 8 (9%) and carcinoma state I in 79 cases (91%). In the stage I cases 57% showed residual carcinoma localized mainly to the myometrium of the uterine body and cornuae. Histological changes inthe carcinoma were unevenly distributed, but tumors growing close to the external uterine surface were well preserved. In 40.5%, no carcinoma was present, and in 2.5% the changes found could not be classified. Correlated to disparity in irradiation technique, no significant difference could be demonstrated in the incidence or localization of residual carcinoma. The most important single pretreatment factor combined with the presence of residual carcinoma was myometrial fragments with infiltrating carcinoma in the curettage material (MICC). Computerized analysis of combinations of factors showed that all patients aged over 60 years with a uterine-sound length exceeding 8 cm had residual carcinoma. Only 2 patients with infiltration of the myometrium had a completely destroyed carcinoma. The findings indicate that it is unlikely that endometrial carcinoma state I with myometrial invasion will be eradicated by intracavitary irradiation.

Early local and systemic fibrinolytic response to high energy missile trauma.

In dogs, tissue and blood fibrinolysis was studied after a high velocity missile injury to one hind leg. Plasminogen activator activity in skeletal muscle at different distances from the wound was estimated with a histochemical method. Samples from venous blood draining both the injured and the not injured leg were regularly analysed for fibrinogen, plasminogen, alpha 2-antiplasmin, alpha 2-macroglobulin, fibrinolysis on fibrin plates and fibrin(ogen) degradation products. Tissue close to the bullet track judged as devitalized displayed a significant and persistent reduction of plasminogen activator activity. The same pattern of reaction was observed in muscle without apparent lesions 3 cm from the wound but no uniform pattern could be discerned at a distance of 10 cm. In the blood a very early activation of fibrinolysis took place. There was no difference between the two sides, thus the trauma induced a general activation of fibrinolysis in the organism. Towards the end of the experiments a continuous decrease of fibrinolysis in the euglobulin fraction of plasma on fibrin plates was noted. Concomitantly an increase of plasmin inhibitors took place. It was concluded that a high velocity missile injury to skeletal muscle induced a reduction of plasminogen activator activity in the tissue surrounding the wound. A very early and general activation of systemic fibrinolysis was followed by a decreased fibrinolysis within a few hours.

Pulmonary microvascular leakage after microembolization and hemodilution.

This study examined the role of colloids versus crystalloids in pulmonary edema associated with the increased pulmonary microvascular permeability secondary to thrombin-induced pulmonary microembolism. Each of 23 healthy dogs received an intravenous injection of thrombin and a fibrinolysis inhibitor, which induced a microembolic state with increased (fivefold) pulmonary lymphatic flow and a lymph/plasma (L/P) protein ratio typical of a permeability change. Seven dogs received no treatment, eight received 15 ml/kg 10% dextran 40 (D40), and eight received 60 ml/kg Ringer's lactate solution (RL). Pulmonary water was measured serially by thermal conductivity and terminally by wet/dry weights. This preparation produced significant hemolysis; however, L/P ratios of hemoglobin approached unity in all groups. Initially there was hemoconcentration, which was reversed by RL and even more so by D40. Both D40 and RL temporarily raised the pulmonary artery and pulmonary artery wedge pressures to 15 mm Hg; D40 more than doubled the cardiac output of control or RL subjects--this was associated with a reduced pulmonary arteriolar resistance (P less than 0.05). In the early stage PaO2 was better maintained with D40 (P less than 0.02). Lymph flow increased and was comparable in all groups, as were lung water and lung weight, which tripled in all three groups. Results of this study indicated that in the presence of a pulmonary microvascular leak, colloids in doses that produced comparable microvascular pressures did not increase lung water and did not accumulate in the pulmonary interstitium. Colloids were superior to crystalloids in maintaining cardiac output, pulmonary vascular resistance, and oxygen tension in the early period after microembolism.U

[Effects of oestrogens on cardiovascular risk factors in patients with carcinoma of the prostate (author's transl)].

16 patients with advanced carcinoma of the prostate were studied in a prospective trial during treatment with oestrogens. Changes in plasma levels of gonadotropins (LH and FSH) and testosterone as well as salt-water balance, antithrombin III, fibrinolytic activity, plasma lipoproteins known to influence the risk of cardiovascular complications, were recorded during the initial 2 months. The plasma testosterone, LH and FSH concentrations were suppressed. The plasma volume was increased and the plasma albumin concentration was decreased. The antithrombin III concentration and the tissue fibrinolytic activity were decreased. The low density lipoprotein (LDL) level decreased and the high density lipoprotein (HDL) level increased. The recorded changes of cardiovascular risk factors indicate that the risk of water retention and thereby congestive heart failure and the risk for thromboembolic disease are increased during oestrogen treatment. The changes in lipoproteins with a marked elevation of the HDL/LDL ratio are thought to retard the development of atherosclerosis.

Studies on the pulmonary capillary permeability after induced microembolism.

Pulmonary microembolization was induced by infusion of thrombin during inhibition of the fibrinolytic system. After embolization cardiac output decreased, pulmonary vascular resistance and pulmonary arterial pressure increased. There was a transient decrease in PaO2. Immediately after embolization there was an increased permeability indicated by tremendous increase in lymph flow with a constant lymph/plasma protein ratio. The lymph/plasma ratio for hemoglobin and for FITC-Dextran (mw 150 000) also increased indicating leakage of large molecules. The increased permeability was accompanied by a significant increase in extra-vascular lung-water as measured both with the thermal conductivity and the dry/wet weight method.

The effects of orchiectomy, oestrogens and cyproterone-acetate on the antithrombin-III concentration in carcinoma of the prostate.

The incidence of thromboembolic complications is increased in patients with oestrogen-treated prostatic carcinoma. Because reduced antithrombin-III (AT-III) levels are associated with increased risk of thromboembolism we have determined AT-III concentrations during oestrogen therapy and other treatments. Forty-six patients with carcinoma of the prostate were allocated to either treatment with subcapsular orchiectomy, oestrogen administration, or cyproterone acetate, AT-III was determined before treatment, at 2 weeks and 2 months later. During oestrogen therapy there was a significant reduction in AT-III to 77% of the base-line value. No significant changes were found after orchiectomy. During cyproterone-acetate treatment there was a slight but significant increase in AT-III at 2 months. The reduction in AT-III could indicate an increased risk of thromboembolism during oestrogen treatment of patients with carcinoma of the prostate. On the other hand, the unchanged AT-III levels after orchiectomy and the increased levels during cyproterone acetate therapy could mean that the risk of thromboembolism is less with these two forms of treatment.

Tissue fibrinolysis in experimental gastric ulcer: a study in the rat.

Gastric mucosal lesions were induced in rats by pyloric ligature and intragastric instillation of hydrochloric acid. Within 4 h all rats developed focal mucosal lesions. Early regeneration was observed 72 h after release of the pyloric ligature and replacement of the hydrochloric acid by a phosphate buffer. A significantly increased gastric mucosal fibrinolytic activity was found 4 h after pylorus ligation. The submucosal vascular fibrinolysis remained unchanged. Following release of the pyloric ligature the increased mucosal fibrinolysis returned to normal values after 72 h. Intravenous administration of tranexamic acid significantly decreased the mucosal and vascular fibrinolytic activity without influencing the formation of induced gastric lesions. Increased mucosal fibrinolysis is probably not involved in the development of mucosal lesions.

Comparison on the pulmonary effects of rapid infusion of a crystalloid and a colloid solution.

The cardio-pulmonary effects of a rapid infusion of a crystalloid (Ringer's lactate) and a colloid solution (Dextran 40) were compared in healthy dogs. The colloid solution produced a long lasting volume expansion and hemodilution. The improved circulation in this group was reflected in an increased cardiac output and arterial blood pressure. The ventilation/perfusion ratios were reduced and the pulmonary vascular resistance was significantly lower in the colloid group in comparison both to the crystalloid group as well as to the control animals. The total oxygen consumption increased slightly in the colloid group but decreased in the crystalloid group. The postmortem extravascular lung water content did not differ in the groups.

[Effects of cyproterone acetate on cardiovascular risk factors in patients with carcinoma of the prostate (author's transl)].

15 patients with advanced carcinoma of the prostate were studied in a prospective trial during treatment with 200 mg cyproterone acetate daily by mouth. Changes in variables (salt-water balance, antithrombin III, fibrinolytic activity, plasma lipoproteins) known to influence the risk of cardiovascular complications were recorded during the initial two months. Blood and plasma volume were unchanged, whereas the 24 h urine volume and the urinary sodium excretion were transitory increased. The body weight was decreased. The antithrombin III concentration and the fibrinolytic activity were increased. The lipoprotein fractions HDL and LDL and the HDL/LDL quotient decreased. The recorded changes of cardiovascular risk factors indicate that the risk of water retention and thereby congestive heart failure is not increased during cyproterone acetate treatment. The risk for thrombembolic disease is rather decreased. The small changes in lipoprotein fractions can hardly affect the incidence of cardiovascular disease in elderly men with prostatic carcinoma.

Effects of estrogen, orchidectomy, and cyproterone acetate on tissue fibrinolysis in patients with carcinoma of the prostate.

Twenty-six patients with carcinoma of the prostate were allocated to treatment by subcapsular orchidectomy, estrogen administration, or cyproterone acetate administration. Tissue fibrinolytic activity in skin biopsy specimens was estimated by a histochemical method before treatment was initiated and 2 and 8 weeks later. Orchidectomy caused no significant changes in the fibrinolytic activity. During estrogen therapy the fibrinolytic activity in skin was significantly reduced at 8 weeks. In the group given cyproterone acetate the activity was significantly increased at 8 weeks. In the group given cyproterone acetate the activity was significantly increased at 8 weeks. The reduction in fibrinolytic activity in skin could contribute to the increased risk of thromboembolism during estrogen treatment of patients with cancer of the prostate.

Vascular trauma induced by clamping- correlation between surface ultrastructure and fibrinolytic activity.

The effects of application of a vascular clamp for 30 sec were studied on the femoral vessels in rats. Control and injured vessel segments were studied with scanning electron microscopy and with a histochemical method to estimate the fibrinolytic activity in the vessel walls. Endothelial injury with loss of endothelial cells was induced by application of the clamp. This was correlated to a reduction of the fibrinolytic activity in the vessel wall. In arteries re-endothelialization was seen three weeks after induction of the trauma, and it was completed after eight weeks. Venous lesions healed earlier. The re-endothelialization was followed by a return to normal fibrinolytic activity.
