[Expression of various monoclonal antibodies in nodules and band stage in Dupuytren's disease].

In 42 seronegative patients (n = 6 normal fascia, n = 12 nodal stage of Dupuytren's contracture, n = 24 stage IV) we performed immunohistochemical examinations of specimens of four locations of the digitopalmar fascia by means of indirect immunofluorescence tests. We determined the expression of monoclonal antibodies against vimentin, leucocyte common antigen (LCA, CD 45), four macrophage-antigens (27E10, RM3/1, 25F9, CD68), PDGFR-beta subunit (platelet-derived growth factor receptor). In 12 specimens of nodal stage disease macrophage-antigens typical for late inflammatory phase was detected. Other macrophage phenotypes were found to a lesser degree. All of these antigens were negative in normal palmar fascia and only sporadically positive in stage IV disease. PDGFR, which has not been investigated in digitopalmar flexion contracture before, has been expressed in cells of Dupuytren's disease as well as in normal fascia to a minor degree. In nodal stage disease, PDGFR-expression was not generally increased, although the majority of these cases showed intensively dyed clusters. EGFR could neither be detected in nodal or cord-stage of Dupuytren's disease, nor in normal digitopalmar fascia.

[What is the advantage of free microvascular tissue transfer after pelvic exenteration? A argument for delayed primary management].

Locoregional pelvic recurrences are amenable to re-resection even if musculoskeletal fixation is present. This report updates our experience with pelvic composite resections and microsurgical closure of the perineal cavity by nine latissimus dorsi flaps, one as a bilobed type combined with a scapula flap. All flaps survived. There was one rupture of the vascular pedicle following repositioning of the patient. Another flap healed completely, but presented a wound rupture with secondary healing in the area of the rima ani. Three major advantages to this method have been proven: (1) Healing and hospitalization could be reduced compared to conservative treatment; (2) The latissimus dorsi transfer allows stable perineal coverage and closure of dead space, preventing any pelvic herniation; (3) Performed as a delayed procedure the free tissue transfer is well tolerated by the patient.

Drug-resistant pulmonary tuberculosis in Berlin, Germany, 1987-1993.

Resistance of Mycobacterium tuberculosis (M.tb) strains is an increasing problem worldwide. Since no public health data are available for urban populations in Germany, we investigated resistance in our hospitalized patients (n = 1,011) over the last 7 yrs. We therefore evaluated clinical data and results of susceptibility tests (breakpoint technique/proportion method) for isoniazid, streptomycin, rifampin, pyrazinamide, protionamide and ethambutol. Since 1987, there has been a relatively constant rate of 5.9% (3.9%-7.8%) for single-drug resistance (SDR), but an increasing rate of multidrug-resistant (MDR) strains (> or = 2 first-line drugs) from 1.7% in 1987 to 5.8% in 1993. Sixty nine percent of patients with MDR strains showed resistance to two drugs, and 31% to three or more drugs. Risk factors for SDR and MDR tuberculosis revealed previous therapy (odds ratio (OR) (95% confidence interval (95% CI)); SDR 2.2 (1.7-4.0); MDR 4.5 (2.3-8.8)); and foreign-born status (SDR 2.2 (1.3-3.6); MDR 3.5 (1.8-6.8)) to be the most important factors associated with resistance. Both primary and acquired resistance were higher in foreign-born than in German-born patients. We conclude that there was a considerable increase in multidrug-resistant tuberculosis in our hospital from 1987 to 1993. Since previously treated patients and patients born in countries with a high level of primary resistance had an increased risk of drug-resistant tuberculosis, we would advise a four drug regimen as initial therapy in those patients.

Rapid drug susceptibility testing of Mycobacterium tuberculosis using conventional solid media.

Radiometric methods for M. tuberculosis drug susceptibility testing yield much faster results than standard techniques; however, these methods require sophisticated equipment and are expensive. We investigated a rapid drug susceptibility testing method for isoniazid, rifampin, ethambutol, streptomycin and pyrazinamide in specimens from 197 patients with pulmonary tuberculosis using a simplified agar-dilution method. Middlebrook 7H11 agar solid medium and microcolony detection were used to test sputum from 64 smear-positive, and from 70 culture-positive but smear-negative patients. Culture-positive material from bronchoscopy, surgical biopsy, pleural fluid or gastric fluid of 63 patients was tested. In 64 smear-positive patients, the median time for final susceptibility results was 11 days (95% confidence interval (95% CI) 10-12 days) compared to 62 days (95% CI 56-66 days) with the standard method. In 133 smear-negative patients, results were available after a median of 35 days (95% CI 32-40 days) in contrast to 72 days (95% CI 62-83 days) with the standard method, regardless of whether or not sputum or other materials were used for primary culture. The rapid method detected all cases of single-drug resistance (n = 20) and multidrug resistance (n = 14) within 13 days (95% CI 9-17 days) in smear-positive patients (n = 8), or within 38 days (95% CI 35-48 days) in smear-negative patients (n = 26). Only one discrepancy was encountered in 985 resistance tests. Moreover, contamination was not observed. Our rapid susceptibility testing method for M. tuberculosis on Middlebrook 7H11 agar is fast, practical and inexpensive. It provides an alternative when more sophisticated techniques are not available or affordable.

[Replantation in multiple injuries?].

While amputations of extremity parts are not uncommon in multiple trauma, replantation of these parts is not always feasible. Initial therapy will concentrate on plain survival. We suggest an aid to identify those patients with multiple trauma and extremity amputation, who should undergo replantation.

[Microsurgical tissue transplantation for correction of hand abnormalities].

Established procedures in hand surgery are dominating the management of congenital anomalies of the hand. In complex deficiencies and in certain forms of thumb hypoplasia however, replacement of structures capable of growing is needed, as can be free transplantation of vascularized metatarsal bone or free toe transplantation as substitute for a whole part of a limb. Capabilities of and limitations to microsurgical procedures are presented in selected cases.

Correction of hypoplastic thumb by free transfer of metatarsal bone: a case report.

A new form of correction of hypoplasia of the thumb is presented in a case of grade 3 thumb hypoplasia. The distal half of the second metatarsal together with its epiphyseal plate was used to lengthen the first metacarpal bone and improve thumb function. This method represents a promising alternative to pollicization in this type of thumb hypoplasia.

Heterotopic finger transfer in ulnar ray deficiency associated with contralateral postaxial polydactyly: a case report.

A case of type-I ulnar ray deficiency, combined with postaxial polydactyly of the opposite hand, is described. Heterotopic transfer of the supernumerary finger was performed, in order to improve function in the defective hand and to treat the hexadactyly of the other hand. A combination of these malformations has apparently not previously been reported in the literature.

[Measurement of serum protein-bound hexose--an aid in the diagnosis and after-care of colorectal cancers?].

In a group of 80 patients with colorectal carcinoma and a control group of 103 persons Protein-bound Hexose (PHex) was measured in the serum. The level was, as reported previously by other authors, significantly elevated in the tumour group. The aim of this study was to compare the sensitivity of PHex with that of the oncofetal antigens CEA and CA 19/9. The sensitivity of PHex turned out to be clearly lower than that of CEA. It was about as high as the sensitivity of CA 19/9 in colorectal carcinoma. In view of these results there is no benefit to be expected from an introduction of the PHex determination into clinical practice.

Activities of APh, gamma-GT, GlDH and GPT and bile acid concentrations in serum after bile duct obstruction and cycloheximide in the rat.

The aim of this study was to investigate the effect of the protein synthesis inhibitor cycloheximide on the serum activities of gamma-Glutamyltransferase, Glutamate dehydrogenase and Glutamate pyruvate transaminase after bile duct obstruction in rats. The results showed that the increase of these enzyme activities after bile duct obstruction is further augmented by cycloheximide. Determinations of total bile acids in serum and liver showed that also the bile acid concentrations rise to a higher level after bile duct obstruction + cycloheximide than after bile duct obstruction alone. It is concluded that the effect of cycloheximide on the serum enzyme activities in extrahepatic cholestasis is produced via an influence on the metabolism of bile acids. This stresses the central role of bile acids for alterations of enzyme activities in extrahepatic cholestasis. Further, the effect of cycloheximide on bile acids after bile duct obstruction has not been reported before.

[The activity of serum cholinesterases following experimental choledochus ligation in the rat].

The activity of serum cholinesterase was measured over three days after ligature of the common bile duct in female Wistar rats. A specific effect of the ligature was observed on the third day postop.: the activity decreased. The delayed reaction can be explained by the long half-life of the enzyme. Further, in all operated rats (sham-operated, too) a decrease of activity on the first postop. day was noticed. Possible causes are an influence of the anesthetics or sequels of the operative trauma.

[Functional reconstruction by neurovascular muscle and myocutaneous flaps of the lower extremity].

The management of resurfacing problems is simplified in using free, microvascular flaps in complex trauma or tumor surgery. To restore motor function neurovascular muscle or myocutaneous flaps are imperative. Therefore 29 neurovascular latissimus dorsi flaps in 28 patients among more than 570 free tissue transfers have been applied to the lower extremity. One patient had a monoblock transfer, while another had bilateral latissimus dorsi flaps to both legs. Reanimation of the neurovascular flaps was perceptible at an average of 13.5 months. Of the 29 neurovascular muscle, or myocutaneous flaps 27 developed a joint excurvation according to muscle grading scale: M3 to M5.
