Based on the variation in the optical density due to erythrocyte concentration and movement, the axial tomographic and image velocimetry techniques are respectively applied to determine the flow field, i.e., the distribution of erythrocytes and axial and radial velocity components, in steady blood flow through a curved glass capillary with a diameter of 180 microns. The data at four positions (two straight and two curved segments of the capillary) are recorded by a video-microscopic system on a video cassette. The erythrocyte and velocity distribution profiles change from symmetric at the straight position to an asymmetric shape at the curved sections. These profiles become symmetric again at the straight section of the capillary. The increase in the radial velocity component at curved portions is attributed to the secondary flow. The tomograms obtained by concentration profiles show respective changes in the cellular population at various cross-sectional positions. The kinetic energy dissipation, as calculated based on a determination of the flow field, is the minimum for the observed profiles. Any deviation towards parabolic form leads to the dissipation of a higher amount of energy.
OBJECTIVE: This study was carried out to evaluate the role of magnetic resonance imaging (MRI) in preoperative assessment of fistula-in-ano. SUBJECTS AND METHODS: Twenty-six patients (21 male and 5 female, age 19-65 years) were prospectively studied from July 1999 to December 2001 using a 1.0-tesla superconducting magnet. T(1)-weighted fast spin echo (T1W FSE) images before and after gadolinium injection and fat suppressed T2-weighted fast spin echo (T2W FSE) images were obtained in transverse and coronal planes. MRI findings were correlated with surgical findings. RESULTS: Twenty-one of the 26 patients demonstrated active fistulas. The MRI findings were in accordance with the examination findings under anesthesia and/or surgery in 15 of 16 cases. Both coronal and transverse planes were useful in assessing the location and direction of tracts and abscesses. Both contrast-enhanced T1W FSE and fat-suppressed T2W FSE images were useful in assessing the activity of lesions and the course of tracts. CONCLUSION: MRI showed accurate correlation with surgical findings and aided in preoperative management and planning for surgery.
Nearly three-fourth of the indigenous medical practitioners (74.37%) in the rural area of Agra were treating 15 patients in a day. Maximum number of patients was attended by trained practitioners. Only 21.82 percent of the practitioners were providing preventive services apart from curative services. Inspite of wide variations in level of training with consequent difference in knowledge, skills and practice, these practitioners still make a significant contribution to health care of the community provided they get some training of modern health system and state patronage. To start with, at least they should be given orientation towards vaccination as they are still giving tetanus toxoid only at the time of injury. In the same way training should be given regarding distribution of vitamin 'A' for prevention of night blindness and iron folic acid tablets for control of anaemia in vulnerable groups.
