[The health survey in Finnmark--how is the population satisfied with community health services?].

Global satisfaction with primary health care services among men and women aged 20 to 62 was examined in a comprehensive population study in the county of Finnmark in 1987-88. Access to general practitioners was found to be the most important determinant of global satisfaction. People were least satisfied in larger municipalities, in municipalities with low stability of doctors, and, when they had experienced difficulties in getting in touch with a doctor when the need arose. There was positive association between satisfaction with life in general and satisfaction with primary health care services. Various measures of health status, chronic disease and use of health services were found to have marginal or no impact on the overall assessment.

[Institutionalized treatment of agoraphobia with panic anxiety].

We describe an integrated behavioral-psychodynamic treatment programme for inpatients suffering from agoraphobia and panic anxiety. This programme emphasizes intensive drug-free exposure training followed by work with anxiety-laden dynamic conflicts. We present promising results of evaluation studies, and discuss the important role of cognitive reconstruction of anxiety and of passive and active avoidant attitudes in exposure training. Lastly, we make certain recommendations concerning the approach to anxiety problems in general practice.

Plasma levels of estradiol, estrone, estrone sulfate and sex hormone binding globulin in patients receiving rifampicin.

Plasma estrone, estradiol, estrone sulfate, androstenedione, testosterone and sex hormone binding globulin were measured in 14 patients (3 postmenopausal women, 11 men) with tuberculosis who received rifampicin. During treatment a moderate, but significant increase in the plasma level of estradiol (mean increase 32%, P less than 0.01) and estrone (mean increase 13%, P less than 0.01) were seen. In contrast, plasma estrone sulfate was significantly reduced (mean reduction of 25%, P less than 0.05). No alteration in plasma testosterone was observed, but there was a slight (mean 15%) increase in plasma androstenedione of borderline significance (P = 0.052). In eight patients, from whom all tuberculostatic treatment except rifampicin had been withdrawn, plasma sex hormone binding globulin was found to be increased by 75% by rifampicin treatment. Further, the results obtained in this part of the study confirmed the alteration in plasma estrone sulfate to be caused by rifampicin alone without any contribution from other tuberculostatic drugs. While plasma estradiol could be increased due to elevation of sex hormone binding globulin, plasma estrone was probably increased secondary to the increase in plasma androstenedione. A reduced plasma estrone sulfate level suggests that rifampicin enhances the rate of estrone sulfate metabolism. The possibility that treatment with drugs which reduce plasma estrone sulfate might be beneficial for hormone dependent cancers is discussed.

The prevalence of gallstones in autopsies from a Danish urban area. Frederiksberg, 1959-1985.

The prevalence of gallstones and cholecystectomies (S + R) was reviewed in nine annual autopsy series, from 1959 to 1985. The investigation confirmed the expectancy of more S + R with increasing age and among women. The median sex ratio was 1.6 (range, 1.3-1.7). It is noteworthy, however, that although the number of elderly people in the autopsy material increased during the period, there was a significant decrease in the prevalence of S + R (p = 2 X 10(-8]. The total prevalence for men decreased from 26% to 18% (p = 2 X 10(-3] and for women from 44% to 31% (p less than 10(-10]. The most striking decrease was seen in the age groups from 60 to 80 years. After a possible culmination during the period from 1920 to 1960, the number of gallstones seemed to level off again. The rates were the same as at present in other Danish investigations from the Copenhagen area (1889, 1911, and 1920-21).

Femoral shaft fracture after hip arthroplasty.

Femoral shaft fractures after hip arthroplasties were treated in 74 noncemented hemiarthroplasties and 65 cemented arthroplasties. In loose prostheses the best clinical results and the least number of operations were achieved with revision arthroplasty with a long-stem prosthesis, combined with simple internal fixation methods when applicable. In firmly fixed prostheses the results of revision arthroplasty and traction treatment were similar. Cemented revision arthroplasty did not interfere with fracture union. Internal fixation with the prosthesis in situ cannot be recommended because of a large number of secondary revision arthroplasties and nonunions. Removal of the femoral stem prosthesis and internal fixation nearly always require a secondary revision and cannot be recommended.

Ultrastructure of germ cells and adjacent somatic cells correlated to initiation of meiosis in the fetal pig.

The ultrastructure of female and male germ cells and associated somatic cells were studied in morphologically sex differentiated fetal pig gonads from day 27 to day 95 post insemination. Before meiosis starts in the ovary, the organelles of germ cells and somatic cells of both sexes are poorly developed. In oocytes in leptotene stage, the endoplasmic reticulum attains close proximity to the plasma membrane forming characteristic contact areas which in addition are only seen in male germ cells of the same age. As meiosis progresses, the organelles of the oocyte increase in number and degree of differentiation. In particular the ER is prominent in the diplotene stage. At midgestation the male germ cells become polarized, the organelles gathering at one side of the nucleus. In the granulosa cells the number and extension of organelles increase concomitantly with the oocytes proceeding through meiosis. The Sertoli cells grow progressively complex in shape, with numerous mitochondria and a prominent ER, whereas the Golgi complex remains poorly developed. Small dense bodies are present both in germ cells and somatic cells. They are electron dense, membrane bounded, rounded or elongated granules often connected with endoplasmic reticulum. No activity of peroxidase, catalase or acid phosphatase could be traced in the granules, making them improbable candidates as peroxisomes or lysosomes. Their number is high in all germ cells and somatic cells on day 27 post insemination and in germ cells in leptotene stage.

Hemi-arthroplasty of the hip followed by ipsilateral fracture of the femoral shaft.

In a series of 74 ipsilateral fractures of the femoral shaft in relation to hemi-arthroplasties, treatment by a cemented long-stem total hip replacement was found to be superior to conservative treatment or internal fixation without removal of the prosthesis. Acceptable clinical results were obtained in 89 per cent of these cases.

Serum amyloid A protein in acute myocardial infarction.

Tissue injury including myocardial infarction leads to a variety of changes in plasma proteins commonly referred to as "the acute phase response". In this report the concentrations of serum amyloid A protein (SAA) were measured serially in 6 patients with myocardial infarction and 4 with angina. SAA was found to be increased in all patients with infarction, but in no patients with angina. Significantly increased SAA levels were detected 12 hours after the peak level of creatine kinase, and the concentrations of SAA seemed to correlate to the amount of damaged tissue. The SAA-response was both faster and more extensive than the response of C-reactive protein (CRP), but the correlation between SAA and CRP was very good.
