Electrosurgical dissection to reduce blood loss in head and neck surgery.

We prospectively determined the intraoperative blood loss in 250 patients who underwent major head and neck surgical procedures over a 13-month period to demonstrate the efficacy of electrosurgical dissection for reducing blood loss and to determine those factors predictive of the need for blood replacement. Transfusions were required in 30 (12%) of the 250 patients, and a total of 66 units of packed red blood cells was administered. Two patients were transfused preoperatively, 16 patients intraoperatively, and 14 patients postoperatively. Factors predicting the necessity for blood replacement included the patient's preoperative hematocrit level, intraoperative blood loss, the duration and type of procedure, and the surgeon's level of experience. The principles of electrosurgical dissection are discussed.

A highly specialized and motivated volunteer population: platelet donors.

The plateletpheresis program of the M.D. Anderson Hospital was established in 1963. Approximately 20,000 units of platelet concentrates are produced per year. Credit for the nonreplacement fee generates the greatest number of donors. No organized recruitment campaign exists. Each donor becomes a recruiter for the program. Also involved in donor recruitment are all laboratory personnel, as well as Donor Room exployees.

Extended storage of human platelets at 22 C: changes in glycogen and adenine nucleotide metabolism.

The energy metabolism of human platelets stored at 22 C with continuous agitation has been studied. Freshly prepared platelets contain 9.7 mg of glycogen, 9.1 mu moles of ATP, 3.7 mu moles of ADP, 0.9 mu moles of AMP per 10(11) platelets and have an adenylate charge of 0.8. In the first 72 hours of storage there is no appreciable change in the glycogen content, adenylate charge and rate of ATP pool labeling. Therefore, on the basis of these results, the early impairment of function reported by others cannot be ascribed to alterations in the platelet energy metabolism. Upon further storage, however, there is a progressive loss of adenylate and glycogen while hypoxanthine accumulates extracellularly.

Modified fluid gelatin. An alternative macromolecular agent for centrifugal leukapheresis.

We studied a French modified fluid gelatin (MFG), substituting it for hydroxyethyl starch (HES) in leukapheresis procedures using three currently available blood cell separators, and observing its effects on the function of platelets and granulocytes. As a cell-collecting agent, we found MFG to be as effective as HES with intermittent flow centrifugation (Haemonetics), and slightly less so with one continuous flow device (IBM 2997). MFG was clearly less effective than HES with the Fenwal CS-3000 continuous flow separator, although we have reason to believe it would be possible to improve efficiency with this machine by changing the operating variables. Tests of platelet and granulocyte function showed negligible alteration with either agent and no difference between them. MFG disappears much more rapidly from the circulation than HES (after a single injection, it is undetectable by the third day). Reaction frequency with MFG was about the same as that of HES, with perhaps somewhat more frequent allergic manifestations.

Perioperative blood transfusion in patients with colon carcinoma.

The effect of perioperative blood transfusion on 5- and 10-year survival following curative resection for intra-abdominal colon carcinoma was investigated retrospectively. Two hundred and seven patients with Dukes' stages A, B, and C carcinoma were divided into two groups: those who received perioperative blood transfusion and those who did not. The data indicated that survival was not altered by blood transfusion during primary surgical tumor resection. Location of tumor in intra-abdominal colon, size of tumor, units of blood received, and stage of disease also were considered in our analysis but did not change this conclusion.

T and B cell populations in platelet donors.

T and B lymphocyte populations were monitored in the peripheral blood of two long-term plateletpheresis donors. The studies showed minimal fluctuation of both populations compared to the normal controls. The evidence so far accumulated seems to indicate that long-term manual plateletpheresis has no appreciable effect on the level of the T and B cell population of the peripheral blood. Further studies are needed to evaluate larger donor pools.
