Measurements have been made of the distribution of natural ultraviolet radiation (UVR) at 41 sites on the face of a large fibreglass model of a head. The dosemeter used was the polymer film polysulphone. The results have been correlated with published values of the distribution of basal cell carcinomas on the face. Although the comparison has highlighted the problems of trying to relate frequency of skin tumour occurrence to sunlight exposure, the present results are compatible with the hypotheses that human skin cancer incidence increases with environmental UVR exposure, and that sunlight is not the only factor in the aetiology of basal cell carcinomas of the face.
Seventy-six patients with advanced gastric adenocarcinoma were studied in a prospecitive, randmoised, controlled trial using vincristine, methotrexate, cyclophosphamide, and 5-fluorouracil in an initiation course and mitomycin-C with 5-fluorouracil as maintenance therapy. Thirty-seven patients were inoperable and 39 had the primary tumour resected with histological evidence of residual disease. Survival in the inoperable group was short and showed no significant difference between treated and control patients. The median survival times for treated and control groups were 9.5 and 9.0 weeks respectively. In the resected patients there was no difference in ultimate overall survival between the groups but up to 20 weeks there was a suggestion that the probability of survival in treated patients was higher (P = 0.06). The patients were well-matched and it is concluded that chemotherapy has had an early effect but that a further trial with more detailed stratification, particularly of staging and histological grade, is needed. No patient received treatment for longer than two years and unacceptable toxicity occurred in only two patients. Nausea occurred more frequently in the treated group but was short-lived and clinically manageable.

The response of plasma-25-hydroxyvitamin D (25[OH]D) to different exposures to ultraviolet irradiation has been studied in patients in long-stay geriatric wards. Increases sufficient to bring the plasma-25(OH)D into the normal range may be obtained with doses less than those required to produce erythema. The provision of such background irradiation may be a suitable method of preventing vitamin-D deficiency in elderly subjects who receive very little exposure to sunlight.

The production and optical properties of a dosemeter for natural ultraviolet radiation are descirbed. The physical form of the dosemeter is a thin polymer film which incorporates 8-methoxypsoralen as the chromophore. The dosemeter responds to ultraviolet radiation at wavelengths up to 380 nm and is suitable for quantifying the UV exposure of patients suffering from some idiopathic photodermatoses or drug photosensitivity.

Measurements have been made of the anatomical distribution of ultraviolet radiation resulting from the irradiation of an inanimate manikin in a PUVA 6000 unit and a PUVA 4000 unit. The dosimeter used was phenothiazine film. The results indicated a very similar pattern of relative UV dose on the surface of the manikin from both units.
Preliminary work on a dosimeter suitable for measuring UVR exposure on individual persons is described. It is based on the use of polysulfone film which, on exposure, shows a change in absorbance that is determined spectrophotometrically. The results of studies of groups of persons with vastly different degrees of solar exposure, e.g., outdoor and indoor workers and geriatric patients, and psychiatric patients with photosensitivity from phenothiazine drugs are presented. We consider the dosimeter may also be suitable for epidemiologic studies in sunlight-induced skin cancer.

Empirical equations based upon experimental measurements of Bener (1972) have been developed which allow the computation of the spectral distribution of global ultraviolet radiation encountered at sea level under clear day conditions for any desired time of day, day of year and geographical latitude.

The relative distribution of natural ultraviolet radiation has been measured at ten sites on the surface of an unclothed manikin for 2 h around solar noon on 19 different days during late summer in Canterbury, U.K. The dosimeter used was polysulphone film. The results obtained indicate that vertical surfaces of the body receive about one half of the dose relative to the vertex and that the relative doses at different sites are independent of weather conditions.
A hypothetical action spectrum for the regression of lesions in photochemotherapy of psoriasis is proposed. An ideal detector should have a wavelength response which matches this action spectrum but this is generally not the case with commercially available photometers. The problems that may arise by indiscriminate use of such detectors are discussed, together with a suggested solution.
