Pilot study comparing the photoelastic stress distribution for four endodontic post systems.

This pilot study compared stress distribution using photoelastic blocks for both vertical loading and oblique loading (26 degrees angle) for four groups of post systems (Flexi-Post, Flexi-Flange, ParaPost and AccessPost dowels). For each group, three test blocks of photoelastic material were prepared with simulated endodontic canals, and the specimens were then photographed. Minimal stresses were observed for all the posts after cementation in the unloaded state. Stress distribution for the Flexi-Post, Flexi-Flange and AccessPost were favourable under vertical loading at 134 N and oblique loadings of 89 N and 134 N. Stress patterns were symmetric over the entire length of these posts. Stress distribution for the ParaPost under vertical loading at 134 N generated stress patterns that were asymmetric with concentration of stresses apically compared to coronally. Stress distribution for the ParaPost during oblique loadings of 89 N and 134 N were asymmetric with stress concentrated apically as compared to coronally. Stress distribution for the Flexi-Post, Flexi-Flange and AccessPost were all favourable under loading conditions studied.

Comparison of the fractural strength for three combination cast post systems.

This study compared the fractural strength measured on a 45 degree angle for three combination cast post systems (Parapost, ExactaCast and Luminex 2000). A force applied at a 45 degree angle on the bench was used to simulate the clinical situation. Many similar studies are found in literature. The experiment was divided into three groups made up of 10 samples each. Castings were fabricated in semi-precious metal from: Parapost burnout post / Duralay patterns (group 1), ExactaCast / a new butyl methacrylate acrylic (AccuSet) patterns (group 2) and Luminex 2000 / Lumin-X light cured gel patterns (group 3). The fractural strength [tests] were performed using a MTS testing machine and failure was measure in pounds. Statistical analysis including a student's test were performed to ensure complete significance between groups. The ExactaCast system had the highest mean (283.9 pounds), while the Luminex 2000 system had the lowest (255.1 pounds). There was no statistical difference in the fractural strength for all three groups studied. It seemed that the fractural strength of tooth structure represents the highest level of failure obtained regarding this experimental test constraint.

15 alpha-hydroxylation of a bile acid analogue, sodium 3 alpha,7 alpha-dihydroxy-25,26-bishomo-5 beta-cholane-26-sulfonate in the hamster.

The metabolism of 3 alpha,7 alpha-dihydroxy-25,26-bishomo-5 beta-cholane-26-sulfonate (bishomoCDC-sul), the sulfonate analogue of bishomochenodeoxycholic acid, and its effect on biliary bile acid composition were studied during chronic administration in the hamster. After oral administration of radiolabeled bishomoCDC-sul, more than 80% of the radioactivity was excreted into the feces within 7 days, both as the unchanged sulfonate (38.5%) and two more polar metabolites (50.0% and 11.5%). The half time of the fecal excretion was 1.6 days. In gallbladder bile, the unchanged sulfonate and its major metabolite accounted for 19.1% and 19.8% of total bile acids, respectively. In another experiment, hamsters were fed bishomoCDC-sul with antibiotics to evaluate the site of biotransformation. Even when the number of intestinal microorganisms was greatly reduced, the same three metabolites were found in the feces: bishomoCDC-sul (44.0%) and the two polar metabolites (30.8% and 25.1%). The major metabolite was isolated from feces of the hamsters fed bishomoCDC-sul without antibiotics. Its chemical structure was identified by mass spectrometry and nuclear magnetic resonance spectroscopy as the 15 alpha-hydroxylated derivative, namely sodium 3 alpha,7 alpha,15 alpha-trihydroxy-25,26-bishomo-5 beta-cholane-26-sulfonate. These results indicate that after oral administration, the sulfonate analogue of bishomochenodeoxycholic acid underwent enterohepatic circulation like a natural bile acid and was transformed, in part, into the 15 alpha-hydroxylated derivative and another more polar metabolite in the liver of hamsters. There was no evidence that bishomoCDC-sul was dehydroxylated to a lithocholic acid analogue during enterohepatic cycling.

Hydrophilic bile acids: prevention and dissolution experiments in two animal models of cholesterol cholelithiasis.

The effects of beta-muricholic acid and hyocholic acid on cholesterol cholelithiasis were examined in two animal models. The following experiments were carried out: A) In a gallstone prevention study, prairie dogs were fed the lithogenic diet with or without 0.1% beta-muricholic or 0.1% hyocholic acid for eight weeks. B) In a second prevention study, hamsters were fed the lithogenic diet with or without 0.1% beta-muricholic acid or 0.1% hyocholic acid for six weeks. C) In a gallstone dissolution study, hamsters were fed the lithogenic diet for six weeks to induce stones; stone dissolution was examined during administration of a cholesterol-free purified diet with or without 0.1% beta-muricholic acid or 0.1% hyocholic acid. In the prevention study in prairie dogs (A), both bile acids failed to prevent stone formation, the cholesterol saturation index of bile was 0.89 in the lithogenic controls, remained unchanged with hyocholic acid and increased to 1.52 in the beta-muricholic acid group. In the prevention study in hamsters (B), beta-muricholic acid completely inhibited the cholesterol cholelithiasis (0% stone incidence); the cholesterol saturation index of bile was 1.78 (compared to lithogenic controls, 1.37). Hyocholic acid reduced stone incidence to 16% with a cholesterol saturation index of 0.98. In the dissolution study in hamsters (C), preexisting cholesterol gallstones were not dissolved by either hydrophilic bile acid after feeding these bile acids for an additional six weeks; at the end of the experiment, the cholesterol saturation indices were below unity.(ABSTRACT TRUNCATED AT 250 WORDS)

Dietary fat alters the distribution of cholesterol between vesicles and micelles in hamster bile.

The type of dietary fat strongly affects the incidence of gallstones in the hamster model of cholesterol cholelithiasis. The present study was designed to determine whether dietary fats could affect gallstone formation by altering the microstructure (vesicular/micellar ratio) of cholesterol in bile. Golden Syrian hamsters from Sasco (Omaha, NE) or Charles River (Wilmington, MA) were fed nutritionally adequate semipurified diets to which were added: (i) 4.0% butterfat without added cholesterol; (ii) 1.2% palmitic acid plus 0.3% cholesterol; or (iii) 4.0% safflower oil plus 0.3% cholesterol. Gallstone incidence and the percentage of cholesterol in vesicles and micelles were determined after two- or six-week feeding periods. Three out of ten Sasco hamsters fed the 1.2% palmitic acid diet for two weeks had cholesterol stones, while none of the eight Charles River animals had stones. In the Sasco hamsters, a significant proportion of the biliary cholesterol was found in void volume vesicles (28.8%) and small vesicles (17.1%); Charles River hamsters had negligible proportions (1.1%) of cholesterol in void volume vesicles and 15.4% in small vesicles. Cholesterol gallstones were most abundant in Sasco hamsters fed 1.2% palmitic acid for six weeks (nine out of ten animals); the mean cholesterol saturation index of the bile was 1.27. A significant proportion of the biliary cholesterol was eluted in the void volume vesicles (21.4%) and in small vesicles (15.0%). Five of the eight identically treated Charles River hamsters had cholesterol stones; the cholesterol saturation index averaged 1.36, and the biliary cholesterol was present in void volume vesicles (31.3%) and small vesicles (14.3%).(ABSTRACT TRUNCATED AT 250 WORDS)

Synthesis and metabolism of sodium 3 alpha,7 alpha-dihydroxy-25,26-bishomo-5 beta-cholane-26-sulfonate in the hamster.

This paper reports the chemical synthesis of a new bile acid analogue, namely sodium 3 alpha,7 alpha-dihydroxy-25,26-bishomo-5 beta-cholane-26-sulfonate (bishomoCDC-sul) from chenodeoxycholic acid and describes its metabolism in the hamster. The structure of the new compound was confirmed by proton and carbon-13 nuclear magnetic resonance spectroscopy. After intravenous infusion of [3H]-labeled sulfonate into bile fistula hamsters, it was extracted by the liver and secreted into the bile; more than 65% of the radioactivity was recovered in the bile within 1 h. Following intraduodenal administration of the [3H]sulfonate and [14C]chenodeoxycholyltaurine, both compounds were excreted into the bile more slowly; only 41 and 43% of the radioactivity, respectively, were recovered in the bile during the four-hour experimental period. In contrast, when the labeled compounds were injected into the terminal ileum, both the sulfonate and chenodeoxycholyltaurine were rapidly absorbed and secreted into the bile; 84 and 97%, respectively, of the radioactivity were recovered during a four-hour period. Chromatographic analysis demonstrated that in these short-term experiments most (> 95%) of the sulfonate was secreted into the bile without biotransformation regardless of the route of administration. When infused intravenously at increasing doses, bishomoCDC-sul induced cholestasis at an infusion rate of 1 mumol/min/kg. These results suggest that sodium 3 alpha,7 alpha-dihydroxy-25,26-bishomo-5 beta-cholane-26-sulfonate was absorbed from the terminal ileum by active transport, extracted by the liver, and secreted into the bile in a manner similar to that of the natural bile acids.

Comparison of the torsional forces at failure for seven endodontic post systems.

Resistance to torsional forces is critical in restoration of endodontically treated teeth. Seven post designs (Flexi-Post, Flexi-Flange, Para-Post, AccessPost, World Post, Vlock, and Dentatus posts) were cemented in roots of natural teeth with zinc phosphate and Flexi-Flow cements, which resulted in 11 different groupings that were subjected to torsional forces in a clockwise direction. In addition, clockwise and counterclockwise torque was applied to the Flexi-Post and Flexi-Flange systems cemented with zinc phosphate and Flexi-Flow cements. A total of 150 samples were prepared, and groups for this study were: group 1, Flexi-Post/zinc phosphate clockwise and counterclockwise; group 2, Flexi-Post/Flexi-Flow clockwise and counterclockwise; group 3, Flexi-Flange/zinc phosphate clockwise and counterclockwise; group 4, Flexi-Flange/Flexi-Flow clockwise and counterclockwise; group 5, Para-Post/zinc phosphate clockwise; group 6, AccessPost/zinc phosphate clockwise; group 7, AccessPost/Flexi-Flow with grooved dentin clockwise; group 8, World Post/zinc phosphate clockwise; group 9, World Post/Flexi-Flow with grooved dentin clockwise; group 10, Vlock Post/zinc phosphate clockwise; and group 11, Dentatus post/zinc phosphate clockwise. Torsional forces for the groups ranged from 17 ounce-inches (Dentatus/zinc phosphate) to 81 ounce-inches (Flexi-Post/zinc phosphate). Flexi-Post and Flexi-Flange threaded posts exhibited statistically greater resistance to torsional forces. Analyses were computed with one-way and three-way analysis of variance followed by Duncan's multiple range test. Duncan's multiple range test indicated that Flexi-Post/zinc phosphate/clockwise was similar to Flexi-Post/Flexi-Flow/clockwise and that both had significantly higher torque levels than the other treatment conditions in the clockwise direction (p 0.0001). Dentatus/zinc phosphate/clockwise had significantly lower torque levels than the other treatment conditions (p 0.031).

Presentation of a new Flexi-Flange post system for clinical use.

Advantages of a new post design, Flexi-Flange are described. It incorporates a split-shank parallel threaded post with a flange that seats into an occlusal countersunk dentine space. This case report describes placement of three Flexi-Flange posts in a first bicuspid, canine and central incisor. The Flexi-Flange system was designed to offer high retention of the post in the root, superior distribution of stresses on insertion and under function, and long-term resistance to metal fatigue.

Dimensional accuracy of three different alginate impression materials.

PURPOSE: The purpose of this study was to evaluate the dimensional stability of three different alginate impression materials (Jeltrate [Caulk Dentsply, LD Caulk Division, Dentsply International, Milford, DE], Hydrogum [Zhermack, Padoua, Italy], and a new hydrophilic alginate material [Essential Dental Systems, South Hackensack, NJ] under five different storage conditions. MATERIALS AND METHODS: The study was divided into four groups with at least eight samples in each group. The five storage conditions were as follows: immediate, 10 minutes with a wet towel, 30 minutes without a wet towel, 1 hour with a wet towel, and 24 hours with a wet towel. A 4 X 5 factorial two-way analysis of variance (P<.05) was used on each dependent variable. Dunnett's test was used to compare each alginate with Jeltrate. RESULTS: The new hydrophilic alginate (group 4, 2:2) showed smaller measurements in the buccolingual and diagonal dimensions compared with Jeltrate (control). CONCLUSIONS: The immediate method (measured buccolingually and diagonally) produced more accurate casts for the alginate materials studied. All other materials studied were similar statistically. The new hydrophilic alginate in group 3 resulted in similar dimensional accuracy compared with Jeltrate and Hydrogum and incorporated 25% less water.

Hormonal control of cholesterol cholelithiasis in the female hamster.

Male golden Syrian hamsters from Sasco form cholesterol gallstones when fed a lithogenic diet; in contrast, female hamsters are resistant to stones when fed the identical diet. Upon addition of the synthetic androgen, methyl-testosterone, to the diet, the incidence of cholesterol gallstones in female hamsters increased from 0% to 40% after 3 weeks and from 0% to 86% after 6 weeks. Cholesterol cholelithiasis remained high in the males. Biliary cholesterol and phospholipid levels were elevated in the females fed the hormone and approached those of the males. The cholesterol saturation of bile in the females increased from 36% to 75% after 3 weeks and from 54% to 109% after 6 weeks. In addition, an appreciable proportion of the cholesterol in the bile of female hamsters was now present in the form of vesicles. The bile acid composition was significantly altered by methyltestosterone even though the total bile acid concentration did not change; the bile acid composition of the female hamsters approached that of the males. The glycine to taurine ratio of the bile acids was drastically reduced by methyltestosterone in the females and to a lesser extent in males. In summary, in female hamsters the addition of methyltestosterone to the lithogenic diet induced cholesterol gallstones, elevated total biliary phospholipid and cholesterol, altered the bile acid composition, and changed the distribution of cholesterol from micelles to vesicles. The data obtained so far do not enable us to define the precise mechanism of action of methyltestosterone.

A 10-year literature review of a split-shanked threaded post.

A review of 10 years of literature on prefabricated threaded and passive post systems is presented along with recent literature supporting the validity of the split-shanked threaded post design, Flexi-Post. It is important to note that a split-shanked threaded post is altogether different from a solid-shanked threaded post. The research discussed in this article confirms the high retention of the Flexi-Post and dispels the notion that high retention must be accompanied by high stress.

Comparison of the photoelastic stress properties for two combination prefabricated cast post systems.

The reason for a post and core preparation is to provide an anchoring system for a final restoration to the tooth root. This experiment was divided into two groups with the following systems; ExactaCast and Parapost. Casts for these systems were fabricated in photoelastic plastic and cemented with zinc phosphate cement. These photoelastic blocks were placed in a special jig on a universal testing machine. The experiment compared the photoelastic stresses generated for these two systems in two different stress modes; vertically at 30 pounds and obliquely (at a 26 degree angle) at 20 and 30 pounds. Specimens were then photographed after cementation in the unloaded and loaded states. Minimal stresses were observed for the ExactaCast in the unloaded state, while symmetric, even patterns of stresses concentrated coronally were present in the loaded vertical and oblique states. Minimal stresses were observed for the Parapost in the unloaded state, while asymmetric, uneven patterns of stresses concentrated apically were present in the loaded vertical and oblique states. The presence of the multiple tiered system for the Exacta-Cast clearly directs stresses in a symmetric pattern, while the single tiered Parapost system directs stresses in asymmetrical pattern. The symmetric even stresses for the ExactaCast are more favorable than the asymmetric, uneven stresses for the Parapost.

Fluoride release from four reinforced composite resins; a one year study.

PURPOSE: Four fluoridated composite resins were described and separated into four groups. Two composite resins were high viscosity core materials Ti-Core (Group 1) and Ti-Core Natural (Group 2) and two were low viscosity core materials Flexi-Flow Cement with titanium (Group 3) and Flexi-Flow Natural (Group 4). MATERIALS AND METHODS: This study recorded the fluoride release measured in parts per million in deionized water for each of four fluoridated composite resins for one year. RESULTS: Repeated measures analysis of variance (RMANOVA) was computed to separately analyze interval release and cumulative release over time and between groups. One-way analysis of variance (ANOVA) (P<0.0001) was selected to compare average weekly release between the four groups. The student-Newman-Keuls (SNK) test revealed that the mean weekly release was similar for both Flexi-Flow Natural and Flexi-Flow with titanium. CONCLUSIONS: The Ti-Core systems of groups 1 and 2 were both different from the Flexi-Flow systems and different from each other. Ti-Core with titanium Group 1 yielded the greatest fluoride release over a 52-week period, followed closely by Ti-Core Natural of Group 2. The fluoride released for these systems in Groups 1-4 was similar to the ranges reported in other fluoridated dental cements and composite resins.

Distribution of cholesterol among its carriers in the bile of male and female hamsters.

The distribution of cholesterol among its carriers was studied in the bile of male and female hamsters. Sasco hamsters (Sasco Inc., Omaha, NE) were fed a semipurified diet with 0.0% cholesterol and 4% butterfat (group 1, males; group 4, females); a semipurified diet with 0.3% cholesterol and 1.2% palmitic acid (group 2, males; group 5, females); and a semipurified diet with 0.3% cholesterol and 4% safflower oil (group 3, males; group 6, females). At the end of six weeks, gallstones were found only in male hamsters receiving both cholesterol and dietary fat (fatty acid) (incidence of cholesterol stones: 90% in group 2; 22% in group 3). The biliary cholesterol carriers were separated and isolated from the bile of the hamsters by gel filtration chromatography, using the method of Pattinson [Pattinson, N.R., Willis, K.E., and Frampton, C.M. (1991) J. Lipid Res. 32, 205-214]. In those male hamsters that formed cholesterol gallstones, significant amounts of cholesterol were present in the void volume which contained large cholesterol phospholipid vesicles (void volume vesicles) (23% in group 2 and 15% in group 3). Smaller cholesterol/phospholipid vesicles were eluted next (fractions 30-45) and contained 15% of biliary cholesterol in group 2 and 21% in group 3. The remainder of the cholesterol was associated with mixed cholesterol/phospholipid/bile salt micelles. The cholesterol/phospholipid ratio was larger in both the void volume vesicles and small vesicles (2.40 and 1.48 in group 2; 2.56 and 1.33 in group 3, respectively) compared to the micelles (about 0.3 in groups 2 and 3).(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of dietary fat and fatty acids on sterol balance in hamsters.

Sterol balance studies, using both isotopic and chromatographic techniques, were carried out in hamsters fed semipurified diets to detect changes in sterol metabolism during the early period of the lithogenic stimulus. The balance studies examined animals in the first two weeks on the experimental lithogenic diets. The variables were as follows: dose of cholesterol (group 1, 0.05% vs. group 2, 0.2%); dietary fat (fatty acid) (group 2, butterfat vs. group 4, palmitic acid); source of hamster [group 2, Sasco (Omaha, NE) vs. group 3, Charles River (Wilmington, MA)]; average weight of animals (group 4, 60 g vs. group 5, 119 g). Animals in groups 1, 2, 3 and 5 maintained almost constant weight throughout the two-week balance study. Liver and plasma cholesterol levels increased in groups 2-5 with increasing dose of dietary cholesterol. The highest levels were found in group 4 (liver cholesterol, 32.7 mg/g; plasma cholesterol, 367 mg/dL). Sterol balance measurements showed that bile acid synthesis remained low (range 0.55-1.01 mg/d) for all groups regardless of the intake of dietary cholesterol (range, 3.27-20.90 mg/d). The dietary cholesterol absorbed from the intestine (range, 2.91-18.91 mg/d) was stored in the liver; this storage was reflected in the negative values for cholesterol balance for all groups (range, -0.70 to -14.97 mg/d). These studies did not reveal any correlations between parameters of sterol balance and cholelithiasis.

Comparison of the photoelastic stress for a split-shank threaded post versus a threaded post.

PURPOSE: This study compared the stress distribution during insertion and after cementation for two prefabricated endodontic posts, (1) a split-shank threaded post, and (2) a threaded post with no split in the shank. MATERIALS AND METHODS: Three test blocks of photoelastic material were prepared with simulated endodontic canals for each group studied. Specimens were then photographed after insertion and cementation. RESULTS: Minimal stresses were observed for the post with a split (group 1) throughout the length of the shaft, whereas asymmetric patterns of stresses for the post without a split (group 2) were present. Stress distribution for the post without a split were concentrated more apically rather than coronally. CONCLUSIONS: The presence of a split reduces insertion and cementation stresses for this post system. The split also acts as a vent during the cementation of the post into the canal and minimizes stress concentration during cementation.
