[Comparative studies of various suture techniques in transthoracic esophagogastric anastomoses in dogs (author's transl)].

In 27 dogs esophagogastric anastomoses were performed in a single-layered end-on, a three-layered end-on and a double-layered inverting technique. The anastomoses were reinvestigated on the 2., 4., 7., 15. and 30. postoperative day. Suture line leakages were not observed. The inverted suture resulted in considerable anastomotic stenosis, ulceration of the mucosa, disordered and initially incomplete revascularisation, and widespread granulomatous inflammatory reaction. In contrast, the end-on technique led to almost no stenosis, ordered revascularisation, and narrow and delicate scar tissue formation while maintaining a normally layered structure. The single-layered methode was superior to the three-layered one. The best results were obtained by the extramucous technique.

[Colonic anastomoses. Mechanical and morphological findings].

In healthy mongrel dogs 45 colonic anastomoses were performed in one--and two layered inverting and single layered end-on technique. They were compared 4, 8, and 18 days after surgery macroscopically, histologically and by tension testing. All criteria showed the single layered end-on technique to be superior. The principle of Lembert, aiming for contact of large areas of serosa, should be replaced by a technique resulting in exact reconstruction of intestinal wall without mucosal damage.

[Internal complications following jejunoileostomy in the treatment of extreme obesity].

More than 250 patients with extreme obesity were treated at the Chir. Univ.-Klinik Erlangen by 30 + 20 cm jejunoileostomy. The patients lose overweight and reach nearly normal weight after 9-12 months. Carbohydrate intolerance and hypertriglyceridema disappear. Ensuing malabsorption and also the surgical procedure are responsible for complications like wound infection or intussuception. The resulting chronic vomitting causes hypoproteinemia, hypokaliemia and liver dysfunction. Continuous therapeutical substitution is necessary, especially of potassium, to avoid deficiency. The diarrhea is treated by drug administration, i.e Reasec. The long time results are not yet sufficiently known. Calcium deficiency may occur many years later. The rate of cholelithiasis and nephrolithiasis ranges from 2 to 10%. The over-all lethality over 5 years is 2,8% as seen in the patients of our clinic during the past 6 years.

[Surgical disconnection of the small intestine as a contribution to plastic surgery].

The limitations of conservative therapy of extreme obesity compel the patient to seek a surgeon. He can do the following: Resect deposits of fat, create a condition of malabsorption by jejunoileostomy or perform a gastric by-pass, which reduces the capacity of the stomach. The gastric by-pass is the method of choice for treating the adipose child. We perform the jejunoileostomy only on patients with more than 100% overweight. With the weight reduction the pathologic metabolism is normalized. A plastic correction of pendant skin is necessary.

[Behaviour of intestinal goblet cells following application of oxidated water soluble terpenes. A pilot study (author's transl)].

1 and 3 h after i.p. application of Ozothin, an oxidation product of Bordeaux terpin oil, in rats there is a significant increase in goblet cells both totally and in the tightly filled ones (respectively in the villi of the small intestine). Double i.v. injection of therapeutic doses of Ozothin (first 14 h and booster 2 h preoperatively) induces a significant decrease in the number of the tightly filled goblet cells. These contradictory results may partly be explained as an initially timed ischesis of mucigenous glands by Ozothin. Statistically significant differences among the different series of tests could be found only in the epithelium of the villi. In the crypts there were similar results lacking statistical significance.

[Survival time and life quality after operative treatment of colonic cancer (author's transl)].

In regard to other intestinal malignomas colon cancer shows the chance of a long survival time. In cases of stage Dukes A and B the patient has a 5 year survival time of 100 to 82% depending on his age. In carcinoma of the rectum preoperative biopsy is necessary. In cases with grade III we prefer the exstirpation. The Erlangen magnetic closure of the colostomy gives continence in 70%.

Colonic Neoplasms↗

[Clinical importance of passage time and reflux after small intestine bypass surgery].

1. After jejunoileostomy patients rapidly loose body weight. This loss of weight may turn out to be too small in cases where there is a reflux. 2. There is a direct relationship between the rapidity of the loss of body weight and the duration of gastro-intestinal passage. The shorter the duration of passage, the quicker the patient's loss of body weight. 3. Duration of passage remains approximately constant during the first 9 months after jejunoileostomy as shown by the evaluation of 52 patients. From the 15th month onward the duration of passage rises significantly and reaches 2-3 times its initial value from the 18th month onward. Simultaneously, a cessation of the loss of body weight is to be observed owing to an adaptation to the malabsorption.

Adaptation, Physiological↗

[Radiological findings after jejunoileal shunt for the treatment of extreme adiposity].

Bypassing the small intestine is used for the treatment of extreme obesity. Because of the altered morphological and functional situation, the assessment of radiological sequelae is different. Passage is rapid, continuous observation is essential. Passage time is related to weight loss. Multiple small fluid levels in the small intestine are an expression of the stasis in the bypassed intestinal loops. They are of clinical importance only when there are other complaints at the same time. A reflux is of particular importance in relation to the expected loss in weight. Absorption studies with radioactive isotopes show the extent of the malabsorption.

Cecum↗

[Surgical therapy of obesity].

The experimental series by dogs and pigs show no specific absorption place in the small intestine--except the high selective absorption of vitamin B12 in the last ileum loop. Potassium absorption is better in the upper jejunum. For all other substances the conditions for absorption are the same in the jejunum as in the ileum. Calcium, carotin and cholesterol need a sufficiently extensive surface and show therefore a direct relation to the length of the small intestine. For the treatment of the extreme obesity we perform the jejunoileostomy with different lengths of jejunum and ileum. The hypercholesterinaemia type II can be treated by a partial ileum bypass. By the diminished absorption of the bile salts a sufficient decrease of the cholesterol level is possible, which amounts to about half the earlier value. The jejunoileostomy reduces not only overweight, but also improves the carbohydrate metabolism and lowers the increased fat levels.

[Results of palliative operations concerning the gastrointestinal tract].

At the Surgical Clinic of the University Erlanten-Nürnber 61 per cent of the carcinoma of the oesophagus, 57 per cent of malignant tumors of the stomach and 28 per cent of all tumors of the large bowel and the rectum could not be operated radically. If there are no metastases in other organs or a wide-spread peritoneal dissemination a palliative procedure is advisable. Resection of the tumor itself has been the most effectful prodedure in cases of palliative treatment. Gastrostomies, bypass-anastomoses and protheses have been comparatively less successful. Radiation is indicated only in cases of squamoun cell-carcinoma of the oesophagus. In cases of the carcinoma of the rectum located in the middle and lower part causing a severe stenosis coagulation is an effective treatment.

[The physiology of the isolated dog pancreas--the influence of the actual blood glucose level on the blood circulation in the pancreas].

1. Glucose loading tests were undertaken on isolated pancreas or pancreas-duodenal preparations. 2. In 75% of cases a vasodilatation can be observed which leads to enhanced blood circulation under constant pressure in the isolated organ. 3. This vasodilatation persists until the level of blood sugar has normalized. 4. The experiment being carried out on an isolated organ, external factors such as the vagus nerve, do not become active.
